1Discovery of laryngeal carcinoma by serum proteomic pattern analysis显示文摘Laryngeal carcinoma is the most common malignancy among head and neck tu-mors. The purpose of this study is to find biomarkers for laryngeal carcinoma in patient blood serum using the Surface Enhanced Laser Desorption/Ionization (SELDI) technique. Serum samples from 33 laryngeal carcinoma (12 cases of glottis, 18 of supraglottis and 3 of subglottis) patients and 31 age- and sex-matched healthy people were analyzed by SELDI-TOF on a Pro-teinChip reader, PBSII-C. Protein profiles were generated using WCX2 protein chips. Protein peak clustering and classification analyses were performed utilizing the Biomarker Wizard and Biomarker Pattern software packages, respectively. The results showed that sixteen peaks had significant difference between laryngeal cancer patients and healthy group, eight of which were up-regulated in the patient samples, and the others were down-regulated. Two protein peaks 8153 Da and 2035 Da were automatically chosen for the system training and development of a classification tree. The analysis yielded a correct percentage of 96.9% for patients and 96.7% for control. The results suggest that serum is a useful resource for the detection of specific bio-markers for laryngeal carcinoma. 5Adenoviral-mediated Hath1-EGFP gene transfer into guinea pig cochlea through intact round window membrane显示文摘Objective To study expression of adenoviral-mediated Hath1-EGFP gene in the guinea pig cochlea after transfer through intact round window membrane(RWM), and to assess its effects on hearing. Methods Twenty adult guinea pigs were used, of which: 12 were surgically inoculated with Ad-Hath1-EGFP in the bony groove of round window niche, and 8 with artificial perilymph. Auditory brainstem response(ABR) thresholds were determined in all animals before and 5 days after surgery. On post-surgery day 5 and day 14, animals were sacrificed and whole mounts of cochlea and frozen sections were examined. Results ABR tests showed no significant change of hearing after the surgery. Strong fluorescence staining in the cochleae was seen in Ad-Hath1-EGFP groups. The highest levels of gene expression were seen in the post-surgery day 5 group with little decrease on post-surgery day 14.The contralateral cochlea and those in the control groups were free of fluorescence staining. 6Hearing and Cochlear structures in a Swine Model显示文摘Objective To study the audition and inner ear structure in a normal swine model.Methods Auditory brainstem responses(ABRs) were determined in swine at 1 day and 1 month after birth.The form of the cochlea and hair cells were examined under a scanning electron microscope and on cochlear slices.Results ABR thresholds at 1 day and 1 month post-birth were between 40 and 50 dB SPL.The latencies of waves I,III and V in 1 day old swine were 1.97 ± 0.13,3.01± 0.16 and 4.26±0.20 ms,respectively.At 1 month,the lantancies of waves I,III and V were 2.01 ± 0.05,3.11±0.08 and 4.65 ± 0.14 ms,respectively,slightly longer than those at 1 day,although not statistically significant(p>0.05).The swine cochlea was constituted of 3 and a half turns and the cochlear hair cells lined up in four rows.Hair cell cilia in the apical turn were longer than those in other turns.Conclusions The swine cochlea is mature at the time of birth.Swine ABR thresholds are slightly higher than humans and rats.Swine appears to be a precocial animal species.GUO Wei-wei1,Cheng Lei2,Liu Hui-zhan1,Sun Jian-he1,YANG Shi-ming1,1 Department of Otolaryngology,Head & Neck Surgery,Institute of Otolaryngology,Chinese PLA General Hospital,Beijing 100853,China 2 State Rey laboratory for Agrobio technology China Agrical universiy Beijing 100193,PR,China 2010Journal of Otology2010,5,2:7
7Backward Propagation of Otoacoustic Emissions显示文摘Normal mammalian ears not only detect but also generate sounds. The ear-generated sounds, i.e., otoacoustic emissions (OAEs), can be measured in the external ear canal using a tiny sensitive microphone. In spite of wide applications of OAEs in diagnosis of hearing disorders and in studies of cochlear functions, the question of how the cochlea emits sounds remains unclear. The current dominating theory is that the OAE reaches the cochlear base through a backward traveling wave. However, recently published works, including experimental data on the spatial pattern of basilar membrane vibrations at the emission frequency, demonstrated only forward traveling waves and no signs of backward traveling waves. These new findings indicate that the cochlea emits sounds through cochlear fluids as compression waves rather than through the basilar membrane as backward traveling waves. 8Development of a script of phonemically balanced monosyllable lists of Mandarin-Chinese显示文摘Objectives To develop a set of monosyllable audiometry test material that can be used in speech recognition testing in Mandarin Chinese. Materials and Methods In an effort to improve reliability and efficiency in clinical practice, a set of 30 test lists of 25 monosyllable test items each was designed with consideration of the following: length of the list, phonemical balance (PB), word utility frequency, coverage of common words, and equivalency among lists. The 30 lists contained 750 monosyllabic words and were constructed on the basis of Chinese phoneme distribution probability summarized in the Manual of Acoustic. The occurrence incidence of the 22 consonants, 36 vowels, and 4 tones of the Chinese Mandarin characters in the 30 test lists were calculated. To achieve PB, 489 monosyllables were selected from the 2500 most commonly used Chinese characters and the 4000 most commonly used Chinese phrases to compile the 30 monosyllable test lists using a computer algorithm with manual adjustment. Results Thirty phonemical balanced word lists of common Mandarin Chinese monosyllabic words were compiled. Each list consisted of 25 monosyllables. A total of 489 phonemes (consonants, vowels and tones) were included in the set. 9Cochlear implantation in a child with auditory neuropathy spectrum disorder显示文摘Objective To report outcomes of cochlear implantation (CI) in a child with auditory neuropathy spectrum disorder (ANSD) and to provide preliminary clinical evidence of the efficacy of CI in ANSD patients.Methods A 4-year-old boy with diagnosed auditory neuropathy spectrum disorder (ANSD) received implantation of a Nucleus CI24R after an unsatisfactory trial of amplification.Post-implantation performance in both hearing sensitivity and speech recognition was assessed in different sessions.Aided hearing thresholds were tested by behavioral audiometry.Mandarin Early Speech Perception Test (MESP),Meaningful Auditory Integration Scale (MAIS),category of auditory performance (CAP) and Speech Intelligibility Rating (SIR) were used to assess the benefits in auditory skills or speech recognition the boy obtained from CI.The tests were administered before surgery and at 3 months and 7 months after opening.Results The boy demonstrated improved auditory sensitivity by using CI.Concerning speech recognition and communication,both speech audiometry and questionnaires showed an obvious benefit from CI.Conclusions CI has worked efficiently in this ANSD boy.But because of limited understanding of ANSD and rehabilitation effect by cochlear implantation in this condition,the clinical decision to implant should be cautious and only after a thorough evaluation.Meanwhile,well controlled and long term studies are needed to confirm the efficacy of cochlear implantation in patients with ANSD.JI Fei CHEN Ai-ting HONG Meng-di SHI Wei LI Jia-nan YANG Shi-ming Department of Otolaryngology/Head and Neck Surgery,Chinese PLA Institute of Otolaryngology,Chinese PLA General Hospital,28 Fuxing Road,Beijing 100853,China 2011Journal of Otology2011,6,2:4
10Hearing Preservation in Acoustic Neuroma Surgery显示文摘Objective To report the authors’ experiences in hearing preservation during acoustic neuroma (AN) resection procedures. Methods Two cases of AN removal via retrosigmoid approach were reviewed. Hearing preservation was attempted in the aid of endoscopic technique along with continuous monitoring of the compound action potential (CAP) and auditory brainstem response(ABR) during the surgery. Results The tumor in Case 1 was 1.5 cm in diameter. The average pure-tone hearing threshold was 30 dB HL and ABR was normal. Waves I, III and V of ABR were present following tumor removal. At 7th month follow-up, audiometric thresholds and ABR inter-peak intervals had recovered to pre-operative levels, with normal facial nerve function. The patient in Case 2 had bilateral AN. The tumors measured 4.0 cm(left) and 5.0 cm (right) on MRI scans. The AN on the right side was removed first, followed by removal of the left AN four months later. Intraoperative CAP monitoring was employed during removal of the left AN. While efforts to preserve the cochlear nerve were not successful, CAPs were still present after tumor removal. Conclusions Intraoperatively recorded CAPs are not reliable in predicting postoperative hearing outcomes. In contrast, ABRs are an indicator of function of the peripheral auditory pathway. 11Prevalence of the GJB2 Mutations in Deafness Patients of Different Ethnic Origins in Xinjiang显示文摘Objective To investigate GJB2 mutation prevalences in the Uigur and Han ethnic groups in Xinjiang, China, and determine the relationship between ethnicity and GJB2 gene mutations. Methods Information regarding ethnicity of patients' families was obtained through medical records review and/or patient interview. Blood samples were collected from 61 Uigurs and 66 Hans for direct sequencing of the coding region and intron/exon boundaries of the GBJ2 gene. Results Carrier frequency of GJB2 mutations was similar between the Uigur and Han subjects. The GJB2 35delG mutation was seen only in Uigur patients with hearing loss, whereas the 235delC mutation was identified in both Uigur and Han patients. The allelic Frequency of 35delG mutation was 7.4% (9/122) in Uigur deaf students, but none in Han deaf students (0/128) and Uigur controls (0/196). The allelic frequency of GJB2 235delC mutation in Uigur and Han deaf students was 5.7% and 9.8%, and that of 299-300delAT mutation was 0.8% and 5.5%, respectively. V27I and E114G were the most frequent types of polymorphism. Conclusion We found an Asian-specific GJB2 diversity among Uigurs, and comparable GJB2 contribution to deafness in Uigur and Han patients. The high carrier frequency of 35delG in Uigurs (11.5%) is probably defined by gene drift/founder effect in a particular group. 12甲状腺乳头状腺癌颈淋巴结清扫术显示文摘我院在1952~1997年共治疗甲状腺癌2097例,其中乳头状腺癌1732例1954~1992年间乳头状腺癌1219例,男317,女902例,男女之比为1:2.85.年龄最小6岁,最大80岁,中位年龄37.68岁,S=12.72,行颈淋巴结清除术913例(71例行双颈清除术),共984例次。通过对913例(984例次)颈淋巴结清除术的分析及随访。预防性组淋巴结阳性率为66.32%,治疗性组为93.98%,预防性组10年以上无瘤生存率远高于治疗性组。治疗性组5年、10年无瘤生存率功能性组优于传统性组(P<0.01),预防性组10、15、20年无瘤生存率功能性组优于传统性组(P<0.005),说明功能性颈淋巴结清除术即保留功能又不降低生存率,是一种值得提倡的治疗方法。另外通过109例死亡原因回顾,死于复发和转移62例(56.88%),术后10年内死亡者占83.87%(52/62),术后10年以上复发机会明显减少。转移瘤多以肺转移为主,占71.4%。徐本义 高明 张艳 张学忠 姜辉春. 72) .
