1成人脊柱畸形环形微创手术矫形分期治疗方案显示文摘为降低开放手术并发症的发生风险,过去十余年人们一直致力于成人脊柱畸形(adult spinal deformity,ASD)微创手术(minimally invasive surgery,MIS)方法的研究。在众多微创术式中,环形MIS(circumferential MIS,cMIS)相对于常规开放手术,可有效发挥微创优势。我们采用最新cMIS手术方案治疗1例严重ASD患者,并对其术前评估、手术规划及优化术式选择进行了总结。术前评估包括详细病史采集、体格检查及影像学检查等。其中影像学检查有站立位全脊柱X线检查(测量患者畸形程度和脊柱骨盆参数)、MRI检查(评估椎管狭窄情况,确保无椎间盘碎片游离于椎管内),如疑有节段融合,则需行CT扫描,以观察关节面融合或椎体间融合情况,指导椎间融合器的合理应用,实现对脊柱矫形手术的有效干预。cMIS手术分两期进行,一期经斜外侧微创入路行腰椎椎间融合,融合范围包括参与测量Cobb角的所有节段。上端椎水平位于X线片及MRI中第一个正常平行椎间盘的下位椎体;远端如有局部椎管狭窄,椎体不稳、倾斜或退行性变,矢状面失衡超过10 cm或骨质疏松,则可将融合范围延长至L5~S1水平。患者一期手术结束后当天即可下地行走,术后2 d或3 d复查站立位X线片,可根据新的影像学测量参数适当调整手术方案。一期术后2~3 d开始做二期手术,对涉及的所有节段行经皮椎弓根螺钉固定,置钉同时经肌间隙显露相应的椎弓根峡部及关节突关节,对其进行去皮质操作,以实现胸椎节段的后路融合。Anand N Kong C Fessler RG 梁达轩 吴增晖 2017中国骨科临床与基础研究杂志2017,9,6:4
