| 1 | Poor performance of the modified early warning score for predicting mortality in critically ill patients presenting to an emergency department显示文摘BACKGROUND:This study was undertaken to validate the use of the modified early warning score(MEWS) as a predictor of patient mortality and intensive care unit(ICU)/ high dependency(HD)admission in an Asian population.METHODS:The MEWS was applied to a retrospective cohort of 1 024 critically ill patients presenting to a large Asian tertiary emergency department(ED) between November 2006 and December2007.Individual MEWS was calculated based on vital signs parameters on arrival at ED.Outcomes of mortality and ICU/HD admission were obtained from hospital records.The ability of the composite MEWS and its individual components to predict mortality within 30 days from ED visit was assessed.Sensitivity,specificity,positive and negative predictive values were derived and compared with values from other cohorts.A MEWS of ≥4 was chosen as the cut-off value for poor prognosis based on previous studies.RESULTS:A total of 311(30.4%) critically ill patients were presented with a MEWS ≥4.Their mean age was 61.4 years(SD 18.1) with a male to female ratio of 1.10.Of the 311 patients,53(17%)died within 30 days,64(20.6%) were admitted to ICU and 86(27.7%) were admitted to HD.The area under the receiver operating characteristic curve was 0.71 with a sensitivity of 53.0%and a specificity of 72.1%in addition to a positive predictive value(PPV) of 17.0%and a negative predictive value(NPV)of 93.4%(MEWS cut-off of ≥4) for predicting mortality.CONCLUSION:The composite MEWS did not perform well in predicting poor patient outcomes for critically ill patients presenting to an ED. | Le Onn Ho Huihua Li Nur Shahidah Zhi Xiong Koh Papia Sultana Marcus Eng Hock Ong | 2013 | World Journal of Emergency Medicine2013,4,4: | 12 |
| 2 | Preparation of Chitin-PLA laminated composite for implantable application显示文摘The present study explores the possibilities of using locally available inexpensive waste prawn shell derived chitin reinforced and bioabsorbable polylactic acid(PLA)laminated composites to develop new materials with excellent mechanical and thermal properties for implantable application such as in bone or dental implant.Chitin at different concentration(1e20%of PLA)reinforced PLA films(CTP)were fabricated by solvent casting process and laminated chitin-PLA composites(LCTP)were prepared by laminating PLA film(obtained by hot press method)with CTP also by hot press method at 160C.The effect of variation of chitin concentration on the resulting laminated composite's behavior was investigated.The detailed physico-mechanical,surface morphology and thermal were assessed with different characterization technique such as FT-IR,XRD,SEM and TGA.The FTIR spectra showed the characteristic peaks for chitin and PLA in the composites.SEM images showed an excellent dispersion of chitin in the films and composites.Thermogravimetric analysis(TGA)showed that the complete degradation of chitin,PLA film,5%chitin reinforced PLA film(CTP2)and LCTP are 98%,95%,87%and 98%respectively at temperature of 500C.The tensile strength of the LCTP was found 25.09 MPa which is significantly higher than pure PLA film(18.55 MPa)and CTP2 film(8.83 MPa).After lamination of pure PLA and CTP2 film,the composite(LCTP)yielded 0.265e1.061%water absorption from 30 min to 24 h immerse in water that is much lower than PLA and CTP.The increased mechanical properties of the laminated films with the increase of chitin content indicated good dispersion of chitin into PLA and strong interfacial actions between the polymer and chitin.The improvement of mechanical properties and the results of antimicrobial and cytotoxicity of the composites also evaluated and revealed the composite would be a suitable candidate for implant application in biomedical sector. | Romana Nasrin Shanta Biswas Taslim Ur Rashid Sanjida Afrin Rumana Akhter Jahan Papia Haque Mohammed Mizanur Rahman | 2017 | Bioactive Materials2017,2,4: | 4 |

| 4 | Control Algorithm Based on Limit Cycle Oscillator-FLL for UPQC-S with Optimized PI Gains显示文摘This paper involves a limit cycle oscillator-frequency lock loop(LCO-FLL)based control algorithm for unified power quality controllers-S type(UPQC-S)for compensation of reactive power,to maintain a unity power factor,regulate constant voltage at the PCC,mitigate sag,swell and to eliminate harmonics.In this approach,an extraction of fundamental in-phase and quadrature components for the estimation of reference signals are taken from the LCO-FLL circuit.The control LCO-FLL provides a high grade of protection against sag-swell voltages,unbalance loading and harmonics present in the utility grid.In addition to that,it has advantageous characteristics of synchronization with the grid frequency at any previously mentioned conditions without use of phase locked loops or trigonometric functions.Other advantages of the LCO-FLL are to give useful information to estimate fundamental components from a highly polluted grid scenario.The values obtained from the JAYA optimization algorithm are used to fine-tune the proportional integral(PI)controller gains,so that it maintains DC link voltage to the desired level.The mean square error(MSE)is employed as an objective function for optimizing the error between actual and reference values.The control algorithm based on LCO-FLL is developed in MATLAB/Simulink software and it is tested for power conditioning features. | Sabha Raj Arya Sayed Javed Alam Papia Ray | 2020 | CSEE Journal of Power and Energy Systems2020,6,3: | 2 |
