3PI3K/Akt/mTOR inhibitors in breast cancer显示文摘Activation of the phosphoinositide 3 kinase(PI3K)/Akt/mammalian target of rapamycin(mTOR) pathway is common in breast cancer. There is preclinical data to support inhibition of the pathway, and phase Ⅰ to Ⅲ trials involving inhibitors of the pathway have been or are being conducted in solid tumors and breast cancer. Everolimus, an mTOR inhibitor, is currently approved for the treatment of hormone receptor(HR)-positive, human epidermal growth factor receptor 2(HER2)-negative breast cancer. In this review, we summarise the efficacy and toxicity findings from the randomised clinical trials, with simplified guidelines on the management of potential adverse effects. Education of healthcare professionals and patients is critical for safety and compliance. While there is some clinical evidence of activity of mTOR inhibition in HR-positive and HER2-positive breast cancers, the benefits may be more pronounced in selected subsets rather than in the overall population. 5Vibrational characteristics of piping system in air conditioning outdoor unit显示文摘The modal analysis of piping system in air conditioner (AC) outdoor unit is essential to investigate the vibration properties of the system. In view of the growing significance of numerical finite element (FE) model for vibration behaviour prediction, the AC piping elastic end support characterization has been explored. The axial and radial stiffness variables (ka, kr1, kr2) of the compressor-piping mounting are obtained and represented by dynamic stiffness of compressor grommet. They are obtained from dynamic load deflection test based on compressor operating condition such as excitation frequency and amplitude. The unknown stiffness variables of the other tube end (chassis-piping mounting) are determined by parameter fine tuning. An experimental modal analysis using impact hammer test has also been employed to determine the vibration properties such as natural frequencies, mode shapes and damping ratio of the piping structures. The modal parameters acquisition using SCADAS mobile acquisition system and LMS Impact Testing software is compared with the corresponding simulated modal properties using Abaqus. Most of the simulated natural frequencies achieve good correlation with the measured frequencies and it is reasonably a good prediction model to predict vibration behaviour of AC piping structures.S. 6Preliminary experience with drug-coated balloon angioplasty in primary percutaneous coronary intervention显示文摘We evaluated the clinical feasibility of using drugcoated balloon(DCB) angioplasty in patients undergoingprimary percutaneous coronary intervention(PPCI). Between January 2010 to September 2014,89 STelevation myocardial infarction patients(83% male,mean age 59 ± 14 years) with a total of 89 coronary lesions were treated with DCB during PPCI. Clinical outcomes are reported at 30 d follow-up. Left anterior descending artery was the most common target vessel for PCI(37%). Twenty-eight percent of the patients had underlying diabetes mellitus. Mean left ventricular ejection fraction was 44% ± 11%. DCB-only PCI was the predominant approach(96%) with the remaining 4% of patients receiving bail-out stenting. Thrombolysis in Myocardial Infarction(TIMI) 3 flow was successfully restored in 98% of patients. An average of 1.2 ± 0.5 DCB were used per patient,with mean DCB diameter of 2.6 ± 0.5 mm and average length of 23.2 ± 10.2 mm. At 30-d follow-up,there were 4 deaths(4.5%). No patients experienced abrupt closure of the infarctrelated artery and there was no reported target-lesion failure. 7尖锐心肌的梗塞: 在在新加坡的年轻成年人的临床的特征和结果显示文摘 AIM: To investigate the clinical features and in-hospital outcomes of young adults with acute myocardial infarction (AMI) in Singapore. METHODS: Between January 2005 to September 2010, 333 consecutive patients aged ≤ 45 years old were diagnosed to have AMI at our institution. As Singapore is a multi-ethnic society, we also analysed whether ethnic differences exist between the three dominant ethnic groups, Malay, Chinese and Indian with regards to the clinical features. Clinical data was collected retrospectively on demographic characteristics, presenting signs and symptoms, blood investigation, angiographic findings and in-hospital clinical outcomes. RESULTS: The mean age at presentation was 40.2 ± 4.0 years with male predominance (94%). The majority of patients were Chinese (51%) followed by Indians (31%) and Malays (18%). The most common risk factor was smoking (74%) followed by hypertension (28.5%) and hyperlipidemia (20.0%). 37% of patients were obese. The majority of patients had single vessel disease (46%) on coronary angiography. The mean total cholesterol, low-density lipoprotein and highdensity lipoprotein levels were 5.6±1.2 mmol/L, 3.8±1.1 mmol/L and 0.93±0.25 mmol/L respectively. The mean left ventricular function was 44%±10% with the incidence of heart failure 3% and cardiogenic shock 4.5%. Overall in-hospital mortality was low with 4 deaths (1.2%). For ethnic subgroup analysis, Indians have a 3-fold risk of developing premature AMI when compared to other ethnic groups. CONCLUSION: Young AMI patients in Singapore are characterized by male predominance, high incidence of smoking and obesity. Overall in-hospital clinical outcomes are favourable. 8不平衡负载工况下三相变流器有源功率解耦控制显示文摘三相电压源型变流器(VSC)对于微电网的稳定运行有着重要的作用。然而,孤岛工况下带三相不平衡负载运行时,交流侧产生的二次脉动功率耦合至直流侧,导致系统的工作性能降低并影响系统的寿命。针对该问题,在二次功率脉动分析的基础上,提出了一种基于虚拟三相瞬时功率的解耦控制策略,通过控制虚拟三相瞬时功率脉动分量为0,在不增加开关器件的情况下,利用LCL滤波器中电容提供负载所需二次脉动功率,实现交直流侧二次脉动功率解耦。在此基础上进一步分析了该拓扑结构直流侧所需的最低电压以及电流应力影响因素。最后仿真和实验结果均表明所提控制策略可以较好地实现功率解耦控制,直流侧源电流二次脉动分量大大减小,有利于提高系统控制性能和实现直流侧电容轻量化。许泽富 唐芬 周家宇 辛振 Poh Chiang Loh 2019电网技术2019,43,3:11
9Effect of near-fault earthquake on bridges:lessons learned from Chi-Chi earthquake显示文摘The objective of this paper is to describe the lessons learned and actions that have been taken related to the seismicdesign of bridge structures after the Chi-Chi, Taiwan earthquake. Much variable near-fault ground motion data was collected fromthe rupture of Chelungpu fault during the Chi-Chi earthquake, allowing the seismic response of bridge structures subjected to thesenear-fault ground motions to be carefully examined. To study the near-fault ground motion effect on bridge seismic design codes, atwo-level seismic design of bridge structures was developed and implemented. This design code reflects the near-fault factors in theseismic design forces. 11二维材料最新研究进展显示文摘Research on two-dimensional(2D) materials has been explosively increasing in last seventeen years in varying subjects including condensed matter physics, electronic engineering, materials science, and chemistry since the mechanical exfoliation of graphene in 2004. Starting from graphene, 2D materials now have become a big family with numerous members and diverse categories. The unique structural features and physicochemical properties of 2D materials make them one class of the most appealing candidates for a wide range of potential applications. In particular, we have seen some major breakthroughs made in the field of 2D materials in last five years not only in developing novel synthetic methods and exploring new structures/properties but also in identifying innovative applications and pushing forward commercialisation. In this review, we provide a critical summary on the recent progress made in the field of 2D materials with a particular focus on last five years. After a brief backgroundintroduction, we first discuss the major synthetic methods for 2D materials, including the mechanical exfoliation, liquid exfoliation, vapor phase deposition, and wet-chemical synthesis as well as phase engineering of 2D materials belonging to the field of phase engineering of nanomaterials(PEN). We then introduce the superconducting/optical/magnetic properties and chirality of 2D materials along with newly emerging magic angle 2D superlattices. Following that, the promising applications of 2D materials in electronics, optoelectronics, catalysis, energy storage, solar cells, biomedicine, sensors, environments, etc. are described sequentially. Thereafter, we present the theoretic calculations and simulations of 2D materials. 12MMP-2和TIMP-2基因多态性与胃癌发病风险的研究显示文摘目的探讨西南汉族人群中基质金属蛋白酶-2(matrix metallopmteinase-2,MMP-2)和金属蛋白酶组织抑制剂-2(tissue inhibitor of metalloproteinase-2,TIMP-2)基因启动子区域单核苷酸多态性与胃癌发病风险的关系。方法在美国Sequenom MassARRAY系统上利用基质辅助激光解吸电离飞行时间质谱法(MALDI-TOF MS)针对308对胃癌病例和对照分析MMP-2-1306C/T和TIMP-2-418G/C基因多态特征。同时利用自行设计的问卷收集研究对象的人口学特征和日常生活习惯。结果 MMP-2-1306C/T的CC、CT和TT基因型在病例组和对照组中的频率分别为:79.5%、19.8%、0.6%和79.8%、19.2%、1%;TIMP-2-418G/C的GG、GC和CC基因型在病例组和对照组中的频率分别为:71.4%、26.6%、2.0%和70.5%、27.8%、1.7%;MMP-2-1306C/T(P=0.936和0.984)和TIMP-2-418G/C(P=0.817和0.898)的基因型和等位基因频率分布在胃癌组与对照组间均无统计学差异(P>0.05)。同时未发现基因-基因联合作用(OR=1.004;95%CI:0.460~2.191)和基因-环境之间的交互作用。结论本研究未发现MMP-2-1306C/T和TIMP-2-418G/C多态性与胃癌发病风险有关,且两单核苷酸多态之间无基因-基因联合作用,也未发现基因与环境的交互作用。陈凤 杨淑娟 田智 温圆圆 谢瑶 潘雄飞 Marie Loh 黄荷 兰慧 温莹 赵志梅 Richie Soong 杨春霞 2014现代预防医学2014,41,1:9
13Diffraction-limited imaging with monolayer 2D material-based ultrathin flat lenses显示文摘Ultrathin flat optics allow control of light at the subwavelength scale that is unmatched by traditional refractive optics.To approach the atomically thin limit,the use of 2D materials is an attractive possibility due to their high refractive indices.However,achievement of diffraction-limited focusing and imaging is challenged by their thickness-limited spatial resolution and focusing efficiency.Here we report a universal method to transform 2D monolayers into ultrathin flat lenses.Femtosecond laser direct writing was applied to generate local scattering media inside a monolayer,which overcomes the longstanding challenge of obtaining sufficient phase or amplitude modulation in atomically thin 2D materials.We achieved highly efficient 3D focusing with subwavelength resolution and diffractionlimited imaging.The high focusing performance even allows diffraction-limited imaging at different focal positions with varying magnifications.Our work paves the way for downscaling of optical devices using 2D materials and reports an unprecedented approach for fabricating ultrathin imaging devices.Han Lin Zai-Quan Xu Guiyuan Cao Yupeng Zhang Jiadong Zhou Ziyu Wang Zhichen Wan Zheng Liu Kian Ping Loh Cheng-Wei Qiu Qiaoliang Bao Baohua Jia 2020Light(Science & Applications)2020,9,1:9
14Differences in neuroplasticity after spinal cord injury in varying animal models and humans显示文摘Rats have been the primary model to study the process and underlying mechanisms of recovery after spinal cord injury. Two weeks after a severe spinal cord contusion, rats can regain weight-bearing abilities without therapeutic interventions, as assessed by the Basso, Beattie and Bresnahan locomotor scale. However, many human patients suffer from permanent loss of motor function following spinal cord injury. While rats are the most understood animal model, major differences in sensorimotor pathways between quadrupeds and bipeds need to be considered. Understanding the major differences between the sensorimotor pathways of rats, non-human primates, and humans is a start to improving targets for treatments of human spinal cord injury. This review will discuss the neuroplasticity of the brain and spinal cord after spinal cord injury in rats, non-human primates, and humans. 16三电平并网变流器中点电位自平衡特性分析及拓扑改进显示文摘传统三电平并网变流器中点电位自平衡时间较长,中点电位可能产生较大偏移,使并网电流低次谐波增加,常需采用中点电位控制防止中点电位偏移。为充分发挥拓扑的自平衡能力,省去中点电位控制算法,文中首先定量分析了载波同相层叠脉宽调制输出电压的谐波分布特性,在此基础上,给出了传统拓扑中点电位自平衡机理及影响因素。针对传统拓扑自平衡时间的制约因素,提出了三电平并网变流器中点电位自平衡改进拓扑,通过新增零序通路在载波频率处产生串联谐振,减小阻抗,从而显著提升自平衡速度。最后,仿真和实验结果表明改进拓扑能快速有效地将中点电位稳定于直流侧电源中点,且自平衡速度不受直流侧电压不平衡度影响,中点电位平衡后并网电流质量较高。相对传统拓扑,自平衡速度提高10倍以上,与传统中点电位控制速度相当,因此,可省去中点电位控制。尽管所提拓扑需增加一个小电容,但其无需电压偏差检测,无需增加或调整控制算法,适用于全功率因数范围,易于实现。陈鹏飞 唐芬 吴学智 武亮亮 王玮 Poh Chiang LOH 2019电力系统自动化2019,43,24:7

18以系统为导向的反转课堂教学在MBBS心脏诊断学教学中的运用评价显示文摘目的评价以系统为导向的反转课堂教学在MBBS心脏诊断学教学中的运用效果。方法对昆明医科大学2014年9月进入诊断学学习阶段的80名东南亚医学留学生采用'以系统为导向'的'反转课堂'教学方法教授心脏诊断学部分。结果 73.80%的学生认为采用的一个病例串联与临床工作很吻合,23.81%的学生认为比较吻合;91.67%的学生认为授课过程中师资有引导学生讨论,其中69.05%认为非常有用;95.24%的学生认为教师的讲课有使用多元化教学方式,其中78.57%认为很有效;96.43%的学生认为教师有进行小结和总结,其中67.86%认为很有效;86.90%的学生认为所讲内容没有超负荷;95.24%的学生认为师资有寻求学生反馈;96.43%的学生认为教学课件设计清晰;96.43%的学生认为教师对课题内容掌握透彻;97.62%的学生认为教师有充分时间给学生提问答疑。结论以系统为导向的反转课堂教学方法能很好地发挥学生的积极主动性,更好地从纵向扩充MBBS留学生对心脏诊断学部分的学习,并且是有效的和容易被接受的。该教学方法更符合外国留学生的医学教学,更好地优化了教学时间和教学资源,从而提高了医学教学质量和学生对知识的运用能力。周娜 林玥 林秋果 James D.Smith Loh Cheng David KNIGHT 张红珊 陈江 周晓鸿 李炯明 2015中国继续医学教育2015,7,7:7
19Overview of milling techniques for improving the solubility of poorly water-soluble drugs显示文摘Milling involves the application of mechanical energy to physically break down coarse particles to finer ones and is regarded as a“topedown”approach in the production of fine particles.Fine drug particulates are especially desired in formulations designed for parenteral,respiratory and transdermal use.Most drugs after crystallization may have to be comminuted and this physical transformation is required to various extents,often to enhance processability or solubility especially for drugs with limited aqueous solubility.The mechanisms by which milling enhances drug dissolution and solubility include alterations in the size,specific surface area and shape of the drug particles as well as millinginduced amorphization and/or structural disordering of the drug crystal(mechanochemical activation).Technology advancements in milling now enable the production of drug micro-and nano-particles on a commercial scale with relative ease.This review will provide a background on milling followed by the introduction of common milling techniques employed for the micronization and nanonization of drugs.Salient information contained in the cited examples are further extracted and summarized for ease of reference by researchers keen on employing these techniques for drug solubility and bioavailability enhancement.Zhi Hui Loh Asim Kumar Samanta Paul Wan Sia Heng 2015Asian Journal of Pharmaceutical Sciences2015,10,4:7
20低开关频率下LCL型并网变流器复矢量分析及改进解耦控制显示文摘同步旋转坐标系(dq)下的比例积分控制在并网变流器中得到广泛应用。由于滤波器和数字延迟的dq模型均存在轴间耦合并且两者相互影响,低开关频率下LCL型并网变流器的建模和解耦控制变得极具挑战。针对该问题,在对同步旋转坐标系下的LCL型并网变流器采用复矢量进行建模的基础上,通过双边频响特性曲线和零极点图对传统状态反馈和相位补偿进行了系统分析,揭示了不同位置相位补偿对系统相位裕度和解耦性能的影响关系,在此基础上提出了一种改进解耦控制方法,通过在状态反馈环内和调节器输出端依次设计相位补偿,在有效调节和改善动态性能的同时保证系统具有较好的解耦性能。最后,仿真及实验结果表明所提方法具有很好的解耦效果,同时能有效减小系统超调量和调节时间。肖琦 唐芬 辛振 周家宇 陈鹏飞 Poh Chiang Loh 姜久春 2018电网技术2018,42,9:7
