3功能性电刺激改善脑卒中早期患者偏瘫下肢功能的随机对照研究显示文摘目的:探讨功能性电刺激(FES)对脑卒中偏瘫患者下肢功能的影响。方法:37例脑卒中早期患者分层后随机分为FES组(19例,年龄60.8±10.8岁,病程25.9±21.3d)和对照组(18例,年龄64.1±9.7岁,病程22.7±16.6d)。2组常规治疗相同,对照组不给予任何电刺激,FES组采用日本生产的低频电刺激治疗仪(KR7型)治疗,电极放在患侧胫前肌及腓骨长、短肌的运动点上;刺激参数为频率30Hz,脉宽200μs,通电/断电比5s/5s,波升/波降(1s/1s),电流以患者最大耐受强度为限。治疗每天1次,每次30min,共3周(15次)。用综合痉挛量表(CSS)评定踝关节痉挛,Fugl-Meyer运动评定量表中下肢部分(FMA)评定下肢运动功能,脑卒中患者姿势评定量表(PASS)和Berg平衡量表(BBS)评定平衡功能。结果:2组患者一般资料及治疗前各项评定结果差异无显著性意义。治疗2周和3周后,FES组与对照组(FESvs对照)CSS、FMA评分差异有显著性意义(P<0.05),增加率为2周时CSS:8.9%±23.2%vs36.3%±47.3%,FMA:105.5%±75.7%vs51.4%±47.3%。3周时CSS:10.4%±18.3%vs47.7%±56.4%,FMA:127.1%±89.4%vs64.3%±51.8%。结论:FES能延缓早期脑卒中患者偏瘫下肢痉挛的发生、减轻痉挛程度,改善下肢运动能力。游国清 燕铁斌 Christina WY HUI-CHAN 2007中国康复医学杂志2007,22,10:31

5低频电刺激改善早期脑卒中偏瘫患者日常生活活动能力的随机对照研究显示文摘目的 探讨低频电刺激(LES)对脑卒中偏瘫患者日常生活活动(ADL)能力的影响.方法 37例早期脑卒中患者经minimize软件分层后随机分为电刺激组19例和对照组18例.2组常规治疗相同,对照组不行电刺激治疗,电刺激组采用LES治疗,电极置于偏瘫侧胫前肌及腓骨长、短肌的运动点上,刺激参数为频率30 Hz,脉宽200μs,通电/断电比5 s/5s,波升/波降为1s/1s,电流以患者最大耐受强度为限.每天治疗1次,每次30 min,共3周(15次).用Fugl-Meyer运动功能评定量表(FMA)中下肢部分评定下肢运动功能,用改良Barthel指数(MBI)评定ADL能力.结果 2组患者一般资料及治疗前各项评定结果差异无统计学意义(P〉0.05).治疗2周和3周后,电刺激组与对照组的FMA评分及MBI总分比较,差异均有统计学意义(P〈0.05),电刺激组与对照组在MBI各项中的床椅转移、平地行走及上下楼梯等评分比较,差异有统计学意义(P〈0.05).结论 LES能提高早期脑卒中偏瘫患者下肢运动能力及ADL能力.游国清 燕铁斌 Christina WY Hui-Chan 2010中华物理医学与康复杂志2010,32,9:13
6经皮电神经刺激穴位治疗对脑卒中患者偏瘫侧上肢体感诱发电位的影响显示文摘目的:比较单次(45min)经皮电神经刺激(TENS)穴位治疗对脑卒中患者偏瘫侧上肢体感诱发电位(SEP)的影响。方法:符合入选条件并签署知情同意书的29例初发脑卒中患者分层后随机分为治疗组和安慰组。治疗组患者接受1次45minTENS治疗,刺激电极放在患侧上肢穴位上,刺激频率为100Hz,脉宽为200μs,强度为患者最大耐受量;安慰组患者接受45min安慰治疗,仪器与治疗组相同,有指示灯闪,但无电流输出。两组患者分别在治疗前后接受1次SEP检测,并比较其治疗前后的差异。结果:治疗前两组患者偏瘫侧与健侧SEP值相比,除N9潜伏期外差异均有显著性意义(P<0.05);治疗组治疗前后SEP值差异有显著性意义(P<0.05);治疗后两组患者组间SEP值N20潜伏期差异有显著性意义(P<0.05)。结论:单次45minTENS治疗能改善脑卒中患者偏瘫侧上肢的SEP,推测TENS改善脑卒中偏瘫患者肢体功能可能与TENS增加脑电活动,改善SEP,激活脑细胞的功能活动有关。魏妮 燕铁斌 Christina WY HUI-CHAN 陈月桂 2007中国康复医学杂志2007,22,10:11
7经皮电神经刺激不同部位对脑卒中患者体感诱发电位的影响显示文摘目的比较单次(45min)经皮电神经刺激(TENS)不同部位治疗脑卒中患者患侧肢体对体感诱发电位(SEP)的影响。方法按TENS的治疗部位将14例脑卒中患者分为2组:同时刺激偏瘫侧上、下肢组(上下肢组)8例和单独刺激偏瘫侧上肢组(上肢组)6例。2组患者分别接受1次45min的TENS治疗,刺激频率为100Hz,脉宽为200μs,强度为患者最大耐受量。分析双侧上、下肢或双上肢SEP检测过程中N9和N20的波幅及潜伏期,比较同一测试时间点SEP的重复检测信度,并比较两种不同治疗方法所得到的SEP结果差异。结果2组患者2次检测的SEP值等级间相关系数(ICC)值为0.9006—1.0000,上肢组优于上下肢组。治疗前、后比较,上下肢组的N9和N20波幅及潜伏期无明显变化,而上肢组N9及N20波幅增高,潜伏期缩短,以N20的改变更为显著。治疗前2组患者SEP值比较,差异有统计学意义(P〈0.05),故以治疗前SEP值为协变量进行统计学分析,治疗后2组患者SEP值差异有统计学意义(P〈0.01)。结论单次45min的TENS治疗能改善脑卒中患者偏瘫肢体的SEP值,单独刺激上肢或同时刺激上下肢均有较高的可重复性,但单独刺激上肢的方法可重复性更高,SEP值的改变更明显。魏妮 燕铁斌 Christina WY Hui-Chan 陈月桂 2007中华物理医学与康复杂志2007,29,1:11

10神经肌肉电刺激对脑卒中患者上肢功能影响的随机研究显示文摘目的探讨神经肌肉电刺激(NMES)对脑卒中偏瘫患者上肢功能的影响。方法将37例初发脑卒中患者分层后随机分为NMES组(n=19)和对照组(n=18)。2组常规治疗相同,NMES组同时给予NMES治疗,电极分别放在患侧肩外展肌群和伸腕肌群处;刺激频率为30Hz,脉宽为300μs,通电/断电比为5s/5S,波升/波降比为1s/1S,强度为0~90mA,随患者感觉而定。治疗每天1次,每次30min,每周5次,共3周。入选患者分别于治疗前、治疗2周和3周后进行评定:采用Fugl-Meyer运动功能评定量表(FMA)中上肢部分评定上肢运动功能,采用改良Barthel指数(MBI)评定日常生活活动能力;23例(NMES组11例,对照组12例)在治疗前及治疗3周后接受体感诱发电位(SEP)评测。结果治疗3周后,NMES组与对照组FMA评分、MBI评分以及SEP参数比较,差异均有统计学意义(P〈0.05)。结论NMES可以改善脑卒中患者偏瘫上肢运动功能,提高日常生活活动能力。林子玲 燕铁斌 郑正涛 Christina WY Hui-chan 2009中华物理医学与康复杂志2009,31,10:5
11Consensus on the digestive endoscopic tunnel technique显示文摘With the digestive endoscopic tunnel technique(DETT), many diseases that previously would have been treated by surgery are now endoscopically curable by establishing a submucosal tunnel between the mucosa and muscularis propria(MP). Through the tunnel, endoscopic diagnosis or treatment is performed for lesions in the mucosa, in the MP, and even outside the gastrointestinal(GI) tract.At present, the tunnel technique application range covers the following:(1)Treatment of lesions originating from the mucosal layer, e.g., endoscopic submucosal tunnel dissection for oesophageal large or circular early-stage cancer or precancerosis;(2) treatment of lesions from the MP layer, per-oral endoscopic myotomy, submucosal tunnelling endoscopic resection, etc.; and(3) diagnosis and treatment of lesions outside the GI tract, such as resection of lymph nodes and benign tumour excision in the mediastinum or abdominal cavity. With the increasing number of DETTs performed worldwide, endoscopic tunnel therapeutics, which is based on DETT, has been gradually developed and optimized. However, there is not yet an expert consensus on DETT to regulate its indications, contraindications, surgical procedure, and postoperative treatment.The International DETT Alliance signed up this consensus to standardize the procedures of DETT. 12Dust mite allergen-specific immunotherapy increases IL4 DNA methylation and induces Der p-specific T cell tolerance in children with allergic asthma显示文摘Allergen-specific immunotherapy(allergen-SIT)is a highly effective treatment for children with allergic asthma(AA),an immune-mediated chronic disease leading to bronchial muscle hypertrophy and airway obstruction in response to specific allergens.T helper cells and secreted cytokines play important roles in the pathogenesis of asthma,and epigenetic modulation controls genes important for T cell development and cytokine expression.This study evaluated T helper cell-secreted cytokines and DNA methylation patterns in children treated with Dermatophagoides pteronyssinus(Der p)allergen-SIT.Our results showed that after Der p challenge,peripheral blood mononuclear cells(PBMCs)from the SIT group,compared with the non-SIT AA group,produced lower levels of IL-4,IL-5 and IL-2.The SIT group,compared with the AA group,exhibited decreased sensitivity to the Der p allergen,concurrent with IL-4 down-modulation due to increased promoter DNA methylation,as estimated in PBMCs.Our results showed that SIT decreased IL-4 and IL-5,and inhibited T cell proliferation,by inhibiting IL-2 production after the specific allergen challenge.These results suggest that decreased IL-2 production and increased IL-4 cytokine promoter methylation is a potential mechanism of Der p-specific allergen desensitization immunotherapy.Chuang-Ming Wang Chia-Bin Chang Michael WY Chan Zhi-Hong Wen Shu-Fen Wu 2018Cellular & Molecular Immunology2018,15,11:5
13Mechanistic links between gut microbial community dynamics, microbial functions and metabolic health显示文摘Gut microbes comprise a high density, biologically active community that lies at the interface of an animal with its nutritional environment. Consequently their activity profoundly influences many aspects of the physiology and metabolism of the host animal. A range of microbial structural components and metabolites directly interact with host intestinal cells and tissues to influence nutrient uptake and epithelial health. Endocrine, neuronal and lymphoid cells in the gut also integrate signals from these microbial factors to influence systemic responses. Dysregulation of these host-microbe interactions is now recognised as a major risk factor in the development of metabolic dysfunction. This is a two-way process and understanding the factors that tip host-microbiome homeostasis over to dysbiosis requires greater appreciation of the host feedbacks that contribute to regulation of microbial community composition. To date, numerous studies have employed taxonomic profiling approaches to explore the links between microbial composition and host outcomes(especially obesity and its comorbidities), but inconsistent host-microbe associations have been reported. Available data indicates multiple factors have contributed to discrepancies between studies. These include the high level of functional redundancy in host-microbiome interactions combined with individual variation in microbiome composition; differences in study design, diet composition and host system between studies; and inherent limitations to the resolution of r RNA-based community profiling. Accounting for these factors allows for recognition of the common microbial and host factors driving community composition and development of dysbiosis on high fat diets.
