1Disease impact on the quality of life of children with inflammatory bowel disease显示文摘AIM To assess the impact of disease characteristics on the quality of life (QOL) in children with inflammatory bowel diseases(IBD).METHODS This was a cross-sectional study conducted at the First Department of Pediatrics of the University of Athens at the 'Aghia Sophia' Children's Hospital. Children diagnosed with Crohn's disease (CD) or ulcerative colitis (UC), who were followed as outpatients or during a hospitalization, participated, after informed consent was obtained from their legal representative. QOL was assessed by the IMPACT-III questionnaire. Demographic data and disease characteristics were also collected. Statistical analyses included parametric(Student's t-test and Pearson's r) and non-parametric(Mann-Whitney test, Fisher's test and Spearman's rho) procedures.RESULTS Ninety-nine patients (UC: 37, 73.0% females, CD: 62, 51.6% females), aged 12.8±2.6 years were included.Overall, as well as, sub-domain scores did not differ between UC and CD(overall score: 73.9±13.3 vs 77.5±11.2, respectively, P=0.16). In the entire sample, total score was related to physician's global assessment(PGA, patients classified as 'mild/moderate' active disease had, on average, 14.8±2.7 points lower total scores compared to those 'in remission', P<0.001) and age at IMPACT completion (Pearson's r=0.29, P=0.05). Disease activity assessed by the indices Pediatric Ulcerative Colitis activity index, Pediatric Crohn's disease activity index or PGA was significantly associated with all subdomains scores. Presence of extraintestinal manifestations had a negative impact on emotional and social functioning domains.CONCLUSION Disease activity is the main correlate of QOL in children with IBD, underlining the importance of achieving and sustaining clinicalGiorgos Chouliaras Daphne Margoni Konstantina Dimakou Smaragdi Fessatou Ioanna Panayiotou Eleftheria Roma-Giannikou 2017World Journal of Gastroenterology2017,23,6:4
2Impact and clinical usefulness of genetic data in the surgical management of colorectal cancer liver metastasis: a narrative review显示文摘Importance:In patients who undergo surgery for colorectal cancer liver metastases(CRLM),a number of somatic mutations have been associated with worse overall(OS)and recurrence-free survival(RFS).Although useful,an association with prognosis does not necessarily equate to an impact on surgical management. Objective:The aim of this review was to investigate whether the best-studied somatic mutations impact surgical management of CRLM by informing:(I)post-hepatectomy surveillance;(II)selection of surgical technique;(III)selection of optimal margin width;and(IV)selection of patients for surgery.Lastly,we discuss the refinement of genetic data from overall mutation status to specific variants,as well as lesser studied somatic mutations.Evidence Review:We conducted a computerized search using PubMed and Google Scholar for reports published so far, using mesh headings and keywords related to genetic data and CRLM.Findings:Genetic data may impact surgical management of CRLM in three ways.Firstly,KRAS mutations can predict lung recurrences.Secondly,KRAS mutations may help tailor margin width.Thirdly,KRAS mutations may help tailor surgical technique. Conclusions:Although genetic data may impact post-hepatectomy surveillance, selection of surgical technique and optimal margin width, their use to guide surgical selection remains elusive,as the data cannot support denying surgery to patients according to their somatic mutation profile.Georgios Antonios Margonis Martin E.Kreis Jaeyun Jane Wang Carsten Kamphues Christopher L.Wolfgang Matthew J.Weiss 2020Hepatobiliary Surgery and Nutrition2020,9,6:3
3Licensing FAIR Data for Reuse显示文摘The last letter of the FAIR acronym stands for Reusability.Data and metadata should be made available with a clear and accessible usage license. But,what are the choices? How can researchers share data and allow reusability? Are all the licenses available for sharing content suitable for data?Data can be covered by different layers of copyright protection making the relationship between data and copyright particularly complex. Some research data can be considered as a work and therefore covered by full copyright while other data can be in the public domain due to their lack of originality. Moreover,a collection of data can be protected by special rights in Europe to acknowledge the investment in time and money in obtaining, presenting, arranging or verifying the data. The need of using a license when sharing data comes from the fact that, under current copyright laws, when rights exist, the absence of any legal notice must be understood as the default“all rights reserved”regime.Unless an exception applies, the authorisation of right holders is necessary for reuse.Right holders could use any text to state the reusability of data but it is advisable to use some of the existing licenses, and especially the ones that are suitable for data and databases. We hope that with this paper we can bring some clarity in relation to the rights involved when sharing research data.Ignasi Labastida Thomas Margoni 2020Data Intelligence2020,2,1:2
