1Biliary cysts:Etiology,diagnosis and management显示文摘Biliary cysts(BC) are rare dilatations of different parts of a biliary tract.They account for approximately 1% of all benign biliary diseases.BC occur the most frequently in Asian and female populations.They are an important problem for pediatricians,gastroenterologists,radiologists and surgeons.Clinical presentation and management depend on the BC type.Cholangiocarcinoma is the most serious and dangerous BC complication.The other complications associated with BC involve cholelithiasis and hepatolithiasis,cholangitis,acute and chronic pancreatitis,portal hypertension,liver fibrosis and secondary liver cirrhosis and spontaneous cyst perforation.Different BC classifications have been described in the literature.Todani classification dividing BC into five types is the most useful in clinical practice.The early diagnosis and proper treatment are very important,because BC are associated with a risk of carcinogenesis.A malignancy risk increases with the age.Radiological investigations(ultrasonography,computed tomography,endoscopic retrograde cholangiopancreatography and magnetic resonance cholangiopancreatography) play an important role in BC diagnostics.Currently,prenatal diagnosis using ultrasonography is possible.It allows to differentiate biliary disorders in fetals and to perform the early surgical treatment that improves results.In most patients,total cyst excision with Roux-Y hepaticojejunostomy is the treatment of choice.Surgical treat-ment of BC is associated with high success rate and low morbidity and mortality.The early treatment is associated with a lower number of complications.Patients following BC surgery require permanent and careful postoperative observation using laboratory and imaging investigations because of possibility of biliary anastomosis stricture and biliary cancer in tissue remnant.Beata Jabońska 2012World Journal of Gastroenterology2012,18,35:26
2Response of soil catalase activity to chromium contamination显示文摘The impact of chromium(III) and (VI) forms on soil catalase activity was presented. The Orthic Podzol, Haplic Phaeozem and Mollic Gleysol from different depths were used in the experiment. The soil samples were amended with solution of Cr(III) using CrCl3, and with Cr(VI) using K2Cr2O7 in the concentration range from 0 to 20 mg/kg, whereas the samples without the addition of chromium served as control. Catalase activity was assayed by one of the commonly used spectrophotometric methods. As it was demonstrated in the experiment, both Cr(III) and Cr(VI) have an ability to reduce soil catalase activity. A chromium dosage of 20 mg/kg caused the inhibition of catalase activity and the corresponding contamination levels ranged from 75% to 92% for Cr(III) and 68% to 76% for Cr(VI), with relation to the control. 3Endoscopic submucosal dissection for the treatment of neoplastic lesions in the gastrointestinal tract显示文摘AIM: To investigate the indications, resection rate, and safety of endoscopic submucosal dissection (ESD) for neoplastic lesions in the gastrointestinal tract at a European referral center. METHODS: We carried out a retrospective analysis of the ESD procedures performed in our center for mucosal neoplastic and submucosal lesions of the gastrointestinal tract. The duration of the procedure, en bloc and complete (R0) resection rates, and complication rates were evaluated. Variables were reported as mean ± SD or simple proportions. Univariate analysis and comparisons of procedure times and resection rates were performed using Mann-Whitney U tests, or χ2 tests for dichotomous variables.RESULTS: Between 2007 and 2011, ESD was performed in a total of 103 patients (46.7% male, mean age 64.0 ± 12.7 years). The indications for the procedure were epithelial tumor (n = 54), submucosal tumor (n = 42), or other (n = 7). The total en bloc resection rate was 90.3% (93/103) and R0 resection rate 80.6% (83/103). The median speed of the procedure was 15.0 min/cm2 . The complete resection rate was lower for submucosal tumors arising from the muscle layer (68%, 15/22, P < 0.05). Resection speed was quicker for submucosal tumors localized in the submucosal layer than for lesions arising from the muscularis propria layer (8.1 min/cm 2 vs 17.9 min/cm 2 , P < 0.05). The R0 resection rate and speed were better in the last 24 mo (90.1%, 49/54 and 15.3 min/cm 2 ) compared to the first 3 years of treatment (73.5%, 36/49, P < 0.05 and 22.0 min/cm2 , P < 0.05). Complications occurred in 14.6% (n = 15) of patients, including perforation in 5.8% (n = 6), pneumoperitoneum in 3.9% (n = 4), delayed bleeding in 1.9% (n = 2), and other in 2.9% (n = 3). Only one patient with delayed perforation required surgical treatment. During the mean follow-up of 26 ± 15.3 mo, among patients with R0 resection, recurrence occurred in one patient (1.2%).CONCLUSION: ESD is an effective and safe method for resection of neoplastic lesions with low recurrence. 4Iatrogenic bile duct injuries:Etiology,diagnosis and management显示文摘Iatrogenic bile duct injuries (IBDI) remain an important problem in gastrointestinal surgery. They are most frequently caused by laparoscopic cholecystectomy which is one of the commonest surgical procedures in the world. The early and proper diagnosis of IBDI is very important for surgeons and gastroenterologists, because unrecognized IBDI lead to serious complications such as biliary cirrhosis, hepatic failure and death. Laboratory and radiological investigations play an important role in the diagnosis of biliary injuries. There are many classifications of IBDI. The most popular and simple classification of IBDI is the Bismuth scale. Endoscopic techniques are recommended for initial treatment of IBDI. When endoscopic treatment is not effective, surgical management is considered. Different surgical reconstructions are performed in patients with IBDI. According to the literature, Roux- en-Y hepaticojejunostomy is the most frequent surgical reconstruction and recommended by most authors. In the opinion of some authors, a more physiological and equally effective type of reconstruction is end- to-end ductal anastomosis. Long term results are the most important in the assessment of the effectiveness of IBDI treatment. There are a few classifications for the long term results in patients treated for IBDI; the Terblanche scale, based on clinical biliary symptoms, is regarded as the most useful classification. 5云南常见牛肝菌属真菌中汞含量及食用安全评价显示文摘汞(Hg)是危害人类健康的主要重金属元素之一,多数食用菌对Hg有很强的富集能力,测定食用菌中Hg含量,并对其进行食用安全性评价具有重要意义。采用冷原子吸收直接测汞仪系统测定85份云南常见牛肝菌属真菌菌盖、菌柄中总Hg含量;以同一牛肝菌子实体菌盖与菌柄总Hg含量比(Q(C/S))分析牛肝菌属真菌对Hg的富集特征;根据联合国粮农组织和世界卫生组织(FAO/WHO)现行每周Hg允许摄入量(provisional tolerable week—lyintake,PTWI)标准和中国GB2762—2012规定的食用菌中Hg限量标准评价样品的食用安全性。结果表明,菌盖和菌柄中总Hg含量分别在0.13—22.00、0.20~8.40mg·kg^-1DW,不同产地、种类及不同采集年份的样品中总Hg含量存在明显差异;同一牛肝菌菌盖、菌柄总Hg含量比(Q(C/S))在0.28~4.08,92%的样品Q((C/S))〉1,表明多数样品中菌盖对Hg的富集能力强于菌柄。根据GB2762-2012规定的食用菌及其制品中总Hg含量限量标准(≤0.1mg·kg^-1),发现所有测试样品的总Hg含量均超标;根据PTWI标准,假设成年人(60kg)每周食用300g新鲜牛肝菌,则多数样品Hg摄入量低于允许摄入量,无安全风险,而少数样品Hg摄入量高于允许摄入量,食用有一定的潜在风险。同时,牛肝菌总Hg含量与种类、产地、采集时间等因素密切相关,采食及安全评价需综合考虑这些因素。杨天伟 张霁 Jerzy FALANDYSZ Grazyna KRASIИSKA Martyna SABA 王元忠 刘鸿高 2015生态学杂志2015,34,12:12
6Role of environmental pollution in irritable bowel syndrome显示文摘Irritable bowel syndrome(IBS),with the prevalence of 10%-20 % of the population has become an emerging problem worldwide. IBS is a functional gastrointestinal(GI) disorder characterized by abdominal pain or discomfort and altered bowel habits. The etiology of IBS contains genetic,psychological,and immunological factors,and has not been fully elucidated; of note,recent studies also point at environmental pollution and its role in the development of functional GI diseases. In this review we focus on several environmental factors,such as bacterial contamination,air pollution,radiation and even stress as potential triggers of IBS. We discuss associated disturbances in homeostasis,such as changes in intestinal microbiome and related pathophysiological mechanisms. 7Developmental Toxicity in Mice Following Paternal Exposure to Di-N-Butyl-Phthalate (DBP)显示文摘Objective The aim of the present study was to investigate the effects of paternal Di‐N‐butyl‐phthalate (DBP) exposure pre‐ and postnatally on F1 generation offspring,and prenatally on F2 generation offspring.Methods Male mice were exposed to either 500 mg/kg or 2 000 mg/kg of DBP for 8 weeks,and mated with non‐exposed females.Three‐quarters of the females were sacrificed a day prior to parturition,and examined for the number of living and dead implantations,and incidence of gross malformations.Pups from the remaining females were assessed for developmental markers,growth parameters,as well as sperm quantity and quality.Results There were no changes in the fertility of parents and in intrauterine development of the offspring.Pups of DBP‐exposed males demonstrated growth‐retardation.Following paternal exposure to 500 mg/kg bw of DBP,there were almost twice the number of males than females born in the F1 generation.F1 generation females had a 2.5‐day delay in vaginal opening.Paternal exposure to 2 000 mg/kg bw of DBP increased the incidence of sperm head malformations in F1 generation males;however,there were no changes in the fertility and viability of foetuses in the F2 generation.Conclusion Paternal DBP exposure may disturb the sex ratio of the offspring,delay female sexual maturation,and deteriorate the sperm quality of F1 generation males.Malgorzata M.DOBRZY SKA Ewa J.TYRKIEL Krzysztof A.PACHOCKI 2011Biomedical and Environmental Sciences2011,24,5:9
8Nitrogen removal from wastewater and bacterial diversity in activated sludge at different COD/N ratios and dissolved oxygen concentrations显示文摘The impact of the organic carbon to nitrogen ratio (chemical oxygen demand (COD)/N) in wastewater and dissolved oxygen (DO) concentration on carbon and nitrogen removal efficiency,and total bacteria and ammonia-oxidizing bacteria (AOB) communities in activated sludge in constantly aerated sequencing batch reactors (SBRs) was determined.At DO of 0.5 and 1.5 mg O2 /L during the aeration phase,the efficiency of ammonia oxidation exceeded 90%,with nitrates as the main product.Nitrification and denitrification achieved under the same operating conditions suggested the simultaneous course of these processes.The most effective nitrogen elimination (above 50%) was obtained at the COD/N ratio of 6.8 and DO of 0.5 mg O2 /L.Total bacterial diversity was similar in all experimental series,however,for both COD/N ratios of 6.8 and 0.7,higher values were observed at DO of 0.5 mg O2 /L.The diversity and abundance of AOB were higher in the reactors with the COD/N ratio of 0.7 in comparison with the reactors with the COD/N of 6.8.For both COD/N ratios applied,the AOB population was not affected by oxygen concentration.Amplicons with sequences indicating membership of the genus Nitrosospira were the determinants of variable technological conditions.Magdalena Zielińska Katarzyna Bernat Agnieszka Cydzik-Kwiatkowska Joanna Sobolewska Irena Wojnowska-Baryla 2012Journal of Environmental Sciences2012,24,6:9
9Virulence factors of Enterococcus strains isolated from patients with inflammatory bowel disease显示文摘AIM: To determine the features of Enterococcus that contribute to the development and maintenance of the inflammatory process in patients with inflammatory bowel disease (IBD). METHODS: Multiplex polymerase chain reaction (PCR) was applied to assess the presence of genes that encode virulence factors [surface aggregating protein (asa1), gelatinase (gelE), cytolysin (cylA), extracellular surface protein (esp) and hyaluronidase (hyl)] in the genomic DNA of 28 strains of Enterococcus isolated from the intestinal tissues of children with IBD (n =16) and of children without IBD (controls; n = 12). Additionally, strains with confirmed presence of the gelE gene were tested by PCR for the presence of quorum sensing genes (fsrA, fsrB, fsrC) that control the gelatinase production. Gelatinase activity was tested on agar plates containing 1.6% gelatin. We also analysed the ability of Enterococcus strains to release and decompose hydrogen peroxide (using Analytical Merckoquant peroxide test strips) and tested their ability to adhere to Caco-2 human gut epithelium cells and form biofilms in vitro. RESULTS: A comparison of the genomes of Enterococcus strains isolated from the inflamed mucosa of patients with IBD with those of the control group showed statistically significant differences in the frequency of theasa1 gene and thegelE gene. Furthermore, the cumulative occurrence of different virulence genes in the genome of a single strain ofEnterococcus isolated from the IBD patient group is greater than in a strain from the control group, although no significant difference was found. Statistically significant differences in the decomposition of hydrogen peroxide and adherence to the Caco-2 epithelial cell line between the strains from the patient group and control group were demonstrated. The results also showed that profuse biofilm production was more frequent amongEnterococcus strains isolated from children with IBD than in control strains. 10Scale of seismic and rock burst hazard in the Silesian companies in Poland显示文摘Presently the seismic and rock burst hazard appears still to be important in most of hard coal mines in Poland. Recently, there was a significant increase of seismic activity of the Silesian rock massive, when compared with the previous years. In the period 1999–2008 the hard coal mines experienced 34 rock bursts. The causes of rockburst occurrence are presented based on the analysis of the rockbursts occurring in the Polish hard coal mines. The scale of the rockburst hazard has been characterized with respect to the mining and geological conditions of the existing exploitation. Of the factors influencing the state of rockburst hazard, the most essential one is considered the depth interval ranging from 600 m to 900 m. 11Synchronous and metachronous neoplasms in gastric cancer patients:A 23-year study显示文摘AIM:To determine the prevalence and characteristics of additional primary malignancies in gastric cancer(GC)patients.METHODS:GC patients(862 total;570 men,292women;mean age 59.8±12.8 years)diagnosed at the Department of Gastroenterology at Pomeranian Medical University over a period of 23 years were included in this retrospective analysis of a prospectively maintained database.Mean follow-up time was 31.3±38.6 mo(range 1-241 mo).The following clinicopathological features of patients with synchronous tumors were compared to those with metachronous tumors:age,sex,symptom duration,family history of cancer,tumor site,stage(early vs advanced),histology,and blood group.GC patients with and without a second tumor were compared in terms of the same clinicopathological features.RESULTS:Of 862 GC patients,58(6.7%)developed a total of 62 multiple primary tumors,of which 39(63%)were metachronous and 23(37%)synchronous.Four(6.9%)of the 58 multiple GC patients developed two or more neoplasms.The predominant tumor type of the secondary neoplasms was colorectal(n=17),followed by lung(n=9),breast(n=8),and prostate(n=7).Age was the only clinicopathological feature that differed between GC patients with synchronous vs metachronous malignancies;GC patients with synchronous neoplasms were older than those with metachronous neoplasms(68.0±10.3 years vs 59.9±11.1years,respectively,P=0.008).Comparisons between patients with and without a second primary cancer revealed that the only statistically significant differences were in age and blood group.The mean age of the patients with multiple GC was higher than that of those without a second primary tumor(63.4±11.4years vs 59.5±13.0 years,respectively,P=0.026).GC patients with a second primary tumor were more commonly blood group O than those without(56.2%vs 31.6%,respectively,P=0.002).CONCLUSION:GC patients may develop other primary cancers;appropriate preoperative and postoperative diagnostic modalities are thus required,particularly if patients are older and blood group O.Magorzata Lawniczak Alicja Gawin Halina Jaroszewicz-Heigelmann Wiesawa Rogoza-Mateja Joanna Raszeja-Wyszomirska Andrzej Biaek Katarzyna Karpińska-Kaczmarczyk Teresa Starzyńska 2014World Journal of Gastroenterology2014,20,23:8
12Intestinal epithelium, intraepithelial lymphocytes and the gut microbiota-Key players in the pathogenesis of celiac disease显示文摘Celiac disease(CD) is a chronic immune-mediated disorder triggered by the ingestion of gluten in genetically predisposed individuals. Before activating the immune system, gluten peptides are transferred by the epithelial barrier to the mucosal lamina propria, where they are deamidated by intestinal tissue transglutaminase 2. As a result, they strongly bind to human leucocyte antigens(HLAs), especially HLA-DQ2 and HLA-DQ8, expressed on antigen-presenting cells. This induces an inflammatory response, which results in small bowel enteropathy. Although gluten is the main external trigger activating both innate and adaptive(specific) immunity, its presence in the intestinal lumen does not fully explain CD pathogenesis. It has been hypothesized that an early disruption of the gut barrier in genetically susceptible individuals, which would result in an increased intestinal permeability, could precede the onset of gluten-induced immune events. The intestinal barrier is a complex functional structure, whose functioning is dependent on intestinal microbiotahomeostasis, epithelial layer integrity, and the gutassociated lymphoid tissue with its intraepithelial lymphocytes(IELs). 13Lung epithelial cell-derived IL-25 negatively regulates LPS-induced exosome release from macrophages显示文摘Background: Acute lung injury(ALI) is a major component of multiple organ dysfunction syndrome(MODS) following pulmonary and systemic infection. Alveolar macrophages(AMφ) are at the center of ALI pathogenesis. Emerging evidence has shown that cell-cell interactions in the lungs play an important regulatory role in the development of acute lung inflammation. However, the underneath mechanisms remain poorly addressed. In this study, we explore a novel function of lung epithelial cells(LEPCs) in regulating the release of exosomes from AMφ following LPS stimulation.Methods: For the in vivo experiments, C57 BL/6 wildtype(WT) mice were treated with lipopolysaccharide(LPS)(2 mg/kg) in 0.2 ml of saline via intratracheal aerosol administration. Bronchoalveolar lavage fluid was collected at 0–24 h after LPS treatment, and exosomes derived from AMφ were measured. For the in vitro studies, LEPCs and bone marrowderived Mφ(BMDM) were isolated from WT or TLR4-/-mice and were then cocultured in the Transwell? system. After coculture for 0–24 h, the BMDM and supernatant were harvested for the measurement of exosomes and cytokines.Results: We demonstrate that LPS induces macrophages(Mφ) to release exosomes, which are then internalized by neighboring Mφ to promote TNF-α expression. The secreted interleukin(IL)-25 from LEPCs downregulates Rab27 a and Rab27 b expression in Mφ, resulting in suppressed exosome release and thereby attenuating exosome-induced TNF-α expression and secretion.Conclusion: These findings reveal a previously unidentified crosstalk pathway between LEPCs and Mφ that negatively regulates the inflammatory responses of Mφ to LPS. 14Evaluation of enterochromaffin cells and melatonin secretion exponents in ulcerative colitis显示文摘AIM: To study an assessment of the number of enterochromaffin cells and expression of hydroxyindole-Omethyltransferase in colonic mucosa and urine excretion of 6-sulfatoxymelatonin in patients with ulcerative colitis. METHODS: The study included 30 healthy subjects (groupⅠ-C), 30 patients with ulcerative proctitis [group Ⅱ-ulcerative proctitis (UP)] and 30 patients with ulcerative colitis [group Ⅲ-ulcerative colitis (UC)] in acute phases of these diseases. The number of enterochromaffin cells (EC) was estimated in rectal and colonic mucosa. Bioptates were assembled from many different parts of the large intestine. Immunorective cells collected from various parts of the colon were counted according to the Eurovision DAKO (Dako A/S, Copenhagen, Denmark) System in the range of 10 fields in each bioptate at × 200 magnification. The level of mRNA expression of hydroxyindole-O-methyltransferase (HIOMT) in colonic mucosa was estimated with RT-PCR. Urine 6-sulfatoxymelatonin (6-HMS) excretion was determined immunoenzymatically using an IBL (IBL International GmbH, Hamburg, Germany) kit (RE 54031). RESULTS: The number of EC cells in healthy subjects (C) was 132.40 ± 31.26. In patients of group Ⅱ (UP) and group Ⅲ (UC) the number of these cells was higher 225.40 ± 37.35 (P < 0.001) and 225.24 ± 40.50 (P < 0.001) respectively. Similar differences were related to HIOMT expression, which was 1.04 ± 0.36 in group C, 1.56 ± 0.56 (P < 0.01) in group UP and 2.00 ± 0.35 (P < 0.001) in group UC. Twenty-four hour 6-HMS urinary excretion was as follows: C 16.32 ± 4.95 μg/24 h, UP 26.30 ± 7.29 μg/24 h (P < 0.01), UC 42.30 ± 12.56 μg/24h (P < 0.001). A correlation between number of EC cells and 6-HMS excretion was noted in all groups: r = 0.766 in patients with UP, r = 0.703 with UC and r = 0.8551 in the control group; the correlation between the results is statistically significant. CONCLUSION: In the acute phases of both UP and UC, proliferation of EC cells and high expression of HIOMT and urine excretion of 6-HMS is noted. 15Is liver biopsy still needed in children with chronic viral hepatitis?显示文摘Liver biopsy is a standard method used for obtaining liver tissue for histopathological evaluation. Since reliable serological and virological tests are currently available, liver biopsy is no longer needed for the etiological diagnosis of chronic hepatitis B and C. However, liver histology remains the gold standard as a prognostic tool, providing information about the liver disease progression(grading of necroinflammatory activity and staging of fibrosis) and serving clinicians in the management and therapeutic decisions. In general, histopathological evaluation is indicated before starting the antiviral treatment. Main limitations of the liver biopsy include its invasive and painful procedure, sampling errors and the inter- and intra-observer variability. In addition, indications for the liver biopsy in pediatric patients with chronic viral hepatitis were questioned recently, and efforts have been made toward the development of non-invasive methods as an alternative to the liver biopsy. The most commonly used methods are novel imaging studies(elastography) and combinations of biomarkers. However, to date, none of these tests was validated in children with chronic viral hepatitis. 16Impact of corneal parameters on intraocular pressure measurements in different tonometry methods显示文摘AIM: To evaluate the impact of central corneal thickness(CCT) and corneal curvature on intraocular pressure(IOP) measurements performed by three different tonometers. METHODS: IOP in 132 healthy eyes of 66 participants was measured using three different tonometry techniques: Goldmann applanation tonometer(GAT), Pascal dynamic contour tonometer(DCT), and ICare rebound tonometer(RT). CCT and corneal curvature were assessed. RESULTS: In healthy eyes, DCT presents significantly higher values of IOP than GAT(17.34±3.69 and 15.27±4.06 mm Hg, P<0.0001). RT measurements are significantly lower than GAT(13.56±4.33 mm Hg, P<0.0001). Compared with GAT, DCT presented on average 2.51 mm Hg higher values in eyes with CCT<600 μm and 0.99 mm Hg higher results in eyes with CCT≥600 μm. The RT results were lower on average by 1.61 and 1.95 mm Hg than those obtained by GAT, respectively. Positive correlations between CCT in eyes with CCT<600 μm were detected for all IOP measurement techniques, whereas a similar relationship was not observed in eyes with thicker corneas. A correlation between IOP values and keratometry in the group with CCT<600 μm was not detected with any of the tonometry methods. In thicker corneas, a positive correlation was found for GAT and mean keratometry values(R=0.369, P=0.005). CONCLUSION: The same method should always be chosen for routine IOP control, and measurements obtained by different methods cannot be compared. 17Sorption Properties of Hard Coals with Regard to Gases Present in the Mine Atmosphere显示文摘In this study, the sorption capacities of hard coals with reference to gases present in the mine atmosphere: carbon dioxide, methane, ethane, ethene, propane, propene, carbon monoxide and hydrogen were examined. For laboratory tests, 4 samples of hard coals collected from exploitable coal seams were selected. Among considered gases the mostly sorbed is carbon dioxide, in slightly smaller amounts are sorbed unsaturated hydrocarbons: ethene and propene. Their large sorption results from i.a. interactions of ? electrons in the double bond between carbon atoms in the ethene or propene molecules with energetic centers of the coal surface. The amounts of the sorbed saturated hydrocarbons: ethane and propane are small, which is probably related to the adsorption mechanism of their sorption by coals. In the smallest amounts are sorbed hydrogen and carbon monoxide, the values of their sorption being maximum 5% of hard coals sorption capacity in relation to carbon dioxide. Based on the experiments it was found that the largest amounts of gases are sorbed by lowrank, highly porous coals with high oxygen content. The process of sorption of unsaturated hydrocarbons: ethene and propene can lead to a reduction in concentration of those gases in the mine atmosphere and thus may affect correctness of the self-heating phenomenon assessment.agnieszka dudzińska 2017Journal of Earth Science2017,28,1:5

19Hepatectomy for bile duct injuries: When is it necessary?显示文摘Iatrogenic bile duct injuries(IBDI)are still a challenge for surgeons.The most frequently,they are caused by laparoscopic cholecystectomy which is one of the commonest surgical procedure in the world.Endoscopic techniques are recommended as initial treatment of IBDI.When endoscopic treatment is not effective,surgery is considered.Different surgical biliary reconstructions are performed in most patients in IBDI.Roux-Y hepaticojejunostomy is the commonest biliary reconstruction for IBDI.In some patients with complex IBDI,hepatectomy is required.Recently,Liet al analyzed the factors that had led to hepatectomy for patients with IBDI after laparoscopic cholecystectomy(LC).Authors concluded that hepatectomy might be necessary to manage early or late complications after LC.The study showed that proximal IBDI(involving hepatic confluence)and IBDI associated with vascular injuries were the two independent risk factors of hepatectomy in this series.Authors distinguished two main groups of patients that require liver resection in IBDI:those with an injury-induced liver necrosis necessitating early intervention,and those in whom liver resection is indicated for treatment of liver atrophy following long-term cholangitis.In this commentary,indications for hepatectomy in patients with IBDI are discussed.Complex biliovascular injuries as indications for hepatectomy are presented.Short-and long-term results in patients following liver resection for IBDI are also discussed.Hepatectomy is not a standard procedure in surgical treatment of IBDI,but in some complex injuries it should be considered.Beata Jablońska 2013World Journal of Gastroenterology2013,19,38:4
20Inhibition of ileal bile acid transporter:An emerging therapeutic strategy for chronic idiopathic constipation显示文摘Chronic idiopathic constipation is a common disorder of the gastrointestinal tract that encompasses a wide profile of symptoms. Current treatment options for chronic idiopathic constipation are of limited value; therefore, a novel strategy is necessary with an increased effectiveness and safety. Recently, the inhibition of the ileal bile acid transporter has become a promising target for constipation-associated diseases. Enhanced delivery of bile acids into the colon achieves an accelerated colonic transit, increased stool frequency, and relief of constipationrelated symptoms.
