1抗氧化剂一、二级预防随机试验中的死亡率系统回顾及汇总分析显示文摘背景:抗氧化剂正用于多种疾病的预防。 目的:评估补充抗氧化剂对一、二级预防随机临床试验参试者死亡率的影响。数据来源及试验选择:检索2005年10月前的电子数据库以及发表的文献目录。所有于成人中进行的、比较单独或联合使用β胡萝卜素、维生素A、维生素C(抗坏血酸)、维生素E及硒剂的安慰剂或空白治疗对照随机试验均纳入分析。使用随机、盲法及随访作为评估入选试验偏倚的指标。采用随机效应分析法分析抗氧化剂对全因死亡率的影响,结果以相对危险度(relative risk,RR)及其95%可信区间(confidence interval,CI)表示。采用汇总回归评估试验协变量的影响。 数据提取:共入选68项随即试验,包括232606例参试者(385篇文章)。 数据整合:将所有低和高偏倚风险抗氧化剂试验汇总后发现,抗氧化剂对死亡率无显著影响(RR,1.02;95%CI,0.98~1.06)。多变量汇总回归分析表明,低偏倚风险试验(RR,1.16;95%CI,1.05~1.29)和硒剂(RR,0.998;95%CI,0.997~0.9995)与死亡率显著相关。在包括180938例参试者在内的47项低偏倚试验中,补充抗氧化剂显著增加死亡率(RR,1.05;95%CI,1.02~1.08)。排除硒剂试验后,在低偏倚风险试验中,β胡萝卜素(RR,1.07;95%CI,1.02~1,11)、维生素A(RR,1.16;95%CI,1.10~1.24)以及维生素E(RR,1.04;95%CI,1.01~1.07)单独或联合使用均显著增加死亡率。维生素C及硒剂对死亡率无显著影响。 结论:β胡萝卜素、维生素A及维生素E治疗可能增加死亡率。维生素C和硒剂对死亡率的影响需要进一步研究。Goran Bjelakovic Dimitrinka Nikolova Lise Lotte Gluud Rose G. 2Chronic Epstein-Barr virus-related hepatitis in immunocompetent patients显示文摘AIM: To investigate reactivated Epstein-Barr virus (EBV) infection as a cause for chronic hepatitis. METHODS: Patients with occasionally established elevated serum aminotransferases were studied. HIV, HBV and HCV-infections were excluded as well as any other immunosuppressive factors, metabolic or toxic disorders. EBV viral capsid antigen (VCA) IgG and IgM, EA-R and EA-D IgG and Epstein-Barr nuclear antigen (EBNA) were measured using IFA kits. Immunophenotyping of whole blood was performed by multicolor flow cytometry. CD8+ T cell responses to EBV and PHA were determined according to the intracellular expression of IFN-γ. RESULTS: The mean alanine aminotransferase (ALT) and gamma glutamyl transpeptidase (GGTP) values exceeded twice the upper normal limit, AST/ALT ratio < 1. Serology tests showed reactivated EBV infection in all patients. Absolute number and percentages of T, B and NK cells were within the reference ranges. Fine subset analysis, in comparison to EBV+ healthy carriers, revealed a significant decrease of naive T cells (P < 0.001), accompanied by increased percentage of CD45RA- (P < 0.0001), and terminally differentiated CD28-CD27- CD8+ T cells (P < 0.01). Moderately elevated numbers of CD38 molecules on CD8+ T cells (P < 0.05) proposed a low viral burden. A significantly increased percentage of CD8+ T cells expressing IFN-γ in response to EBV and PHA stimulation was registered in patients, as compared to controls (P < 0.05). 3Tissue with high intelligence quotient Adipose-derived stem cells in neural regeneration显示文摘The aim of the present review is to highlight the possible neuroregenerative potential ofadipose-derived stem cells. The key property of stem cells is plasticity including self-renewal,multilineage differentiation, and migration, whereas the required property is transplantability. For along time, embryonic stem cells were thought to be the only source of pluripotency, a dogma that hasbeen challenged during the last decade. Today, an alternative option might be adipose-derived stemcells, as easily accessible, ethical and autologous cellular source. Recent knowledge ofadipobiology increasingly recognizes that adipose tissue is the major endo- and paracrine organ ofthe human body. Likewise, numerous neuropetides, neurotrophic factors, neurotransmitters,hypothalamic and steroid hormones and their receptors are shared by adipose tissue and brain.Accordingly, the regenerative potential of neuroprotective factor-secreting adipose-derived stemcells is outlined. 4Modular architecture for fully non-blocking silicon photonic switch fabric显示文摘Integrated photonics offers the possibility of compact,low energy,bandwidth-dense interconnects for large port count spatial optical switches,facilitating flexible and energy efficient data movement in future data communications systems.To achieve widespread adoption,intimate integration with electronics has to be possible,requiring switch design using standard microelectronic foundry processes and available devices.We report on the feasibility of a switch fabric comprised of ubiquitous silicon photonic building blocks,opening the possibility to combine technologies,and materials towards a new path for switch fabric design.Rather than focus on integrating all devices on a single silicon chip die to achieve large port count optical switching,this work shifts the focus towards innovative packaging and integration schemes.In this work,we demonstrate 1×8 and 8×1 microring-based silicon photonic switch building blocks with software control,providing the feasibility of a full 8×8 architecture composed of silicon photonic building blocks.The proposed switch is fully non-blocking,has path-independent insertion loss,low crosstalk,and is straightforward to control.We further analyze this architecture and compare it with other common switching architectures for varying underlying technologies and radices,showing that the proposed architecture favorably scales to very large port counts when considering both crosstalk and architectural footprint.Separating a switch fabric into functional building blocks via multiple photonic integrated circuits offers the advantage of piece-wise manufacturing,packaging,and assembly,potentially reducing the number of optical I/O and electrical contacts on a single die.Dessislava Nikolova David M.Calhoun Yang Liu Sébastien Rumley Ari Novack Tom Baehr-Jones Michael Hochberg Keren Bergman 2017Microsystems & Nanoengineering2017,3,1:3
5Mountain Water Tower and Ecological Risk Estimation of the Mesta-Nestos Transboundary River Basin (Bulgaria-Greece)显示文摘The Mesta-Nestos river basin in Bulgaria and Greece is a case study for transboundary decision-making support in south-eastern Europe and a show-case for the development of methodologies and information-gathering for the integrated regional planning of water resources. Land-use conflicts in this water-scarce region cover a wide spectrum of activities like agricultural irrigation,drinking water production,diversions for industrial water,and risk of pollution from mining,to name a few examples. Measurements of the water quality were carried out in the upper basin. Results will be illustrated by the example of the environmental situation in the alpine region of the Pirin National Park as well as in the Razlog Basin with a stronger anthropogenic impact and pollution around a former uranium mine near the village of Ele?nica. The social and economic development of this transboundary region is a recently established priority for the future. It will mean an increase in water usage and more stress for the water resources if regional impacts of global climate change are verified.
