| 1 | Role of endoscopic ultrasound in idiopathic pancreatitis显示文摘Recurrent acute pancreatitis(RAP) is defined based on the occurrence of two or more episodes of acute pancreatitis. The initial evaluation fails to detect the cause of RAP in 10%-30% of patients, whose condition is classified as idiopathic RAP(IRAP). Idiopathic acute pancreatitis(IAP) is a diagnostic challenge for gastroenterologists. In view of associated morbidity and mortality, it is important to determine the aetiology of pancreatitis to provide early treatment and prevent recurrence. Endoscopic ultrasound(EUS) is an investigation of choice for imaging of pancreas and biliary tract. In view of high diagnostic accuracy and safety of EUS, a EUS based management strategy appears to be a reasonable approach for evaluation of patients with a single/recurrent idiopathic pancreatitis. The most common diagnoses by EUS in IAP is biliary tract disease. The present review aims to discuss the role of EUS in the clinical management and diagnosis of patients with IAP. It elaborates the diagnostic approach to IAP in relation to EUS and other different modalities. | 2 | Efficacy of repetitive transcranial magnetic stimulation in treatmentresistant depression:the evidence thus far显示文摘Depression is a common mental disorder, which attributes to significant morbidity, disability and burden of care. A significant number of patients with depression still remain symptomatic after adequate trials of antidepressant treatment as well as psychotherapy, which is often referred to as treatment-resistant depression. Neuromodulation techniques—like electroconvulsive therapy, vagus nerve stimulation, transcranial magnetic stimulation (TMS) and transcranial direct current stimulation, may be useful augmenting techniques in depression, mostly recommended for treatment-resistant cases. Robust evidence exists regarding the efficacy of electroconvulsive therapy in the management of treatment-resistant depression;however, other techniques are understudied. TMS has been increasingly studied in various psychiatric disorders including depression. It has been approved by the US Food and Drug Administration for use in major depressive disorder. Over the past two decades, TMS has been studied in diverse groups of the population with depression using several research designs. | 3 | Pre-treatment of rocks prior to comminution--A critical review of present practices显示文摘It has been an established fact that comminution processes, crushing and grinding, are most energy intensive processes which account for more than half of the total energy consumed in mineral industries.Various alternative pre-treatment methods have been tried by experts around the globe. Although these methods yielded positive results in terms of reduction in energy consumption in crushing and particularly, in grinding operations at laboratory scale, their industrial application still remains an unresolved issue and challenge. | 4 | Esophageal tuberculosis presenting with hematemesis显示文摘Esophageal tuberculosis is rare, constituting about 0.3% of gastrointestinal tuberculosis. It presents commonly with dysphagia, cough, chest pain in addition to fever and weight loss. Complications may include hemorrhage from the lesion, development of arterioesophageal fistula, esophagocutaneous fistula or tracheoesophageal fistula. There are very few reports of esophageal tuberculosis presenting with hematemesis due to ulceration. We report a patient with hematemesis that was due to the erosion of tuberculous subcarinal lymph nodes into the esophagus. A 15-year-old boy presented with hemetemesis as his only complaint. Esophagogastroduodenoscopy(EGD) revealed an eccentric ulcerative lesion involving 50% of circumference of the esophagus. Biopsy showed caseating epitheloid granulomas with lymphocytic infiltrates suggestive of tuberculosis. Computerised tomography of the thorax revealed thickening of the mid-esophagus with enlarged mediastinal lymph nodes in the subcarinal region compressing the esophagus along with moderate right sided pleural effusion. Patient was treated with anti-tuberculosis therapy(Rifampicin, Isoniazid, Pyrazinamide, Ethambutol) for 6 mo. | 5 | Protective effect of diosmin against diabetic neuropathy in experimental rats显示文摘OBJECTIVE:The present study was undertaken to evaluate the effect of diosmin in diabetic neuropathy in type 2 diabetic rats.METHODS:Type 2 diabetes was induced in male Sprague-Dawley rats by single intraperitoneal injection of streptozotocin(35 mg/kg) and high-fat diet.Four weeks after the confirmation of diabetes,diabetic rats were treated with diosmin(50 and 100 mg/kg,p.o.) for next 4 weeks.Rats were evaluated for biochemical,behavioral and oxidative stress parameters.Eddy's hot plate and tail immersion test were performed on 6th,7th,8th,9th and 10th weeks of experiment to assess thermal hyperalgesia and cold allodynia respectively.Further,the walking function test was performed for assessing the motor responses at the end of the treatment schedule.RESULTS:Rats were fed with high-fat diet throughout the experiment schedule and administration of low-dose streptozotocin induced significant elevation in blood glucose level and insulin resistance which was confirmed by oral glucose tolerance test.Treatment with diosmin at doses of 50 and 100 mg/kg significantly restored the reduced body weight,elevated blood sugar and lipid profiles.Further the dose-dependent improvement was observed in thermal hyperalgesia,cold allodynia and walking function in diabetic rats treated with diosmin.Elevated levels of malondialdehyde,and nitric oxide and decreased glutathione levels and superoxide dismutase activity in diabetic rats were restored significantly after the 4 weeks of diosmin treatment.CONCLUSION:Diosmin has shown beneficial effect in preventing the progression of early diabetic neuropathy in rats. | Dilpesh Jain Manish Kumar Bansal Rahul Dalvi Aman Upganlawar Rahul Somani | 2014 | Journal of Integrative Medicine2014,12,1: | 4 |

| 7 | Partial Protection by Lipoic Acid Against Carboplantin-induced Ototoxicity in Rats显示文摘Objective To investigate the alterations in auditory brainstem evoked responses (ABRs) and the changes of carboplatin-induced ototoxicity in the cochlear oxidant/antioxidant systems and otoprotection by an antioxidant lipoate. Methods Male wistar rats were divided into four groups and treated as follows: 1) vehicle (saline) control, 2) carboplatin (256 mg/kg, i.p.), 3) lipoate (100 mg/kg, i.p.), 4) lipoate + carboplatin. Post-treatment ABRs were performed after four days and rats were sacrificed with their cochleae harvested and analyzed. Results Carboplatin significantly elevated ABR threshold above the pretreatment thresholds. Lipoate+carboplatin treated rats showed decreased elevation of hearing threshold. Carboplatin significantly depleted cochlear reduced to oxizized glutathione (GSH/GSSG) ratio, whereas lipoate+carboplatin treatment increased GSH/GSSG ratio. Carboplatin significantly decreased cochlear copper zinc-superoxide dismutase (CuZn-SOD), catalase (CAT), glutathione peroxidase (GSH-Px), glutathione reductase (GR) and glutathione-S-transferase (GST) activities and enzyme protein expressions and a significant increase in Mn-SOD activity, protein expression and malondialdehyde (MDA) level. Cochlear antioxidant enzyme activities, enzyme protein expressions and MDA level were partially restored in lipoate+carboplatin treated rats, compared to carboplatin alone.
