1Immunoexpression of cyclooxygenase-2 in primary gastric carcinomas and lymph node metastases显示文摘AIM: To evaluate immunoexpression of cyclooxygenase-2 (COX-2) in primary gastric carcinomas and respective lymph node metastases. METHODS: Immunohistochemistry to analyze COX-2 expression was performed on tissue microarray slices obtained from 36 specimens of gastrectomy and satellite lymph nodes from patients with gastric carcinoma. RESULTS: Immunostaining was seen in most cases, and COX-2 expression was higher in lymph node me-tastases than in corresponding primary gastric tumors of intestinal, diffuse and mixed carcinomas, with a statistically signif icant difference in the diffuse histotype (P = 0.0108). 2Short-term Celecoxib intervention is a safe and effective chemopreventive for gastric carcinogenesis based on a Mongolian gerbil model显示文摘AIM:To evaluate the optimal intervention point of a selective cyclooxygenase-2(COX-2) inhibitor,Celecoxib,for inhibiting Helicobacter pylori(H pylori)-associated gastric carcinogenesis in Mongolian gerbils(MGs).METHODS:One hundred and twelve MGs were divided into six groups(A-F).One hundred gerbils were inoculated with H pylori(groups A-E).Twelve gerbils were inoculated with vehicle broth only(group F).After 4 wk,they were given N'-methyl-N' -nitro-N-nitroso-guanidine(MNNG)(50 μg/mL) in the drinking water for 20 wk.In groups B-E,the animals were given the stock Celecoxib(10 mg/kg per day) diet from the 21st,31st,21st and 41st week respectively.The periods of administering Celecoxib were 30,20,20,and 15 wk respectively.On the 51st week,the animals were sacrificed for histological examination.Local PCNA expression was examined by the immunohistochemistry method.The expression of COX-2 protein was assessed by Western Blot.Analysis used the χ2 test.The difference was regarded as signifi cant when P value was less than 0.05.RESULTS:Seventeen percent(17/100) of H pylori-infected MGs developed gastric cancer.All of these lesions were well-differentiated adenocarcinoma.The incidence rates of adenocarcinoma in groups A-F were 40%,0%,0%,20%,25%,and 0% respectively.The inflammatory scores were higher in group B than in other groups.There was no inflammatory response noted in group F.Celecoxib treatment resulted in a significant reduction in the proliferation of H pylori-infected mucosal cells(groups B,C and D)(P < 0.01).The expression of COX-2 protein was significantly attenuated in the groups which were Celecoxib-treated for more than 20 wk(groups B,C,D).The groups treated with Celecoxib had a significantly lower rate of advanced gastric cancer(34% vs 75%,P<0.001) There were no sudden deaths in any of the groups.CONCLUSION:Short-term treatment with Celecoxib has an anti-carcinogenic effect,and resulted in less severe inflammation and inhibited the invasive degree of gastric cancer.Chao-Hung Kuo Huang-Ming Hu Pei-Yun Tsai I-Chen Wu Sheau-Fang Yang Lin-Li Chang Jaw-Yuan Wang Chang-Ming Jan Wen-Ming Wang Deng-Chyang Wu 2009World Journal of Gastroenterology2009,15,39:9
3Hematologic manifestations of Helicobacter pylori infection显示文摘Helicobacter pylori(H. pylori) is the most common infection in humans, with a marked disparity between developed and developing countries. Although H. pylori infections are asymptomatic in most infected individuals, they are intimately related to malignant gastric conditions such as gastric cancer and gastric mucosaassociated lymphoid tissue(MALT) lymphoma and to benign diseases such as gastritis and duodenal and gastric peptic ulcers. Since it was learned that bacteria could colonize the gastric mucosa, there have been reports in the medical literature of over 50 extragastric manifestations involving a variety medical areas of specialization. These areas include cardiology, dermatology, endocrinology, gynecology and obstetrics, hematology, pneumology, odontology, ophthalmology, otorhinolaryngology and pediatrics, and they encompass conditions with a range of clear evidence between the H. pylori infection and development of the disease. This literature review covers extragastric manifestations of H. pylori infection in the hematology field. It focuses on conditions that are included in international consensus and management guides for H. pylori infection, specifically iron deficiency, vitamin B12(cobalamin) deficiency, immune thrombocytopenia, and MALT lymphoma. 4NAT2 polymorphism in Omani gastric cancer patients-risk predisposition and clinicopathological associations显示文摘AIM: To study whether N-acetyltransferase 2 (NAT2) genotypes and phenotypes are associated with increased risk factor for gastric cancer in Omani patients and to study the clinico-pathological correlations and the prognostic signifi cance of NAT2.METHODS: Genomic DNA was extracted from peripheral blood of 100 gastric cancer patients and 100 control subjects. NAT2 genotyping was performed using DNA sequencing. The prognostic significance of NAT2 and other clinicopathological features was assessed by univariate and multivariate analyses.RESULTS: We observed no significant association between NAT2 genotypes and phenotypes and gastric cancer risk. The NAT2 phenotype polymorphisms and gastric cancer risk predisposition were not modified by concomitant H pylori infection and smoking. There was no significant association between NAT2 and clinicopathological features, and NAT2 had no independent prognostic signifi cance.CONCLUSION: In the current study, NAT2 genotypes and phenotypes are not associated with gastric cancer risk predisposition. 5Genetic predisposition to Helicobacter pylori-induced gastric precancerous conditions显示文摘Gastric cancer is the most common malignancy of the gastrointestinal tract in East Asian populations and the second most frequent cause of cancer-related mortality in the world. While previous studies have investigated the genetic factors involved in gastric carcinogenesis, there still exist relatively few studies that have investigated the genetic traits associated with the risk of gastric precancerous conditions. In this paper we will review the biology and genetic polymorphisms involved in the genesis of gastric precancerous conditions reported to date and discuss the future prospects of this field of study. The associations of gastric precancerous conditions with polymorphisms in the cytotoxin-associated gene A-related genes (e.g. PTPN11 G/A at intron 3, rs2301756), those in the genes involved in host immunity against Helico-bacter pylori (H. pylori) infection (e.g.TLR4 +3725G/C, rs11536889) or polymorphisms of the genes essential for the development/ differentiation of the gastric epithelial cells (e.g. RUNX3 T/A polymorphism at intron 3, rs760805) have been reported to date. 6幽门螺杆菌感染与细胞因子的关系显示文摘目的 观察幽门螺杆菌(Hp)感染者胃黏膜、血清中细胞因子的水平变化,并探讨其临床意义.方法 采用荧光定量PCR(FQ-PCR)法检测95例胃病患者胃黏膜Hp,并将患者分为Hp阳性组与Hp阴性组;放射免疫分析(RIA)法测定患者胃黏膜和血清中的白细胞介素(IL)、肿瘤坏死因子(TNF-α)含量.结果 Hp阳性组胃黏膜中IL-2,IL-4,IL-8和TNF-α含量(ng/mg)分别为0.47±0.29,0.26±0.06,0.68±0.21和0.54±0.21,与Hp阴性组胃(0.85±0.23,0.18±0.01,0.41±0.10和0.38±0.18)相比,差异有统计学意义(t分别为9.500,7.500,8.359和4.102,P均〈0.01);同样,Hp阳性组血清中上述四种细胞因子含量(ng/ml)分别为1.29±0.94,0.74±0.32,0.44±0.20和2.78±1.12,与Hp阴性组(2.87±1.12,0.43±0.24,0.30±0.11和1.53±0.76)相比,差异有统计学意义(t分别为7.281,5.438,4.375和6.497,P均〈 0.01).结论 Hp感染后,患者胃黏膜和血清中的IL-2水平有下调趋势,而IL-4,IL-8和TNF-α水平均有上调趋势,提示细胞因子在幽门螺杆菌感染致胃部疾病中发挥着一定的作用.袁建芬 喻海忠 张小洁 洪宏 顾庆华 何秀萍 刘小丽 2013现代检验医学杂志2013,28,1:1
