1Inflammation, oxidative stress and renin angiotensin system in atherosclerosis显示文摘Atherosclerosis is a chronic inflammatory disease associated with cardiovascular dysfunction including myocardial infarction, unstable angina, sudden cardiac death, stroke and peripheral thromboses. It has been predicted that atherosclerosis will be the primary cause of death in the world by 2020. Atherogenesis is initiated by endothelial injury due to oxidative stress associated with cardiovascular risk factors including diabetes mellitus, hypertension, cigarette smoking, dyslipidemia, obesity, and metabolic syndrome. The impairment of the endothelium associated with cardiovascular risk factors creates an imbalance between vasodilating and vasoconstricting factors, in particular, an increase in angiotensin Ⅱ(Ang Ⅱ) and a decrease in nitric oxide. The renin-angiotensin system(RAS), and its primary mediator Ang Ⅱ, also have a direct influence on the progression of the atherosclerotic process via effects on endothelial function, inflammation, fibrinolytic balance, and plaque stability. Anti-inflammatory agents [statins, secretory phospholipase A2 inhibitor, lipoprotein-associated phospholipase A2 inhibitor, 5-lipoxygenase activating protein, chemokine motif ligand-2, C-C chemokine motif receptor 2 pathway inhibitors, methotrexate, IL-1 pathway inhibitor and RAS inhibitors(angiotensin-converting enzyme inhibitors)], Ang Ⅱ receptor blockers and ranin inhibitors may slow inflammatory processes and disease progression. Several studies in human using anti-inflammatory agents and RAS inhibitors revealed vascular benefits and reduced progression of coronary atherosclerosis in patients with stable angina pectoris; decreased vascular inflammatory markers, improved common carotid intima-media thickness and plaque volume in patients with diagnosed atherosclerosis. 5经口腔达芬奇机器人甲状腺手术初步经验显示文摘目的探寻通过达芬奇机器人技术进行经口腔甲状腺切除术(transoral robotic thyroidectomy,TORT)是否可行并报道初步的经验教训。方法从2017年9月到12月,在北京和睦家医院进行了5例TORT。使用了3个完全口内切口连接达芬奇机器人手臂3个端口,回顾性分析手术结果。结果 5例均为女性患者(平均年龄30岁,平均肿瘤大小2. 0 cm),其中行单侧甲状腺腺叶切除4例、全切1例,3例行中央颈清扫。其中3例甲状腺乳头状癌(papillary thyroid carcinomas,PTC)、1例甲状腺滤泡癌、1例甲状腺良性腺瘤。平均手术时间为343 min。PTC患者中央淋巴结数目平均为8. 7枚。术后没有短暂性声带麻痹和低钙症状,3例患者术后有下唇感觉减退和感觉异常、1例因镜头接触发生皮瓣Ⅰ度烫伤。结论 TORT对于选择的甲状腺肿瘤患者初步尝试是可行的,是无瘢痕甲状腺手术选择之一。张彬 韩宗辉 Bikash Rai 2018中华腔镜外科杂志(电子版)2018,11,4:21

7Enhanced and irreversible sorption of pesticide pyrimethanil by soil amended with biochars显示文摘Biochar derived from partial combustion of vegetation is ubiquitous and potentially effective in sequestration of environmental contaminants.Biochars were prepared by burning of red gum(Eucalyptus spp.) woodchips at 450 and 850°C(labeled as BC450 and BC850).These two biochars were found to possess markedly different properties in terms of surface area and porosity.Short-term equilibration tests(24 hr) were conducted to assess the sorption-desorption behavior of pyrimethanil in the soil amended with various amounts of biochar of each type,with a special focus on the desorption behavior of the sorbed pesticide through four times successive desorption by dilution.Sorption coefficient and isotherm nonlinearity of the amended soils progressively increased with the content of biochar in the soil.Biochar BC850 with higher surface area and microporosity showed a stronger effect on the reversibility of sorption pesticide.The soils amended with 5% BC450 and 1% BC850 had nearly the same sorption capacity for pyrimethanil;however,their desorption processes were very different with 13.65% and 1.49% of the sorbed pesticide being released,respectively.This study suggested that biochar in soil could be an important factor for immobilization of a pesticide and thus affecting its environment fate in soil.Xiangyang Yu Ligang Pan Guangguo Ying Rai S. 8Fiber and prebiotic supplementation in enteral nutrition:A systematic review and meta-analysis显示文摘AIM:To investigate fiber and prebiotic supplementation of enteral nutrition(EN) for diarrhea,fecal microbiota and short-chain fatty acids(SCFAs).METHODS:MEDLINE,EMBASE,Cochrane Library,CINAHL,Academic Search Premier,and Web of Science databases were searched for human experimental and observational cohort studies conducted between January 1990 and June 2014.The keywords used for the literature search were fiber,prebiotics and enteral nutrition.English language studies with adult patient populations on exclusive EN were selected.Abstracts and/or full texts of selected studies were reviewed and agreed upon by two independent researchers for inclusion in the meta-analysis.Tools used for the quality assessment were Jadad Scale and the Scottish Intercollegiate Guidelines Network Critical Appraisal of the Medical Literature.RESULTS:A total of 456 possible articles were retrieved,and 430 were excluded due to lack of appropriate data.Of the 26 remaining studies,only eight investigated the effects of prebiotics.Results of the meta-analysis indicated that overall,fiber reduces diarrhea in patients receiving EN(OR = 0.47;95%CI:0.29-0.77;P = 0.02).Subgroup analysis revealed a positive effect of fiber supplementation in EN towards diarrhea in stable patients(OR = 0.31;95%CI:0.19-0.51;P < 0.01),but not in critically ill patients(OR = 0.89;95%CI:0.41-1.92;P = 0.77).Prebiotic supplementation in EN does not improve the incidence of diarrhea despite its manipulative effect on bifidobacteria concentrations and SCFA in healthy humans.In addition,the effect of fiber and/or prebiotic supplementation towards fecal microbiota and SCFA remain disputable.CONCLUSION:Fiber helps minimize diarrhea in patients receiving EN,particularly in non-critically ill patients.However,the effect of prebiotics in moderating diarrhea is inconclusive.Mazuin Kamarul Zaman Kin-Fah Chin Vineya Rai Hazreen Abdul Majid 2015World Journal of Gastroenterology2015,21,17:18
9ERCP in acute pancreatitis:What takes place in routine clinical practice?显示文摘AIM:To evaluate the data from a survey carried out in Italy regarding the endoscopic approach to acute pancreatitis in order to obtain a picture of what takes place after the release of an educational project on acute pancreatitis sponsored by the Italian Association for the Study of the Pancreas.METHODS:Of the 1 173 patients enrolled in our survey,the most frequent etiological category was biliary forms(69.3%) and most patients had mild pancreatitis(85.8%).RESULTS:344/1 173(29.3%) underwent endoscopic retrograde cholangiopancreatography(ERCP).The mean interval between the onset of symptoms and ERCP was 6.7 ± 5.0 d;only 89 examinations(25.9%) were performed within 72 h from the onset of symptoms.The main indications for ERCP were suspicion of common bile duct stones(90.3%),jaundice(44.5%),clini cal worsening of acute pancreatitis(14.2%) and cho langitis(6.1%).Biliary and pancreatic ducts were visua lized in 305 patients(88.7%) and in 93 patients(27.0%) respectively.The success rate in obtaining a cholangio gram was statistically higher(P = 0.003) in patients with mild acute pancreatitis(90.6%) than in patients with severe disease(72.2%).Biliary endoscopic sphinc terotomy was performed in 295 of the 305 patients(96.7%) with no difference between mild and severe disease(P = 0.985).ERCP morbidity was 6.1% and mortality was 1.7%;the mortality was due to the complications of acute pancreatitis and not the endoscopic procedure.CONCLUSION:The results of this survey,as with those carried out in other countries,indicate a lack of compliance with the guidelines for the indications for interventional endoscopy.Armando Gabbrielli Raffaele Pezzilli Generoso Uomo Alessandro Zerbi Luca Frulloni Paolo De Rai Laura Castoldi Guido Costamagna Claudio Bassi Valerio Di Carlo 2010World Journal of Gastrointestinal Endoscopy2010,2,9:14
10MicroRNA aberrations:An emerging field for gallbladdercancer management显示文摘Gallbladder cancer(GBC) is infrequent but most lethal biliary tract malignancy characterized by an advanced stage diagnosis and poor survival rates attributed to absence of specific symptoms and effective treatment options. These necessitate development of early prognostic/predictive markers and novel therapeutic interventions. Micro RNAs(mi RNAs) are small, noncoding RNA molecules that play a key role in tumor biology by functioning like tumor suppressor- or oncogenes and their aberrant expression are associated with the pathogenesis of several neoplasms with overwhelming clinical implications. Since mi RNA signature is tissue specific, here, we focused on current data concerning the mi RNAs abberations in GBC pathogenesis. In GBC, mi RNAs with tumor suppressor activity(mi R-135-5p, mi R-335, mi R-34 a, mi R-26 a, mi R-146b-5p, Mir-218-5p, mi R-1, mi R-145, mir-130a) were found downregulated, while those with oncogenic property(mi R-20 a, mi R-182, mir-155) were upregulated. The expression profile of mi RNAs was significantly associated with GBC prognosis and prediction, and forced over-expression/ inhibition of these mi RNAs was shown to affect tumor growth and development. Further, differential expression of mi RNAs in the blood samples of GBC patients suggest mi RNAs as promising noninvasive biomarker. 12经耳后达芬奇机器人甲状腺手术的初步经验显示文摘目的分析总结耳后入路达芬奇机器人甲状腺手术的经验,探讨此术式的可行性。方法回顾性分析2018年5月—12月,在北京和睦家医院采用无注气耳后入路达芬奇机器人手术的甲状腺癌患者5例,其中男1例,女4例,年龄18~37岁。其中1例为甲状腺癌甲状腺全切除术后颈部淋巴结复发。分析总结临床特点、治疗及预后。结果5例患者中行单侧甲状腺腺叶切除术1例,甲状腺全切除术3例,4例行颈侧清扫术,1例术中中转。5例病理均为甲状腺乳头状癌(papillary thyroid carcinoma,PTC),平均肿瘤大小1.3 cm,每侧平均手术时间约375 min。术后发生声带麻痹和低钙各1例次。结论对部分经选择的甲状腺肿瘤患者,初步尝试经耳后机器人手术是可行的,是颈外入路甲状腺手术的选择之一。张彬 于灏 韩宗辉 Bikash Rai 2020中华耳鼻咽喉头颈外科杂志2020,55,3:13
13Role of Helicobacter pylori in gastric cancer:Updates显示文摘Helicobacter pylori(H. pylori) infection is highly prevalent in human,affecting nearly half of the world's population; however,infection remains asymptomatic in majority of population. During its co-existence with humans,H. pylori has evolved various strategies to maintain a mild gastritis and limit the immune response of host. On the other side,presence of H.pylori is also associated with increased risk for the development of various gastric pathologies including gastric cancer(GC). A complex combination of host genetics,environmental agents,and bacterial virulence factors are considered to determine the susceptibility as well as the severity of outcome in a subset of individuals. GC is one of the most common cancers and considered as the third most common cause of cancer related death worldwide. Many studies had proved H. pylori as an important risk factor in the development of non-cardia GC. Although both H. pylori infection and GC are showing decreasing trends in the developed world,they still remain a major threat to human population in the developing countries. The current review attempts to highlight recent progress in the field of research on H. pylori induced GC and aims to provide brief insight into H. pylori pathogenesis,the role of major virulence factors of H. pylori that modulates the host environment and transform the normal gastric epithelium to neoplastic one. 15Anti-oxidation actions of curcumin in two forms of bed rest:oxidative stress serum and salivary markers显示文摘Objective:To determine the preventive effects of curcumin on peroxidative damage under two bed rest conditions.Methods:20 healthy male(10 with curcumin and 10 without curcumin ) volunteers were selected.They were studied before,during,and just on bed rest conditions at -6°head-down-tilt(HDT) bed rest and bed rest position(BD) for 10 days.We measured the salivary and serum oxidative markers such as Malonaldehyde,8-hydroxydeoxyguanosine, vitamin C and E just before HDT & BD.during HDT & BD experiment,and in course time of recovery with curcumin and without curcumin groups.Results:The values of serum and salivary vitamin C & E showed statistically significant decrease in both bed rest conditions as compared to those of the conditions before and during the recovery stage.However,these levels were not significantly lowered in curcumin groups in contrast to the groups without curcumin (P>0.05).MDA and 8-OHdG levels showed significant increase in simulating microgravity and zero gravity conditions as compared to those before and in the recovery stage.However,these levels were lower in curcumin groups in contrast to the groups without curcumin(P<0.05).Serum and salivary correlation analysis revealed a strong and highly significant correlation for MDA. vitamin C & E and 8 dihydro-2 deoxyguanosine(8-OHdG) in the conditions before,during and in the recovery periods in both bed rest conditions.Since saliva collection is easy and noninvasive, measurements of salivary marker levels may prove to be useful in the space research. 17Potentially fatal haemobilia due to inappropriate use of an expanding biliary stent显示文摘AIM: To highlight the fatal complication caused by expanding biliary stents and the importance of avoiding use of expanding stent in potentially curable diseases.METHODS: Arteriobiliary fistula is an uncommon cause of haemobilia. We describe a case of right hepatic artery pseudoaneurysm causing arteriobiliary fistula and presenting as severe malena and cholangitis, in a patient with a mesh metal biliary stent. The patient had lymphoma causing bile duct obstruction.RESULTS: Gastroduodenoscopy failed to establish the exact source of bleeding and hepatic artery angiography and selective embolisation of the pseudo aneurysm successfully controlled the bleeding.CONCLUSION: Bleeding from the pseudo aneurysm of the hepatic artery can be fatal. Mesh metal stents in biliary tree can cause this complication as demonstrated in this case.So mesh metal stent insertion should be avoided in potentially benign or in curable conditions. 18Alcohol-induced hypertension: Mechanism and prevention显示文摘Epidemiological, preclinical and clinical studies es-tablished the association between high alcohol con-sumption and hypertension. However the mechanism through which alcohol raises blood pressure remains elusive. Several possible mechanisms have been pro-posed such as an imbalance of the central nervous system, impairment of the baroreceptors, enhanced sympathetic activity, stimulation of the renin-angio-tensin-aldosterone system, increased cortisol levels, increased vascular reactivity due to increase in intracel-lular calcium levels, stimulation of the endothelium to release vasoconstrictors and loss of relaxation due to inflammation and oxidative injury of the endothelium leading to inhibition of endothelium-dependent nitric oxide production. Loss of relaxation due to inflamma-tion and oxidative injury of the endothelium by angio-tensin II leading to inhibition of endothelium-dependent nitric oxide production is the major contributors of the alcohol-induced hypertension. For the prevention of alcohol-induced hypertension is to reduce the amount of alcohol intake. Physical conditioning/exercise trainingis one of the most important strategies to prevent/treat chronic alcohol-induced hypertension on physiological basis. 19二肽基肽酶-4水平、超敏C反应蛋白与冠状动脉病变程度的相关性显示文摘目的探讨二肽基肽酶(DPP)-4水平、超敏C反应蛋白(hs-CRP)与冠状动脉病变程度的相关性。方法选取经冠状动脉造影(CAG)后明确诊断为冠心病(CAD)的120例患者为CAD组,CAG阴性的40例患者为对照组,通过SYNTAX积分将CAD组分为低危组、中危组和高危组。检测各组糖化血红蛋白(HbA1c)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、肌酐(SCr)、总胆固醇(TC)、三酰甘油(TG)、hs-CRP、DPP-4水平。结果CAD低、中、高危组的hs-CRP水平均明显高于对照组,中危组的hs-CRP水平明显高于低危组(P<0.05),并随着冠状动脉病变严重程度的增加而上升(P<0.001);CAD高危组和中危组的DPP-4水平均明显高于对照组、低危组(P<0.05)。高危组的DPP-4水平明显高于中危组(P<0.05)。DPP-4水平随着冠状动脉病变严重程度的增加而上升(P<0.001);hs-CRP,DPP-4水平影响冠状动脉病变程度的多重线性回归分析结果示:未调整时,hs-CRP,DPP-4水平与SYNTAX积分呈显著正相关(P<0.01)。调整年龄、性别、β-受体阻滞剂用药史、血管紧张素转换酶抑制剂用药史、SCr、LDL-C因素后,hs-CRP、DPP-4水平与SYNTAX积分仍呈显著正相关(P<0.001)。结论测定血浆hs-CRP,DPP-4水平有利于预测CAD患者冠状动脉病变严重程度,hs-CRP,DPP-4水平与CAD密切相关,DPP-4水平为判断冠状动脉病变程度提供了新的生物学标志物。修倩茹 王誉旋 徐睿 李明杰 孙素娟 Avinas Rai 杨光 成宪武 2019中国老年学杂志2019,39,19:7
20Expression profile of cholecystokinin type-A receptor in gallbladder cancer and gallstone disease显示文摘BACKGROUND:Regulatory peptide receptors have attracted the interest of oncologists as a new promising approach for cancer pathology,imaging and therapy.Although cholecystokinin (CCK) is a potent modulator of gallbladder contractility and plays a potential role in pancreatic carcinogenesis through CCK type-A receptor (CCKAR),its role in gallbladder cancer (GBC) is still unknown and immunohistochemical detection of CCKAR in the gallbladder has not yet been reported.This novel case-control study aimed to investigate the expression profile of CCKAR in GBC and gallstone disease (GSD).METHODS:This study included 162 samples of gallbladder:94 from GBC and 68 from GSD.Expression of CCKAR was analyzed by immunohistochemistry and immunoblotting.The results were statistically correlated with disease history including age,sex,presence of gallstone,stage and differentiation.RESULTS:CCKAR was positive in 30/68 (44.1%) of GSD and 72/94 (76.6%) of GBC samples.Fifty-one of the 72 (70.8%) CCKAR-positive GBC samples showed over-expression.Interestingly,consistent results also appeared in the immunoblotting study.CONCLUSIONS:CCKAR expression was significantly increased in GBC compared to GSD.Moreover,CCKAR expression was associated with the degree of tumor differentiation,i.e.,less expression in poorly-differentiated tumors.Thus,it has future prognostic and therapeutic implications in the management of GBC.Rajani Rai Mallika Tewari Mohan Kumar Tej Bali Singh Hari S Shukla 2011Hepatobiliary & Pancreatic Diseases International2011,10,4:7
