| 2 | Feasibility and safety of autologous bone marrow mononuclear cell transplantation in patients with advanced chronic liver disease显示文摘AIM: To evaluate the safety and feasibility of bone marrow cell (BMC) transplantation in patients with chronic liver disease on the waiting list for liver transplantation. METHODS: Ten patients (eight males) with chronic liver disease were enrolled to receive infusion of autologous bone marrow-derived cells. Seven patients were classified as Child-Pugh B and three as Child-Pugh C. Baseline assessment included complete clinical and laboratory evaluation and abdominal MRI. Approximately 50 ml of bone marrow aspirate was prepared by centrifugation in a ficoll-hypaque gradient. At least of 100 millions of mononuclear-enriched BMCs were infused into the hepatic artery using the routine technique for arterial chemoembolization for liver tumors. Patients were followed up for adverse events up to 4 mo. RESULTS: The median age of the patients was 52 years (range 24-70 years). All patients were discharged 48 h after BMC infusion. Two patients complained ofmild pain at the bone marrow needle puncture site. No other complications or specific side effects related to the procedure were observed. Bilirubin levels were lower at 1 (2.19 ± 0.9) and 4 mo (2.10 ± 1.0) after cell transplantation that baseline levels (2.78 ± 1.2). Albumin levels 4 mo after BMC infusion (3.73 ± 0.5) were higher than baseline levels (3.47 ± 0.5). International normalized ratio (INR) decreased from 1.48 (SD = 0.23) to 1.43 (SD = 0.23) one month after cell transplantation. CONCLUSION: BMC infusion into hepatic artery of patients with advanced chronic liver disease is safe and feasible. In addition, a decrease in mean serum bilirubin and INR levels and an increase in albumin levels are observed. | 3 | Efficacy and safety of endoscopic prophylactic treatment with undiluted cyanoacrylate for gastric varices显示文摘AIM:To evaluate the efficacy and safety of undiluted N-butyl-2 cyanoacrylate plus methacryloxysulfolane(NBCM)as a prophylactic treatment for gastric varices(GV)bleeding.METHODS:This prospective study was conducted at a single tertiary-care teaching hospital between October 2009 and March 2013.Patients with portal hypertension(PH)and GV,with no active gastrointestinal bleeding,were enrolled in primary prophylactic treatment with NBCM injection without lipiodol dilution.Initial diagnosis of GV was based on endoscopy and confirmed with endosonography(EUS); the same procedure was used after treatment to confirm eradication of GV.After puncturing the GV with a regular injection needle,1 mL of undiluted NBCM was injected intranasally into GV.The injection was repeated as necessary to achieve eradication or until a maximum total volume of 3 mL of NBCM had been injected.Patients were followed clinically and evaluated with endoscopy at 3,6 and 12 mo.Later follow-ups were performed yearly.The main outcome measures were efficacy(GV eradication),safety(adverse events related to cyanoacrylate injection),recurrence,bleeding from GV and mortality related to GV treatment.RESULTS:A total of 20 patients(15 male)with PH and GV were enrolled in the study and treated with undiluted NBCM injection.Only 2(10%)patients had no esophageal varices(EV); 18(90%)patients were treated with endoscopic band ligation to eradicate EV before inclusion in the study.The patients were followed clinically and endoscopically for a median of 31 mo(range:6-40 mo).Eradication of GV was observed in all patients(13 patients were treated with 1 session and 7 patients with 2 sessions),with a maximum injected volume of 2 mL NBCM.One patient had GV recurrence,confirmed by EUS,at 6-mo follow-up,and another had late recurrence with GV bleeding after 35 mo of follow-up; overall,GV recurrence was observed in 2 patients(10%),after 6 and 35 mo of follow-up,and GV bleeding rate was 5%(1 patient).Mild epigastric pain was reported by 3 patients(15%).No mortality or major complications,including embolism,or damage to equipment were observed.CONCLUSION:Endoscopic injection with NBCM,without lipiodol,may be a safe and effective treatment for primary prophylaxis of gastric variceal bleeding. | Matheus Cavalcante Franco Gustavo Flores Gomes Frank Shigeo Nakao Gustavo Andrade de Paulo Angelo Paulo Ferrari Jr Ermelindo Della Libera Jr | 2014 | World Journal of Gastrointestinal Endoscopy2014,6,6: | 16 |

| 5 | Prevalence of musculoskeletal injuries and a proposal for neuromuscular training to prevent lower limb injuries in Brazilian Army soldiers:an observational study显示文摘Background: The activities carried out by soldiers in the army involve great physical demands and require intense trainings to perform combat-specific tasks. Musculoskeletal injury is a potential threat to the health and physical integrity of the soldier. This study aimed to evaluate the prevalence of lower limb musculoskeletal injuries among soldiers and to propose a training protocol to prevent the most frequent injuries.Methods: This observational(cross-sectional) study recruited a sample of 103 soldiers who required medical attention,from a total 202 new battalion soldiers. The medical records(paper and online) had a form of running text. All data collected were recorded by the registered physicians of the battalion medical post. The records were analyzed by the following variables: medical diagnosis, injury site, mechanism, type of treatment, time loss, existence of previous injury,and recurring injury.Results: A total of 112 musculoskeletal injuries were diagnosed in 71 soldiers, and other types of diseases/injuries were diagnosed in the other soldiers. Joint pain accounted for 55.4% of the diagnoses. The knee was the most affected site, while trauma and overload were the most common mechanisms of injury. Drug treatment was used most frequently, accounting for 58% of the cases. The majority of the sample obtained a temporary leave of absence for1 to 6 days or not at all. Previous injuries and recurrence were not presented as risk factors for injury. With the data received, a protocol for the prevention of injuries to the lower limbs was proposed.Conclusions: This study indicated that the most frequent site of injury is the knee, and joint pain is the most common diagnosis. | 6 | Cytomegalovirus frequency in neonatal intrahepatic cholestasis determined by serology, histology, immunohistochemistry and PCR显示文摘AIM: To determine cytomegalovirus (CMV) frequency in neonatal intrahepatic cholestasis by serology, histological revision (searching for cytomegalic cells), immunohistochemistry, and polymerase chain reaction (PCR), and to verify the relationships among these methods.METHODS: The study comprised 101 non-consecutive infants submitted for hepatic biopsy between March 1982 and December 2005. Serological results were obtained from the patient's f iles and the other methods were performed on paraffin-embedded liver samples from hepatic biopsies. The following statistical measures were calculated: frequency, sensibility, specif ic positive predictive value, negative predictive value, and accuracy.RESULTS: The frequencies of positive results were as follows: serology, 7/64 (11%); histological revision, 0/84; immunohistochemistry, 1/44 (2%), and PCR, 6/77 (8%). Only one patient had positive immunohistochemical fi ndings and a positive PCR. The following statistical measures were calculated between PCR and serology: sensitivity, 33.3%; specificity, 88.89%; positive predictive value, 28.57%; negative predictive value, 90.91%; and accuracy, 82.35%. CONCLUSION: The frequency of positive CMV varied among the tests. Serology presented the highest positive frequency. | 11 | Multidimensional Projections for Visual Analysis of Social Networks显示文摘Visual analysis of social networks is usually based on graph drawing algorithms and tools.However,social networks are a special kind of graph in the sense that interpretation of displayed relationships is heavily dependent on context.Context,in its turn,is given by attributes associated with graph elements,such as individual nodes,edges,and groups of edges,as well as by the nature of the connections between individuals.In most systems,attributes of individuals and communities are not taken into consideration during graph layout,except to derive weights for force-based placement strategies.This paper proposes a set of novel tools for displaying and exploring social networks based on attribute and connectivity mappings.These properties are employed to layout nodes on the plane via multidimensional projection techniques.For the attribute mapping,we show that node proximity in the layout corresponds to similarity in attribute,leading to easiness in locating similar groups of nodes.The projection based on connectivity yields an initial placement that forgoes force-based or graph analysis algorithm,reaching a meaningful layout in one pass.When a force algorithm is then applied to this initial mapping,the final layout presents better properties than conventional force-based approaches.Numerical evaluations show a number of advantages of pre-mapping points via projections.User evaluation demonstrates that these tools promote ease of manipulation as well as fast identification of concepts and associations which cannot be easily expressed by conventional graph visualization alone.In order to allow better space usage for complex networks,a graph mapping on the surface of a sphere is also implemented. | Rafael Messias Martins Gabriel Faria Andery Henry Heberle Fernando Vieira Paulovich Alneu de Andrade Lopes Helio Pedrini Rosane Minghim | 2012 | Journal of Computer Science & Technology2012,27,4: | 6 |
| 12 | Bone marrow-derived monocyte infusion improves hepatic fibrosis by decreasing osteopontin,TGF-β1,IL-13 and oxidative stress显示文摘AIM To evaluate the therapeutic effects of bone marrowderived CD11b+CD14+ monocytes in a murine model of chronic liver damage. METHODS Chronic liver damage was induced in C57BL/6 mice by administration of carbon tetrachloride and ethanol for 6 mo. Bone marrow-derived monocytes isolated by immunomagnetic separation were used for therapy. The cell transplantation effects were evaluated by morphometry,biochemical assessment,immunohistochemistry and enzyme-linked immunosorbent assay. RESULTS CD11b+CD14+ monocyte therapy significantly reduced liver fibrosis and increased hepatic glutathione levels. Levels of pro-inflammatory cytokines,including tumor necrosis factor-α,interleukin(IL)-6 and IL-1β,in addition to pro-fibrotic factors,such as IL-13,transforming growth factor-β1 and tissue inhibitor of metalloproteinase-1 also decreased,while IL-10 and matrix metalloproteinase-9 increased in the monocytetreated group. CD11b+CD14+ monocyte transplantation caused significant changes in the hepatic expression of α-smooth muscle actin and osteopontin. Unfortunately, recurrence is common and seems to be related to esophageal collateral vessels, easily identified by EUS. Eradication of these vessels might lead to a more durable therapeutic effect. Objective: To compare ES with EUS-guided sclerotherapy of collateral vessels (EUS-ES). Design: Randomized controlled trial. Setting: Endoscopy Unit, Division of Gastroenterology. Universidade Federal de S o Paulo, S o Paulo, Brazil. Patients and Interventions: Fifty cirrhotic patientswith esophageal varices were randomized into 2 groups: ES (n = 25) and EUS-ES (n = 25). EUS-ES was targeted at collateral veins. Patients were followed-up for at least 6 months after eradication. Main Outcome Measurements: Efficacy in eradication, complications, and recurrence of varices. Results: Varices were eradicated in 48 patients who adhered to the study protocol. The mean (SD) number of sessions until eradication was 4.3 (1.5) for the ES group and 4.1 (1.2) for the EUS-ES group. In ES group, 4 patients had mild bleeding. In EUS-ES group, 4 patients had pain. The mean (SD) length of the follow-up period was 22.6 (6.9) months for the ES group and 24.9 (8.1) months for the EUS-ES group. Recurrence was seen in 4 patients after ES and in 2 after EUS-ES (P = .32). The presence of collateral vessels was associated with recurrence (P = .003). Conclusion: EUS-ES is as safe and effective as ES in variceal eradication. Recurrence tends to be less frequent and occurs later. | 15 | Cumulative positive fluid balance is a risk factor for acute kidney injury and requirement for renal replacement therapy after liver transplantation显示文摘AIM To analyze whether fluid overload is an independent risk factor of adverse outcomes after liver transplantation(LT).METHODS One hundred and twenty-one patients submitted to LT were retrospectively evaluated.Data regarding perioperative and postoperative variables previously associated with adverse outcomes after LT were reviewed.Cumulative fluid balance(FB) in the first 12 h and 4 d after surgery were compared with major adverse outcomes after LT.RESULTS Most of the patients were submitted to a liberal approach of fluid administration with a mean cumulative FBover 5 L and 10 L,respectively,in the first 12 h and 4 d after LT.Cumulative FB in 4 d was independently associated with occurrence of both AKI and requirement for renal replacement therapy(RRT)(OR = 2.3;95%CI:1.37-3.86,P = 0.02 and OR = 2.89;95%CI:1.52-5.49,P = 0.001 respectively).Other variables on multivariate analysis associated with AKI and RRT were,respectively,male sex and Acute Physiology and Chronic Health Disease Classification System(APACHE II) levels and sepsis or septic shock.Mortality was shown to be independently related to AST and APACHE II levels(OR = 2.35;95%CI:1.1-5.05,P = 0.02 and 2.63;95%CI:1.0-6.87,P = 0.04 respectively),probably reflecting the degree of graft dysfunction and severity of early postoperative course of LT.No effect of FB on mortality after LT was disclosed.CONCLUSION Cumulative positive FB over 4 d after LT is independently associated with the development of AKI and the requirement of RRT.Survival was not independently related to FB,but to surrogate markers of graft dysfunction and severity of postoperative course of LT. | Liana Codes Ygor Gomes de Souza Ricardo Azevedo Cruz D'Oliveira Jorge Luiz Andrade Bastos Paulo Lisboa Bittencourt | 2018 | World Journal of Transplantation2018,8,2: | 4 |

| 17 | Association of Helicobacter pylori infection and giardiasis: Results from a study of surrogate markers for fecal exposure among children显示文摘AIM: To investigate whether Helicobacter pylori (H pykori)infection is associated with hepatitis A virus (HAV) infection, presence of enteroparasites, and other surrogates of fecal exposure.METHODS: We conducted a cross-sectional study in 121 children consecutively admitted at a pediatric hospital inSalvador, Brazil.H pylori and HAV infection were identified by the presence of serum antibodies. Stool specimens were examined for the presence of ova and parasites. A structured questionnaire inquiring about sanitary conditions and life style was applied to each subject. RESULTS: Fifty-one of the 121 children (42.1%) werefound to be seropositive for H pylori, and 45 (37.2%) for HAV. The seroprevalence ofHpyloriand HAV both increased significantly with age. Cross-tabulation of data showed that 26 (21.5%) were seropositive and 51 (42.1%) were negative for bothHpyloriand HAV antibodies (χ2 = 7.18,OR = 2.8, CI 1.30-5.97). The age adjusted OR for an HAV-infected child being H pylori positive was 2.3 (CI1.02-5.03). The agreement between H pylori and HAVseropositivity was fair (κ = 0.24). After controlling for possible confounding, the variables remaining independently associated with seropositivity to H pyloriwere age,presence of Giardia lamblia in feces (OR = 3.2, 95%CI, 1.1-9.5) and poor garbage disposal quality (OR = 2.4,95%CI, 1.1-5.1).CONCLUSION: Our data suggest that H pylori infection is associated with surrogate markers of fecal exposure. Thus, we conclude that the fecal-oral route is relevant in the transmission of HP among children in an urban setting of a developing country. The association observed between G. lamblia and H pylori infection may have several explanations. | 19 | Sex-specific effects of Eugenia punicifolia extract on gastric ulcer healing in rats显示文摘AIM To evaluate the sex-specific effects of a hydroalcoholic extract from Eugenia punicifolia(HEEP) leaves on gastric ulcer healing.METHODS In this rat study involving males, intact(cycling) females, and ovariectomized females, gastric ulcers were induced using acetic acid. A vehicle, lansoprazole, or HEEP was administered for 14 d after ulcer induction. Body weight was monitored throughout the treatment period. At the end of treatment, the rats were euthanized and the following in vivo and in vitro investigations were performed: macroscopic examination of the lesion area and organ weights, biochemical analysis, zymography, and evaluation of protein expression levels. Additionally, the concentration-dependent effect of HEEP was evaluated in terms of subacute toxicity and cytotoxicity.RESULTS Compared to the vehicle, HEEP demonstrated a great healing capacity by substantially reducing the ulcerative lesion area in males(52.44%), intact females(85.22%), and ovariectomized females(65.47%), confirming that HEEP accelerates the healing of acetic acidinduced gastric lesions and suggesting that this effect is modulated by female sex hormones. The antiulcer effect of HEEP was mediated by prostaglandin E2 only in male rats. Overall, the beneficial effect of HEEP was the highest in intact females. Notably, HEEP promoted the expression of vascular endothelial growth factor(intact vs ovariectomized females) and decreased the expression of Caspase-8 and Bcl-2(intact female vs male or ovariectomized female). Additionally, HEEP enhanced fibroblast proliferation and migration into a wounded area in vitro, confirming its healing effect.
