1海南汉族、黎族人葡萄糖-6-磷酸脱氢酶缺乏症的基因突变型分析及一种新的G6PD基因突变型的鉴定显示文摘目的 阐明海南汉族、黎族人群中葡萄糖 - 6 -磷酸脱氢酶缺乏症的分子基础。方法 用聚合酶链反应、限制性内切酶消化筛查了 1388G→ A、136 0 C→ T、10 2 4C→ T、5 92 C→ T、5 17T→ C、493A→ G、487G→ A、392 G→ T和 95 A→ G突变 ;用单链构象多态性分析筛查其它突变 ;用核苷酸顺序分析鉴定具有SSCP异常区带样品的突变。结果 在 5 9例汉族 G6 PD缺乏症患者中 ,发现 1388G→ A 14例 (2 3.7% )、871G→ A 3例 (5 .1% )、835 A→ T 1例 (1.7% )、5 17T→ C 1例 (1.7% )、392 G→ T 3例 (5 .1% )和 95 A→ G 4例 (6 .8% ) ;在 32例黎族 G6 PD缺乏症患者中 ,发现 1388G→ A6例 (18.8% )、871G→ A3例 (9.4% )和95 A→ G2例 (6 .3% ) ;在 1例汉族患者中发现了一种新的 G6 PD基因突变—— 835 A→ G突变 ,此突变导致第 2 79位的苏氨酸被丙氨酸取代 ,将此突变型命名为 G6 PD-海口 ,其酶活性约是正常的 10 % ,比 835 A→ T突变的活性低 ,后者的酶活性约是正常的 40 %。分析人 G6 PD的三维结构模型表明 ,第 2 79位苏氨酸残基的羟基是维持 G6 PD亚基相互作用的基团。结论 海南汉族、黎族人群中具有共同的常见 G6 PD基因突变型 ;与中国其它地区人群的 G6 PD基因突变谱比较 ,结果表明某些 G6 PD基因突变广?蔡望伟 Stefania Filosa Giuseppe Martini 周玉英 周代锋 蔡兰洁 邝宇 2001中华医学遗传学杂志2001,18,2:24

3Pancreatic neuroendocrine neoplasms: Magnetic resonance imaging features according to grade and stage显示文摘AIM To describe magnetic resonance(MR) imaging features of pancreatic neuroendocrine neoplasms(Pan NENs) according to their grade and tumor-nodes-metastases stage by comparing them to histopathology and todetermine the accuracy of MR imaging features in predicting their biological behavior.METHODS This study was approved by our institutional review board; requirement for informed patient consent was waived due to the retrospective nature of the study. Preoperative MR examinations of 55 Pan NEN patients(29 men, 26 women; mean age of 57.6 years, range 21-83 years) performed between June 2013 and December 2015 were reviewed. Qualitative and quantitative features were compared between tumor grades and stages determined by histopathological analysis.RESULTS Ill defined margins were more common in G2-3 and stage Ⅲ-Ⅳ PanN ENs than in G1 and low-stage tumors(P < 0.001); this feature had high specificity in the identification of G2-3 and stage Ⅲ-Ⅳ tumors(90.3% and 96%, 95%CI: 73.1-97.5 and 77.7-99.8). The mean apparent diffusion coefficient value was significantly lower in G2-3 and stage Ⅲ-Ⅳ lesions compared to well differentiated and low-stage tumors(1.09 × 10-3 mm2/s vs 1.45 × 10-3 mm2/s and 1.10 × 10-3 mm2/s vs 1.53 × 10-3 mm2/s, P = 0.003 and 0.001). 4Diffusion-weighted imaging of pancreatic cancer显示文摘Magnetic resonance imaging(MRI) is a reliable and accurate imaging method for the evaluation of patients with pancreatic ductal adenocarcinoma(PDAC). Diffusion-weighted imaging(DWI) is a relatively recent technological improvement that expanded MRI capabilities, having brought functional aspects into conventional morphologic MRI evaluation. DWI can depict the random diffusion of water molecules within tissues(the so-called Brownian motions). Modifications of water diffusion induced by different factors acting on the extracellular and intracellular spaces, as increased cell density, edema, fibrosis, or altered functionality of cell membranes, can be detected using this MR sequence. The intravoxel incoherent motion(IVIM) model is an advanced DWI technique that consent a separate quantitative evaluation of all the microscopic random motions that contribute to DWI, which are essentially represented by molecular diffusion and blood microcirculation(perfusion). Technological improvements have made possible the routine use of DWI during abdominal MRI study. Several authors have reported that the addition of DWI sequence can be of value for the evaluation of patients with PDAC, especially improving the staging; nevertheless, it is still unclear whether and how DWI could be helpful for identification, characterization, prognostic stratification and follow-up during treatment. 5Current trends in management of hepatitis B virus reactivation in the biologic therapy era显示文摘Hepatitis B virus (HBV) reactivation represents an emerging cause of liver disease in patients undergoing treatment with biologic agents. In particular, the risk ofHBV reactivation is heightened by the use monoclonalantibodies, such as rituximab (anti-CD20) and alemtuzumab (anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat HBV reactivation could also develop following theuse of other biologic agents, such as tumor necrosis factor (TNF)-α inhibitors. When HBV reactivation is di-agnosed, it is mandatory to suspend biologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antibodyadministration is crucial in preventing HBV reactivationand its clinical consequences. Several lines of evidencehave shown that risk of HBV reactivation is greatlyreduced by the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. 8Prognostication and response assessment in liver and pancreatic tumors:The new imaging显示文摘Diffusion-weighted imaging(DWI), dynamic contrastenhanced magnetic resonance imaging(DCE-MRI) and perfusion computed tomography(CT) are technical improvements of morphologic imaging that can evaluate functional properties of hepato-bilio-pancreatic tumors during conventional MRI or CT examinations.Nevertheless, the term 'functional imaging' is commonly used to describe molecular imaging techniques, as positron emission tomography(PET)CT/MRI, which still represent the most widely used methods for the evaluation of functional properties of solid neoplasms; unlike PET or single photon emission computed tomography, functional imaging techniques applied to conventional MRI/CT examinations do not require the administration of radiolabeled drugs or specific equipments. Moreover, DWI and DCE-MRI can be performed during the same session, thus providing a comprehensive 'one-step' morphological and functional evaluation of hepato-bilio-pancreatic tumors. Literature data reveal that functional imaging techniques could be proposed for the evaluation of these tumors before treatment, given that they may improve staging and predict prognosis or clinical outcome. Microscopic changes within neoplastic tissues induced by treatments can be detected and quantified with functional imaging,therefore these techniques could be used also for posttreatment assessment, even at an early stage. 9Present and future of metastatic colorectal cancer treatment: A review of new candidate targets显示文摘In the last two decades, great efforts have been made in the treatment of metastatic colorectal cancer(m CRC) due to the approval of new target agents for cytotoxic drugs. Unfortunately, a large percentage of patients present with metastasis at the time of diagnosis or relapse after a few months. The complex molecular heterogeneity of this disease is not completely understood; to date, there is a lack of predictive biomarkers that can be used to select subsets of patients who may respond to target drugs. Only the RAS-mutation status is used to predict resistance to anti-epidermal growth factor receptor agents in patients with m CRC. 10Role of occult hepatitis B virus infection in chronic hepatitis C显示文摘The development of sensitive assays to detect small amounts of hepatitis B virus(HBV) DNA has favored the identification of occult hepatitis B infection(OBI), a virological condition characterized by a low level of HBV replication with detectable levels of HBV DNA in liver tissue but an absence of detectable surface antigen of HBV(HBs Ag) in serum. The gold standard to diagnose OBI is the detection of HBV DNA in the hepatocytes by highly sensitive and specific techniques, a diagnostic procedure requiring liver tissue to be tested and the use of non-standardized non-commercially available techniques. Consequently, in everyday clinical practice, the detection of anti-hepatitis B core antibody(antiHBc) in serum of HBs Ag-negative subjects is used as a surrogate marker to identify patients with OBI. In patients with chronic hepatitis C(CHC), OBI has been identified in nearly one-third of these cases. Considerable data suggest that OBI favors the increase of liver damage and the development of hepatocellular carcinoma(HCC) in patients with CHC. 11Mechanisms of resistance to anti-epidermal growth factor receptor inhibitors in metastatic colorectal cancer显示文摘The prognosis of patients with metastatic colorectal cancer(m CRC) remain poor despite the impressive improvement of treatments observed over the last 20 years that led to an increase in median overall survival from 6 mo, with the only best supportive care, to approximately 30 mo with the introduction of active chemotherapy drugs and targeted agents. The monoclonal antibodies(mo Abs) cetuximab and panitumumab, directed against the epidermal growth factor receptor(EGFR), undoubtedly represent a major step forward in the treatment of m CRC, given the relevant efficacy in terms of progression-free survival, overall survival, response rate, and quality of life observed in several phase Ⅲ clinical trials among different lines of treatment. However, the anti-EGFR mo Abs were shown only to be effective in a subset of patients. For instance, KRAS and NRAS mutations have been identified as biomarkers of resistance to these drugs, improving the selection of patients who might derive a benefit from these treatments. Nevertheless, several other alterations might affect the response to these drugs, and unfortunately, even the responders eventually become resistant by developing secondary(or acquired) resistance in approximately 13-18 mo. Several studies highlighted that the landscape of responsible alterations of both primary and acquired resistance to anti-EGFR drugs biochemically converge into MEK-ERK and PIK3CA-AKT pathways. 13Reactivation of hepatitis B virus infection in patients with hemolymphoproliferative diseases, and its prevention显示文摘Reactivation of hepatitis B virus(HBV)replication is characterized by increased HBV-DNA serum values of about 1 log or by HBV DNA turning positive if previously undetectable in serum,possibly associated with liver damage and seldom life-threatening.Due to HBV reactivation,hepatitis B surface antigen(HBsAg)-negative/anti-HBc-positive subjects may revert to HBsAg-positive.In patients with hemo-lymphoproliferative disease,the frequency of HBV reactivation depends on the type of lymphoproliferative disorder,the individual's HBV serological status and the potency and duration of immunosuppression.In particular,it occurs in 10%-50%of the HBsAg-positive and in 2%-25%of the HBsAg-negative/anti-HBc-positive,the highest incidences being registered in patients receiving rituximab-based therapy.HBV reactivation can be prevented by accurate screening of patients at risk and by a pharmacological prophylaxis with anti-HBV nucleo(t)sides starting 2-3 wk before the beginning of immunosuppressive treatment and covering the entire period of administration of immunosuppressive drugs and a long subsequent period,the duration of which depends substantially on the degree of immunodepression achieved.Patients with significant HBV replication before immunosuppressive therapy should receive anti-HBV nucleo(t)sides as a long-term(may be life-long)treatment.This review article is mainly directed to doctors engaged every day in the treatment of patients with onco-lymphoproliferative diseases,so that they can broaden their knowledge on HBV infection and on its reactivation induced by the drugs with high immunosuppressive potential that they use in the care of their patients.Caterina Sagnelli Mariantonietta Pisaturo Federica Calò Salvatore Martini Evangelista Sagnelli Nicola Coppola 2019World Journal of Gastroenterology2019,25,26:6
14Clinical impact of occult hepatitis B virus infection in immunosuppressed patients显示文摘Occult hepatitis B infection(OBI), is characterized by low level hepatitis B virus(HBV) DNA in circulating blood and/or liver tissue. In clinical practice the presence of antibody to hepatitis B core antigen in hepatitis B surface antigen(HBsAg)-/anti-HBs-negative subjects is considered indicative of OBI. OBI is mostly observed in the window period of acute HBV infection in blood donors and in recipients of blood and blood products, in hepatitis C virus chronic carriers, in patients under pharmacological immunosuppression, and in those with immunodepression due to HIV infection or cancer. Reactivation of OBI mostly occurs in anti-HIV-positive subjects, in patients treated with immunosuppressive therapy in onco-hematological settings, in patients who undergo hematopoietic stem cell transplantation, in those treated with anti-CD20 or anti-CD52 monoclonal antibody, or anti-tumor necrosis factors antibody for rheumatological diseases, or chemotherapy for solid tumors. Under these conditions the mortality rate for hepatic failure or progression of the underlying disease due to discontinuation of specific treatment can reach 20%. For patients with OBI, prophylaxis with nucleot(s)ide analogues should be based on the HBV serological markers, the underlying diseases and the type of immunosuppressive treatment. Lamivudine prophylaxis is indicated in hemopoietic stem cell transplantation and in onco-hematological diseases when high dose corticosteroids and rituximab are used; monitoring may be indicated when rituximab-sparing schedules are used, but early treatment should be applied as soon as HBsAg becomes detectable. 16Coronary artery calcium score on low-dose computed tomography for lung cancer screening显示文摘AIM: To evaluate the feasibility of coronary artery calcium score(CACS) on low-dose non-gated chest CT(ngCCT).METHODS: Sixty consecutive individuals(30 males; 73 ± 7 years) scheduled for risk stratification by means of unenhanced ECG-triggered cardiac computed to-mography(gCCT) underwent additional unenhanced ngCCT. All CT scans were performed on a 64-slice CT scanner(Somatom Sensation 64 Cardiac, Siemens, Germany). CACS was calculated using conventional methods/scores(Volume, Mass, Agatston) as previ-ously described in literature. The CACS value obtained were compared. The Mayo Clinic classification was used to stratify cardiovascular risk based on Agatston CACS. Differences and correlations between the two methods were compared. A P-value < 0.05 was considered sig-nificant.RESULTS: Mean CACS values were significantly higher for gCCT as compared to ngCCT(Volume: 418 ± 747 vs 332 ± 597; Mass: 89 ± 151 vs 78 ± 141; Agatston: 481 ± 854 vs 428 ± 776; P < 0.05). The correlation between the two values was always very high(Volume: r = 0.95; Mass: r = 0.97; Agatston: r = 0.98). Of the 6 patients with 0 Agatston score on gCCT, 2(33%) showed an Agatston score > 0 in the ngCCT. Of the 3 patients with 1-10 Agatston score on gCCT, 1(33%) showed an Agatston score of 0 in the ngCCT. Overall, 23(38%) patients were reclassified in a different car-diovascular risk category, mostly(18/23; 78%) shifting to a lower risk in the ngCCT. 19有 CT 冠的 angiography 的匾成像: 匾类型分类上的 intra 脉管的变细的效果显示文摘 AIM:To assess the attenuation of non-calcified atherosclerotic coronary artery plaques with computed tomography coronary angiography(CTCA).METHODS:Four hundred consecutive patients underwent CTCA(Group 1:200 patients,Sensation 64 Cardiac,Siemens;Group 2:200 patients,VCT GE Healthcare,with either Iomeprol 400 or Iodixanol 320,respectively) for suspected coronary artery disease(CAD).CTCA was performed using standard protocols.Image quality(score 0-3),plaque(within the accessible non-calcified component of each non-calcified/mixed plaque) and coronary lumen attenuation were measured.Data were compared on a per-segment/perplaque basis.Plaques were classified as fibrous vs lipid rich based on different attenuation thresholds.A P < 0.05 was considered significant.RESULTS:In 468 atherosclerotic plaques in Group 1 and 644 in Group 2,average image quality was 2.96 ± 0.19 in Group 1 and 2.93 ± 0.25 in Group 2(P ≥ 0.05).Coronary lumen attenuation was 367 ± 85 Hounsfield units(HU) in Group 1 and 327 ± 73 HU in Group 2(P < 0.05);non-calcified plaque attenuation was 48 ± 23 HU in Group 1 and 39 ± 21 HU in Group 2(P < 0.05).Overall signal to noise ratio was 15.6 ± 4.7 in Group 1 and 21.2 ± 7.7 in Group 2(P < 0.01).CONCLUSION:Higher intra-vascular attenuation modifies significantly the attenuation of non-calcified coronary plaques.This results in a more difficult characterization between lipid rich vs fibrous type.Erica Maffei Chiara Martini Teresa Arcadi Alberto Clemente Sara Seitun Alessandra Zuccarelli Tito Torri Nico R Mollet Alexia Rossi Onofrio Catalano Giancarlo Messalli Filippo Cademartiri 2012World Journal of Radiology2012,4,6:3
20Coastal plain evolution in southern Hainan Island,China显示文摘The coast of southern Hainan Island is characterized by wide sandy embayments, which consist of ( i) drowned valleys bounded by steep bedrock hills and only locally receiving sediments, and embayments of various dimensions covered either by (ii) alluvial-deltaic deposits or by (iii) sands of coastal beach ridges/barriers and associated elongated lagoons. During the late Tertiary-Pleistocene the area has experienced isostatic and eustatic movements associated with neotectonics and climatic changes. Such history isrecorded in terraces at various altitudes (SO, 40, 20 m asl) and sequences of coastal sand ridges/baymouth bars. The Holocene variations in sea level and climate are recorded in the dated coastal ridges, coral reef and beachrock. Conditions suitable for reef development started about 8000 a BP.
