| 1 | Matched-pair analysis of postoperative morbidity and mortality for pancreaticogastrostomy and pancreaticojejunostomy using mattress sutures in soft pancreatic tissue remnants显示文摘BACKGROUND:After pancreaticoduodenectomy,the incidence of postoperative pancreatic fistula remains high,especially in patients with 'soft' pancreatic tissue remnants.No 'gold standard' surgical technique for pancreaticoenteric anastomosis has been established.This study aimed to compare the postoperative morbidity and mortality of pancreaticogastrostomy and pancreaticojejunostomy for 'soft' pancreatic tissue remnants using modified mattress sutures.METHODS:Seventy-five patients who had undergone pancreaticogastrostomy and 75 who had undergone pancreaticojejunostomy after pancreaticoduodenectomy between 2002 and 2008 were retrospectively compared using matched-pair analysis.A modified mattress suture technique was used for the pancreaticoenteric anastomosis.Patients with an underlying 'hard' pancreatic tissue remnant,as in chronic pancreatitis,were excluded.Both groups were homogeneous for age,gender,and underlying disease.Postoperative morbidity,mortality,and preoperative and operative data were analyzed.RESULTS:There were no significant differences between the groups for the incidence of postoperative pancreatic fistula (10.7% in both).Postoperative morbidity and mortality,median operation time,median length of hospital stay,intraoperative blood loss,and the amount of intraoperatively transfused erythrocyte concentrates also did not significantly differ between the groups.Patient age >65 years (P=0.017),operation time >350minutes (P=0.001),and intraoperative transfusion of erythrocyte concentrates (P=0.038) were identified as risk factors for postoperative morbidity.CONCLUSIONS:Our results showed no significant differences between the groups in the pancreaticogastrostomy and pancreaticojejunostomy anastomosis techniques using mattress sutures for 'soft' pancreatic tissue remnants.In our experience,the mattress sutures are safe and simple to use,and pancreaticogastrostomy in particular is feasible and easy to learn,with good endoscopic accessibility to the anastomosis region.However,the location of the anastomosis and the surgical technique need to be individually evaluated to further reduce the incidence of postoperative pancreatic fistula. | Fritz Klein Marcus Bahra Matthias Glanemann Wladimir Faber Peter Warnick Andreas Andreou Safak Gül Dietmar Jacob | 2012 | Hepatobiliary & Pancreatic Diseases International2012，11，1： | 9 |
| 2 | Muscle force and movement variability before and after total knee arthroplasty: A review显示文摘Variability in muscle force output and movement variability are important aspects of identifying individuals with mobility deficits, central nervous system impairments, and future risk of falling. This has been investigated in elderly healthy and impaired adults, as well as in adults with osteoarthritis(OA), but the question of whether the same correlations also apply to those who have undergone a surgical intervention such as total knee arthroplasty(TKA) is still being investigated. While there is a growing body of literature identifying potential rehabilitation targets for individuals who have undergone TKA, it is important to first understand the underlying post-operative impairments to more efficiently target functional deficits that may lead to improvedlong-term outcomes. The purpose of this article is to review the potential role of muscle force output and movement variability in TKA recipients. The narrative review relies on existing literature in elderly healthy and impaired individuals, as well as in those with OA before and following TKA. | 3 | Comparison between hepaticojejunostomy and hepaticoduodenostomy after excision of choledochal cyst in children:a cohort study显示文摘Objective Hepaticojejunostomy(HJ)and hepaticoduodenostomy(HD)are commonly used biliary reconstruction techniques after choledochal cyst excision.HD has been suggested to be a more physiologic alternative during reconstruction.The objective of this study is to compare operative time,hospital stay,morbidity(leak,cholangitis,ileus,and obstruction),and mortality between HJ and HD after cyst excision.Methods This is a 14-year retrospective cohort study of pediatric patients(≤18 years old)who underwent choledochal cyst excision and subsequent biliary reconstruction at the Philippine Children’s Medical Center.Data were taken from inpatient charts,operative technique,OPD logbook,readmission,and OPD charts.results There were 122 patients:56%HD and 44%HJ.Majority were female(72%),with 1:2.6 male to female ratio.The average age was 36.1 months,with a mean follow-up of 32.8 months(range 6 months-14 years).The most common cyst was type I(87%).Operative time was longer for HJ compared with HD(321.3 vs 203.6 min;p=0.000).Hospital stay was longer with HJ compared with HD(7.7 vs 6.8 days;p=0.002).Mortality rate was low at 1.6%while morbidity was at 13.9%in both groups.Although morbidity was higher among those who underwent HD,there was no significant difference between the two procedures.Anastomotic leak(4%)and cholangitis(7.4%)were observed in HD,and ileus(7.4%)was observed in the HJ group.Conclusions In our series,HD provided less operative time and hospital stay than with HJ.We did not observe bile gastritis after HD as compared with others.It is suggested that longer follow-up is needed to confirm such findings. | Johann Paulo Suico Guzman Leandro L Resurreccion III Marcus Lester R Suntay Renato G Bernaldez | 2019 | World Journal of Pediatric Surgery2019，2，2： | 2 |
