3Endoscopic ultrasound elastography for evaluation of lymph nodes and pancreatic masses:A multicenter study显示文摘AIM:To evaluate the ability of endoscopic ultrasound(EUS) elastography to distinguish benign from malignant pancreatic masses and lymph nodes.METHODS:A multicenter study was conducted and included 222 patients who underwent EUS examination with assessment of a pancreatic mass(n=121) or lymph node(n=101).The classification as benign or malignant,based on the real time elastography pattern,was compared with the classif ication based on the B-mode EUS images and with the fi nal diagnosis obtained by EUS-guided fi ne needle aspiration(EUS-FNA) and/or by surgical pathology.An interobserver study was performed.RESULTS:The sensitivity and specificity of EUS elastography to differentiate benign from malignant pancreatic lesions are 92.3% and 80.0%,respectively,compared to 92.3% and 68.9%,respectively,for the conventional B-mode images.The sensitivity and specificity of EUS elastography to differentiate benign from malignant lymph nodes was 91.8% and 82.5%,respectively,compared to 78.6% and 50.0%,respectively,for the B-mode images.The kappa coefficient was 0.785 for the pancreatic masses and 0.657 for the lymph nodes.CONCLUSION:EUS elastography is superior compared to conventional B-mode imaging and appears to be able to distinguish benign from malignant pancreatic masses and lymph nodes with a high sensitivity,specificity and accuracy.It might be reserved as a second line examination to help characterise pancreatic masses after negative EUS-FNA and might increase the yield of EUS-FNA for lymph nodes.Marc Giovannini Botelberge Thomas Bories Erwan Pesenti Christian Caillol Fabrice Esterni Benjamin Monges Geneviève Arcidiacono Paolo Deprez Pierre Yeung Robert Schimdt Walter Schrader Hanz Szymanski Carl Dietrich Christoph Eisendrath Pierre Van Laethem Jean-Luc Devière Jacques Vilmann Peter Saftoiu Andrian 2009World Journal of Gastroenterology2009,15,13:48
4Hepatoprotective activity of Sapindus mukorossi and Rheum emodi extracts: In vitro and in vivo studies显示文摘AIM: To study the hepatoprotective capacity of Sapindus mukorossi (S. mukorossi) and Rheum emodi (R. emodi) extracts in CCl4 treated male rats. METHODS: The dried powder of S. mukorossi and R. emodi was extracted successively with petroleum ether, benzene, chloroform, and ethanol and concentrated in vacuum. Primary rat hepatocyte monolayer cultures were used for in vitro studies. In vivo, the hepatoprotective capacity of the extract of the fruit pericarp of S. mukorossi and the rhizomes of R. emodi was analyzed in liver injured CCl4-treated male rats. RESULTS: In vitro: primary hepatocytes monolayer cultures were treated with CCl4 and extracts of S. mukorossi & R. emodi. A protective activity could be demonstrated in the CCl4 damaged primary monolayer culture. In vivo : extracts of the fruit pericarp of S. mukorossi (2.5 mg/mL) and rhizomes of R. emodi (3.0 mg/mL) were found to have protective properties in rats with CCl4 induced liver damage as judged from serum marker enzyme activities. 5子宫腺肌病患者药物治疗进展显示文摘药物治疗在缓解子宫腺肌病患者相关症状、提高手术疗效、延缓疾病进展和促进辅助生育方面具有重要的作用。非甾体抗炎药是控制子宫腺肌病相关疼痛的首选药物,是近期有生育要求患者的唯一选择;甾体激素、促性腺激素释放激素激动剂和米非司酮等药物可有效减轻疼痛和控制子宫出血,其中以口服避孕药、左炔诺孕酮宫内节育系统(曼月乐)和地诺孕素等甾体激素的疗效更确切。目前尚无用于子宫腺肌病的特效药物,应根据患者的年龄、症状、子宫大小、生育要求和经济条件等情况选择性应用,但停药后症状易复发。因此,子宫腺肌病患者药物长期应用有待进一步研究。Baboo Kalianee Devi 陈正云 张信美 2019浙江大学学报(医学版)2019,48,2:22
6Biochemical changes in the injured brain显示文摘Brain metabolism is an energy intensive phenomenon involving a wide spectrum of chemical intermediaries. Various injury states have a detrimental effect on the biochemical processes involved in the homeostatic and electrophysiological properties of the brain. The biochemical markers of brain injury are a recent addition in the armamentarium of neuro-clinicians and are being increasingly used in the routine management of neuropathological entities such as traumatic brain injury, stroke, subarachnoid haemorrhage and intracranial space occupying lesions. These markers are increasingly being used in assessing severity as well as in predicting the prognostic course of neuro-pathological lesions. S-100 protein, neuron specific enolase, creatinine phosphokinase isoenzyme BB and myelin basic protein are some of the biochemical markers which have been proven to have prognostic and clinical value in the brain injury. While S-100, glial fibrillary acidic protein and ubiquitin C terminal hydrolase are early biomarkers of neuronal injury and have the potential to aid in clinical decisionmaking in the initial management of patients presenting with an acute neuronal crisis, the other biomarkers are of value in predicting long-term complications and prognosis in such patients. 7Adjuvants to local anesthetics: Current understanding and future trends显示文摘Although beneficial in acute and chronic pain management, the use of local anaesthetics is limited by itsduration of action and the dose dependent adverse effects on the cardiac and central nervous system. Adjuvants or additives are often used with local anaesthetics for its synergistic effect by prolonging the duration of sensory-motor block and limiting the cumulative dose requirement of local anaesthetics. The armamentarium of local anesthetic adjuvants have evolved over time from classical opioids to a wide array of drugs spanning several groups and varying mechanisms of action. A large array of opioids ranging from morphine, fentanyl and sufentanyl to hydromorphone, buprenorphine and tramadol has been used with varying success. However, their use has been limited by their adverse effect like respiratory depression, nausea, vomiting and pruritus, especially with its neuraxial use. Epinephrine potentiates the local anesthetics by its antinociceptive properties mediated by alpha-2 adrenoreceptor activation along with its vasoconstrictive properties limiting the systemic absorption of local anesthetics. Alpha 2 adrenoreceptor antagonists like clonidine and dexmedetomidine are one of the most widely used class of local anesthetic adjuvants. Other drugs like steroids(dexamethasone), anti-inflammatory agents(parecoxib and lornoxicam), midazolam, ketamine, magnesium sulfate and neostigmine have also been used with mixed success. The concern regarding the safety profile of these adjuvants is due to its potential neurotoxicity and neurological complications which necessitate further research in this direction. Current research is directed towards a search for agents and techniques which would prolong local anaesthetic action without its deleterious effects. The understanding of the mechanism of hypotension and the choice of vasopressor has evolved over the years to a point where phenylephrine has become the preferred vasopressor. Due to the absence of definitive evidence showing absolute clinical benefit of one over the other, especially in emergency and high-risk Cesarean sections, our choice of phenylephrine over the other vasopressors like mephentermine, metaraminol, and ephedrine is guided by indirect evidence on fetalacid-base status. 11Antimicrobial potential of selected brown seaweeds from Vedalai coastal waters,Gulf of Mannar显示文摘Objective:To evaluate the antimicrobial activity of Turbinaria conoides(T.conoides).Padina gymnospora(P.gymnospora) and Sargassum tenerrimum against human bacterial and fungal pathogens.Methods:The antimicrobial activities of the extracts against various organisms were tested by using disc diffusion method.Results:The methanol extract showed the better result than the other extracts.Whereas,the strong antibacterial inhibition was noted in methanol extracts of P.gymnospora against Bacillus subtilus(26.33±1.86) and the mild inhibition of ethanol extracts from T.conoides against Klebsiella pneumoniae(2.33±0.51).Acetone extraction of P. gymnospora had strong antifungal inhibition against Cryptococcus neoformans(23.00±1.78), and acetone extract of T.conoides had mild inhibition against Aspergillus niger(3.00±0.89). 13Ischemic colitis masquerading as colonic tumor:Case report with review of literature显示文摘Ischemic colitis can mimic a carcinoma on computed tomographic (CT) imaging or endoscopic examination. A coexisting colonic carcinoma or another potentially obstructing lesion has also been described in 20% of the cases of ischemic colitis. CT scan can differentiate it from colon cancer in 75% of cases. However, colonoscopy is the preferred method for diagnosing ischemic colitis as it allows for direct visualization with tissue sampling. Varied presentations of ischemic colitis have been described as an ulcerated or submucosal mass or as a narrowed segment of colon with ulcerated mucosa on colonoscopy. 14电视辅助胸腔镜钳闭治疗动脉导管未闭显示文摘目的 总结电视辅助胸腔镜钳闭未闭动脉导管的经验。 方法 用电视辅助胸腔镜技术对 130例动脉导管未闭患儿进行了手术。在全麻和单侧肺通气的情况下 ,在左侧第 3肋间脊柱肩胛骨旁做 2cm的切口 ,用以导入器械。在左侧第 4肋间腋中线做相似的切口以导入镜头。在左侧第3肋间胸骨旁 2cm和左侧第 4肋间胸骨旁 1cm各做 1孔 ,用以放入长而钝的神经钩暴露术野。通过器械窗导入持Ethicon 1cm钛夹的持夹器钳闭未闭的动脉导管。整个手术过程在电视荧屏上观看 ,食管B超和床边B超监测术中和术后的效果。 结果 本组患儿 ,平均年龄 (2 8± 1 2 )岁 ,平均体重(7 9± 4 5 )kg,平均肺动脉压 (35± 7)mmHg。合并心内畸形包括 :房间隔缺损 2例 ,室间隔缺损 3例。平均动脉导管直径 (6 8± 3 1)mm。手术平均时间 (30± 19)min。术后残余分流 4例 ,开胸结扎残余分流导管 3例 ,喉返神经麻痹 3例 ,5例转为开胸结扎。本组无手术死亡病例 ,无术后出血、手术输血、乳糜胸患儿。 结论 电视辅助胸腔镜钳闭未闭的动脉导管是治疗动脉导管未闭的一种较好方法 ,具有安全、快速、费用少、效果好。谢斌 张镜芳 庄健 Devi Shetty 2001中华外科杂志2001,39,6:8
15Chlorogenic acid loaded chitosan nanoparticles with sustained release property,retained antioxidant activity and enhanced bioavailability显示文摘In this study,chlorogenic acid(CGA),a phenolic compound widely distributed in fruits and vegetables,was encapsulated into chitosan nanoparticles by ionic gelation method.The particles exhibited the size and zeta potential of 210 nm and 33 mV respectively.A regular,spherical shaped distribution of nanoparticles was observed through scanning electron microscopy(SEM)and the success of entrapment was confirmed by FTIR analysis.The encapsulation efficiency of CGA was at about 59%with the loading efficiency of 5.2%.In vitro ABTS assay indicated that the radical scavenging activity of CAG was retained in the nanostructure and further,the release kinetics study revealed the burst release of 69%CGA from nanoparticles at the end of 100th hours.Pharmacokinetic analysis in rats showed a lower level of Cmax,longer Tmax,longer MRT,larger AUC0et and AUC0e∞for the CGA nanoparticles compared to free CGA.Collectively,these results suggest that the synthesised nanoparticle with sustained release property can therefore ease the fortification of food-matrices targeted for health benefits through effective delivery of CGA in body.Ilaiyaraja Nallamuthu Aishwarya Devi Farhath Khanum 2015Asian Journal of Pharmaceutical Sciences2015,10,3:7
16Neuroendocrine carcinoma of the ampulla of Vater:a clinicopathologic evaluation显示文摘BACKGROUND:Only 105 cases of neuroendocrine tumor(NET)of the ampulla of Vater have been described, mostly as single case reports.The incidence of NET is rising.The changes in incidence may result from changes in detection.This study was to determine the relative incidence and clinicopathological characteristics of high- grade neuroendocrine carcinoma(small cell carcinoma and large cell carcinoma)of the ampulla of Vater at a single institution. METHODS:Sections from paraffin blocks of tumors of the ampulla of Vater taken from 45 patients who underwent Whipple's procedure and 6 patients who underwent palliative bypass between September 2003 and January 2007 were subjected to immunohistochemical analysis.The clinical and pathological data from 5 patients diagnosed with NET of the ampulla of Vater were analyzed. RESULTS:The patients were 3 men and 2 women,ranging in age from 39 to 47 years(mean 44 years).Operative procedures included Whipple's procedure in 4 patients and palliative bypass in 1 patient.Histopathological examination revealed large-cell neuroendocrine carcinoma in 2 patients,small cell carcinoma in 2,and carcinoid in 1.Three patients with high-grade neuroendocrine carcinoma who had undergone Whipple's procedure died at postoperatively 7,11,and 13 months.The patient who had undergone palliative triple bypass died 3 months after surgery.CONCLUSIONS:The relative incidence of high-grade neuroendocrine carcinomas of the ampulla of Vater is higher than that generally expected.The tumors behave aggressively and have a dismal prognosis despite aggressive treatment.Eswaran Selvakumar Shanmugasundaram Rajendran Tirupporur Govindaswamy Balachandar Devy Gounder Kannan Satyanesan Jeswanth Palaniappan Ravichandran Rajagopal Surendran 2008Hepatobiliary & Pancreatic Diseases International2008,7,4:7
17左心室室壁瘤的外科治疗显示文摘目的 探讨左心室室壁瘤 (left ventricular aneurysm,L VA)的外科治疗效果。 方法 33例 L VA患者行闭式折叠术 1例 ,L VA切除后行标准线性修补术 14例 ,心内膜荷包环缩法 10例 ,心内室壁瘤缝合法 8例。其中 2 0例在心脏不停跳下完成手术。所有患者均同期行冠状动脉旁路移植术 (CABG) ,每例移植血管 3.2± 1.1支。应用主动脉内球囊反搏 (IABP) 6例。 结果 32例手术成功 ,手术死亡 1例 ;术后发生并发症 9例 ,其中低心排血量 6例、室性心律失常 2例、脑并发症 1例。术后早期随访 1~ 3个月 ,超声心动图示患者心功能均获改善 ,心绞痛缓解或消失。结论 L VA的外科治疗中 ,应用左心室几何重建技术和心脏不停跳及同期行 CABG能获得较好的临床治疗效果 。王玺胜 何学志 Devi Shetty 2004中国胸心血管外科临床杂志2004,11,4:7
18Solvent extraction of neodymium(Ⅲ) from acidic nitrate medium using Cyanex 921 in kerosene显示文摘The extraction of neodymium(Ⅲ) from acidic nitrate medium was investigated using Cyanex 921 as extractant in kerosene. The metal concentration in the aqueous phase before and after extraction was determined spectrophotometrically by Arsenazo Ⅲ method. The complete equilibration was achieved in 15 min. The effects of shaking time, nitric acid concentration, nitrate concentration, extractant concentration, and temperature on the extraction were studied. The extraction of Nd(Ⅲ) was found to increase very slowly with increase in concentration of HNO3 in the range of 0.001-0.008 mol/L and then decreased when 0.01 mol/L HNO3 was used. The percentage of extraction was increased with increase in nitrate concentration from 0.01-0.45 mol/L and then decreased when nitrate concentration increased to 0.5 mol/L. Quantitative extraction of Nd(Ⅲ) (98%) was obtained from the aqueous phase containing 0.001 mol/L HNO3 and 0.1 mol/L KNO3 using 0.5 mol/L Cyanex 921. On the basis of slope analysis, the extracted complex in the organic phase was proposed to be Nd(NO3)3.2Cyanex 921. The extraction of Nd(III) was found to increase with increase in concentration of metal ion in the range of 0.001-0.05 mol/L from 0.001 mol/L HNO3 and 0.1 mol/L KNO3 with 0.1 mol/L Cyanex 921. The percentage of extraction of neodymium was found to decrease with increase in temperature. From temperature variation studies, the negative value of △H indicated the extraction reaction to be exothermic and the negative value of △S indicated the formation of a stable complex. 19Hierarchical highly ordered SnO_(2) nanobowl branched ZnO nanowires for ultrasensitive and selective hydrogen sulfide gas sensing显示文摘Highly sensitive and selective hydrogen sulfide(H_(2)S)sensors based on hierarchical highly ordered SnO_(2) nanobowl branched ZnO nanowires(NWs)were synthesized via a sequential process combining hard template processing,atomic-layer deposition,and hydrothermal processing.The hierarchical sensing materials were prepared in situ on microelectromechanical systems,which are expected to achieve high-performance gas sensors with superior sensitivity,long-term stability and repeatability,as well as low power consumption.Specifically,the hierarchical nanobowl SnO_(2)@ZnO NW sensor displayed a high sensitivity of 6.24,a fast response and recovery speed(i.e.,14 s and 39 s,respectively),and an excellent selectivity when detecting 1 ppm H_(2)S at 250°C,whose rate of resistance change(i.e.,5.24)is 2.6 times higher than that of the pristine SnO_(2) nanobowl sensor.The improved sensing performance could be attributed to the increased specific surface area,the formation of heterojunctions and homojunctions,as well as the additional reaction between ZnO and H_(2)S,which were confirmed by electrochemical characterization and band alignment analysis.Moreover,the well-structured hierarchical sensors maintained stable performance after a month,suggesting excellent stability and repeatability.In summary,such well-designed hierarchical highly ordered nanobowl SnO_(2)@ZnO NW gas sensors demonstrate favorable potential for enhanced sensitive and selective H_(2)S detection with long-term stability and repeatability.Li-Yuan Zhu Kai-Ping Yuan Jia-He Yang Cheng-Zhou Hang Hong-Ping Ma Xin-Ming Ji Anjana Devi Hong-Liang Lu David Wei Zhang 2020Microsystems & Nanoengineering2020,6,1:7
20In vitro antioxidant activities of selected seaweeds from Southeast coast of India显示文摘Objective:In vitro antioxidant activities of three selected Indian seaweeds viz.,Halimeda tuna(H.tuna),Turbinaria conoides(T.conoides) and Gracilaria foliifera(G.foliifera) were evaluated.Methods:Total antioxidant activity,total phenolic content,and reducing power of crude methanol and diethyl ether extracts were determined.Results:Total phenolic content and total antioxidant activity were higher(1.231±0.173 mg GAE/g,1.675±0.361 mg GAE/g) in T.conoides respectively.Reducing power of crude methanol extract increased with concentrations of the extract.The Fourier transform-infra red spectrum analysis revealed the presence of polyphenolic signals.The seaweed extracts displayed moderate antioxidant activity compared to gallic acid standard.Conclusions:The seaweeds could be considered for curing diseases from oxidative deteriorations.Gopalraj Karthikai Devi Karunamoorthy Manivannan Ganapathy Thirumaran Fatimson Arockiya Aarthi Rajathi Perumal Anantharaman 2011Asian Pacific Journal of Tropical Medicine2011,4,3:6
