| 1 | Adjuvant probiotics improve the eradication effect of triple therapy for Helicobacter pylori infection显示文摘AIM:To investigate whether the addition of probiotics can improve the eradication effect of triple therapy for Helicobacter pylori (H. pylori ) infection. METHODS:This open randomized trial recruited 234 H. pylori positive gastritis patients from seven local centers. The patients were randomized to one-week standard triple therapy (omeprazole 20 mg bid , clarithromycin 500 mg bid , and amoxicillin 1000 mg bid ; OCA group, n = 79); two weeks of pre-treatment with probiotics, containing 3 × 107 Lactobacillus acidophilus per day, prior to one week of triple therapy (POCA group, n = 78); or one week of triple therapy followed by two weeks of the same probiotics (OCAP group, n = 77). Successful eradication was defined as a negative C13 or C14 urease breath test four weeks after triple therapy. Patients were asked to report associated symptoms at baseline and during follow-up, and side effects related to therapy were recorded. Data were analyzed by both intention-to-treat (ITT) and per-protocol (PP) methods. RESULTS:PP analysis involved 228 patients, 78 in the OCA, 76 in the POCA and 74 in the OCAP group. Successful eradication was observed in 171 patients; by PP analysis, the eradication rates were significantly higher (P = 0.007 each) in the POCA (62/76; 81.6%, 95% CI 72.8%-90.4%) and OCAP (61/74; 82.4%, 95% CI 73.6%-91.2%) groups than in the OCA group (48/78; 61.5%, 95% CI 50.6%-72.4%). ITT analysis also showed that eradication rates were significantly higher in the POCA (62/78; 79.5%, 95% CI 70.4%-88.6%) and OCAP (61/77; 79.2%, 95% CI 70%-88.4%) groups than in the OCA group (48/79; 60.8%, 95% CI 49.9%-71.7%), (P = 0.014 and P = 0.015). The symptom relieving rates in the POCA, OCAP and OCA groups were 85.5%, 89.2% and 87.2%, respectively. Only one of the 228 patients experienced an adverse reaction. | 4 | Preliminary experience with drug-coated balloon angioplasty in primary percutaneous coronary intervention显示文摘We evaluated the clinical feasibility of using drugcoated balloon(DCB) angioplasty in patients undergoingprimary percutaneous coronary intervention(PPCI). Between January 2010 to September 2014,89 STelevation myocardial infarction patients(83% male,mean age 59 ± 14 years) with a total of 89 coronary lesions were treated with DCB during PPCI. Clinical outcomes are reported at 30 d follow-up. Left anterior descending artery was the most common target vessel for PCI(37%). Twenty-eight percent of the patients had underlying diabetes mellitus. Mean left ventricular ejection fraction was 44% ± 11%. DCB-only PCI was the predominant approach(96%) with the remaining 4% of patients receiving bail-out stenting. Thrombolysis in Myocardial Infarction(TIMI) 3 flow was successfully restored in 98% of patients. An average of 1.2 ± 0.5 DCB were used per patient,with mean DCB diameter of 2.6 ± 0.5 mm and average length of 23.2 ± 10.2 mm. At 30-d follow-up,there were 4 deaths(4.5%). No patients experienced abrupt closure of the infarctrelated artery and there was no reported target-lesion failure. | 6 | Acute-on-chronic liver failure in a multi-ethnic Asian city:A comparison of patients identified by Asia-Pacific Association for the Study of the Liver and European Association for the Study of the Liver definitions显示文摘AIM To explore the applicability of the Asia-Pacific Association for the Study of the Liver(APASL) and European Association for the Study of the Liver(EASL) guidelines for acute-on-chronic liver failure(ACLF) in profiling patients and determining the outcome.METHODS Patients admitted to a tertiary hospital in Singapore with acute decompensation of liver disease from January 2004to July 2014 are screened for ACLF according to the APASL and EASL criteria. The patients' data(including basic demographics, information about existing chronic liver disease, information about the acute decompensation, relevant laboratory values during admission, treatment, and outcome) are retrospectively analyzed to determine the background, precipitating factors and outcome.RESULTS A total of 458 liver patients is analyzed, and 78 patients with ACLF are identified. Sixty-three patients(80.8%) meet the APASL criteria, 64 patients(82.1%) meet the EASL criteria, and 49 patients(62.8%) fulfilled both criteria. The most common causes of acute liver injury are bacterial infections(59.0%), hepatitis B flare(29.5%), and variceal bleeding(24.4%). The common aetiologies of the underlying chronic disease included hepatitis B(43.6%), alcoholic(20.5%) and cryptogenic(11.5%) liver disease. The overall mortality rate is 61.5%. Increased age, the number of organ failures(as per CLIF-SOFA score), peak creatinine, INR, and amylase levels are associated with increased mortality or the need for liver transplantation. 14.3% of patients undergo liver transplantation with a 100% 1-year survival rate. | 7 | Dynamically reconfigurable topological edge state in phase change photonic crystals显示文摘The observation of topological edge states(TESs) revolutionized our understanding of scattering and propagation of electromagnetic(EM) waves. Supported by topological robustness, the TES at the interface between trivial and non-trivial insulators was not reflected from the structural disorders and imperfections. Recently topological photonic crystals(PhCs) were demonstrated to obtain remarkable one-way propagation of the TES, having the advantages of lossless propagation, dense integration, and high fabrication tolerance over conventional PhCs. Nevertheless, the lack of reversible switching of TES possesses significant limitations in helicity/spin filtering and tunable photonic devices. We proposed a topological PhC based on a prototypical phase-change material, Ge2 Sb2 Te5(GST225) to solve the problem. We find that at a particular frequency, the TES within the structure can be reversibly switched between 'on'and 'off' by transiting the GST225 structural state between amorphous and crystalline. Moreover, the topology of the PhC was maintained since the tuning of TES was achieved by varying the refractive index of GST225 instead of the structural geometry. This provides a continuous change of the spectral position of the photonic bandgap and TES by gradually crystallising the GST225. We show that the phase change of GST225 from amorphous to crystalline and vice versa can be engineered in nanoseconds. Our proof of concept may offer a platform for dynamically tuning the TESs that might otherwise be challenging to attain in photonic systems. | 9 | High-quality Arabidopsis thaliana Genome Assembly with Nanopore and HiFi Long Reads显示文摘Arabidopsis thaliana is an important and long-established model species for plant molecular biology,genetics,epigenetics,and genomics.However,the latest version of reference genome still contains a significant number of missing segments.Here,we reported a high-quality and almost complete Col-0 genome assembly with two gaps(named Col-XJTU)by combining the Oxford Nanopore Technologies ultra-long reads,Pacific Biosciences high-fidelity long reads,and Hi-C data.The total genome assembly size is 133,725,193 bp,introducing 14.6 Mb of novel sequences compared to the TAIR10.1 reference genome.All five chromosomes of the Col-XJTU assembly are highly accurate with consensus quality(QV)scores>60(ranging from 62 to 68),which are higher than those of the TAIR10.1 reference(ranging from 45 to 52).We completely resolved chromosome(Chr)3 and Chr5 in a telomere-to-telomere manner.Chr4 was completely resolved except the nucleolar organizing regions,which comprise long repetitive DNA fragments.The Chrl centromere(CEN1),reportedly around 9 Mb in length,is particularly challenging to assemble due to the presence of tens of thousands of CEN180 satellite repeats.Using the cutting-edge sequencing data and novel computational approaches,we assembled a 3.8-Mb-long CEN1 and a 3.5-Mb-long CEN2.We also investigated the structure and epigenetics of centromeres.Four clusters of CEN180 monomers were detected,and the centromere-specific histone H3-like protein(CENH3)exhibited a strong preference for CEN180 Cluster 3.Moreover,we observed hypomethylation patterns in CENH3-enriched regions.We believe that this high-quality genome assembly,Col-XJTU,would serve as a valuable reference to better understand the global pattern of centromeric polymorphisms,as well as the genetic and epigenetic features in plants. | Bo Wang Xiaofei Yang Yanyan Jia Yu Xu Peng Jia Ningxin Dang Songbo Wang Tun Xu Xixi Zhao Shenghan Gao Quanbin Dong Kai Ye | 2022 | Genomics, Proteomics & Bioinformatics2022,20,1: | 6 |
| 10 | Analysis of Runoff in Ungauged Mountain Watersheds in Sichuan,China using Kinematic-wave-based GIUH Model显示文摘Floods are one of the most common natural hazards occurring all around the world.However,the knowledge of the origins of a food and its possible magnitude in a given region remains unclear yet.This lack of understanding is particularly acute in mountainous regions with large degrees in Sichuan Province,China,where runoff is seldom measured.The nature of streamflow in a region is related to the time and spatial distribution of rainfall quantity and watershed geomorphology.The geomorphologic characteristics are the channel network and surrounding landscape which transform the rainfall input into an output hydrograph at the outlet of the watershed.With the given geomorphologic properties of the watershed,theoretically the hydrological response function can be determined hydraulically without using any recorded data of past rainfall or runoff events.In this study,a kinematic-wave-based geomorphologic instantaneous unit hydrograph (KW-GIUH) model was adopted and verified to estimate runoff in ungauged areas.Two mountain watersheds,the Yingjing River watershed and Tianquan River watershed in Sichuan were selected as study sites.The geomorphologic factors of the two watersheds were obtained by using a digital elevation model (DEM) based on the topographic database obtained from the Shuttle Radar Topography Mission of US's NASA.The tests of the model on the two watersheds were performed both at gauged and ungauged sites.Comparison between the simulated and observed hydrographs for a number of rainstorms at the gauged sites indicated the potential of the KW-GIUH model as a useful tool for runoff analysis in these regions.Moreover,to simulate possible concentrated rainstorms that could result in serious flooding in these areas,synthetic rainfall hyetographs were adopted as input to the KW-GIUH model to obtain the flow hydrographs at two ungauged sites for different return period conditions.Hydroeconomic analysis can be performed in the future to select the optimum design return period for determining the flood control work. | CAO Shuyou LEE Kwan Tun HO Juiyi LIU Xingnian HUANG Er YANG Kejun | 2010 | Journal of Mountain Science2010,7,2: | 5 |
| 11 | Flow Range Enhancement by Secondary Flow Effect in Low Solidity Circular Cascade Diffusers显示文摘High-pressure ratio and wide operating range are highly required for compressors and blowers. The technical issue of the design is achievement of suppression of flow separation at small flow rate without deteriorating the efficiency at design flow rate. A numerical simulation is very effective in design procedure, however, cost of the numerical simulation is generally high during the practical design process, and it is difficult to confirm the optimal design which is combined with many parameters. A multi-objective optimization technique is the idea that has been proposed for solving the problem in practical design process. In this study, a Low Solidity circular cascade Diffuser(LSD) in a centrifugal blower is successfully designed by means of multi-objective optimization technique. An optimization code with a meta-model assisted evolutionary algorithm is used with a commercial CFD code ANSYS-CFX. The optimization is aiming at improving the static pressure coefficient at design point and at low flow rate condition while constraining the slope of the lift coefficient curve. Moreover, a small tip clearance of the LSD blade was applied in order to activate and to stabilize the secondary flow effect at small flow rate condition. The optimized LSD blade has an extended operating range of 114 % towards smaller flow rate as compared to the baseline design without deteriorating the diffuser pressure recovery at design point. The diffuser pressure rise and operating flow range of the optimized LSD blade are experimentally verified by overall performance test. The detailed flow in the diffuser is also confirmed by means of a Particle Image Velocimeter. Secondary flow is clearly captured by PIV and it spreads to the whole area of LSD blade pitch. | 15 | Theoretical study of tunable chirality from graphene integrated achiral metasurfaces显示文摘Control of chirality using metamaterials plays a critical role in a diverse set of advanced photonics applications,such as polarization control, bio-sensing, and polarization-sensitive imaging devices. However, this poses a major challenge, as it primarily involves the geometrical reconfiguration of metamolecules that cannot be adjusted dynamically. Real-world applications require active tuning of the chirality, which can easily manipulate the magnitude, handedness, and spectral range of chiroptical response. Here, enabled by graphene, we theoretically reveal a tunable/switchable achiral metasurface in the near-infrared region. In the model, the achiral metasurface consists of an array of circular holes embedded through a metal/dielectric/metal trilayer incorporated with the graphene sheet, where holes occupy the sites of a rectangular lattice. Circular conversion dichroism(CCD) originates from the mutual orientation between the achiral metasurface and oblique incident wave.The achiral metasurface possesses dual-band sharp features in the CCD spectra, which are tuned over a broad bandwidth by electrically modulating the graphene's Fermi level. | 16 | Hepatitis C virus therapy with peg-interferon and ribavirin in Myanmar: A resource-constrained country显示文摘AIM To investigate peg-interferon(peg-IFN) and ribavirin(RBV) therapy in Myanmar and to predict sustained virologic response(SVR).METHODS This single-center, open-label, study was conducted in Myanmar between 2009 and 2014. A total of 288 patients infected with HCV genotypes 1, 2, 3 and 6 were treated with peg-IFN alpha-2a(180 μg/wk) or alpha-2b(50 to 100 μg as a weight-based dose) and RBV as a weight-based dose(15 mg/kg/d). Treatment duration was 48 wk for genotypes 1 and 6, 24 wk for genotype 2, and 24 or 48 wk for genotype 3 based on rapid virologic response(RVR). Those co-infected with hepatitis B received 48 wk of therapy.RESULTS Overall, SVR was achieved for 82% of patients and the therapy was well tolerated. All patients achieved SVR at equivalent rates regardless of HCV genotype(P = 0.314). Low fibrosis scores(P < 0.001), high baseline albumin levels(P = 0.028) and low baseline viral loads(P = 0.029) all independently predicted SVR. On the other hand, IL-28 B TT and CC genotypes were not found to significantly predict SVR(P = 0.634; P = 0.618). Among those who completed treatment, the occurrence of RVR showed a > 96% positive predictive value for achieving SVR. Treatment duration did not significantly impact the likelihood of achieving SVR for patients infected with genotype 3 HCV(P = 0.371). The most common adverse events were fatigue(71%) and poor appetite(60%). Among patients with genotype 3 HCV, more patients in the 48-wk treatment group required erythropoietin than in the 24-wk treatment group(61.1% vs 49.2%).CONCLUSION SVR rates were high with peg-IFN and RBV therapy in Myanmar. | 20 | Reconstruction method of irregular seismic data with adaptive thresholds based on different sparse transform bases显示文摘Oil and gas seismic exploration have to adopt irregular seismic acquisition due to the increasingly complex exploration conditions to adapt to complex geological conditions and environments.However,the irregular seismic acquisition is accompanied by the lack of acquisition data,which requires high-precision regularization.The sparse signal feature in the transform domain in compressed sensing theory is used in this paper to recover the missing signal,involving sparse transform base optimization and threshold modeling.First,this paper analyzes and compares the effects of six sparse transformation bases on the reconstruction accuracy and efficiency of irregular seismic data and establishes the quantitative relationship between sparse transformation and reconstruction accuracy and efficiency.Second,an adaptive threshold modeling method based on sparse coefficient is provided to improve the reconstruction accuracy.Test results show that the method has good adaptability to different seismic data and sparse transform bases.The f-x domain reconstruction method of effective frequency samples is studied to address the problem of low computational efficiency.The parallel computing strategy of curvelet transform combined with OpenMP is further proposed,which substantially improves the computational efficiency under the premise of ensuring the reconstruction accuracy.Finally,the actual acquisition data are used to verify the proposed method.The results indicate that the proposed method strategy can solve the regularization problem of irregular seismic data in production and improve the imaging quality of the target layer economically and efficiently. | Zhao Hu Yang Tun Ni Yu-Dong Liu Xing-Gang Xu Yin-Po Zhang Yi-Lei Zhang Guang-Rong | 2021 | Applied Geophysics2021,18,3: | 2 |
