1Long-term outcomes of hepatocellular carcinoma that underwent chemoembolization for bridging or downstaging显示文摘BACKGROUND Prospective study of 200 patients with hepatocellular carcinoma(HCC)that underwent liver transplant(LT)after drug-eluting beads transarterial chemoembolization(DEB-TACE)for downstaging versus bridging.Overall survival and tumor recurrence rates were calculated,eligibility for LT,time on the waiting list and radiological response were compared.After TACE,only patients within Milan Criteria(MC)were transplanted.More patients underwent LT in bridging group.Five-year post-transplant overall survival,recurrence-free survival has no difference between the groups.Complete response was observed more frequently in bridging group.Patients in DS group can achieve posttransplant survival and HCC recurrence-free probability,at five years,just like patients within MC in patients undergoing DEB-TACE.AIM To determine long-term outcomes of patients with HCC that underwent LT after DEB-TACE for downstaging vs bridging.METHODS Prospective cohort study of 200 patients included from April 2011 through June 2014.Bridging group included patients within MC.Downstaging group(out of MC)was divided in 5 subgroups(G1 to G5).Total tumor diameter was≤8 cm for G1,2,3,4(n=42)and was>8 cm for G5(n=22).Downstaging(n=64)and bridging(n=136)populations were not significantly different.Overall survival and tumor recurrence rates were calculated by the Kaplan-Meier method.Additionally,eligibility for LT,time on the waiting list until LT and radiological response were compared.RESULTS After TACE,only patients within MC were transplanted.More patients underwent LT in bridging group 65.9%(P=0.001).Downstaging population presented:higher number of nodules 2.81(P=0.001);larger total tumor diameter 8.09(P=0.001);multifocal HCC 78%(P=0.001);more post-transplantation recurrence 25%(P=0.02).Patients with maximal tumor diameter up to 7.05 cm were more likely to receive LT(P=0.005).Median time on the waiting list was significantly longer in downstaging group 10.6 mo(P=0.028).Five-year posttransplant overall survival was 73.5%in downstaging and 72.3%bridging groups(P=0.31),and recurrence-free survival was 62.1%in downstaging and 74.8%bridging groups(P=0.93).Radiological response:complete response was observed more frequently in bridging group(P=0.004).CONCLUSION Tumors initially exceeding the MC down-staged after DEB-TACE,can achieve post-transplant survival and HCC recurrence-free probability,at five years,just like patients within MC in patients undergoing DEB-TACE.Breno Boueri Affonso Francisco Leonardo Galastri Joaquim Mauricio da Motta Leal Filho Felipe Nasser Priscila Mina Falsarella Rafael Noronha Cavalcante Marcio Dias de Almeida Guilherme Eduardo Goncalves Felga Leonardo Guedes Moreira Valle Nelson Wolosker 2019World Journal of Gastroenterology2019,25,37:21

3L^p-decay rates to nonlinear diffusion waves for p-system with nonlinear damping显示文摘In this paper, we study the Lp (2 ≤ p ≤ +∞) convergence rates of the solutions to the Cauchy problem of the so-called p-system with nonlinear damping. 4Efficacy of combined cataract extraction and endoscopic cyclophotocoagulation for the reduction of intraocular pressure and medication burden显示文摘AIM: To report on the efficacy of combined endoscopic cyclophotocoagulation(ECP) and phacoemulsification cataract extraction(PCE) with intraocular lens placement for reduction of intraocular pressure(IOP) and medication burden in glaucoma.·METHODS: A retrospective case review of 91 eyes(73patients) with glaucoma and cataract that underwent combined PCE/ECP surgery was performed. Baseline demographic and ocular characteristics were recorded,as well as intraocular pressure, number of glaucoma medications, and visual acuity postoperatively with 12-month follow-up. Treatment failure was defined as less than 20% reduction in IOP from baseline on two consecutive visits(at 1, 3, 6, or 12 mo postoperatively),IOP ≥21 mm Hg or ≤5 mm Hg on two consecutive visits, or additional glaucoma surgery performed within12 mo after PCE/ECP.·RESULTS: Overall, mean medicated IOP was reduced from 16.65 mm Hg at baseline to 13.38 mm Hg at 12mo(P <0.0001). Mean number of glaucoma medications was reduced from 1.88 medications at baseline to 1.48 medications at 12mo(P =0.0003). At 3mo postoperatively,the success rate was 73.6%(95%CI: 63.3, 81.5), 57.1% at6mo(95% CI: 46.3, 66.6), and 49.7% at 12mo(95% CI:38.9, 59.6). Patient demographic characteristics were not associated with treatment success. The only ocular characteristic associated with treatment success was a higher baseline IOP.· CONCLUSION: Combined PCE/ECP surgery is an effective surgical option for the reduction of IOP and medication burden in glaucoma patients. 5Diabetes and cancer: Epidemiological and biological links显示文摘The incidence of diabetes and cancer has increased significantly in recent years.Furthermore,there are many common risk factors for both diabetes and cancer,such as obesity,sedentary lifestyle,smoking,and ageing.A large body of epidemiological evidence has indicated that diabetes is considered as an independent risk factor for increased rates of heterogeneous types of cancer occurrence and death.The incidence and mortality of various types of cancer,such as pancreas,liver,colorectal,breast,endometrial,and bladder cancers,have a modest growth in diabetics.However,diabetes may work as a protective factor for prostate cancer.Although the underlying biological mechanisms have not been totally understood,studies have validated that insulin/insulin-like growth factor(IGF)axis(including insulin resistance,hyperinsulinemia,and IGF),hyperglycemia,inflammatory cytokines,and sex hormones provide good circumstances for cancer cell proliferation and metastasis.Insulin/IGF axis activates several metabolic and mitogenic signaling pathways;hyperglycemia provides energy for cancer cell growth;inflammatory cytokines influence cancer cell apoptosis.Thus,these three factors affect all types of cancer,while sex hormones only play important roles in breast cancer,endometrial cancer,and prostate cancer.This minireview consolidates and discusses the epidemiological and biological links between diabetes and various types of cancer.Mina Wang Yingying Yang Zehuan Liao 2020World Journal of Diabetes2020,11,6:10
6Autoimmune pancreatitis versus pancreaticcancer: a comprehensive review withemphasis on differential diagnosis显示文摘BACKGROUND: Autoimmune pancreatitis (AIP) is a rare form of chronic pancreatitis with a discrete pathophysiology occasional diagnostic radiological findings, and characteristic histological features. Its etiology and pathogenesis are still under investigation, especially during the last decade Another aspect of interest is the attempt to establish specific criteria for the differential diagnosis between autoimmune pancreatitis and pancreatic cancer, entities that are frequently indistinguishable. DATA SOURCES: An extensive search of the PubMed database was performed with emphasis on articles about the differential diagnosis between autoimmune pancreatitis and pancreatic cancer up to the present. RESULTS: The most interesting outcome of recent research is the theory that autoimmune pancreatitis and its various extra-pancreatic manifestations represent a systemic fibro inflammatory process called IgG4-related systemic disease The diagnostic criteria proposed by the Japanese Pancreatic Society, the more expanded HISORt criteria, the new definitions of histological types, and the new guidelines of the International Association of Pancreatology help to establish the diagnosis of the disease types. 7Prevalence of occult hepatitis B virus infection in haemodialysis patients from central Greece显示文摘AIM:To assess the hepatitis B virus(HBV)-DNA and the prevalence of occult HBV infection in end-stage renal failure(ESRF)patients from Central Greece. METHODS:Sera from 366 ESRF patients attending five out of six dialysis units from Central Greece were investigated for HBV-DNA by real-time polymerase chain reaction.Only serum samples with repeatedly detectable HBV-DNA were considered positive.IgG antibodies to hepatitis C virus(anti-HCV)were tested by a third generation enzyme linked immunosorbent assay(ELISA),while IgG antibodies to hepatitis E virus (anti-HEV)were tested by two commercially available ELISAs.RESULTS:HBV-DNA was detected in 15/366 patient (4.1%)and HBsAg in 20/366(5.5%).The prevalenc of occult HBV infection was 0.9%(3/346 HBsAg negative patients).Occult HBV was not associate with a specific marker of HBV infection or anti-HCV o anti-HEV reactivity.There was no significant differenc in HBV-DNA titres,demographic and biochemica features,between patients with occult HBV infectio and those with HBsAg-positive chronic HBV infection. 8Refractive outcomes among glaucoma patients undergoing phacoemulsification cataract extraction with and without Kahook Dual Blade goniotomy显示文摘Background:Glaucoma patients undergoing phacoemulsification alone have a higher rate of refractive surprise compared to patients without glaucoma.This risk is further increased with combined filtering procedures.Indeed,there are few and conflicting reports on the effect of combined phacoemulsification and micro-invasive glaucoma surgery(MIGS).Here,we look at refractive outcomes of glaucoma patients undergoing phacoemulsification with and without Kahook Dual Blade(KDB)goniotomy.Methods:Retrospective chart review of 385 glaucomatous eyes of 281 patients,which underwent either phacoemulsification alone(n=309)or phacoemulsification with KDB goniotomy(n=76,phaco-KDB)at the University of Colorado.The main outcome was refractive surprise defined as the difference in target and postoperative refraction spherical equivalent greater than±0.5 Diopter(D).Results:Refractive surprise greater than±0.5 D occurred in 26.3%of eyes in the phaco-KDB group and 36.2%in the phacoemulsification group(p=0.11).Refractive surprise greater than±1.0 D occurred in 6.6%for the phacoKDB group and 9.7%for the phacoemulsification group(p=0.08).There was no significant difference in risk of refractive surprise when pre-operative IOP,axial length,keratometry or performance of KDB goniotomy were assessed in univariate analyses.Conclusion:There was no difference between refractive outcomes of glaucomatous patients undergoing phacoemulsification with or without KDB goniotomy.Erin G.Sieck Cara E.Capitena Young Rebecca S.Epstein Jeffrey R.SooHoo Mina B.Pantcheva Jennifer L.Patnaik Anne M.Lynch Malik Y.Kahook Leonard K.Seibold 2019Eye and Vision2019,6,1:7

10Prognostic value of cortisol and thyroid function tests in poisoned patients admitted to toxicology ICU显示文摘BACKGROUND: Prognostic value of cortisol and thyroid function tests(TFTs) has previously been evaluated in medical ICUs. We aimed to evaluate prognostic efficacy of cortisol and TFTs in critically ill poisoned patients admitted to toxicology intensive care unit(ICU).METHODS: In a prospective study of consecutively enrolled subjects admitted to the toxicology ICU, lab analyses included TFTs(total T3 and T4 as well as TSH) and cortisol levels drawn between 8 am–10 am during period of the first 24 hours post-ingestion/exposure. Simplified Acute Physiology Score Ⅱ(SAPS Ⅱ) and Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) were recorded. All scores were compared to detect the best prognostic factor. Type of poisoning was also included.RESULTS: In 200 patients evaluated, 129 were male and mean age was 31 years. In general, SAPS Ⅱ, T4, and cortisol could prognosticate death. After regression analysis, only cortisol had such efficacy(P=0.04; OR=1.06; 95%CI=1.05–1.08; cut-off=42 μg/d L; sensitivity=70%; specificity=82%). Between aluminium phosphide(ALP)-and non ALP-poisoned patients, level of consciousness, mean arterial pressure, and cortisol level could prognosticate death in ALP poisoning(all Ps<0.001 in both uni and multivariate analyses). Median(interquartile range; IQR) GCS was 7(6, 10) and 15(8, 15) in non-ALP and ALP-poisoned patients(P<0.003). SAPS Ⅱ and APACHE Ⅱ could not prognosticate death at all.CONCLUSION: Cortisol best prognosticated outcomes for subjects admitted to the toxicology ICU. 11A nonlinear rheological model of backfill material for retaining roadways and the analysis of its stability显示文摘The long-term stability of backfill material is the key to retaining roadways successfully.In order to study the rheological deformation of backfill material and its long-term stability,given the visco-elastoplastic properties of this material,we introduced a softening and a hardening function for a new nonlinear rheological model with time-varying parameters.Based on this,we presented the instability condition of this model by using the principle of minimum potential energy.Combined with engineering practice,we calculated the unstable time period of backfill material.The results show that the time of instability of the backfill material relate to the initial parameters of the material,the coefficients decided by temperature and the ratio of the plastic zone of the backfill material.Based on the results of our analysis from the point of view of energy,we can quickly obtain the time of instability of this model from our graphical analysis.The time of instability of the backfill material obtained from our investigation coincides with an actual project.Zhang Tianjun Ma Mina Wang Hongsheng Xu Hongjie 2011Mining Science and Technology2011,21,4:5

13Path analysis of relationship among personality, perceived stress, coping, social support, and psychological outcomes显示文摘AIM: To provide a structural model of the relationship between personality traits, perceived stress, coping strategies, social support, and psychological outcomes in the general population.METHODS: This is a cross sectional study in which the study group was selected using multistage cluster and convenience sampling among a population of 4 million. For data collection, a total of 4763 individuals were asked to complete a questionnaire on demographics, personality traits, life events, coping with stress, social support, and psychological outcomes such as anxiety and depression. To evaluate the comprehensive relation-ship between the variables, a path model was fitted.RESULTS: The standard electronic modules showed that personality traits and perceived stress are important determinants of psychological outcomes. Social support and coping strategies were demonstrated to reduce the increasing cumulative positive effects of neuroticism and perceived stress on the psychological outcomes and enhance the protective effect of extraversion through decreasing the positive effect of perceived stress on the psychological outcomes. CONCLUSION: Personal resources play an important role in reduction and prevention of anxiety and depression. 14Spatial patterns and secular trends in human leishmaniasis incidence in Morocco between 2003 and 2013显示文摘Background:Few studies on spatial patterns or secular trends in human leishmanias have been conducted in Morocco.This study aimed to examine spatial patterns and trends associated with the human leishmaniasis incidence rate(HLIR)at the province/prefecture level between 2003 and 2013 in Morocco.Methods:Only the available published country data on the HLIR between 2003 and 2013,from the open access files of the Ministry of Health,were used.Secular trends were examined using Kendall’s rank correlation.An exploratory spatial data analysis was also conducted to examine the spatial autocorrelation(Global Moran’s I and local indicator of spatial association[LISA]),and spatial diffusion at the province/prefecture level.The influence of various covariates(poverty rate,vulnerability rate,population density,and urbanization)on the HLIR was tested via spatial regression(ordinary least squares regression).Results:At the country level,no secular variation was observed.Poisson annual incidence rate estimates were 13 per 100000 population(95%CI=12.9–13.1)for cutaneous leishmaniasis(CL)and 0.4 per 100000 population(95%CI=0.4–0.5)for visceral leishmaniasis(VL).The available data on HLIR were based on combined CL and VL cases,however,as the CL cases totally outnumbered the VL ones,HLIR may be considered as CL incidence rate.At the provincial level,a secular increase in the incidence rate was observed in Al Hoceima(P=0.008),Taounate(P=0.04),Larache(P=0.002),Tétouan(P=0.0003),Khenifra(P=0.008),Meknes(P=0.03),and El Kelaa(P=0.0007),whereas a secular decrease was observed only in the Chichaoua province(P=0.006).Even though increased or decreased rate was evident in these provinces,none of them showed clustering of leishmaniasis incidence.Significant spatial clusters of high leishmaniasis incidence were located in the northeastern part of Morocco,while spatial clusters of low leishmaniasis incidence were seen in some northwestern and southern parts of Morocco;there was spatial randomness in the remaining parts of the country.Significant clustering was seen from 2005 to 2013,during which time the Errachidia province was a permanent‘hot spot’.Global Moran’s I increased from 0.2844(P=0.006)in 2005 to 0.5886(P=0.001)in 2011,and decreased to 0.2491(P=0.004)in 2013.It was found that only poverty had an effect on the HLIR(P=0.0003),contributing only 23%to this(Adjusted R-squared=0.226).Conclusion:Localities showing either secular increase in human leishmaniasis or significant clustering have been identified,which may guide decision-making as to where to appropriately allocate funding and implement control measures.Researchers are also urged to undertake further studies focusing on these localities.Mina Sadeq 2016Infectious Diseases of Poverty2016,5,1:5
15Medicinal properties of Peganum harmala L. in traditional Iranian medicine and modern phytotherapy: a review显示文摘OBJECTIVE: To review the pharmacological activities of Peganum harmala L.(P. harmala, Nitrariaceae) in traditional Iranian medicine(TIM) and modern phytotherapy.METHODS: Opinions of TIM and modern phytotherapy about safety and acceptable dosage of this plant are discussed. Various medical properties of P.harmala were collected from important TIM references and added to scientific reports derived from modern medical databases like Pub Med, Scirus, Science Direct and Scopus.RESULTS: The main medicinal part of the plant is the seed. In TIM resources, this plant possesses various Pharmacological activities such as carminative,galactagogue, diuretic, emmenagogue, antithrom-botic and analgesic. In modern phytotherapy, P. harmala demonstrated numerous medicinal effects including cardiovascular, neurologic, antimicrobial,insecticidal, antineoplasmic, antiproliferative, gastrointestinal and antidiabetic effects. Adverse events such as neuro-sensorial symptoms, visual hallucination, bradycardia, hypotension, agitation,tremors, ataxia, abortion and vomiting cause people to use this plant cautiously. P. harmala is contraindicated during pregnancy, due to its abortive and mutagenic activities. Because of increasing the expression of CYP1A2, 2C19, and 3A4 and inhibition of monoamine oxidase, the pharmacokinetic parameters of drugs which are mainly metabolized by these enzymes may be affected by P. harmala.CONCLUSION: The medicinal properties declared for this plant in TIM are compared with those showed in modern phytotherapy. Some of the TIM properties were confirmed in modern phytotherapy like emetic and analgesic activities and some have not been evaluated in modern phytotherapy such as its therapeutic effects on paralysis, epilepsy and numbness. 17Automatic Visual Leakage Detection and Localization from Pipelines in Chemical Process Plants Using Machine Vision Techniques显示文摘Liquid leakage from pipelines is a critical issue in large-scale process plants.Damage in pipelines affects the normal operation of the plant and increases maintenance costs.Furthermore,it causes unsafe and hazardous situations for operators.Therefore,the detection and localization of leakages is a crucial task for maintenance and condition monitoring.Recently,the use of infrared(IR)cameras was found to be a promising approach for leakage detection in large-scale plants.IR cameras can capture leaking liquid if it has a higher(or lower)temperature than its surroundings.In this paper,a method based on IR video data and machine vision techniques is proposed to detect and localize liquid leakages in a chemical process plant.Since the proposed method is a vision-based method and does not consider the physical properties of the leaking liquid,it is applicable for any type of liquid leakage(i.e.,water,oil,etc.).In this method,subsequent frames are subtracted and divided into blocks.Then,principle component analysis is performed in each block to extract features from the blocks.All subtracted frames within the blocks are individually transferred to feature vectors,which are used as a basis for classifying the blocks.The k-nearest neighbor algorithm is used to classify the blocks as normal(without leakage)or anomalous(with leakage).Finally,the positions of the leakages are determined in each anomalous block.In order to evaluate the approach,two datasets with two different formats,consisting of video footage of a laboratory demonstrator plant captured by an IR camera,are considered.The results show that the proposed method is a promising approach to detect and localize leakages from pipelines using IR videos.The proposed method has high accuracy and a reasonable detection time for leakage detection.The possibility of extending the proposed method to a real industrial plant and the limitations of this method are discussed at the end.Mina Fahimipirehgalin Emanuel Trunzer Matthias Odenweller Birgit Vogel-Heuser 2021Engineering2021,7,6:4
18Surfactant assisted solvothermal synthesis of ZnO nanoparticles and study of their antimicrobial and antioxidant properties显示文摘This study demonstrated a solvothermal method of growth of three different morphologies of zinc oxide nanoparticles(ZnO NPs): i) flower-like nanorod and nanoflakes, ii) assembled hierarchical structure,and iii) nano granule. Oleic acid(C_(18)H_(34)O_2), gluconic acid(C_6H_(12)O_7) and tween 80(C_(64)H_(124)O_(26)) were used as surfactant/capping/reducing agent for the formation of different morphologies of nanoparticles.The as-synthesized ZnO NPs were characterized by different physicochemical techniques such as UV–vis(UV–vis) spectroscopy, X-ray diffraction(XRD), fourier transform infrared spectroscopy(FTIR), field emission scanning electron microscopy(FE-SEM), energy dispersive X-ray(EDX) analysis and dynamic light scattering(DLS) studies. Further, the antioxidant and antimicrobial activity of these nanostructures was evaluated. The antioxidant activity of these nanostructures was assessed via 2,2-diphenyl,1-1 picrylhydrazyl(DPPH), 2,2-azino-bis(3-ethylbenzothiazoline-6-sulphonic acid)(ABTS) and H_2O_2 free radical scavenging activity. The in vitro antimicrobial activity of the obtained nanostructures was demonstrated against both gram negative(Escherichia coli) and gram positive(Staphylococcus aureus) bacterial genera.This study revealed antioxidant and antimicrobial properties of different structures of ZnO NPs suggesting their biomedical and industrial applications.Mina Zare K. 19Genome-Wide Identification of Zn_(2)Cys_(6 ) Class Fungal-Specific Transcription Factors(ZnFTFs)and Functional Analysis of UvZnFTFI in Ustilaginoidea virens显示文摘Transcription factors(TFs)orchestrate the regulation of cellular gene expression and thereby determine cell functionality.In this study,we analyzed the distribution of TFs containing domains,which named as ZnFTFs,both in ascomycete and basidiomycete fungi.We found that ZnFTFs were widely distributed in these fungal species,but there was more expansion of the ZnFTF class in Ascomycota than Basidiomycota.We identified 40 ZnFTFs in Ustilaginoidea virens,and demonstrated the involvement of UvZnFTF1 in vegetative growth,conidiation,pigment biosynthesis and pathogenicity.RNA-Seq analysis suggested that UvZnFTF1 may regulate different nutrient metabolism pathways,the production of secondary metabolites,and the expression of pathogen-host interaction genes and secreted protein-encodi ng genes.Analysis of the distributi on of differe nt fungal TFs in U.virens further dem on strated that UvZnFTFs make up a large TF family and may play essential biological roles in U.virens.SONG Tianqiao ZHANG Xiong ZHANG You LIANG Dong YAN Jiaoling YU Junjie YU Mina CAO Huijuan YONG Mingli PAN Xiayan QI Zhongqiang DU Yan ZHANG Rongsheng LIU Yongfeng 2021Rice science2021,28,6:4
20优化船用柴油机的性能以减少氧化硫SO_x排放物的形成显示文摘Optimization procedures are required to minimize the amount of fuel consumption and exhaust emissions from marine engines.This study discusses the procedures to optimize the performance of any marine engine implemented in a 0D/1D numerical model in order to achieve lower values of exhaust emissions.From that point,an extension of previous simulation researches is presented to calculate the amount of SOx emissions from two marine diesel engines along their load diagrams based on the percentage of sulfur in the marine fuel used.The variations of SOx emissions are computed in g/k W·h and in parts per million(ppm)as functions of the optimized parameters:brake specific fuel consumption and the amount of air-fuel ratio respectively.Then,a surrogate model-based response surface methodology is used to generate polynomial equations to estimate the amount of SOx emissions as functions of engine speed and load.These developed non-dimensional equations can be further used directly to assess the value of SOx emissions for different percentages of sulfur of the selected or similar engines to be used in different marine applications.Mina Tadros Manuel Ventura C.Guedes Soares 2020Journal of Marine Science and Application2020,19,3:4
