1Tumor size as a prognostic factor in patients with advanced gastric cancer in the lower third of the stomach显示文摘AIM: To explore the impact of tumor size on outcomes in patients with advanced gastric cancer in the lower third of the stomach. METHODS: We retrospectively analyzed the clinical records of 430 patients with advanced gastric cancer in the lower third of the stomach who underwent distal subtotal gastrectomy and D2 lymphadenectomy in our hospital from January 1998 to June 2004. Receiver-operating characteristic (ROC) curve analysis was used to determine the appropriate cutoff value for tumor size, which was measured as maximum tumor diameter. Based on this cutoff value, patients were divided into two groups: those with large-sized tumors (LSTs) and those with small-sized tumors (SSTs). The correlations between other clinicopathologic factors and tumor size were investigated, and the 5-year overall survival (OS) rate was compared between the two groups. Potential prognostic factors were evaluated by univariate KaplanMeier survival analysis and multivariate Cox's propor-tional hazard model analysis. The 5-year OS rates in the two groups were compared according to pT stage and pN stage. RESULTS: The 5-year OS rate in the 430 patients with advanced gastric cancer in the lower third of the stomach was 53.7%. The mean ± SD tumor size was 4.9 ± 1.9 cm, and the median tumor size was 5.0 cm. ROC analysis indicated that the sensitivity and specificity results for the appropriate tumor size cutoff value of 4.8 cm were 80.0% and 68.2%, respectively (AUC=0.795, 95%CI: 0.751-0.839, P=0.000). Using this cutoff value, 222 patients (51.6%) had LSTs (tumor size ≥ 4.8 cm) and 208 (48.4%) had SSTs (tumor size<4.8 cm). Tumor size was significantly correlated with histological type (P=0.039), Borrmann type (P=0.000), depth of tumor invasion (P=0.000), lymph node metastasis (P=0.000), tumor-nodes metastasis stage (P=0.000), mean number of metastatic lymph nodes (P=0.000) and metastatic lymph node ratio (P=0.000). Patients with LSTs had a significantly lower 5-year OS rate than those with SSTs (37.1% vs 63.3%, P=0.000). Univariate analysis showed that depth of tumor invasion (c 2=69.581, P=0.000), lymph node metastasis (c 2=138.815, P=0.000), tumor size (c 2=78.184, P=0.000) and metastatic lymph node ratio (c 2=139.034, P=0.000) were significantly associated with 5-year OS rate. Multivariate analysis revealed that depth of tumor invasion (P=0.000), lymph node metastasis (P=0.019) and tumor size (P=0.000) were independent prognostic factors. Gastric cancers were divided into 12 subgroups: pT2N0; pT2N1; pT2N2; pT2N3; pT3N0; pT3N1; pT3N2; pT3N3; pT4aN0; pT4aN1; pT4aN2; and pT4aN3. In patients with pT2-3N3 stage tumors and patients with pT4a stage tumors, 5-year OS rates were significantly lower for LSTs than for SSTs (P<0.05 each), but there were no significant differences in the 5-year OS rates in LST and SST patients with pT23N0-2 stage tumors (P > 0.05). 3Field Measurements of Influence of Sand Transport Rate on Structure of Wind-sand Flow over Coastal Transverse Ridge显示文摘The structure of wind-sand flow under different total sand transport rates was measured with field vertical anemometer and sand trap on the crest of typical coastal transverse ridge in Changli Gold Coast of Hebei Province, which is one of the most typical coastal aeolian distribution regions in China and famous for the tall and typical coastal transverse ridges. The measurement results show that, on the conditions of approximate wind velocities and same surface materials and environments, some changes happen to the structure of wind-sand flow with the increase of total sand transport rate on the crest of coastal transverse ridge. First, the sand transport rates of layers at different heights in the wind-sand flow increase, with the maximum increase at the height layer of 4-8cm. Second, the ratios of sand trans-port rates of layers at different heights to total sand transport rate decrease at the low height layer (0-4cm), but increase at the high height layer (4-60cm). Third, the distribution of the sand transport rate in the wind-sand flow can be expressed by an exponential function at the height layer of 0-40cm, but it changes from power function model to ex-ponential function model in the whole height layer (0-60cm) and changes into polynomial function model at the height layer of 40-60cm with the increase of total sand transport rate. 4Sequential organ failure assessment score is superior to other prognostic indices in acute pancreatitis显示文摘BACKGROUND Acute pancreatitis(AP)is a common surgical condition,with severe AP(SAP)potentially lethal.Many prognostic indices,including;acute physiology and chronic health evaluation II score(APACHE II),bedside index of severity in acute pancreatitis(BISAP),Glasgow score,harmless acute pancreatitis score(HAPS),Ranson’s score,and sequential organ failure assessment(SOFA)evaluate AP severity and predict mortality.AIM To evaluate these indices'utility in predicting severity,intensive care unit(ICU)admission,and mortality.METHODS A retrospective analysis of 653 patients with AP from July 2009 to September 2016 was performed.The demographic,clinical profile,and patient outcomes were collected.SAP was defined as per the revised Atlanta classification.Values for APACHE II score,BISAP,HAPS,and SOFA within 24 h of admission were retrospectively obtained based on laboratory results and patient evaluation recorded on a secure hospital-based online electronic platform.Data with<10%missing data was imputed via mean substitution.Other patient information such as demographics,disease etiology,and patient outcomes were also derived from electronic medical records.RESULTS The mean age was 58.7±17.5 years,with 58.7%males.Gallstones(n=404,61.9%),alcohol(n=38,5.8%),and hypertriglyceridemia(n=19,2.9%)were more common aetiologies.81(12.4%)patients developed SAP,20(3.1%)required ICU admission,and 12(1.8%)deaths were attributed to SAP.Ranson’s score and APACHE-II demonstrated the highest sensitivity in predicting SAP(92.6%,80.2%respectively),ICU admission(100%),and mortality(100%).While SOFA and BISAP demonstrated lowest sensitivity in predicting SAP(13.6%,24.7%respectively),ICU admission(40.0%,25.0%respectively)and mortality(50.0%,25.5%respectively).However,SOFA demonstrated the highest specificity in predicting SAP(99.7%),ICU admission(99.2%),and mortality(98.9%).SOFA demonstrated the highest positive predictive value,positive likelihood ratio,diagnostic odds ratio,and overall accuracy in predicting SAP,ICU admission,and mortality.SOFA and Ranson’s score demonstrated the highest area under receiver-operator curves at 48 h in predicting SAP(0.966,0.857 respectively),ICU admission(0.943,0.946 respectively),and mortality(0.968,0.917 respectively).CONCLUSION The SOFA and 48-h Ranson’s scores accurately predict severity,ICU admission,and mortality in AP,with more favorable statistics for the SOFA score.Thomas Zheng Jie Teng Jun Kiat Thaddaeus Tan Samantha Baey Sivaraj K Gunasekaran Sameer P Junnarkar Jee Keem Low Cheong Wei Terence Huey Vishal G Shelat 2021World Journal of Critical Care Medicine2021,10,6:9

12Seasonal variations of phytoplankton diversity in the Coleroon coastal waters,southeast coast of India显示文摘The results of an investigation carried out during June 2005 to May 2007 on physico-chemical parameters,species composition and community structure of phytoplankton including Chlorophyll a(Chl-a) at the Coleroon coastal waters(Southeast coast of India) are reported.Air and surface water temperatures(°C) varied from 25.1 to 30.1 and from 24.5 to 28.5 respectively.Salinity values varied from 6 to 28.5 and the pH ranged between 7.0 and 8.3.Variation in dissolved oxygen content was from 3.1 to 7.5 mg/dm-3 while the light extinction coefficient values(LEC) ranged between 3.1 and 10.1 cm.The ranges of inorganic nutrients(μmol/dm-3) viz.,nitrate,nitrite,phosphate and silicate were 10.1-23.4,1.2-8.9,0.2-3.1 and 55-125,respectively.The ranges of Chlorophyll a(μg/dm-3) values was 2.0-7.5.Presently,124 phytoplankton species representing different classes viz:Bacillariophyceae(77),Dinophyceae(19),Cyanophyceae(15),Chlorophyceae(10) and Chrysophyceae(3) were recorded.The phytoplankton cell abundance varied from 0.290 to 111.662 cells/cm-3,with peak diversity(3.38 bits/ind.) during summer season.The maximum abundance was found during summer season coinciding with the stable hydrographical conditions.The seasonal distribution and abundance of phytoplankton are discussed in relation to hydrographical parameters.Canonical Correspondence Analysis(CCA) was applied in this paper for discriminating environmental factors having effect on phytoplankton community at species level.Coleroon coastal water is subjected to long term fluctuations in physico-chemical parameters depending upon the seasonal tidal amplitude and freshwater influx resulting in a continuous exchange of organic,inorganic,plant and animal matters.Thillai Rajasekar K Rajkumar M SUN Jun Ashok Prabu V Perumal P 2010Acta Oceanologica Sinica2010,29,5:3

14Ultrafast photonic PCR显示文摘Nucleic acid amplification and quantification via polymerase chain reaction(PCR)is one of the most sensitive and powerful tools for clinical laboratories,precision medicine,personalized medicine,agricultural science,forensic science and environmental science.Ultrafast multiplex PCR,characterized by low power consumption,compact size and simple operation,is ideal for timely diagnosis at the point-of-care(POC).Although several fast/ultrafast PCR methods have been proposed,the use of a simple and robust PCR thermal cycler remains challenging for POC testing.Here,we present an ultrafast photonic PCR method using plasmonic photothermal light-to-heat conversion via photon–electron–phonon coupling.We demonstrate an efficient photonic heat converter using a thin gold(Au)film due to its plasmon-assisted high optical absorption(approximately 65%at 450 nm,the peak wavelength of heat source light-emitting diodes(LEDs)).The plasmon-excited Au film is capable of rapidly heating the surrounding solution to over 150℃ within 3 min.Using this method,ultrafast thermal cycling(30 cycles;heating and cooling rate of 12.7960.93℃ s^(-1) and 6.660.29℃ s^(-1),respectively)from 55℃(temperature of annealing)to 95℃(temperature of denaturation)is accomplished within 5 min.Using photonic PCR thermal cycles,we demonstrate here successful nucleic acid(λ-DNA)amplification.Our simple,robust and low cost approach to ultrafast PCR using an efficient photonic-based heating procedure could be generally integrated into a variety of devices or procedures,including on-chip thermal lysis and heating for isothermal amplifications.Jun Ho Son Byungrae Cho SoonGweon Hong Sang Hun Lee Ori Hoxha Amanda J Haack Luke P Lee 2015Light(Science & Applications)2015,4,1:2
15Fluorescence and SEM correlative microscopy for nanomanipulation of subcellular structures显示文摘Nanomanipulation under scanning electron microscopy(SEM)enables direct interactions of a tool with a sample.We recently developed a nanomanipulation technique for the extraction and identification of DNA contained within sub-nuclear locations of a single cell nucleus.In nanomanipulation of sub-cellular structures,a key step is to identify targets of interest through correlating fluorescence and SEM images.The DNA extraction task must be conducted with low accelerating voltages resulting in low imaging resolutions.This is imposed by the necessity of preserving the biochemical integrity of the sample.Such poor imaging conditions make the identification of nanometer-sized fiducial marks difficult.This paper presents an affine scale-invariant feature transform(ASIFT)based method for correlating SEM images and fluorescence microscopy images.The performance of the image correlation approach under different noise levels and imaging magnifications was quantitatively evaluated.The optimal mean absolute error(MAE)of correlation results is 68634 nm under standard conditions.Compared with manual correlation by skilled operators,the automated correlation approach demonstrates a speed that is higher by an order of magnitude.With the SEM-fluorescence image correlation approach,targeted DNA was successfully extracted via nanomanipulation under SEM conditions.Zheng Gong Brandon K Chen Jun Liu Chao Zhou Dave Anchel Xiao Li Ji Ge David P Bazett-Jones Yu Sun 2014Light(Science & Applications)2014,3,1:2
