| 1 | 陶瓷球头和金属球头配伍聚乙烯臼杯的20年长期对比磨损研究显示文摘背景关节承载面是假体磨损过程中的重要假体相关因素。目前已知聚乙烯磨损是全髋关节置换术后髋臼和股骨假体组件无菌性松动的主要原因。方法本项前瞻性研究中,共随机挑选了80例同时期患者共93个髋关节接受了非骨水泥髋关节置换术,使用钛涂层的RM臼杯和CLS股骨柄。80例髋关节使用陶瓷球头,13例使用钴铬钼合金球头,所有股骨头直径均为32 mm。所有患者平均年龄为52岁,研究平均随访时间为19.3年。所有患者均获得随访。结果随访时使用EBRA系统对髋臼假体组件的磨损、倾斜和前倾角进行影像学评估。其中55例髋关节进行了磨损率的确认。翻修病例的平均磨损率为0.191 mm年/,而未翻修病例的平均磨损率为0.105 mm年/(P=0.041)。钴铬钼合金组平均磨损率为0.190 mm年/,陶瓷组平均磨损率为0.107 mm年/(P=0.025)。钴铬钼合金组的翻修率(13例中6例)明显高于陶瓷组(80例中11例)。股骨球头磨损率与其材料(P=0.025)、股骨柄周围骨溶解(P=0.003)、臼杯骨溶解(P=0.011)、臼杯翻修(P=0.041)以及臼杯倾斜(P=0.012)呈正性相关。结论本长期研究表明,在20年的时间里,陶瓷股骨头与钴铬钼合金股骨头相比造成的线性磨损更少,从而显著减少骨溶解的发生以及任一假体部件的翻修。 | Martin Ihle Sabine Mai Werner E.
