1Transjugular intrahepatic portosystemic shunt for the management of acute variceal hemorrhage显示文摘Acute variceal hemorrhage,a life-threatening condition that requires a multidisciplinary approach for effective therapy,is defined as visible bleeding from an esophageal or gastric varix at the time of endoscopy,the presence of large esophageal varices with recent stigmata of bleeding,or fresh blood visible in the stomach with no other source of bleeding identified.Transfusion of blood products,pharmacological treatments and early endoscopic therapy are often effective;however,if primary hemostasis cannot be obtained or if uncontrollable early rebleeding occurs,transjugular intrahepatic portosystemic shunt(TIPS)is recommended as rescue treatment.The TIPS represents a major advance in the treatment of complications of portal hypertension.Acute variceal hemorrhage that is poorly controlled with endoscopic therapy is generally well controlled with TIPS,which has a 90%to 100%success rate.However,TIPS is associated with a mortality of 30%to 50%in such a setting.Emergency TIPS should be considered early in patients with refractory variceal bleeding once medical treatment and endoscopic sclerotherapy failure,before the clinical condition worsens.Furthermore,admission to specialized centers is mandatory in such a setting and regional protocols are essential to be organized effectively.This review article discusses initial management and then focuses on the specific role of TIPS as a primary therapy to control acute variceal hemorrhage,particularly as a rescue therapy following failure of endoscopic approaches.Romaric Loffroy Louis Estivalet Violaine Cherblanc Sylvain Favelier Pierre Pottecher Samia Hamza Anne Minello Patrick Hillon Pierre Thouant Pierre-Henri Lefevre Denis Krausé Jean-Pierre Cercueil 2013World Journal of Gastroenterology2013,19,37:25
2BAS-ADAM:An ADAM Based Approach to Improve the Performance of Beetle Antennae Search Optimizer显示文摘In this paper,we propose enhancements to Beetle Antennae search(BAS)algorithm,called BAS-ADAIVL to smoothen the convergence behavior and avoid trapping in localminima for a highly noin-convex objective function.We achieve this by adaptively adjusting the step-size in each iteration using the adaptive moment estimation(ADAM)update rule.The proposed algorithm also increases the convergence rate in a narrow valley.A key feature of the ADAM update rule is the ability to adjust the step-size for each dimension separately instead of using the same step-size.Since ADAM is traditionally used with gradient-based optimization algorithms,therefore we first propose a gradient estimation model without the need to differentiate the objective function.Resultantly,it demonstrates excellent performance and fast convergence rate in searching for the optimum of noin-convex functions.The efficiency of the proposed algorithm was tested on three different benchmark problems,including the training of a high-dimensional neural network.The performance is compared with particle swarm optimizer(PSO)and the original BAS algorithm.Ameer Hamza Khan Xinwei Cao Shuai Li Vasilios N.Katsikis Liefa Liao 2020IEEE/CAA Journal of Automatica Sinica2020,7,2:20

6Pancreatic injury in patients with septic shock: A literature review显示文摘Sepsis and septic shock are life threatening condition associated with high mortality rate in critically-ill patients. This high mortality is mainly related to the inadequacy between oxygen delivery and cellular demand leading to the onset of multiorgan dysfunction. Whether this multiorgan failure affect the pancreas is not fully investigated. In fact, pancreatic injury may occur because of ischemia, overwhelming inflammatory response, oxidative stress, cellular apoptosis and/or metabolic derangement. Increased serum amylase and/or lipase levels are common in patients with septic shock. However, imaging test rarely reveal significant pancreatic damage. Whether pancreatic dysfunction does affect the prognosis of patients with septic shock or not is still a matter of debate. In fact, only few studies with limited sample size assessed the clinical relevance of the pancreatic injury in this group of patients. In this review, we aimed to describe the epidemiology and the physiopathology of pancreatic injury in septic shock patients, to clarify whether it requires specific management and to assess its prognostic value. Our main finding is that pancreatic injury does not significantly affect the outcome in septic shock patients. 9Hypoxia inducible factor-1 alpha stabilization for regenerative therapy in traumatic brain injury显示文摘Mild traumatic brain injury(TBI), also called concussion, initiates sequelae leading to motor deficits, cognitive impairments and subtly compromised neurobehaviors. While the acute phase of TBI is associated with neuroinflammation and nitroxidative burst, the chronic phase shows a lack of stimulation of the neurorepair process and regeneration. The deficiency of nitric oxide(NO), the consequent disturbed NO metabolome, and imbalanced mechanisms of S-nitrosylation are implicated in blocking the mechanisms of neurorepair processes and functional recovery in the both phases. Hypoxia inducible factor-1 alpha(HIF-1α), a master regulator of hypoxia/ischemia, stimulates the process of neurorepair and thus aids in functional recovery after brain trauma. The activity of HIF-1α is regulated by NO via the mechanism of S-nitrosylation of HIF-1α. S-nitrosylation is dynamically regulated by NO metabolites such as S-nitrosoglutathione(GSNO) and peroxynitrite. GSNO stabilizes, and peroxynitrite destabilizes HIF-1α. 11Tracking control of redundant manipulator under active remote center-of-motion constraints: an RNN-based metaheuristic approach显示文摘In this paper, we propose a recurrent neural network(RNN) for the tracking control of surgical robots while satisfying remote center-of-motion(RCM) constraints. RCM constraints enforce rules suggesting that the surgical tip should not go beyond the region of incision while tracking the commands of the surgeon.Violations of RCM constraints can result in serious injury to the patient. We unify the RCM constraints with the tracing control by formulating a single constrained optimization problem using a penalty-term approach.The penalty-term actively rewards the optimizer for satisfying the RCM constraints. We then propose an RNN-based metaheuristic optimization algorithm called 'Beetle Antennae Olfactory Recurrent Neural Network(BAORNN)' for solving the formulated optimization problem in real time. The proposed control framework can track the surgeon’s commands and satisfy the RCM constraints simultaneously. Theoretical analysis is performed to demonstrate the stability and convergence of the BAORNN algorithm. 12Trends in incidence and management of cancer of the ampulla of Vater显示文摘AIM:To provide trends in incidence,management and survival of cancer of the ampulla of Vater in a welldefined French population.METHODS:Data were obtained from the populationbased digestive cancer registry of Burgundy over a34-year period.Age-standardized incidence rates were computed using the world standard population.Average annual variations in incidence rates were estimated using a poisson regression.A univariate and multivariate relative survival analysis was performed.RESULTS:Age-standardized incidence rates were0.46 and 0.30 per 100000 inhabitants for men and women,respectively.Incidence rate increased from0.26(1976-1984)to 0.58(2003-2009)for men and remained stable for women.Resection for cure was performed in 48.3%of cases.This proportion was stable over the study period.Among cases with curative resection,pancreatico-duodenectomy was performed in94.0%of cases and ampullectomy in 6.0%of cases.A total of 50.8%of cancers of the ampulla of Vater were diagnosed at an advanced stage.Their proportion remained stable throughout the study period.The overall1-and 5-year relative survival rates were 60.2%and27.7%,respectively.Relative survival did not vary over time.Treatment and stage at diagnosis were the most important determinants of survival.The 5-year relative survival rate was 41.5%after resection for cure,9.5%after palliative surgery and 6.7%after symptomatic treatment.In multivariate analysis,only stage at diagnosis significantly influenced the risk of death.CONCLUSION:Cancer of the ampulla of Vater is still uncommon,but its incidence increased for men in Burgundy.Diagnosis is often made at an advanced stage,dramatically worsening the prognosis.Florian Rostain Samia Hamza Antoine Drouillard Jean Faivre Anne-Marie Bouvier C?me Lepage 2014World Journal of Gastroenterology2014,20,29:5
13Nickel ferrite spinel as catalyst precursor in the dry reforming of methane:Synthesis,characterization and catalytic properties显示文摘Dry reforming of methane by CO2 using nickel ferrite as precursor of catalysts was investigated.Nickel ferrite crystalline particles were prepared by coprecipitation of nitrates with NaOH or ammonia followed by calcination,or by hydrothermal synthesis without calcination step.The textural and structural properties were determined by a number of analysis methods,including X-ray diffraction (XRD),Raman spectroscopy and X-ray photoelectron spectroscopy (XPS),among which X-ray diffraction (XRD) was at room and variable temperatures.All synthesized oxides showed the presence of micro or nanoparticles of NiFe2O4 inverse spinel,but Fe2O3 (hematite) was also present when ammonia was used for coprecipitation.The reducibility by hydrogen was studied by temperature-programmed reduction (TPR) and in situ XRD,which showed the influence of the preparation method.The surface area (BET),particle size (Rietveld refinement),as well as surface Ni/Fe atomic ratio (XPS) and the behavior upon reduction varied according to the synthesis method.The catalytic reactivity was investigated using isopropanol decomposition to determine the acid/base properties.The catalytic performance of methane reforming with CO2 was measured with and without the pre-treatment of catalysts under H2 in 650-800 C range.The catalytic conversions of methane and CO2 were quite low but they increased when the catalysts were pre-reduced.A significant contribution of reverse water gas shift reaction accounted for the low values of H2 /CO ratio.No coking was observed as shown by the reoxidation step performed after the catalytic reactions.The possible formation of nickel-iron alloy observed during the study of reducibility by hydrogen was invoked to account for the catalytic behavior.Rafik Benrabaa Hamza Boukhlouf Axel Lfberg Annick Rubbens Rose-Nelle Vannier Elisabeth Bordes-Richard Akila Barama 2012Journal of Natural Gas Chemistry2012,21,5:5

16Surface Flood and Underground Flood in Xiangxi River Karst Basin: Characteristics, Models, and Comparisons显示文摘Xiangxi River Basin, located in western Hubei Province in central China, is a karst ridge-trough area with an inhomogeneous and complicated distribution of water resources. This paper compares the characteristics of surface and subsurface floods in this karst basin, utilizing a one-parameter Darcian model and the traditional exponential model. The observed hydrographs and inferred water components are strikingly similar for surface and subsurface floods. The Darcian model and the exponential model are based on different views of the flood generation process, with the former fitting the entire hydrograph with a single time constant, and the latter fitting only the recession limb with multiple time constants. 17Weed detection in canola fields using maximum likelihood classification and deep convolutional neural network显示文摘Herbicide use is rising globally to enhance food production,causing harm to environment and the ecosystem.Precision agriculture suggests variable-rate herbicide application based on weed densities to mitigate adverse effects of herbicides.Accurate weed density estimation using advanced computer vision techniques like deep learning requires large labelled agriculture data.Labelling large agriculture data at pixel level is a time-consuming and tedious job.In this paper,a methodology is developed to accelerate manual labelling of pixels using a two-step procedure.In the first step,the background and foreground are segmented using maximum likelihood classification,and in the second step,the weed pixels are manually labelled.Such labelled data is used to train semantic segmentation models,which classify crop and background pixels as one class,and all other vegetation as the second class.This paper evaluates the proposed methodology on high-resolution colour images of canola fields and makes performance comparison of deep learning meta-architectures like SegNet and UNET and encoder blocks like VGG16 and ResNet-50.ResNet-50 based SegNet model has shown the best results with mean intersection over union value of 0.8288 and frequency weighted intersection over union value of 0.9869.Muhammad Hamza Asad Abdul Bais 2020Information Processing in Agriculture2020,7,4:3
18Synthesis, Characterization and X-ray Structure of N-(4-bromobutanoyl-N'-(2-3-and 4-methylphenyl)thiourea显示文摘N-4-(bromobutanoyl)-N'-(o-, m- and p-tolyl)thioureas 3a, 3b and 3c respectively, were synthesized by the reaction of 4-bromobutanoylisothiocyanate with p-, m- and o-toludine. The products were characterized by IR, and NMR spectroscopic techniques. The two carbonoylthiourea isomers N-(4-bromobutanoyl)-N'-(3-methylphenyl)thiourea(3b) and N-(4-bromobutanoyl)-N'-(4-methylphenyl)thiourea(3c) were obtained in crystalline form by recrystallization from DMSO. Xray crystallographic studies showed that both compounds 3b and 3c crystallize in triclinic system with space group of P1. The molecules adopt trans-cis configuration with respect to the positions of 4-bromobutanoyl and tolyl groups respectively, against the thiono C=S bond across their C–N bonds. The configuration is attributed by the intrahydrogen bond between the carbonyl oxygen and amide hydrogen atoms. 19Concealing arbitrary objects remotely with multi-folded transformation optics显示文摘An invisibility cloak that can hide an arbitrary object external to the cloak itself has not been devised before.In this Letter,we introduce a novel way to design a remote cloaking device that makes any object located at a certain distance invisible.This is accomplished using multi-folded transformation optics to remotely generate a hidden region around the object that no field can penetrate and that does not disturb the far-field scattering electromagnetic field.As a result,any object in the hidden region can stay in position or move freely within that region and remain invisible.Our idea is further extended in order to design a remote illusion optics that can transform any arbitrary object into another one.Unlike other cloaking methods,this method would require no knowledge of the details of the object itself.The proposed multi-folded transformation optics will be crucial in the design of remote devices in a variety of contexts.Bin Zheng Hamza Ahmad Madni Ran Hao Xianmin Zhang Xu Liu Erping Li Hongsheng Chen 2016Light(Science & Applications)2016,5,1:3
20Visual outcomes in patients with open globe injuries compared to predicted outcomes using the Ocular Trauma Scoring system显示文摘AIM: To determine the visual outcomes in adult patients who sustained open globe injuries and to determine whether the visual prognosis following an eye injury in an African setting differs from the predicted outcomes according to the Ocular Trauma Score(OTS)study. A secondary aim was to establish the evisceration rate for these injuries and assess how this form of intervention affected outcomes in comparison to the OTS.·METHODS: A prospective case series of all patients admitted with open globe injuries over a two-year(July2009 to June 2011) period. Injuries were scored using the OTS and the surgical intervention was recorded. The best corrected visual acuity at three months was regarded as visual outcome.· RESULTS: There were 249 open globe injuries, of which 169 patients(169 eyes) completed the 3-month follow-up. All patients underwent primary surgery, 175(70.3%) repairs, 61(24.5%) eviscerations and 13(5.2%)other procedures. Globe eviscerations were mainly done on OTS Category 1 cases, but outcomes in this category were not found to be different from OTS outcomes.Outcomes were significantly worse in Category 2, but when the entire distribution was tested, the differences were not statistically significant. The overall association between OTS outcomes and the final visual outcomes in this study was found to be a strong(P <0.005).· CONCLUSION: Reliable information regarding the expected outcomes of eye injuries will influence management decisions and patient expectations.
