2Induction of apoptosis by arsenic trioxide and hydroxycamptothecin in gastric cancer cells in vitro显示文摘AIM To study the effects of arsenic trioxide andHCPT on different degrees of differentiated gastriccancer cells(SGC-7901,MKN-45,MKN-28)withrespect to both cytotoxicity and induction ofapoptosis in vitro.METHODS The cytotoxicity of As2O3 and HCPTon gastric cancer cells was determined by MTTassay.Morphologic changes of apoptosis ofgastric cancer cells were observed by lightmicroscopy and transmission electron microscopy.Apoptosis and cell cycle changes of gastric cancercells induced by HCPT and As2O3 were investigatedby TUNEL method and flow cytometry.RESULTS As2O3 and HCPT had remarkablecytotoxic effects on different degrees ofdifferentiated gastric cancer cells.The IC50ofAs2O3 on well differentiated gastric cancer cellMKN-28,moderately differentiated gastric cancercell SGC-7901,and poorly differentiated gastriccancer cell MKN-28 were 8.91 μmol/L,10.57μmol/L,and 11.65 μmol/L,respectively.The IC50of HCPT on MKN-28,SGC-7901,and MKN-45 were9.35 mg/L,10.21 mg/L,and 12.63 mg/Lrespectively after 48 h treatment.After 12 h ofexposure to both drugs,gastric cancer cellsexhibited morphologic features of apoptosis,including cell shrinkage,nuclear condensation, and formation of apoptotic bodies.A typicalsubdiploid peak before G0/G1 phase was observedby flow cytometry.The apoptotic rates of SGC-7901,MKN-45,and MKN-28 were 13.84%,22.52%,and 9.68%,respectively after 48 hexposure to 10 μmol/L As2O3.The apoptotic ratesof SGC-7901,MKN-45,and MKN-28 were 21.88%,12.35%,and 30.26%,respectively after 48 hexposure to 10 mg/L HCPT.The apoptotic indicewere 7%-15% as assessed by TUNEL method.The effect of As2O3 on SGC-7901 showedremarkable cell cycle specificity,which inducedcell death in G1 phase,and blocked G2/M phase.HCPT also showed a remarkable cell cyclespecificity,by inducing cell death and apoptosis inG1 phase and arrest of proliferation at S phase.CONCLUSION As2O3 and HCPT exhibitsignificant cytotoxicity on gastric cancer cells byinduction of apoptosis.As2O3 and HCPT mighthave a promising prospect in the treatment ofgastric cancer,which needs to be further studied.Tu SP Zhong J Tan JH Jiang XH Qiao MM Wu YX Jiang SH 2000World Journal of Gastroenterology2000,6,4:43
3Management of hepatocellular carcinoma with portal vein tumor thrombosis: Review and update at 2016显示文摘Portal vein tumor thrombosis(PVTT) is a common phenomenon in hepatocellular carcinoma(HCC). Compared to HCC without PVTT, HCC with PVTT is characterized by an aggressive disease course, worse hepatic function, a higher chance of complications related to portal hypertension and poorer tolerance to treatment. Conventionally, HCC with PVTT is grouped together with metastatic HCC during the planning of its management, and most patients are offered palliative treatment with sorafenib or other systemic agents. As a result, most data on the management of HCC with PVTT comes from subgroup analyses or retrospective series. In the past few years, there have been several updates on management of HCC with PVTT. First, it is evident that HCC with PVTT consists of heterogeneous subgroups with different prognoses. Different classifications have been proposed to stage the degree of portal vein invasion/thrombosis, suggesting that different treatment modalities may be individualized to patients with different risks. Second, more studies indicate that more aggressive treatment, including surgical resection or locoregional treatment, may benefit select HCC patients with PVTT. 4High-intensity focused ultrasound ablation:An effective bridging therapy for hepatocellular carcinoma patients显示文摘AIM:To analyze whether high-intensity focused ultrasound(HIFU) ablation is an effective bridging therapy for patients with hepatocellular carcinoma(HCC).METHODS:From January 2007 to December 2010,49 consecutive HCC patients were listed for liver transplantation(UCSF criteria).The median waiting time for transplantation was 9.5 mo.Twenty-nine patients received transarterial chemoembolization(TACE) as a bringing therapy and 16 patients received no treatment before transplantation.Five patients received HIFU ablation as a bridging therapy.Another five patients with the same tumor staging(within the UCSF criteria) who received HIFU ablation but not on the transplant list were included for comparison.Patients were comparable in terms of Child-Pugh and model for end-stage liver disease scores,tumor size and number,and cause of cirrhosis.RESULTS:The HIFU group and TACE group showed no difference in terms of tumor size and tumor number.One patient in the HIFU group and no patient in the TACE group had gross ascites.The median hospital stay was 1 d(range,1-21 d) in the TACE group and two days(range,1-9 d) in the HIFU group(P < 0.000).No HIFU-related complication occurred.In the HIFU group,nine patients(90%) had complete response and one patient(10%) had partial response to the treatment.In the TACE group,only one patient(3%) had response to the treatment while 14 patients(48%) had stable disease and 14 patients(48%) had progressive disease(P = 0.00).Seven patients in the TACE group and no patient in the HIFU group dropped out from the transplant waiting list(P = 0.559).CONCLUSION:HIFU ablation is safe and effective in the treatment of HCC for patients with advanced cirrhosis.It may reduce the drop-out rate of liver transplant candidate.Tan To Cheung Sheung Tat Fan See Ching Chan Kenneth SH Chok Ferdinand SK Chu Caroline R Jenkins Regina CL Lo James YY Fung Albert CY Chan William W Sharr Simon HY Tsang Wing Chiu Dai Ronnie TP Poon Chung Mau Lo 2013World Journal of Gastroenterology2013,19,20:23
5Effects of Silicon Content and Intercritical Annealing on Manganese Partitioning in Dual Phase Steels显示文摘Steels of constant manganese and carbon contents with silicon content of 0.34%-2.26% were cast.The as-cast steels were then hot rolled at 1100 ℃ in five passes to reduce the cast ingot thickness from 80 to 4 mm, air cooled to room temperature and cold rolled to 2 mm in thickness. Dual phase microstructures with different volume fraction of martensite were obtained through the intercritical annealing of the steels at different temperatures for 15 min followed by water quenching. In addition to intercritical annealing temperature, silicon content also altered the volume fraction of martensite in dual phase steels. The partitioning of manganese in dual phase silicon steels was investigated using energy-dispersive spectrometer (EDS). The partitioning coefficient, defined as the ratio of the amounts of alloying element in the austenite to that in the adjacent ferrite, for manganese increased with increasing intercritical annealing temperature and silicon content of steels. It was also found that the solubility of manganese in ferrite and austenite decreased with increasing intercritical temperature. 10Evaluation of Four Coals for Blast Furnace Pulverized Coal Injection显示文摘One of the effective methods of reducing coke consumption is pulverized coal injection. The most important problems encountered in this method are reduced permeability,unburned and high ash content. To select the best coal for injection,suitable tests can be used. Therefore,experiments such as proximate and ultimate analysis,Rock-Eval and combustion tests were performed on four kinds of coals from different mines,including Sarakhs,Sangrood,Karmozd,and Tabas. The results of proximate and ultimate analysis indicated that although the sulfur content and ash content of selected coals were a little high,they were suitable for coal injection. The results of combustion experiments and Rock-Eval tests showed that Karmozd coal was the best one to be injected into blast furnace. 11Cytotoxicity evaluation and hepatoprotective potential of bioassay guided fractions from Feronia limmonia Linn leaf显示文摘Objective:To evaluate the cytotoxicity and hepatoprotective potentials of extracts,fractions or isolated compound from the leaves of Feronia limonia(F.limonia).Methods:Qualitative phytochemical analysis of extracts,fractions or compound was performed by means of thin layer chromatography and spectroscopic assays.The%purity of compound was measured by analytical HPLC.Extracts,fractions or compound have been individually evaluated for their cytotoxicity effects(10,20,100,250,500,750 and 1 000 μg/mL).Based on the inhibitory concentration(IC_(50)) obtained from the cell viability assay,graded concentrations of extracts,fractions or isolated compound were assessed(10,20,50,100,200 μg/mL) for its hepatoprotective potential against CCl_4-induced hepatotoxicity by monitoring activity levels of serum glutamatic pyruvatic transaminase(SGPT) and serum glutamic oxaloacetic transaminase(SGOT).Results:Results indicated that the methanol extract of F.limonia was non-toxic and hepatoprotective in nature as compared with the petroleum ether extract.The acetone fraction of methanolic extract also showed similar properties but the subsequent two fractions were cytotoxic.However,the pure compound isolated from the penultimate fraction of methanolic extract was non-toxic and hepatoprotective in nature.Biochemical investigations(SCOT,SCPT) further corroborated these cytological observations.Conclusions:It can be concluded from this study that F.limonia methanol extract,some fractions and pure isolated compound herein exhibit hepatoprotective activity.However,cytotoxicity recorded in the penultimate fraction and investigation of structural details of pure compound warrants further study.Mahendra Jain Rakhee Kapadia Ravirajsinh N Jadeja Menaka C Thounaojam Ranjitsinh V Devkar SH Mishra 2011Asian Pacific Journal of Tropical Biomedicine2011,1,6:11

13Strategies to increase the resectability of hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC) is best treated by liver transplantation, but the applicability of transplantation is greatly limited. Tumor resection in partial hepatectomy is hence resorted to. However, in most parts of the world, only 20%-30% of HCCs are resectable. The main reason for such a low resectability is a future liver remnant too small to be sufficient for the patient. To allow more HCC patients to undergo curative hepatectomy, a variety of ways have been developed to increase the resectability of HCC, mainly ways to increase the future liver remnants in patients through hypertrophy. They include portal vein embolization, sequential transarterial chemoembolization and portal vein embolization, staged hepatectomy, two-staged hepatectomy with portal vein ligation, and Associating Liver Partition and Portal Vein Ligation in Staged Hepatectomy. 15小胶质细胞对星形胶质细胞的免疫和神经保护功能的调节作用(英文)显示文摘在中枢神经系统中,小胶质细胞和星形胶质细胞对神经元的发育、功能维持及细胞存活具有重要的作用。但在炎症反应过程中,星形胶质细胞增生的作用及其对神经元的影响目前并不十分清楚。因此,本研究对不同种类的细胞进行培养,给予细菌脂多糖(lipopolysaccharide,LPS)刺激,观察各种细胞的反应。结果显示,星形胶质细胞可以减轻炎症反应的神经元毒性,提示星形胶质细胞增生主要具有神经保护作用。如果去掉上清液中由星形胶质细胞分泌的胶质细胞源性神经营养因子(glial cell line-derived neurotrophic factor,GDNF),这种神经保护作用显著降低。为了研究星形胶质细胞的免疫功能,本研究培养了高浓度的星形胶质细胞,并发现LPS不能诱导肿瘤坏死因子(tumor necrosis factor,TNF)-α及诱导型一氧化氮合酶(inducible nitric oxide synthase,iN OS)/一氧化氮(nitric oxide,NO)(iN OS/NO)释放。但如果在培养的高浓度星形胶质细胞中增加0.5%~1%的小胶质细胞,则可以诱导TNF-α及iN OS/NO的释放。这提示小胶质细胞和星形胶质细胞间的相互作用对于LPS诱导星形胶质细胞释放前炎症因子及GDNF是必须的。我们还发现,小胶质细胞释放的TNF-α可以通过旁分泌作用调节星形胶质细胞的神经保护功能。综上所述,本研究结果提示星形胶质细胞可能不会直接被LPS激活,而是通过与小胶质细胞相互作用被激活,释放神经营养因子,减少因炎症反应激活的小胶质细胞释放的毒性物质对神经元的损害作用。Chen SH Oyarzabal EA Sung YF Chu CH Wang Q Chen SL Lu RB Hong JS 唐颖馨 2015神经损伤与功能重建2015,10,1:9
16Pancreaticoduodenectomy with vascular reconstruction for adenocarcinoma of the pancreas with borderline resectability显示文摘AIM:To analyze whether pancreaticoduodenectomy with simultaneous resection of tumor-involved vessels is a safe approach with acceptable patient survival.METHODS:Between January 2001 and March 2012,136 patients received pancreaticoduodenectomy for adenocarcinoma at our hospital.Seventy-eight patients diagnosed with pancreatic head carcinoma were included in this study.Among them,46 patients received standard pancreaticoduodenectomy(group 1)and32 patients received pancreaticoduodenectomy with simultaneous resection of the portal vein or the superior mesenteric vein or artery(group 2)followed by reconstruction.The immediate surgical outcomes and survivals were compared between the groups.Fifty-five patients with unresectable adenocarcinoma of the pancreas without liver metastasis who received only bypassoperations(group 3)were selected for additional survival comparison.RESULTS:The median ages of patients were 67 years(range:37-82 years)in group 1,and 63 years(range:35-86 years)in group 2.All group 2 patients had resection of the portal vein or the superior mesenteric vein and three patients had resection of the superior mesenteric artery.The pancreatic fistula formation rate was21.7%(10/46)in group 1 and 15.6%(5/32)in group2(P=0.662).Two hospital deaths(4.3%)occurred in group 1 and one hospital death(3.1%)occurred in group 2(P=0.641).The one-year,three-year and five-year overall survival rates in group 1 were 71.1%,23.6%and 13.5%,respectively.The corresponding rates in group 2 were 70.6%,33.3%and 22.2%(P=0.815).The one-year survival rate in group 3 was13.8%.Pancreaticoduodenectomy with simultaneous vascular resection was safe for pancreatic head adenocarcinoma.CONCLUSION:The short-term and survival outcomes with simultaneous resection were not compromised when compared with that of standard pancreaticoduodenectomy.Tan To Cheung Ronnie TP Poon Kenneth SH Chok Albert CY Chan Simon HY Tsang Wing Chiu Dai See Ching Chan Sheung Tat Fan Chung Mau Lo 2014World Journal of Gastroenterology2014,20,46:8
17Effects of partial portal vein arterialization on the hilar bile duct in a rat model显示文摘BACKGROUND: Liver revascularization is frequently required during the enlarged radical operation for hilar cholangio carcinoma involving the hepatic artery. Researchers have carried out a number of experiments applying partial porta vein arterialization (PVA) in clinical practice. In this study we aimed to establish a theoretical basis for clinical application o partial PVA and to investigate the effects of partial PVA on ra hilar bile duct and hepatic functions. METHODS: Thirty rats were randomly and equally assigned into 3 groups: control (group A), hepatic artery ligation+bile duct recanalization (group B), and partial PVA+bile duc recanalization (group C). Proliferation and apoptosis o rat hilar bile duct epithelial cells, arteriolar counts of the peribiliary plexus (PBP) of the bile duct wall, changes in serum biochemistry, and pathologic changes in the bile duc were assessed 1 month after operation. RESULTS: The proliferation of hilar bile duct epithelial cells in group B was greater than in groups A and C (P<0.01). No apoptotic hilar bile duct epithelial cells were detected in any of the groups. The PBP arteriolar counts of the hilar bile duc wall were similar in groups A and C (P>0.05), but the coun was lower in group B than in group A (P<0.01). No statistically significant differences in alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase and albumin were found in the 3 groups. The gamma-glutamyltransferase value was higher in group B than in groups A and C (P<0.01) The hepatic tissues of groups A and C showed no significan abnormality. Chronic inflammatory changes in the hilar bile duct walls were observed only in group B. 18Rapid measurement of indocyanine green retention by pulse spectrophotometry: a validation study in 70 patients with Child-Pugh A cirrhosis before hepatectomy for hepatocellular carcinoma显示文摘BACKGROUND: The indocyanine green (ICG) retention test is the most popular liver function test for selecting patients for major hepatectomy. Traditionally, it is done using spectrophotometry with serial blood sampling. The newly- developed pulse spectrophotometry is a faster alternative, but its accuracy on Child-Pugh A cirrhotic patients undergoing hepatectomy for hepatocellular carcinoma has not been well documented. This study aimed to assess the accuracy of the LiMON , one of the pulse spectrophotometry systems, in measuring preoperative ICG retention in these patients and to devise an easy formula for conversion of the results so that they can be compared with classical literature records where ICG retention was measured by the traditional method. METHODS: We measured the liver function of 70 Child-Pugh A cirrhotic patients before hepatectomy for hepatocellular carcinoma from September 2008 to January 2009. ICG retention at 15 minutes measured by traditional spectrophotometry (ICGR15) was compared with ICG retention at 15 minutes measured by the LiMON (ICGR15(L)). RESULTS: The median ICGR15 was 14.7% (5.6%-32%) and the median ICGR15(L) was 10.4% (1.2%-28%). The mean difference between them was -4.3606. There was a strong correlation between ICGR15 and ICGR15(L) (correlation coefficient, 0.844; 95% confidence interval, 0.762-0.899). The following formula was devised: ICGR15=1.16×ICGR15(L)+2.73.CONCLUSIONS: The LiMON provides a fast and repeatable way to measure ICG retention at 15 minutes, but with constant underestimation of the real value. 20Management of recurrent hepatocellular carcinoma after liver transplant显示文摘Hepatocellular carcinoma(HCC) is the leading cause of deaths in patients with hepatitis B or C, and its incidence has increased considerably over the past decade and is still on the rise. Liver transplantation(LT) provides the best chance of cure for patients with HCC and liver cirrhosis. With the implementation of the MELD exception system for patients with HCC waitlisted for LT, the number of recipients of LT is increasing, so is the number of patients who have recurrence of HCC after LT. Treatments for intrahepatic recurrence after transplantation and after other kinds of surgery are more or less the same, but long-term cure of posttransplant recurrence is rarely seen as it is a 'systemic' disease. Nonetheless, surgicalresection has been shown to be effective in prolonging patient survival despite the technical difficulty in resecting graft livers. Besides surgical resection, different kinds of treatment are also in use, including transarterial chemoembolization, radiofrequency ablation, highintensity focused ultrasound ablation, and stereotactic body radiation therapy.
