| 1 | 全节段经椎弓根螺纹钉固定融合术治疗老年脊柱侧凸性腰椎管狭窄症显示文摘目的系统评价全节段经椎弓根螺钉固定融合术治疗老年脊柱侧凸性腰椎管狭窄症(SSS)的临床疗效。方法 2004年1月至2012年1月该院收治的老年SSS患者113例,根据经椎弓根固定与融合方式分为A组(短节段固定组)45例,B组(全节段固定组)68例,评估手术疗效及4年随访观察。主要观察指标:①减压节段椎体前缘高度、椎间隙前缘与后缘高度;②脊柱矢状位与冠状位Cobbs角;③腰腿痛情况JOA评分;④Os-westry功能障碍指数;⑤术后并发症。结果 93例患者获得4年以上随访,所有患者随访期内矫正度数、椎间隙高度无丢失;全部获得植骨融合,平均融合时间3.5个月。术后两组脊柱侧凸Cobb's角明显改善。矢状位上腰椎前凸得到不同程度的重建,恢复并且保持了腰椎生理前凸。A、B组疼痛缓解率分别为33%、75%,临床症状改善率75%、80%。JOA评分术前11~13分,术后4年平均26分。95%患者自诉生活质量得到明显改善,恢复了生活自理能力。4年内A组6例X线片检查出现内植物松动,6例出现继发性代偿侧凸;而B组无1例发生。B组术后骨吸收现象较A组明显(P<0.05)。结论脊柱后路全节段固定融合技术手术疗效优于脊柱短节段固定融合技术,尤其在控制脊柱畸形进展、防止继发性代偿侧凸、维持后期脊柱稳定性,减轻症状等方面居优势;但是远期脊柱固定区域内的应力遮挡效应应引起注意。 | 李英谱 包晓群 杨小玉 李雪鹏 王永明 Rune Hedlund | 2012 | 中国老年学杂志2012，32，24： | 7 |
| 2 | 斜齿轮副轮齿啮合刚度变量计算用的参数化的数值模型显示文摘在齿轮传动设计中的最通常的问题之一是确定齿轮几何参数的最佳组合。这些参数应该有可能改变效能,以及与齿轮啮合刚度变化有关改变激励和振动分析。而齿轮啮合刚度和传动误差的精确预测要求有效的数值方法,本文为斜齿轮设计目的采用啮合刚度变化估算激励开发了参数化数值模型。该模型采用线性有限元(FE)方法计算轮齿变形,包括基础柔性。该方法用结构分析综合了赫茨(Hertzian)接触从而避免了大量的FE网格。因此,在时间和频率范围内求得了啮合刚度的变量,它是由测量范围比较给出的弹性。计算表明足够少的单元数满足估算啮合刚度的变量,它使设计趋向和计算方面达到合理的协调。 | J Hedlund A Lelical | 2012 | 传动技术2012，26，1： | 4 |
