| 1 | Combined laparoscopic and endoscopic treatment for bile duct diseases显示文摘BACKGROUND: Clinical application of laparoscopy, duo- denoscopy and choledochoscopy has been accepted as a mini-invasive surgical therapy for bile duct diseases; but ei- ther endoscopic or laparoscopic therapy alone is disadvanta- geous in its narrow indications and in failure to give full play to the individual superiority. The present study was to evaluate the procedures and therapeutic results of combined laparoscopic and endoscopic treatment for bile duct disea- ses. METHODS: Clinical data of 1990 patients with bile duct diseases treated by combination of laparoscopy, duodenos- copy and choledochoscopy in two hospitals were reviewed and analyzed. RESULTS: Patients with cholecystolithiasis and choledo- cholithiasis were treated with combined laparoscopy and duodenoscopy (n =1350) in a single operation with a cure rate of 93.6%. Those with choledocholithiasis (n =332) were treated with combined laparoscopy and choledocho- scopy with a cure rate of 100%. Combined laparoscopy, duo- denoscopy and choledochoscopy was used in 258 patients with choledocholithiasis (29 of them complicated with pan- creatitis) and 24 patients with Mirizzi’s syndrome, with a cure rate of 100%. Laparoscopic choledochoenterostomy and preoperative endoscopic nasobiliary drainage were done in 26 patients with a cure rate of 100%. There were no serious operative complications. A follow-up study of 1051 patients for 3 months to 12 years (mean 7.8 years) showed that 10 patients had recurrence of stones but no stenosis of the bile duct. | 8 | Effect of human autologous serum and fetal bovine serum on human corneal epithelial cell viability,migration and proliferation in vitro显示文摘AIM: To analyze the concentration-dependent effects of autologous serum(AS) and fetal bovine serum(FBS) on human corneal epithelial cell(HCEC) viability, migration and proliferation.METHODS: AS was prepared from 13 patients with nonhealing epithelial defects Dulbecco's modified eagle medium/Ham's F12(DMEM/F12) with 5% FBS, 0.5% dimethyl sulphoxide(DMSO), 10 ng/m L human epidermal growth factor, 1% insulin-transferrin-selenium, then were incubated in serum media: DMEM/F12 supplemented by 5%, 10%, 15% or 30% AS or FBS. HCEC viability was analyzed using cell proliferation kit XTT, migration using a wound healing assay, proliferation by the cell proliferation enzyme-linked immunosorbent assay(ELISA) Brd U kit. Statistical analysis was performed using the generalized linear model, the values at 30% AS or 30% FBS were used as the baselines.RESULTS: HCEC viability was the highest at 30% AS or 15% FBS and the lowest at 10% AS or 30% FBS application. HCEC migration was the quickest through 30% AS or 30% FBS and the slowest through 5% AS or 5% FBS concentrations. Proliferation was the most increased through 15% AS or 5% FBS and the least increased through 30% AS or 30% FBS concentrations. HCEC viability at 10% and 15% AS was significantly worse(P=0.001, P=0.023) compared to baseline and significantly better at 15% FBS(P=0.003) concentrations. HCEC migration was significantly worse(P≤0.007) and HCEC proliferation significantly better(P<0.001) in all concentration groups compared to baseline.CONCLUSION: For the best viability of HCEC 30% AS or 15% FBS, for HCEC migration 30% AS or 30% FBS, for proliferation 15% AS or 5% FBS should be used. | 11 | Intralesional steroid is beneficial in benign refractory esophageal strictures:A meta-analysis显示文摘AIM To analyze the effect of intralesional steroid injections in addition to endoscopic dilation of benign refractory esophageal strictures.METHODS A comprehensive search was performed in three databases from inception to 10 April 2017 to identify trials, comparing the efficacy of endoscopic dilation to dilation combined with intralesional steroid injections. Following the data extraction, meta-analytical calculations were performed on measures of outcome by the randomeffects method of Der Simonian and Laird. Heterogeneity of the studies was tested by Cochrane's Q and I^2 statistics. Risk of quality and bias was assessed by the Newcastle Ottawa Scale and JADAD assessment tools.RESULTS Eleven articles were identified suitable for analyses, involving 343 patients, 235 cases and 229 controls in total. Four studies used crossover design with 121 subjects enrolled. The periodic dilation index(PDI) was comparable in 4 studies, where the pooled result showed a significant improvement of PDI in the steroid group(MD:-1.12 dilation/month, 95% CI:-1.99 to -0.25 P = 0.012; I^2 = 74.4%). The total number of repeat dilations(TNRD) was comparable in 5 studies and showed a non-significant decrease(MD:-1.17, 95%CI:-0.24-0.05, P = 0.057; I^2 = 0), while the dysphagia score(DS) was comparable in 5 studies and did not improve(SMD: 0.35, 95%CI:-0.38, 1.08, P = 0.351; I^2 = 83.98%) after intralesional steroid injection.CONCLUSION Intralesional steroid injection increases the time between endoscopic dilations of benign refractory esophageal strictures. | 14 | Effects of aging on the microstructures and mechanical properties of extruded AM50+xCa magnesium alloys显示文摘The effects of aging treatment on the microstructures and mechanical properties of extruded AM50 + xCa alloys (x = 0, 1, 2 wt.%) were studied. The results indicated the secondary phase Mg17Al12 precipitated from the saturated α-Mg solid solution while Al2Ca changed slightly when the aging time was increased. The hardness of extruded AM50 + xCa al-loys increased initially to its peak, and then dropped to reach its original hardness with the increase in aging time. With the increase in aging temperature, the hardness of the AM50 + 2Ca alloy decreased, whereas the hardness of AM50 and AM50 + 1Ca alloys decreased in the initial stages of aging treatment and increased in the later stages of aging treatment. The tensile strengths of AM50 and AM50 + 1Ca alloys increased after aging treatment for the precipitation of Mg17Al12 phase, which increases the resistance against dislocation movement at the grain boundary; with increase in aging temperature, their tensile strengths in-creased. For AM50 + 2Ca alloy, the tensile strength declined after aging at 150°C and 175°C, while it increased slightly at 200°C. Long-term loss of electrical insulation and of further supportive functions myelin provides to axons, as seen in demyelinating diseases such as multiple sclerosis(MS), leads to neurodegeneration and results in progressive disabilities. Multiple lines of evidence have demonstrated the increasing inability of oligodendrocyte precursor cells(OPCs) to replace lost oligodendrocytes(OLs) in order to restore lost myelin. Much research has been dedicated to reveal potential reasons for this regeneration deficit but despite promising approaches no remyelination-promoting drugs have successfully been developed yet. In addition to OPCs neural stem cells of the adult central nervous system also hold a high potential to generate myelinating OLs. There are at least two neural stem cell niches in the brain, the subventricular zone lining the lateral ventricles and the subgranular zone of the dentate gyrus, and an additional source of neural stem cells has been located in the central canal of the spinal cord. While a substantial body of literature has described their neurogenic capacity, still little is known about the oligodendrogenic potential of these cells, even if some animal studies have provided proof of their contribution to remyelination. Redox process Sm3++e-→Sm2+ was recognized and analysed. Cyclic voltammetry data suggested that at potential sweep rates lower than 0.25 V/s, the reduction was limited by the diffusion of Sm3+ ions. It was not possible to observe reduction process of Sm2++2e-→Sm0 due to insufficient electrochemical stability of LiF-BeF2. Diffusion coefficients of Sm3+ ions in LiF-BeF2 were calculated from voltammetric and chronopotentiometric data in the temperature range 804-872 K. Diffusion coefficient values obeyed Arrhenius law.
