1Challenges in the diagnosis and treatment of gestational trophoblastic neoplasia worldwide显示文摘Gestational trophoblastic neoplasia(GTN) is a rare tumor that originates from pregnancy that includes invasive mole, choriocarcinoma(CCA), placental site trophoblastic tumor and epithelioid trophoblastic tumor(PSTT/ETT). GTN presents different degrees of proliferation, invasion and dissemination, but, if treated in reference centers, has high cure rates, even in multi-metastatic cases.The diagnosis of GTN following a hydatidiform molar pregnancy is made according to the International Federation of Gynecology and Obstetrics(FIGO)2000 criteria: four or more plateaued human chorionic gonadotropin(hCG)concentrations over three weeks; rise in hCG for three consecutive weekly measurements over at least a period of 2 weeks or more; and an elevated but falling hCG concentrations six or more months after molar evacuation. However,the latter reason for treatment is no longer used by many centers. In addition,GTN is diagnosed with a pathological diagnosis of CCA or PSTT/ETT. For staging after a molar pregnancy, FIGO recommends pelvic-transvaginal Doppler ultrasound and chest X-ray. In cases of pulmonary metastases with more than 1cm, the screening should be complemented with chest computed tomography and brain magnetic resonance image. Single agent chemotherapy, usually Methotrexate(MTX) or Actinomycin-D(Act-D), can cure about 70% of patients with FIGO/World Health Organization(WHO) prognosis risk score ≤ 6(low risk), reserving multiple agent chemotherapy, such as EMA/CO(Etoposide,MTX, Act-D, Cyclophosphamide and Oncovin) for cases with FIGO/WHO prognosis risk score ≥ 7(high risk) that is often metastatic. Best overall cure rates for low and high risk disease is close to 100% and > 95%, respectively. The management of PSTT/ETT differs and cure rates tend to be a bit lower. 2Prognostic value of increased carbohydrate antigen in patients with heart failure显示文摘AIM:To study the prognostic value of carbohydrateantigen 125(CA125) and whether it adds prognostic information to N-terminal pro-brain natriuretic peptide(NT-proBNP) in stable heart failure(HF) patients.METHODS:The predictive value of CA125 was retrospectively assessed in 156 patients with stable HF remitted to the outpatient HF unit for monitoring from 2009 to 2011.Patients were included in the study if they had a previous documented episode of HF and received HF treatment.CA125 and NT-proBNP concentrations were measured.The independent association between NT-proBNP or CA125 and mortality was assessed with Cox regression analysis,and their combined predictive ability was tested by the integrated discrimination improvement(IDI) index.RESULTS:The mean age of the 156 patients was 72 ± 12 years.During follow-up(17 ± 8 mo),27 patients died,1 received an urgent heart transplantation and 106 required hospitalization for HF.Higher CA125 values were correlated with outcomes:58 ± 85 KU/L if hospitalized vs 34 ± 61 KU/L if not(P < 0.05),and 94 ± 121 KU/L in those who died or needed urgent heart transplantation vs 45 ± 78 KU/L in survivors(P < 0.01).After adjusting for propensity scores,the highest risk was observed when both biomarkers were elevated vs not elevated(HR = 8.95,95%CI:3.11-25.73; P < 0.001) and intermediate when only NT-proBNP was elevated vs not elevated(HR = 4.15,95%CI:1.41-12.24; P < 0.01).Moreover,when CA125 was added to the clinical model with NT-proBNP,a 4%(P < 0.05) improvement in the IDI was found.CONCLUSION:CA125 > 60 KU/L identified patients in stable HF with poor survival.Circulating CA125 level adds prognostic value to NT-proBNP level in predicting HF outcomes.Ana B Méndez Jordi Ordonez-Llanos Andreu Ferrero Mariana Noguero Teresa Mir Josefina Mora Antoni Bayes-Genis Sònia Mirabet Juan Cinca Eulàlia Roig 2014World Journal of Cardiology2014,6,4:13
3Hepatitis B virus prevalence and transmission risk factors in inflammatory bowel disease patients at Clementino Fraga Filho university hospital显示文摘AIM: To evaluate the prevalence of hepatitis B virus (HBV) infection in inflammatory bowel disease (IBD) patients that followed up in our hospital and try to identify the possible risk factors involved in this infection transmission. METHODS: This was a cross-sectional study for which 176 patients were selected according to their arrival for the medical interview. All these patients had already IBD diagnosis. The patient was interviewed and a questionnaire was filled out. RESULTS: In the group of 176 patients whom we examined, we found that 17% (30) were anti-HBc positive. Out of 30 patients with positive anti-HBc, 2.3% (4) had positive HBsAg and negative HBV-DNA. In an attempt to identify the possible HBV infection transmission risk factors in IBD patients, it was observed that 117 patients had been submitted to some kind of surgical procedure, but only 24 patients had positive anti-HBc (P = 0.085). It was also observed that surgery to treat IBD complications was not a risk factor for HBV infection transmission, since we did not get a statically significant P value. However, IBDpatients that have been submitted to surgery to treat IBD complications received more blood transfusions then patients submitted to other surgical interventions (P = 0.015). 5Acute effects of physical exercise in type 2 diabetes: A review显示文摘The literature has shown the efficiency of exercise in the control of type 2 diabetes(T2D), being suggested as one of the best kinds of non-pharmacological treatments for its population. Thus, the scientific production related to this phenomenon has growing exponentially. However, despite its advances, still there is a lack of studies that have carried out a review on the acute effects of physical exercise on metabolic and hemodynamic markers and possible control mechanisms of these indicators in individuals with T2 D, not to mention that in a related way, these themes have been very little studied today. Therefore, the aim of this study was to organize and analyze the current scientific production about the acute effects of physical exercise on metabolic and hemodynamic markers and possible control mechanisms of these indicators in T2 D individuals. For such, a research with the following keywords was performed:-exercise; diabetes and post-exercise hypotension; diabetes and excess post-exercise oxygen consumption; diabetes and acute effects in PUBMED, SCIELO and HIGHWIRE databases. From the analyzed studies, it is possible to conclude that, a single exercise session can promote an increase in the bioavailability of nitric oxide and elicit decreases in postexercise blood pressure. 6Ophthalmic manifestations in patients with inflammatory bowel disease: a review显示文摘Clinical manifestations of inflammatory bowel disease(IBD) are not locally restricted to the gastrointestinal tract, and a significant portion of patients have involvement of other organs and systems. The visual system is one of the most frequently affected, mainly by inflammatory disorders such as episcleritis, uveitis and scleritis. A critical review of available literature concerning ocular involvement in IBD, as it appears in Pub Med, was performed. Episcleritis, the most common ocular extraintestinal manifestation(EIM), seems to be more associated with IBD activity when compared with other ocular EIMs. In IBD patients, anterior uveitis has an insidious onset, it is longstanding and bilateral, and not related to the intestinal disease activity. Systemic steroids or immunosuppressants may be necessary in severe ocular inflammation cases, and control of the underlying bowel disease is important to prevent recurrence. Our review revealed that ocular involvement is more prevalent in Crohn's disease than ulcerative colitis, in active IBD, mainly in the presence of other EIMs. The ophthalmic symptoms in IBD are mainly nonspecific and their relevance may not be recognized by the clinician; most ophthalmic manifestations are treatable, and resolve without sequel upon prompt treatment. 8Stem cell therapy in the management of shoulder rotator cuff disorders显示文摘Rotator cuff tears are frequent shoulder problems that are usually dealt with surgical repair. Despite improved surgical techniques, the tendon-to-bone healing rate is unsatisfactory due to difficulties in restoring the delicate transitional tissue between bone and tendon. It is essential to understand the molecular mechanisms that determine this failure. The study of the molecular environment during embryogenesis and during normal healing after injury is key in devising strategies to get a successful repair. Mesenchymal stem cells(MSC) can differentiate into different mesodermal tissues and have a strong paracrine, anti-inflammatory, immunoregulatory and angiogenic potential. Stem cell therapy is thus a potentially effective therapy to enhance rotator cuff healing. Promising results have been reported with the use of autologous MSC of different origins in animal studies: they have shown to have better healing properties, increasing the amount of fibrocartilage formation and improving the orientation of fibrocartilage fibers with less immunologic response and reduced lymphocyte infiltration. All these changes lead to an increase in biomechanical strength. However, animal research is still inconclusive and more experimental studies are needed before human application. 9Perivascular epithelioid cell tumor of the liver coexisting with a gastrointestinal stromal tumor显示文摘Approximately 10% of patients with gastrointestinal stromal tumors (GIST) develop other neoplasms, either synchronously or metachronously. In this report we describe coexistence of a gastrointestinal stromal tumor and a hepatic perivascular epithelioid cell tumor (PEComa) in a 51-year-old woman with no evidence of tuberous sclerosis. A subcapsular hepatic nodule (0.8 cm in diameter) was found during surgery for symptomatic gastric neoplasm (15 cm in diameter) arising from the lesser curvature. Both tumors revealed histomorphological and immunohistochemical features confirming a diagnosis of a small incidental hepatic PEComa and a high risky extramural gastric GIST, respectively. The patient remained disease-free 25 mo after surgery with no evidence of tumor recurrence or new neoplasms. To our knowledge, this is the first report of PEComa in a patient with GIST. 11Lattice Solid/Boltzmann Microscopic Model to Simulate Solid/Fluid Systems——A Tool to Study Creation of Fluid Flow Networks for Viable Deep Geothermal Energy显示文摘Realizing the potential of geothermal energy as a cheap,green,sustainable resource to provide for the planet's future energy demands that a key geophysical problem be solved first: how to develop and maintain a network of multiple fluid flow pathways for the time required to deplete the heat within a given region. We present the key components for micro-scale particle-based numerical modeling of hydraulic fracture,and fluid and heat flow in geothermal reservoirs. They are based on the latest developments of ESy S-Particle—the coupling of the lattice solid model(LSM) to simulate the nonlinear dynamics of complex solids with the lattice Boltzmann method(LBM) applied to the nonlinear dynamics of coupled fluid and heat flow in the complex solid-fluid system. The coupled LSM/LBM can be used to simulate development of fracture systems in discontinuous media,elastic stress release,fluid injection and the consequent slip at joint surfaces,and hydraulic fracturing; heat exchange between hot rocks and water within flow pathways created through hydraulic fracturing; and fluid flow through complex,narrow,compact and gouge- or powder-filled fracture and joint systems. 12Effects of diabetic ketoacidosis in the respiratory system显示文摘Diabetes affects approximately 30 million persons in the United States. Diabetes ketoacidosis is one of the most serious and acute complications of diabetes. At the time of presentation and during treatment of diabetic ketoacidosis(DKA), several metabolic and electrolyte derangements can ultimately result in respiratory compromise. Most commonly, hypokalemia, hypomagnesemia and hypophosphatemia can eventually lead to respiratory muscles failure.Furthermore, tachypnea, hyperpnea and more severely, Kussmaul breathing pattern can develop. Also, hydrostatic and non-hydrostatic pulmonary edema can occur secondary to volume shifts into the extracellular space and secondary to increased permeability of the pulmonary capillaries. The presence of respiratory failure in patients with DKA is associated with higher morbidity and mortality. 13A NETWORK ANALYSIS OF THE 2010 FIFA WORLD CUP CHAMPION TEAM PLAY显示文摘This paper analyzes the network of passes among the players of the Spanish team during the last FIFA World Cup 2010,where they emerged as the champion,with the objective of explaining the results obtained from the behavior at the complex network level.The team is considered a network with players as nodes and passes as(directed) edges.A temporal analysis of the resulting passes network is also done,looking at the number of passes,length of the chain of passes,and to network measures such as player centrality and clustering coefficient.Results of the last three matches(the decisive ones) indicate that the clustering coefficient of the pass network remains high,indicating the elaborate style of the Spanish team.The effectiveness of the opposing team in negating the Spanish game is reflected in the change of several network measures over time,most importantly in drops of the clustering coefficient and passing length/speed,as well as in their being able in removing the most talented players from the central positions of the network.Spain's ability to restore their combinative game and move the focus of the game to offensive positions and talented players is shown to tilt the balance in favor of the Spanish team.COTTA Carlos MORA Antonio M MERELO Juan Julin MERELO-MOLINA Cecilia 2013Journal of Systems Science & Complexity2013,26,1:4
14Complex interactomes and post-translational modifications of the regulatory proteins HABP4 and SERBP1 suggest pleiotropic cellular functions显示文摘The 57 kDa antigen recognized by the Ki-1 antibody,is also known as intracellular hyaluronic acid binding protein 4 and shares 40.7%identity and 67.4%similarity with serpin mRNA binding protein 1,which is also named CGI-55,or plasminogen activator inhibitor type-1-RNA binding protein-1,indicating that they might be paralog proteins,possibly with similar or redundant functions in human cells.Through the identification of their protein interactomes,both regulatory proteins have been functionally implicated in transcriptional regulation,mRNA metabolism,specifically RNA splicing,the regulation of mRNA stability,especially,in the context of the progesterone hormone response,and the DNA damage response.Both proteins also show a complex pattern of post-translational modifications,involving Ser/Thr phosphorylation,mainly through protein kinase C,arginine methylation and SUMOylation,suggesting that their functions and locations are highly regulated.Furthermore,they show a highly dynamic cellular localization pattern with localizations in both the cytoplasm and nucleus as well as punctuated localizations in both granular cytoplasmic protein bodies,upon stress,and nuclear splicing speckles.Several reports in the literature show altered expressions of both regulatory proteins in a series of cancers as well as mutations in their genes that may contribute to tumorigenesis.This review highlights important aspects of the structure,interactome,post-translational modifications,sub-cellular localization and function of both regulatory proteins and further discusses their possible functions and their potential as tumor markers in different cancer settings.Carolina Colleti Talita Diniz Melo-Hanchuk Flavia Regina Moraes da Silva Angela Saito Jorg Kobarg 2019World Journal of Biological Chemistry2019,10,3:4
15加卸载响应比(Load/Unload Response Ratio)、能量加速释放(AE/MR)的临界区尺度及地震预测显示文摘加卸载响应比理论的主要思路是 :系统在稳定状态时加载响应与卸载响应的比值与非稳定状态时加载响应与卸载响应的比值是完全不同的。大震前加卸载响应比升高和能量加速释放这两种现象可以用来对地震进行中期预报。同时 ,加卸载响应比理论和能量加速释放可能有相同的物理机制。为了验证这种地震预报方法的可行性 ,我们研究了几例发生在澳大利亚与中国 ,M 5 0~ 7 9之间的地震 ,其中包括破坏严重的澳大利亚纽卡斯尔地震和中国的唐山地震。我们利用以震源中心一定范围内的数据计算了震前的加卸载响应比和能量加速释放的幂律拟合。能量幂律加速释放存在一组最佳的拟合 ,一定范围内加卸载响应比达最大值表明加卸载响应比也有一个临界区尺度。进一步讲 ,加卸载响应比与能量加速释放的临界区尺度是相似的。这些结果表明加卸载响应比与能量加速释放有相同的物理机制。进一步的研究可能会对这种物理机制提供更好的解释 。尹祥础 Peter Mora 彭克银 王裕仓 Dion Weatherley 张晖辉(翻译) 2004国际地震动态2004,,5:4

17Rice Drying,Storage and Processing:Effects of Post-Harvest Operations on Grain Quality显示文摘Various post-harvest processes of rice are commonly employed,especially during the off-season,to ensure its consumption feasibility,which often affect the grain quality.Different forms of drying,storage and processing of rice are evaluated to identify their effects on grain quality.Microwave drying has emerged as an alternative to the widely-used intermittent-drying and fixed-bed-dryer methods of drying paddy rice.Control of drying-air temperatures(between 40℃ and 60℃)according to the rice variety can improve quality,especially for exotic varieties.Keeping stored grain in hygroscopic balance,with water content between 11%to 15%,at temperatures between 16℃ and 20℃ and with intergranular relative humidity near 60%,allows 12 months of storage in a controlled environment without significant deterioration.Other innovations,notably the application of artificial refrigeration to grain stored in bulk in vertical cylindrical silos and the use of impermeable packaging for storage,ensure the conservation of grain mass.The different stages and equipments used to obtain polished,brown and parboiled rice result in significant changes in the nutritional value of rice because of the removal of the outermost layers of the grains.Polishing reduces the nutritional value and physical homogeneity of rice.Brown rice retains more bioactive compounds and nutrients because it does not lose the outer layer of the grains in the polishing processes.Parboiled rice,although less nutritious than brown rice,has better grain integrity and milling yield and less loss of nutrients than white rice.Amanda MÜLLER Marcela Trojahn NUNES Vanessa MALDANER Paulo Carteri CORADI Rosana Santos de MORAES Samuel MARTENS Andressa Fernandes LEAL Vladison Fogliato PEREIRA Cristielle König MARIN 2022Rice science2022,29,1:3
