| 1 | 匹维溴铵、培菲康联合黛力新治疗腹泻型肠易激综合征的疗效观察显示文摘目的:观察匹维溴铵、培菲康联合黛力新治疗腹泻型肠易激综合征的临床疗效。方法:选择符合罗马Ⅲ诊断标准的D-IBS 68例,随机分为观察组和对照组各34例。观察组予以匹维溴铵、培菲康(双歧杆菌三联活菌胶囊)、黛力新(氟哌塞吨+美利曲辛)口服。对照组予以匹维溴铵和培菲康口服。分别在治疗第1周、2周、4周对比有效率,同时记录两组治疗后腹痛和腹泻的症状评分及症状总积分并进行对比。结果:观察组和对照组各34例患者完成了4周的治疗随访。两组治疗后症状均有所改善,但治疗1周末有效率差别尚未见有统计学意义;治疗2周末,观察组的有效率即明显高于对照组(79.4%,50%),差别有统计学意义(χ2=6.434,P<0.05);治疗4周末差别更为明显(91.2%,58.8%;χ2=9.490,P<0.01)。观察组在治疗后1周、2周、4周腹痛和腹泻症状评分和症状总积分均低于对照组,但治疗1周末,仅腹痛的症状评分下降较对照组有统计学意义(t=2.376,P<0.05);治疗2周末,症状总积分与对照组相比差别有统计学意义(t=2.607,P<0.05);治疗4周末,腹泻症状评分较对照组差别方有统计学意义(t=2.433,P<0.05),而腹痛的症状评分较对照组差别进一步明显(t=5.143,P<0.001)。结论:匹维溴铵、培菲康联合黛力新治疗腹泻型肠易激综合征疗效高于匹维溴铵、培菲康联合方案,尤其腹痛症状的改善表现得较早且明显,而腹泻的症状改善表现得较晚并不如前者突出。 | 王崇强 李庭赞 徐胡根 | 2015 | 皖南医学院学报2015,34,4: | 12 |
| 2 | 肠易激综合征患者肠黏膜超微结构改变的电镜观察显示文摘目的观察肠易激综合征(IBS)患者小肠和结肠黏膜杯状细胞、浆细胞、神经内分泌细胞和肥大细胞超微结构的变化,探讨它们在IBS发病机制中的作用。方法用活检钳分别钳取IBS患者腹泻型、便秘型和对照组各10例回肠末端及升结肠黏膜,在透射电镜下观察肠黏膜杯状细胞、浆细胞、神经内分泌细胞和肥大细胞超微结构的变化。结果与正常组比较,两型IBS患者结肠和小肠黏膜上皮均可见到杯状细胞内黏液泡和融合的黏液分泌泡明显增多;神经内分泌细胞和肥大细胞超微结构有改变,呈现功能活跃状态。4例发病前有急性感染性腹泻病史的腹泻型IBS,肠黏膜中浆细胞多见,并有超微结构的改变。结论小肠和结肠黏膜内杯状细胞、浆细胞、神经内分泌细胞、肥大细胞等超微结构的改变,可能在IBS症状发生的神经免疫、内脏感觉过敏等机制中有一定的作用。 | 孔武明 龚均 陈明霞 董蕾 秦斌 | 2008 | 西安交通大学学报(医学版)2008,29,3: | 10 |
| 3 | 雌激素受体β在女性慢传输性便秘患者乙状结肠壁内的分布及其蛋白表达显示文摘目的研究女性慢传输性便秘(slow transit constipation,STC)患者雌激素受体β(estrogen receptor β,ERβ)在乙状结肠组织的分布及其蛋白表达。方法采用免疫组织化学及Western blot方法检测20例STC患者以及20例对照组患者乙状结肠ERβ的分布及其蛋白表达。结果免疫组化结果显示ERβ在STC组及对照组乙状结肠黏膜、肌间神经丛及黏膜下神经丛均有表达,肌层未见表达。与对照组相比,STC组ERβ表达均明显降低,差异有统计学意义(P<0.05)。Western blot结果显示STC组乙状结肠ERβ蛋白表达较对照组明显降低,差异有统计学意义(P<0.01)。结论肌间及黏膜下神经丛ERβ蛋白表达降低可能参与STC发病。 | 张广军 王崇树 魏寿江 刘宝华 童卫东 | 2010 | 中国普外基础与临床杂志2010,17,2: | 9 |
| 4 | Effect of low-dose amitriptyline on globus pharyngeus and its side effects显示文摘AIM:To compare the efficacy and side effects of lowdose amitriptyline(AMT)with proton pump inhibitor treatment in patients with globus pharyngeus.METHODS:Thirty-four patients who fulfilled the RomeⅢcriteria for functional esophageal disorders were included in this study.Patients were randomly assigned to receive either 25 mg AMT before bedtime(AMT group)or 40 mg Pantoprazole once daily for 4 wk(conventional group).The main efficacy endpoint was assessed using the Glasgow Edinburgh Throat Scale(GETS).The secondary efficacy endpoints included the Medical Outcomes Study 36-item short form health survey[social functioning(SF)-36]and the Pittsburgh Sleep Quality Index.Treatment response was defined as a>50%reduction in GETS scores.All patients entering this study recorded side effects at days 1,8,15,22 and 29 using a visual analogue scale.RESULTS:Thirty patients completed the study.After 4 wk of treatment,the AMT group had a greater response than the conventional group(75%vs 35.7%,P=0.004).At day 3,the AMT group showed significantly more improvement than the Conventional group in GETS score(3.69±1.14 vs 5.64±1.28,P=0.000).After 4 wk of treatment,the AMT group showed significantly greater improvement in GETS score and sleep quality than the Conventional group(1.25±1.84 vs 3.79±2.33,4.19±2.07 vs 8.5±4.97;P<0.01 for both).Additionally,the AMT group was more likely than the Conventional group to experience improvement in the SF-36,including general health,vitality,social functioning and mental health(P=0.044,0.024,0.049 and 0.005).Dry mouth,sleepiness,dizziness and constipation were the most common side effects.CONCLUSION:Low-dose AMT is well tolerated and can significantly improve patient symptoms,sleep and quality of life.Thus,low-dose AMT may be an effective treatment for globus pharyngeus. | Le-Qing You Jing Liu Lin Jia Shu-Man Jiang Gui-Qin Wang | 2013 | World Journal of Gastroenterology2013,19,42: | 7 |

| 6 | Coupling analysis of transcutaneous energy transfer coils with planar sandwich structure for a novel artificial anal sphincter显示文摘This paper presents a set of analytical expressions used to determine the coupling coefficient between primary and secondary Litz-wire planar coils used in a transcutaneous energy transfer(TET) system. A TET system has been designed to power a novel elastic scaling artificial anal sphincter system(ES-AASS) for treating severe fecal incontinence(FI), a condition that would benefit from an optimized TET. Expressions that describe the geometrical dimension dependence of self- and mutual inductances of planar coils on a ferrite substrate are provided. The effects of ferrite substrate conductivity, relative permeability, and geometrical dimensions are also considered. To verify these expressions, mutual coupling between planar coils is computed by 3D finite element analysis(FEA), and the proposed expressions show good agreement with numerical results. Different types of planar coils are fabricated with or without ferrite substrate. Measured results for each of the cases are compared with theoretical predictions and FEA solutions. | 7 | Autofluorescence imaging endoscopy can distinguish non-erosive reflux disease from functional heartburn: A pilot study显示文摘AIM: To investigate whether autofluorescence imaging(Af I) endoscopy can distinguish non-erosive reflux disease(NERD) from functional heartburn(fh).METHODS: In this prospective observational trial, 127 patients presenting with typical reflux symptoms for > 6 mo were screened. All the participants underwent endoscopy, during which white light imaging(WLI) was followed by Af I. finally 84 patients with normal esophageal appearance on WLI were enrolled. It was defined as being suggestive of NERD if one or more longitudinal purple lines longer than one centimeter were visualized in the distal part of the esophagus during Af I endoscopy. Ambulatory 24-h multichannel intraluminal impedance and ph monitoring was also performed. After standard proton-pump inhibitor(PPI) tests, subjects were divided into an NERD group and an fh group and the diagnostic performance of Af I endoscopy to differentiate NERD from fh was evaluated.RESULTS: Of 84 endoscopy-negative patients, 36(42.9%) had a normal ph/impedance test. Of these, 26 patients with favorable responses to PPI tests were classified as having NERD. finally 10 patients were diagnosed with fh and the others with NERD. Altogether, 68(81.0%) of the 84 patients were positive on Af I endoscopy. In the NERD group, there were 67(90.5%) patients with abnormal esophageal findings on Af I endoscopy while only 1(10%) patient was positive on Af I endoscopy in the fh group. The sensitivity and specificity of AFI in differentiating NERD from FH were 90.5%(95%CI: 81.5%-96.1%) and 90.0%(95%CI: 55.5%-99.7%), respectively. | 8 | Expression of intestinal mucosa tight junctions claudin proteins and mRNA in patients with irritable bowel syndrome显示文摘Objective:To investigate the changes of intestinal mucosa tight junctions (TJs) claudin-1, -3,-4 proteins and mRNA changes in patients with irritable bowel syndrome (IBS) and to elucidate their possible roles in the changes of bowel evacuation habit and formation.Methods:Claudin-1,-3,-4 proteins and mRNA were evaluated in intestinal mucosa in control group,D-IBS (diarrhea IBS) group and C-IBS (constipation IBS) group with immunohistochemical assay and Realtime-PCR.Results:It was observed that claudin-1,-3,-4 proteins were localized in the membranes of epithelial cells along the entire length of the plasma membrane including the apical end of the epithelial cells.The claudins were concentrated at the site of TJs only.Claudin-1,-3,-4 mRNA and claudin-1 protein in small intestinal mucosa and colonal mu- cous in D-IBS group were significantly downregulated (P<0.05).Claudin-1,-3,-4 mRNA and proteins in small intestinal mucosa and colonal mucous in C-IBS group were significantly upregulated (P<0.05). There was no significant difference in the expression of claudin-3 protein in both small intestinal mucosa and colonal mucous between D-IBS group and control group(P>0.05).Similarly,no significantly differ- ent expression of claudin-4 protein in colonal mucous in D-IBS group was found compared with control group (P>0.05).Otherwise,the expression of claudin-4 protein in small intestinal mucosa decreased in D-IBS group (P<0.05).Conclusion:Claudin-1,-3,-4 may play a potential important role in the changes of bowel evacuation habit and formation in patients with IBS.It is not due to the localization changes of claudin proteins in T J,but may be caused by the quantitative changes of the expression of TJ proteins and mRNA. | 孔武明 龚均 董蕾 鲁晓岚 徐俊荣 | 2007 | Journal of Medical Colleges of PLA(China)2007,22,3: | 0 |
