| 2 | Interventional treatment for unresectable hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC) is the sixth most common cancer and third leading cause of cancer-related death in the world. The Barcelona clinic liver cancer classification is the current standard classification system for the clinical management of patients with HCC and suggests that patients with intermediate-stage HCC benefit from transcatheter arterial chemoembolization(TACE). Interventional treatments such as TACE, balloon-occluded TACE, drug-eluting bead embolization, radioembolization, and combined therapies including TACE and radiofrequency ablation, continue to evolve, resulting in improved patient prognosis. However, patients with advanced-stage HCC typically receive only chemotherapy with sorafenib, a multi-kinase inhibitor, or palliative and conservative therapy. Most patients receive palliative or conservative therapy only, and approximately 50% of patients with HCC are candidatesfor systemic therapy. However, these patients require therapy that is more effective than sorafenib or conservative treatment. Several researchers try to perform more effective therapies, such as combined therapies(TACE with radiotherapy and sorafenib with TACE), modified TACE for HCC with arterioportal or arteriohepatic vein shunts, TACE based on hepatic hemodynamics, and isolated hepatic perfusion. | 5 | Microcosmic analysis of ductile shearing zones of coal seams of brittle deformation domain in superficial lithosphere显示文摘The ductile shearing zones of coal seams in a brittle deformation domain in super-ficial lithosphere are put forward based on the study on bedding shearing and ductile rheology of coal seams. The macrocosmic and microcosmic characteristics include wrinkle fold, mymonitized zones and ductile planar structure of coal seams, etc., while the microcosmic characteristics may also include different optic-axis fabrics and the anisotropy of vitrinite reflectance as well as the change of chemical structure and organic geochemistry components. The forming mechanism is analyzed and the strain environment of ductile shearing zones of coal seams discussed. The result indicates that, in the superficial brittle deformation domain, the coal seams are easy to be deformed, resulting in not only brittle deformation but also ductile shearing deformation under the action of force. Because of simple shearing stress, the interlayer gliding or ductile rheology may take place between coal seams and wall rocks. Therefore, many ductile shearing zones come into being in superficial lithosphere (<5 km). | 6 | Edge Effect Correction in the S-A Method for Geochemical Anomaly Separation显示文摘Anomaly separation using geochemical data often involves operations in the frequency domain, such as filtering and reducing noise/signal ratios. Unfortunately, the abrupt edge truncation of an image along edges and holes (with missing data) often causes frequency distribution distortion in the frequency domain. For example, bright strips are commonly seen in frequency distribution when using a Fourier transform. Such edge effect distortion may affect information extraction results; sometimes severely, depending on the edge abruptness of the image. Traditionally, edge effects are reduced by smoothing the image boundary prior to applying a Fourier transform. Zero-padding is one of the most commonly used smoothing methods. This simple method can reduce the edge effect to some degree but still distorts the image in some cases. Moreover, due to the complexity of geoscience images, which can include irregular shapes and holes with missing data, zero-padding does not always give satisfactory results. This paper proposes the use of decay functions to handle edge effects when extracting information from geoscience images. As an application, this method has been used in a newly developed multifractal method (S-A) for separating geochemical anomalies from background patterns. | 7 | Geochemistry and mineralogy of the Late Permian coals from the Songzao Coalfield, Chongqing, southwestern China显示文摘Mineralogy and geochemistry of the four main workable coal seams (No.6, No.7, No.8, and No.11) of Late Permian age from the Songzao Coalfield, Chongqing, Southwest China, were examined using in- ductively coupled plasma-mass spectrometry (ICP-MS), X-ray fluorescence spectrometry (XRF), cold-vapor absorption spectrometry (CV-AAS), ion-selective electrode (ISE), scanning electron mi- croscopy equipped with an energy-dispersive X-ray spectrometer (SEM-EDX), and X-ray diffraction analysis (XRD). The results showed that the main workable No.8 Coal that accounts for about 60% of the total coal reserves in the Songzao Coalfield was not enriched in hazardous trace elements. The No.11 Coal has high concentrations of alkaline elements, Be (9.14 μg/g), Sc (12.9 μg/g), Ti (9508 μg/g), Mn (397 μg/g), Co (23.7 μg/g), Cu (108 μg/g), Zn (123 μg/g), Ga (32 μg/g), Zr (1304 μg/g), Nb (169 μg/g), Hf (32.7 μg/g), Ta (11.4 μg/g), W (24.8 μg/g), Hg (0.28 μg/g), Pb (28.1 μg/g), Th (24.1 μg/g), and rare earth elements (509.62 μg/g). The concentration of Nb and Ta in the No. 11 Coal is higher than the industrial grade, and their potential utilization should be further studied. Besides pyrite, quartz, calcite, and clay minerals, trace minerals including chalcopyrite, marcasite, siderite, albite, mixed-layer clay minerals of illite and smectite, monazite, apatite, anatase, chlorite, and gypsum were found in the No.11 Coal. It should be noted that alabandite of hydrothermal origin and anatase occurring as cement were identi- fied in coal. In addition, the clayey microbands derived from alkaline volcanic ashes were identified in the coal. The dominant compositions of these clayey microbands were mixed-layer clay minerals of illite and smectite, which were interlayered with organic bands. The modes of occurrence of alkaline volcanic ash bands indicate that the volcanic activities were characterized by the multiple eruptions, short time interval and small scale for each eruption during peat accumulation. | 9 | High precision slope deformation monitoring model based on the GPS/Pseudolites technology in open-pit mine显示文摘As the number and geometric intensity of visual satellites are susceptible to large slopes in open-pit mines, we propose integration of GPS/Pseudolites (PLs) positioning technology which can increase the number of visible satellites, strengthen the geometric intensity of satellites and provide a precision solution for slope deformation monitoring. However, the un-modeled systematic errors are still the main limiting factors for high precision baseline solution. In order to eliminate the un-modeled systematic error, the Empirical Mode Decomposition (EMD) theory is employed. The multi-scale decomposition and reconstruction architecture are defined here on the basis of the EMD theory and the systematic error mitigation model is demonstrated as well. A standard of the scale selection for the systematic error elimination is given in terms of the mean of the accumulated standardized modes. Thereafter, the scheme of the GPS/PLs baseline solution based on the EMD is suggested. The simulation and experiment results show that the precision factors (DOP) are reduced greatly when PLs is located suitably. | 10 | Randomized controlled trial of pancreatic stenting to prevent pancreatitis after endoscopic retrograde cholangiopancreatography显示文摘AIM:To determine the effectiveness of pancreatic duct(PD) stent placement for the prevention of pancreatitis after endoscopic retrograde cholangiopancreatography(ERCP) in high risk patients.METHODS:Authors conducted a single-blind,randomized controlled trial to evaluate the effectiveness of a pancreatic spontaneous dislodgement stent against post-ERCP pancreatitis,including rates of spontaneous dislodgement and complications.Authors defined high risk patients as having any of the following:sphincter of Oddi dysfunction,difficult cannulation,prior history of post-ERCP pancreatitis,pre-cut sphincterotomy,pancreatic ductal biopsy,pancreatic sphincterotomy,intraductal ultrasonography,or a procedure time of more than 30 min.Patients were randomized to a stent group(n = 60) or to a non-stent group(n = 60).An abdominal radiograph was obtained daily to assessspontaneous stent dislodgement.Post-ERCP pancreatitis was diagnosed according to consensus criteria.RESULTS:The mean age(± standard deviation) was 67.4 ± 13.8 years and the male:female ratio was 68:52.In the stent group,the mean age was 66 ± 13 years and the male:female ratio was 33:27,and in the non-stent group,the mean age was 68 ± 14 years and the male:female ratio was 35:25.There were no significant differences between groups with respect to age,gender,final diagnosis,or type of endoscopic intervention.The frequency of post-ERCP pancreatitis in PD stent and non-stent groups was 1.7%(1/60) and 13.3%(8/60),respectively.The severity of pancreatitis was mild in all cases.The frequency of post-ERCP pancreatitis in the stent group was significantly lower than in the non-stent group(P = 0.032,Fisher's exact test).The rate of hyperamylasemia were 30%(18/60) and 38.3%(23 of 60) in the stent and non-stent groups,respectively(P = 0.05,χ2 test).The placement of a PD stent was successful in all 60 patients.The rate of spontaneous dislodgement by the third day was 96.7%(58/60),and the median(range) time to dislodgement was 2.1(2-3) d.The rates of stent migration,hemorrhage,perforation,infection(cholangitis or cholecystitis) or other complicationss were 0%(0/60),0%(0/60),0%(0/60),0%(0/60),0%(0/60),respectively,in the stent group.Univariate analysis revealed no significant differences in high risk factors between the two groups.The pancreatic spontaneous dislodgement stent safely prevented post-ERCP pancreatitis in high risk patients.CONCLUSION:Pancreatic stent placement is a safe and effective technique to prevent post-ERCP pancreatitis.Therefore authors recommend pancreatic stent placement after ERCP in high risk patients. | Yoshiaki Kawaguchi Masami Ogawa Fumio Omata Hiroyuki Ito Tooru Shimosegawa Tetsuya Mine | 2012 | World Journal of Gastroenterology2012,18,14: | 25 |
| 11 | Deformation mechanism and excavation process of large span intersection within deep soft rock roadway显示文摘The FLAC3D software was used to simulate and analyze numerically the displacement, stress and plastic zone distribu-tion characteristics of a large span intersection in a deep soft rock roadway after the surrounding rock was excavated. Our simula-tion results show that there are two kinds of dominating factors affecting roadway stability at points of intersection, one is the angle between horizontal stress and axial direction of the roadway and the other are the angles at the points of intersection. These results are based on a study we carried out as follows: first, we analyzed the failure mechanism of a large span intersection and then we built a mechanical model of a rock pillar at one of the points of intersection. At the end of this analysis, we obtained the failure characteristics of the critical parts on the large span intersection. Given these failure characteristics, we proposed a new supporting method, i.e., a Double-Bolt Control Technology (DBCT). | 12 | On three-dimensional geological modeling and visualization显示文摘The technology of 3D geological modeling will bring about great changes in the methods of geological data acquiring, storing, processing and displaying. However, no perfect or convenient software systems have been developed up to now since the geologic data which re-flect geological entities bear the feature of diversity, uncertainty and complexity. Some supervoxel models, mathematical models of fault and geometrical models of fold have been contrived so as to show the space geometric configuration of the complicated geologic structures. The application-oriented system architecture for 3D geological modeling is established; and a novel design concept based on spatial data processing is also proposed with the technology of solid modeling as its core and the application of models as its objective. Theories and methods for 3D geological modeling will hopefully be enriched and developed. In the light of these theories and methods, a feature-based navigation visualization technique is also put forward in the paper. | 13 | End-Permian catastrophic event of marine acidification by hydrated sulfuric acid:Mineralogical evidence from Meishan Section of South China显示文摘The event Permian-Triassic boundary (EPTB) is well marked by the famous 'white clay' of bed 25 in Mei-shan Section located in Changxing county, Zhejiang province of China. In this note, the white clay as well as its overlying and underlying sequences is investigated particularly for mineralogical records. The investigation yields three findings that contribute to better understanding the scenario of the EPTB mass extinction. (i) A red goethite-rich microlayer (0.3 mm) is first recognized to be horizontally widespread on the base of the white clay in the section. The microlayer should be considered as a macro geochemical indicator naturally tracing a catastrophic initiation at the EPTB. (ii) An interruption of marine carbonate deposition is discovered due to blank of carbonate minerals in the white clay. | 14 | A statistical model for porous structure of rocks显示文摘The geometric features and the distribution properties of pores in rocks were in- vestigated by means of CT scanning tests of sandstones. The centroidal coordi- nates of pores, the statistic characterristics of pore distance, quantity, size and their probability density functions were formulated in this paper. The Monte Carlo method and the random number generating algorithm were employed to generate two series of random numbers with the desired statistic characteristics and prob- ability density functions upon which the random distribution of pore position, dis- tance and quantity were determined. A three-dimensional porous structural model of sandstone was constructed based on the FLAC3D program and the information of the pore position and distribution that the series of random numbers defined. On the basis of modelling, the Brazil split tests of rock discs were carried out to ex- amine the stress distribution, the pattern of element failure and the inosculation of failed elements. The simulation indicated that the proposed model was consistent with the realistic porous structure of rock in terms of their statistic properties of pores and geometric similarity. | 15 | Removal of diminutive colorectal polyps: A prospective randomized clinical trial between cold snare polypectomy and hot forceps biopsy显示文摘AIM To compare the efficacy and safety of cold snare polypectomy(CSP) and hot forceps biopsy(HFB) for diminutive colorectal polyps.METHODS This prospective, randomized single-center clinical trial included consecutive patients ≥ 20 years of age with diminutive colorectal polyps 3-5 mm from December 2014 to October 2015. The primary outcome measures were en-bloc resection(endoscopic evaluation) and complete resection rates(pathological evaluation). The secondary outcome measures were the immediate bleeding or immediate perforation rate after polypectomy, delayed bleeding or delayed perforation rate after polypectomy, use of clipping for bleeding or perforation, and polyp retrieval rate. Prophylactic clipping after polyp removal wasn't routinely performed.RESULTS Two hundred eight patients were randomized into the CSP(102), HFB(106) and 283 polyps were evaluated(CSP: 148, HFB: 135). The en-bloc resection rate was significantly higher with CSP than with HFB [99.3%(147/148) vs 80.0%(108/135), P < 0.0001]. The complete resection rate was significantly higher with CSP than with HFB [80.4%(119/148) vs 47.4%(64/135), P < 0.0001]. The immediate bleeding rate was similar between the groups [8.6%(13/148) vs 8.1%(11/135), P = 1.000], and endoscopic hemostasis with hemoclips was successful in all cases. No cases of perforation or delayed bleeding occurred. The rate of severe tissue injury to the pathological specimen was higher HFB than CSP [52.6%(71/135) vs 1.3%(2/148), P < 0.0001]. | 17 | ^(13)C NMR spectra of tectonic coals and the effects of stress on structural components显示文摘High-resolution 13C Nuclear Magnetic Resonance (NMR) spectra of different kinds of tectonic coals were obtained using the NMR (CP/MAS+TOSS) method. On the basis of this, after simulation synthesis and division of spectra, the relative contents of carbon functional groups were calculated. Combined with results of Ro, max, XRD testing and element analysis, stress effects on the composition of macromolecular structures in tectonic coals were studied further. The results showed that Ro, max was not only the important index for describing coal rank, but was also effective for estimating the stress effect of tectonic coals. Under tectonic stress ac- tion, Ro, max was the most direct indicator of the coal structure and chemical components. Changes in the stacking Lc of the coal basic structure unit (BSU) and La/Lc parameters could distinguish the temperature and stress effects on metamorphic-deformed environments, and re- flected the degree of structural deformation. Therefore, on the whole, Lc and La/Lc can be used to index of the degree of structural deformation of tectonic coals. In different metamorphic and de- formed environments, different kinds of tectonic coals are formed under structural stress. The changes in characteristics of the macromolecular structure and chemical composition are such that as the increase in structural deformation becomes stronger, from the brittle deformation coal to ductile deformation coal, the ratio of width at the half height of the aromatic carbon and ali- phatic carbon peaks (Hfa/Hfal ) was increased. As carbon aromaticity was raised further, carbon aliphaticity reduced obviously and different compositions of macromolecular structure appeared as a jump and wave pattern except for in wrinkle structure coal, which might result chiefly from stress effects on the macromolecular structure of different kinds of tectonic coals. The macro- moecular changes of wrinkle structure coal are reflected mainly on physical structure. In the metamorphic and deformed environments of the middle and high rank coals, different kinds of tectonic coals, when compared with the metamorphic and deformed environments of low rank coal, have a greater change in Hfa/Hfal, carbon aromaticity, carbon aliphaticity and a different composition of macromolecular structure. | 19 | Characteristics of acoustic emission signals in damp cracking coal rocks显示文摘A uniaxial load experiment on coal rocks at different stress rates was carried out, based on the characteristics of acoustic emission (AE) signals in cracking coal rocks, decomposition, de-noising and reconstruction for the AE signals through wavelet packet transform for solving the current problems created by the presence of noise in AE signals and the existing problems in AE signal processing. The results show that the various characteristics of AE signals in coal rocks cracking under different situations can be clearly reflected, after the AE signals are de-noised by the wavelet packet. Compared to dry coal rocks, the number of AE occurrences in damp coal rocks was significantly reduced, as well as the average amplitude. The number of AE occurrences in damp and dry coal rocks clearly increased with increases in the loading rate, but the largest amplitude of the AE signals in damp coal rocks has been reduced. | 20 | Intrahepatic biliary cystic neoplasms:Surgical results of 9 patients and literature review显示文摘AIM: To investigate the eligible management of the cystic neplasms of the liver. METHODS: The charts of 9 patients who underwent surgery for intrahepatic biliary cystic liver neoplasms between 2003 and 2008 were reviewed retrospectively. Informed consent was obtained from the patients and approval was obtained from the designated review board of the institution. RESULTS: All patients were female with a median (range) age of 49 (27-60 years). The most frequent symptom was abdominal pain in 6 of the patients. Four patients had undergone previous laparotomy (with other diagnoses) which resulted in incomplete surgery or recurrences. Liver resection (n = 6) or enucleation (n = 3) was performed. The final diagnosis was intrahepaticbiliary cystadenoma in 8 patients and cystadenocarcinoma in 1 patient. All symptoms resolved after surgery. There has been no recurrence during a median (range) 31 (7-72) mo of follow up. CONCLUSION: In spite of the improvement in imaging modalities and increasing recognition of biliary cystadenoma and cystadenocarcinoma, accurate preoperative diagnosis may be difficult.
