| 1 | Role of diffusion-weighted magnetic resonance imaging in the differential diagnosis of focal hepatic lesions显示文摘AIM:To evaluate the utility of diffusion-weighted im-aging(DWI)in screening and differential diagnosis of benign and malignant focal hepatic lesions.METHODS:Magnetic resonance imaging(MRI)ex-aminations were performed using the Signa Excite Xl Twin Speed 1.5T system(GE Healthcare,Milwaukee,WI,USA).Seventy patients who had undergone MRI of the liver [29 hepatocellular carcinomas(HCC),four cholangiocarcinomas,34 metastatic liver cancers,10 hemangiomas,and eight cysts] between April 2004 and August 2008 were retrospectively evaluated.Visu-alization of lesions,relative contrast ratio(RCR),and apparent diffusion coefficient(ADC)were compared between benign and malignant lesions on DWI.Su-perparamagnetic iron oxide(SPIO)was administered to 59 patients,and RCR was compared pre-and post-administration.RESULTS:DWI showed higher contrast between ma-lignant lesions(especially in multiple small metastatic cancers)and surrounding liver parenchyma than did contrast-enhanced computed tomography.ADCs(mean ± SD × 10-3 mm2/s)were signif icantly lower(P < 0.05)in malignant lesions(HCC:1.31 ± 0.28 and liver me-tastasis:1.11 ± 0.22)and were signifi cantly higher in benign lesions(hemangioma:1.84 ± 0.37 and cyst:2.61 ± 0.45)than in the surrounding hepatic tissues.RCR between malignant lesions and surrounding he-patic tissues signifi cantly improved after SPIO adminis-tration,but RCRs in benign lesions were not improved.CONCLUSION:DWI is a simple and sensitive method for screening focal hepatic lesions and is useful for dif-ferential diagnosis. | Naoto Koike Akihiro Cho Katsuhiro Nasu Kazuhiko Seto Shigeyuki Nagaya Yuji Ohshima Nobuhiro Ohkohchi | 2009 | World Journal of Gastroenterology2009,15,46: | 38 |
| 2 | Pure laparoscopic hepatectomy for hepatocellular carcinoma with chronic liver disease显示文摘Pure laparoscopic hepatectomy is a less invasive procedure than conventional open hepatectomy for the resection of hepatic lesions. Increases in experiences with the technique, in combination with advances in technology, have promoted the popularity of pure laparoscopic hepatectomy. However, indications for usage and potential contraindications of the procedure remain unresolved. The characteristics and specific advantages of the procedure, especially for hepatocellular carcinoma(HCC) patients with chronic liver diseases,are reviewed and discussed in this paper. For cirrhotic patients with liver tumors, pure laparoscopic hepatectomy minimizes destruction of the collateral blood and lymphatic flow from laparotomy and mobilization, and mesenchymal injury from compression. Therefore, pure laparoscopic hepatectomy has the specific advantage of minimal postoperative ascites production that leads to lowering the risk of disturbance in water or electrolyte balance and hypoproteinemia. It minimizes complications that routinely trigger postoperative serious liver failure. Under adequate patient positioning and port arrangement, the partial resection of the liver in the area of subphrenic space, peri-inferior vena cava area or next to the attachment of retro-peritoneum is facilitated in pure laparoscopic surgery by providing good vision and manipulation in the small operative field.Furthermore, the features of reduced post-operative adhesion, good vision, and manipulation within the small area between the adhesions make this procedure safer in the context of repeat hepatectomy procedures.These improved features are especially advantageous for patients with liver cirrhosis and multicentric and/or metachronous HCCs. | Zenichi Morise Norihiko Kawabe Jin Kawase Hirokazu Tomishige Hidetoshi Nagata Hisanori Ohshima Satoshi Arakawa Rie Yoshida Masashi Isetani | 2013 | World Journal of Hepatology2013,5,9: | 21 |

| 4 | Caudal approach to pure laparoscopic posterior sectionectomy under the laparoscopy-specific view显示文摘AIM:To study our novel caudal approach laparoscopic posterior-sectionectomy with parenchymal transection prior to mobilization under laparoscopy-specific view.METHODS:Points of the procedure are:(1) Patients are put in left lateral position and posterior sector is not mobilized;(2) Glissonian pedicle of the sector is encircled and clamped extra-hepatically and divided afterward during the transection;(3) Dissection of inferior vena cava(IVC) anterior wall behind the liver is started from caudal.Simultaneously,liver transection is performed to search right hepatic vein(RHV) from caudal;(4) Liver transection proceeds to the bifurcation of the vessels from caudal to cranial,exposing the surfaces of IVC and RHV.Since the remnant liver sinks down,the cutting surface is well-opend;and(5) After the completion of transection,dissection of the resected liver from retroperitoneum is easily performed using the gravity.This approach was performed for a 63 years old woman with liver metastasis close to RHV.RESULTS:RHV exposure is required for R0 resection of the lesion.Although the cutting plane is horizontal in supine position and the gravity obstructs the exposure in the small subphrenic space,the use of specific characteristics of laparoscopic hepatectomy,such as the good vision for the dorsal part of the liver and IVC and facilitated dissection using the gravity with the patient positioning,made the complete RHV exposure during the liver transection easy to perform.The operation time was 341 min and operative blood loss was 1356 mL.Her postoperative hospital stay was uneventfull and she is well without any signs of recurrences 14 mo after surgery.CONCLUSION:The new procedure is feasible and useful for the patients with tumors close to RHV and the need of the exposure of RHV. | Hirokazu Tomishige Zenichi Morise Norihiko Kawabe Hidetoshi Nagata Hisanori Ohshima Jin Kawase Satoshi Arakawa Rie Yoshida Masashi Isetani | 2013 | World Journal of Gastrointestinal Surgery2013,5,6: | 7 |
| 5 | 伴放线放线杆菌菌落生长形态变化的观察显示文摘目的 观察伴放线放线杆菌 (Actinobacillusactinomycetemcomitans,Aa)从粗糙型到光滑型的转变过程 ,识别Aa在实验室传代过程中出现的不同生长形态。方法 从牙周炎患者龈下菌斑中分离出的原代菌株 8株 ,应用固体及液体培养基连续传代 ,液体培养每次传代的同时接种固体培养基观察相应的菌落形态。结果 液体培养获得 3株光滑型转变株。菌落的变化从粘附的小菌落到沉淀的大菌落到完全的均匀生长 ,转化过程大约需要 7~ 8代。在这一过程中相应传至固体培养基上生长的Aa从粘附的半透明的小菌落变大、不透明并失去粘附的特性 ,又随着边缘的扩散变为扁平 ,透明度也增加 ;与此同时内部的星形结构逐渐变简单、变小 ,最后消失。固体培养未获得典型的转变株。结论 Aa从粗糙型到光滑型的转变是一个菌落湿度逐渐增加 ,体积逐渐增大 ,并逐渐失去内部结构的过程。这一过程至少可以看到半透明突起的粗糙型、不透明突起的光滑型和近乎透明的扁平光滑型 | 王者玲 杨圣辉 李金路 Nobuko Maeda Tomoko Ohshima Ayako Takao | 2003 | 中华口腔医学杂志2003,38,1: | 3 |
| 6 | 基于G1亚型的猪流行性腹泻病毒活疫苗对日本G2亚型田间毒株的效力观察显示文摘背景:猪流行性腹泻(PED)是由猪流行性腹泻病毒(PEDV)引起的一种致死性传染病,其主要症状是哺乳仔猪水样腹泻。自20世纪90年代末以来,日本开始使用基于G1亚型的PED活疫苗,甚至在2013年新的基因亚型出现之后,G1亚型活疫苗仍在广泛使用。该研究我们评价了一种传统的PED活疫苗在试验和田间条件下对新流行的G2亚型野毒株的免疫效果。方法:两头怀孕母猪在分娩前接种了两次活疫苗。一头怀孕母猪作为阴性对照。所有新生仔猪攻毒G2亚型病毒,免疫后7 d监测临床症状。用病毒中和试验检测母猪血清和乳汁中PEDV特异性免疫应答。此外,该研究比较了接种和不接种PED活疫苗猪场的哺乳期仔猪PED发病后死亡率,以评价PED活疫苗的田间使用效果。结果:无论接种与否,受到攻击的仔猪都表现出水样腹泻。然而,来自接种疫苗的母猪所产仔猪攻毒后第4天临床评分有了明显的提高。试验结束时,免疫组仔猪存活率为80%,对照组为0。试验期间,未免疫母猪血清和乳汁中和抗体效价均为阴性,而免疫母猪的抗体效价较高。与未接种疫苗的饲养场相比,接种疫苗的饲养场在PED发病后显著降低了哺乳仔猪的死亡率。结论:传统的PED活疫苗在试验和田间条件下均能诱导母猪产生乳汁免疫,并对G2亚型病毒有部分保护作用。 | Tetsuo Sato Kazuki Oroku Yoshiyuki Ohshima 牟迪(译) 夏伟(校) | 2018 | 养猪2018,,6: | 3 |

| 10 | Skull metastasis from hepatocellular carcinoma with chronic hepatitis B显示文摘A 56-year-old male visited our hospital for evaluation of an occipital mass.Contrast computed tomography showed hypervascular enhancement with osteolytic change in the skull and a huge enhanced mass in the liver.Magnetic resonance imaging showed bone metastasis in the thoracic vertebrae.Assays for hepatitis B surface antigen and hepatitis B core antibody were positive and his liver condition was Child-Pugh grade A.Our diagnosis was hepatocellular carcinoma(HCC) with skull and vertebrae metastases on chronic hepatitis B.He was treated with radiation therapy for bone metastases and transcatheter arterial chemoembolization for HCC.But he developed acute respiratory failure because of aspiration pneumonia,congestion and oedema with haemorrhage of the lungs and died.Dissection showed HCC with multiple bone metastases.The liver tumor was categorized as well-differentiated HCC,Edmondson classification Ⅰ,trabecular type and pseudoglandular type.In the liver mild infiltration of lymphocytes was seen in Glisson's capsules which were signif icantly enlarged with well preserved limiting plates.Piecemeal necrosis was not obvious.No fibrosis was noted.An 8 cm × 7 cm × 3 cm metastatic lesion had formed in the left occipitotemporal part of the cranial bone.The lesion was osteolytic and showed invasion into the dura mater.Neither the subdural cavity nor the brain showed involvement from the metastatic tumor.However,skull metastasis from HCC is very rare and it affects the patient's prognosis and the quality of life.Therefore,it is very important to make an early diagnosis and carry out proper management of skull metastasis from HCC. | Takashi Goto Takahiro Dohmen Kouichi Miura Shigetoshi Ohshima Kazuo Yoneyama Tomomi Shibuya Ei Kataoka Daisuke Segawa Wataru Sato Yumiko Anezaki Hajime Ishii Daigo Kon Ikuhiro Yamada Kentaro Kamada Hirohide Ohnishi | 2010 | World Journal of Gastrointestinal Oncology2010,2,3: | 2 |
