1Acute calculous cholecystitis: Review of current best practices显示文摘Acute calculous cholecystitis(ACC) is the most frequent complication of cholelithiasis and represents one-third of all surgical emergency hospital admissions, many aspects of the disease are still a matter of debate. Knowledge of the current evidence may allow the surgical team to develop practical bedside decision-making strategies, aiming at a less demanding procedure and lower frequency of complications. In this regard, recommendations on the diagnosis supported by specific criteria and severity scores are being implemented, to prioritize patients eligible for urgency surgery. Laparoscopic cholecystectomy is the best treatment for ACC and the procedure should ideally be performed within 72h. Early surgery is associated with better results in comparison to delayed surgery. In addition, when to suspect associated common bile duct stones and how to treat them when found are still debated. The antimicrobial agents are indicated for high-risk patients and especially in the presence of gallbladder necrosis. The use of broad-spectrum antibiotics and in some cases with antifungal agents is related to better prognosis. Moreover, an emerging strategy of not converting to open, a difficult laparoscopic cholecystectomy and performing a subtotal cholecystectomy is recommended by adept surgical teams. 2Role of mi RNAs and their potential to be useful as diagnostic and prognostic biomarkers in gastric cancer显示文摘Alterations in epigenetic control of gene expression play an important role in many diseases, including gastric cancer. Many studies have identified a large number of upregulated oncogenic mi RNAs and downregulated tumour-suppressor mi RNAs in this type of cancer. In this review, we provide an overview of the role of mi RNAs, pointing to their potential to be useful as diagnostic and/or prognostic biomarkers in gastric cancer. 4Open abdomen in gastrointestinal surgery:Which technique is the best for temporary closure during damage control?显示文摘AIM To compare the 3 main techniques of temporary closure of the abdominal cavity,vacuum assisted closure(vacuum-assisted closure therapy- VAC),Bogota bag and Barker technique,in damage control surgery.METHODS After systematic review of the literature,33 articles were selected to compare the efficiency of the three procedures.Criteria such as cost,infections,capacity of reconstruction of the abdominal wall,diseases associated with the technique,among others were analyzed.RESULTS The Bogota bag and Barker techniques present as advantage the availability of material and low cost,what is not observed in the VAC procedure.The VAC technique is the most efficient,not only because it reduces the tension on the boarders of the lesion,but also removes stagnant fluids and debris and acts at cellular level increasing cell proliferation and division.Bogota bag presents the higher rates of skin laceration and evisceration,greater need for a stent for draining fluids and wash-ups,higher rates of intestinal adhesion to the abdominal wall.The Barker technique presents lack of efficiency in closing the abdominal wall and difficulty on maintaining pressure on the dressing.The VAC dressing can generate irritation and dermatitis when the drape is applied,in addition to pain,infection and bleeding,as well as toxic shock syndrome,anaerobic sepsis and thrombosis.CONCLUSION The VAC technique,showed to be superior allowing a better control of liquid on the third space,avoiding complications such as fistula with small mortality,low infection rate,and easier capability on primary closure of the abdominal cavity.Marcelo A F Ribeiro Junior Emily Alves Barros Sabrina Marques de Carvalho Vinicius Pereira Nascimento José Cruvinel Neto Alexandre Zanchenko Fonseca 2016World Journal of Gastrointestinal Surgery2016,8,8:9
5Comparison between endoscopic sphincterotomy vs endoscopic sphincterotomy associated with balloon dilation for removal of bile duct stones:A systematic review and meta-analysis based on randomized controlled trials显示文摘AIM To compare gallstones removal rate and incidence of bleeding, pancreatitis, use of mechanical lithotripsy, cholangitis and perforation between isolated sphincterotomy vs sphincterotomy associated with balloon dilation of papilla in choledocholithiasis through the meta-analysis of randomized clinical trials. METHODS We conducted a systematic review according to the PRISMA guidelines. Literature search was restricted to randomized controlled trials(RCTs) on Med Line, Cochrane Library, LILACS, and EMBASE database platforms in July 2017. The manual search included references of retrieved articles. We extracted data focusing on outcomes: The primary endpoint was the stones removal rate; Secondary endpoints were rates of pancreatitis, bleeding, use of mechanical lithotripsy(ML), perforation and cholangitis. RESULTS Eleven RCTs with 1824 patients were included. EST was associated with more post-endoscopic retrograde cholangiopancreatography(ERCP) bleeding [FE RD-0.02, CI(-0.03,-0.00), I2 = 33%, P = 0.05] and more need of mechanical lithotripsy in general [RE RD-0.16, CI(-0.25,-0.06), I2 = 90%, P = 0.002] and in subgroup analysis of stones greater than 15 mm [RE RD-0.20, CI(-0.38,-0.02), I2 = 82%, P = 0.003]. Incidence of pancreatitis [FE RD-0.01, CI(-0.03, 0.01), I2 = 0, P = 0.36], cholangitis [FE RD-0.00, CI(-0.01, 0.01), I2 =0, P = 0.97] and perforation [FE RD-0.01, CI(-0.01, 0.00), I2 = 0, P = 0.23] was similar between the groups as well as similar stone removal rates in general [FE RD-0.01, CI(-0.01, 0.04), I2 = 0, P = 0.23] and pooled analysis of stones greater than 15 mm [FE RD-0.02, CI(-0.02, 0.07), I2 = 11%, P = 0.31]. CONCLUSION Through meta-analysis of randomized clinical trials we found that isolated sphincterotomy was associated with more post-ERCP bleeding and more need for mechanical lithotripsy. 7Mercury in aquatic fauna contamination:A systematic review on its dynamics and potential health risks显示文摘Mercury is an important pollutant,released into aquatic ecosystems both naturally and by anthropogenic action.This element is transferred to aquatic organisms in different ways,causing potential health risks.In addition,mercury can be accumulated by humans,especially through the consumption of contaminated food.This systematic review aims to present mercury pathways,the major routes through which this element reaches the aquatic environment and its transformations until becoming available to living animals,leading to bioaccumulation and biomagnification phenomena.The key biotic and abiotic factors affecting such processes,the impact of mercury on animal and human health and the issue of seafood consumption as a source of chronic mercury contamination are also addressed.A total of 101 articles were retrieved from a standardized search on three databases(PubMed,Emabse,and Web of Science),in addition to 28 other studies not found on these databases but considered fundamental to this review(totaling 129 articles).Both biotic and abiotic factors display fundamental importance in mediating mercurial dynamics,i.e.,muscle tropism,and salinity,respectively.Consequently,mercurial contamination in aquatic environments affects animal health,especially the risk of extinction species and also on human health,with methylmercury the main mercury species responsible for acute and chronic symptomatology.Paloma de Almeida Rodrigues Rafaela Gomes Ferrari Luciano Neves dos Santos Carlos Adam Conte Junior 2019Journal of Environmental Sciences2019,31,10:6
8Transcranial direct current stimulation in psychiatric disorders显示文摘The interest in non-invasive brain stimulation techniques is increasing in recent years. Among these techniques, transcranial direct current stimulation(t DCS) has been the subject of great interest among researchers because of its easiness to use, low cost, benign profile of side effects and encouraging results of research in the field. This interest has generated several studies and randomized clinical trials, particularly in psychiatry. In this review, we provide a summary of the development of the technique and its mechanism of action as well as a review of the methodological aspects of randomized clinical trials in psychiatry, including studies in affective disorders, schizophrenia, obsessive compulsive disorder, child psychiatry and substance use disorder. Finally,we provide an overview of t DCS use in cognitive enhancement as well as a discussion regarding its clinical use and regulatory and ethical issues. 10Modeling novel methodologies for unmanned aerial systems–Applications to the UAS-S4 Ehecatl and the UAS-S45 Bálaam显示文摘The rising demand for Unmanned Aerial Systems(UASs) to perform tasks in hostile environments has emphasized the need for their simulation models for the preliminary evaluations of their missions. The efficiency of the UAS model is directly related to its capacity to estimate its flight dynamics with minimum computational resources. The literature describes several techniques to estimate accurate aircraft flight dynamics. Most of them are based on system identification. This paper presents an alternative methodology to obtain complete model of the S4 and S45 unmanned aerial systems. The UAS-S4 and the UAS-S45 models were divided into four sub-models, each corresponding to a specific discipline: aerodynamics, propulsion, mass and inertia, and actuator. The‘‘aerodynamic' sub-model was built using the Fderivatives in-house code, which is an improvement of the classical DATCOM procedure. The ‘‘propulsion' sub-model was obtained by coupling a two-stroke engine model based on the ideal Otto cycle and a Blade Element Theory(BET) analysis of the propeller. The ‘‘mass and the inertia' sub-model was designed utilizing the Raymer and DATCOM methodologies. A sub-model of an actuator using servomotor characteristics was employed to complete the model. The total model was then checked by validation of each submodel with numerical and experimental data. 12Intestinal anti-inflammatory activity of Ground Cherry(Physalis angulata L.)standardized CO2 phytopharmaceutical preparation显示文摘AIM To investigate the effects of Ground Cherry(Physalis angulata L.)standardized supercritical CO_2 extract in trinitrobenzenesulphonic acid(TNBS)model of rat intestinal inflammation.METHODS The animals were divided into groups that received vehicle or P.angulata extract(PACO_2)orally at the doses 25,50 and 100 mg/kg daily by 5 d before TNBS damage.Protective effects of PACO_2 were assessed by macroscopic analysis,biochemical determinations of the levels of myeloperoxidase(MPO),alkaline phosphatase(ALP),glutathione and cytokines(such as INF-γ,IL-1β,IL-6,IL-10 and TNF-α),gene expression evaluation(including Hsp70,heparanase,NF-κB,mitogenactivated protein kinases(Mapk)1,3,6 and 9,and the mucins genes Muc 1,2,3 and 4)and histopathological studies using optical,and electronic(transmission and scanning)microscopy.RESULTS PACO2 extract promoted a significant reduction in MPO and ALP activities,reducing oxidative stress and neutrophil infiltration.These effects were accompanied by significant reduction of colonic levels of IFN-γand IL-6 and down-regulation of heparanase,Hsp70,Mapk3,Mapk9,Muc1 and Muc2 genes expression when compared with TNBS-control animals.In addition,protective effects were also evidenced by reduced neutrophil infiltration,recovery of cell architecture and replacement of mucin by histopathological and ultrastructural analysis.CONCLUSION Physalis angulata supercritical CO2 extract is an intestinal anti-inflammatory product that modulates oxidative stress,immune response and expression of inflammatory mediators,with potentially utility for treating inflammatory bowel disease.luiz domingues almeida junior ana elisa valencise quaglio celso acácio rodrigues de almeida costa Luiz Claudio Di Stasi 2017World Journal of Gastroenterology2017,23,24:4
13一种基于SGX的轻量Fabric链码可信执行环境构建方法显示文摘Hyperledger Fabric是一个开源分布式账本平台,其不仅拥有公有链防篡改、分布式记账的特点,还具有身份识别、数据保密、低延迟、高吞吐率等优点。传统Fabric架构中的链码缺乏安全执行环境,其容器执行环境会带来隐私泄露风险,而现有的智能合约隐私保护方案无法适用于Go语言链码架构,且性能开销较大。因此,文章提出一种基于SGX的轻量Fabric链码可信执行环境构建方法及E-Fabric架构,搭建支持原生Go语言的可信镜像和容器,为链码创建可信执行环境,并通过远程认证协议验证链码是否可信。理论评估和数据测试结果表明,SGX Enclave的构建会适当增加开销,与原Fabric架构相比,E-Fabric的延迟升高了8%左右,吞吐率下降了4%左右,但整体性能达到原网络的94%,并且具有较小的可信计算基和更好的安全性。KELEKET GOMA Christy Junior Yannick 易文哲 王鹃 2022信息网络安全2022,,7:3

15Origin and evolution of new exons in the rodent zinc fingerprotein 39 gene显示文摘The origin of new structures and functions is an important process in evolution. In the past decades, we have obtained some preliminary knowledge of the origin and evo- lution of new genes. However, as the basic unit of genes, the origin and evolution of exons remain unclear. Because young exons retain the footprints of origination, they can be good materials for studying origin and evolution of new exons. In this paper, we report two young exons in a zinc finger protein gene of rodents. Since they are unique sequences in mouse and rat genome and no homologous sequences were found in the orthologous genes of human and pig, the young exons might originate after the divergence of primates and rodents through exonization of intronic sequences. Strong positive selection was detected in the new exons between mouse and rat, suggesting that these exons have undergone significant functional divergence after the separation of the two species. On the other hand, population genetics data of mouse dem- onstrate that the new exons have been subject to functional constraint, indicating an important function of the new exons in mouse. Functional analyses suggest that these new exons encode a nuclear localization signal peptide, which may me- diate new ways of nuclear protein transport. 16Physiological Effect of Kinetin on the Photosynthetic Apparatus and Antioxidant Enzymes Activities During Production of Anthurium显示文摘The results observed in the literature raise the hypothesis according to which cytokinin plays important roles in photosynthetic metabolisms and antioxidant enzymes. Thus, the study aimed to evaluate the effect of foliar application of the isolated cytokinin kinetin at the production cycle, seeking to analyze its effects on enzyme activity and photosynthetic parameters. The plants treated with CK presented reduction of leaf CO_2 assimilation rate(Pn) and stomatal conductance(Gs), while that transpiration rate(Tr) was unaffected. The internal CO_2 concentrations decreased with the increase in cytokinin levels, but were maintained under CK 50 mg·L^(-1). The plants treated with CK 75 mg·L^(-1) was verified higher carboxylation efficiency(Pn/Ci), which was associated to values of CO_2 assimilation and transpiration unaltered. Apparent electron transport rate showed variations in the concentration of 25 mg·L^(-1). Considering the study of enzyme activity, on the other hand, it cannot be stated that kinetin has an effective action in delaying oxidative damage. It presents mixed results, since an efficiency in the application of cytokinin was not observed, presenting induction levels of ascorbate peroxidase activity. 17Detection of anti-Helicobacter pyloriantibodies in serum and duodenal fluid in peptic gastroduodenal disease显示文摘AIM: To study the diagnosis of He/icobacterpy/ori(H pylori) infection through the determination of serum levels of anti-H pylori IgG and IgA antibodies, and the levels of anti- H pylori IgA antibodies in duodenal fluid. METHODS: Data were collected from 93 patients submitted to upper digestive endoscopy due to dyspeptic symptoms. The patients were either negative (group A) or positive (group B) to Hpyloriby means of both histological detection and urease tests. Before endoscopy, peripheral blood was collected for the investigation of anti-H pylori IgG and IgA antibodies. To perform the urease test, biopsies were obtained from the gastric antrum. For the histological evaluation, biopsies were collected from the gastric antrum (greater and lesser curvatures) and the gastric body. Following this, duodenal fluid was collected from the first and second portions of the duodenum. For the serological assaying of anti-Hpy/oriIgG and IgA, and anti-Hpy/oriIgA in duodenal fluids, the ELISA method was utilized. RESULTS: The concentration of serum IgG showed sensitivity of 64.0%, specificity of 83.7%, positive predictive value of 82.0%, negative predictive value of 66.6% and accuracy of 73.1% for the diagnosis of H pyloriinfection. For the same purpose, serum IgA showed sensitivity of 72.0%, specificity of 65.9%, positive predictive value of 72.0%, negative predictive value of 67.4% and accuracy of 69.8%. If the serological tests were considered together, i.e. when both were positive or negative, the accuracy was 80.0%, sensitivity was 86.6%, specificity was 74.2%, positive predictive value was 74.2% and negative predictive value was 86.6%. When values obtained in the test for detecting IgA in the duodenal fluid were analyzed, no significant difference (P=0.43) was observed between the values obtained from patients with or without Hpyloriinfection. CONCLUSION: The results of serum IgG and IgA tests for H pylori detection when used simultaneously, are more efficient in accuracy, sensitivity and negative predictive value, than those when used alone. 18Starch Digestibility and Physicochemical and Cooking Properties of Irradiated Rice Grains显示文摘This study evaluated the starch digestibility, physicochemical properties and cooking characteristics of polished rice(varieties IRGA417 and IAC202) subjected to the doses of 0(as the control), 1, 2 and 5 k Gy of gamma radiation. The highest dose decreased the apparent amylose content, peak viscosity, water absorption and volume expansion. Irradiation increased the solid loss by 119% and 187% for IAC202 and IRGA417, respectively, when comparing the higher dose with the control. For IAC202, irradiation decreased the rapidly digestible starch and increased the slowly digestible starch(SDS) and resistant starch(RS). IRGA417 showed an elevation of SDS and a reduction in RS.
