| 1 | Ultrasonically activated scalpel versus monopolar electrocautery shovel in laparoscopic total mesorectal excision for rectal cancer显示文摘AIM: To investigate the feasibility and safety of monopolar electrocautery shovel (ES) in laparoscopic total mesorectal excision (TME) with anal sphincter preservation for rectal cancer in order to reduce the cost of the laparoscopic operation, and to compare ES with the ultrasonically activated scalpel (US). METHODS: Forty patients with rectal cancer, who underwent laparoscopic TME with anal sphincter preservation from June 2005 to June 2007, were randomly divided into ultrasonic scalpel group and monopolar ES group, prospectively. White blood cells (WBC) were measured before and after operation, operative time, blood loss, pelvic volume of drainage, time of anal exhaust, visual analogue scales (VAS) and surgery-related complications were recorded. RESULTS: All the operations were successful; no one was converted to open procedure. No significant differences were observed in terms of preoperative and postoperative d 1 and d 3 WBC counts (P = 0.493, P = 0.375, P = 0.559), operation time (P = 0.235), blood loss (P = 0.296), anal exhaust time (P = 0.431), pelvicdrainage volume and VAS in postoperative d 1 (P = 0.431, P = 0.426) and d 3 (P = 0.844, P = 0.617) between ES group and US group. The occurrence of surgery-related complications such as anastomotic leakage and wound infection was the same in the two groups. CONCLUSION: ES is a safe and feasible tool as same as US used in laparoscopic TME with anal sphincter preservation for rectal cancer on the basis of the skillful laparoscopic technique and the complete understanding of laparoscopic pelvic anatomy. | 2 | Extracorporeal shock-wave versus pneumatic ureteroscopic lithotripsy in treatment of lower ureteral calculi显示文摘Aim:To compae the efficacy and complications of extracorporeal shock-wave lithotripsy (SWL) and pneumatic ureteroscopic lithotripsy(URS) in the treatment of lower ureteral calculi.Methods:From August 1997 to June 1999,210 patients with calculi in the distal third of the ureter were treated with SWL and other 180 with URS.The stones were fragmented with either HB-ESWL-V lithotripter or JML-93 pneumatic lithotripter through Wolf 7.5-9.0 Fr ureteroscope.The outcome was assessed in terms of stone clearance rate,re-treatment rate and complication incidence.Results:the stone clearance rate was 78.1% with SWL and 93.3% with URS (p<0.05).SWL had a re-treatment rate of 11.9%,vs 2.2% in the URS group (P<0.05).URS caused ureteral perforation in 3.3% of patients,while it was o with SWL (P<0.05).The differences in the incidence of other complications such as infection and stricture between the two groups were insignificant.conclusion:Though the selection of these two options depends on equipments available and the expertise of the operator,we recomment URS as the optimal treatment for distal ureteral calculi. | Zeng, GQ Zhong, WD Cai, YB Dai, QS Hu, JB Wei, HA | 2002 | Asian Journal of Andrology2002，4，4： | 17 |

Matrix metalloproteinases (MMPs) are one of the most important mediators. MMP-28, the new member of MMPs, was sequenced and identified recently. Expression of MMP-28 mRNA and protein in the cytotrophoblast cells and a choriocarcinoma cell line, JEG-3 cell, was conducted by zymography, RT-PCR and Northern blot. There is MMP-28 mRNA expression in both the cytotrophoblast cells and JEG-3 cells by RT-PCR. The activity of MMP-28 in cytotrophoblast cells was significantly weaker than that in JEG-3 (P < 0.01) by zymography. Furthermore, mRNA expression of MMP-28 was significantly stronger (P < 0.001) in JEG-3 than in human cytotrophoblast cells in a time-dependent way by Northern Blot. METHODS: The biocompatibility of PHBHHx with both rat tenocytes (rT) and human mesenchymal stem cells (hMSC) was explored by monitoring adhesive characteristics on films of varying weight/volume ratios coupled to a culture atmosphere of either 21% O2 (air) or 2% O2 (physiological normoxia). The diameter and stiffness of PHBHHx films was established using optical coherence tomography and mechanical testing, respectively. RESULTS: Film thickness correlated directly with weight/volume PHBHHx (r2 = 0.9473) ranging from 0.1 mm (0.8% weight/volume) to 0.19 mm (2.4% weight/volume). Film stiffness on the other hand displayed a biphasic response which increased rapidly at values > 1.6% weight/volume. Optimal cell attachment of rT required films of ≥ 1.6% and ≥ 2.0% weight/volume PHBHHx in 2% O2 and 21% O2 respectively. A qualitative adhesion increase was noted for hMSC in films ≥ 1.2% weight/volume, becoming significant at 2% weight/volume in 2% O2. An increase in cell adhesion was also noted with ≥ 2% weight/volume PHBHHx in 21% O2. Cell migration into films was not observed.
