| 11 | From intestinal stem cells to inflammatory bowel diseases显示文摘The pathogenesis of both entities of inflammatory bowel disease (IBD), namely Crohn's disease (CD) and ulcerative colitis (UC), is still complex and under investigation. The importance of the microbial flora in developing IBD is beyond debate. In the last few years, the focus has changed from adaptive towards innate immunity. Crohn's ileitis is associated with a deficiency of the antimicrobial shield, as shown by a reduced expression and secretion of the Paneth cell defensin HD5 and HD6, which is related to a Paneth cell differentiation defect mediated by a diminished expression of the Wnt transcription factor TCF4. In UC, the protective mucus layer, acting as a physical and chemical barrier between the gut epithelium and the luminal microbes, is thin- ner and in part denuded as compared to controls. This could be caused by a missing induction of the goblet cell differentiation factors Hath1 and KLF4 leading to immature goblet cells. This defective Paneth and goblet cell differentiation in Crohn's ileitis and UC may enablethe luminal microbes to invade the mucosa and trigger the inflammation. | 12 | Current status of adoption of no-till farming in the world and some of its main benefits显示文摘In 1999 no-tillage farming,synonymous of zero tillage farming or conservation agriculture,was adopted on about 45 million ha world wide,growing to 72 million ha in 2003 and to 111 million ha in 2009,corresponding to an growth rate of 6 million ha per annum.Fastest adoption rates have been experienced in South America where some countries are using no-tillage farming on about 70%of the total cultivated area.Opposite to countries like the USA where often fields under no-tillage farming are tilled every now and then,more than two thirds of the area under no-tillage systems in South America is permanently not tilled;in other words once adopted,the soil is never tilled again.The spread of no-tillage systems on more than 110 million ha world-wide shows the great adaptability of the systems to all kinds of climates,soils and cropping conditions.No-tillage is now being practiced from the artic circle over the tropics to about 50ºlatitude south,from sea level to 3,000 m altitude,from extremely rainy areas with 2,500 mm a year to extremely dry conditions with 250 mm a year.No-till farming offers a way of optimizing productivity and ecosystem services,offering a wide range of economic,environmental and social benefits to the producer and to the society.At the same time,no-till farming is enabling agriculture to respond to some of the global challenges associated with climate change,land and environmental degradation,and increasing cost of food,energy and production inputs.The wide recognition of no-till farming as a truly sustainable system should ensure the spread of the no-till technology and the associated practices of organic soil cover and crop rotation,as soon as the barriers to its adoption have been overcome,to areas where adoption is currently still low.The widespread adoption globally also shows that no-tillage farming cannot any more be considered a temporary fashion or a craze;instead largely through farmers’own effort,the system has established itself as a farming practice and a different way of thinking about sustainable agro-ecosystem management that can no longer be ignored by scientists,academics,extension workers,farmers at large as well as equipment and machine manufacturers and politicians. | Rolf Derpsch Theodor Friedrich Amir Kassam Li Hongwen | 2010 | International Journal of Agricultural and Biological Engineering2010,3,1: | 18 |
| 13 | Mineral and bone disorder and management in the China Dialysis Outcomes and Practice Patterns Study显示文摘Background:Despite a growing population of patients starting hemodialysis in China,little is known about markers of mineral bone disease(MBD)and their management.We present data on prevalence and correlates of hypocalcemia,hyperphosphatemia,and secondary hyperparathyroidism from the China Dialysis Outcomes and Practice Patterns Study(DOPPS),with evaluation of whether these laboratory markers triggered changes in management.Methods:We compared the frequency of measurement and prevalence of poor control of MBD markers in China DOPPS with other DOPPS regions.We also used generalized estimating equations to assess correlates of MBD markers,and separate models to assess predictors of vitamin D and phosphate binder prescriptions in the China DOPPS.Results:Severe hyperphosphatemia(>7 mg/dL)and secondary hyperparathyroidism(>600 pg/mL)were common(27%and 21%prevalence,respectively);both were measured infrequently(14.9%and 3.2%of patients received monthly measurements in China).Frequency of dialysis sessions was positively associated with hyperphosphatemia;presence of residual kidney function was negatively associated with both hyperphosphatemia and secondary hyperparathyroidism.Laboratory measures indicating poor control of MBD were not associated with subsequent prescription of active vitamin D or phosphate binder.Conclusions:There are substantial opportunities for improvement and standardization of MBD management in China.Development of country-specific guidelines may yield realistic targets and standardization of medication use accounting for availability and cost. | Jun Wang Brian A.Bieber Fan-Fan Hou Friedrich K.Port Shuchi Anand | 2019 | Chinese Medical Journal2019,,23: | 15 |

| 18 | Increased liver stiffness in alcoholic liver disease:Differentiating fibrosis from steatohepatitis显示文摘AIM:To test if inflammation also interferes with liver stiffness (LS) assessment in alcoholic liver disease (ALD) and to provide a clinical algorithm for reliable fibrosis assessment in ALD by FibroScan (FS).METHODS:We first performed sequential LS analysis before and after normalization of serum transaminases in a learning cohort of 50 patients with ALD admitted for alcohol detoxification. LS decreased in almost all patients within a mean observation interval of 5.3 d. Six patients (12%) would have been misdiagnosed with F3and F4 fibrosis but LS decreased below critical cut-off values of 8 and 12.5 kPa after normalization of trans-aminases. RESULTS:Of the serum transaminases,the decrease in LS correlated best with the decrease in glutamic oxaloacetic transaminase (GOT). No significant chang-es in LS were observed below GOT levels of 100 U/L. After establishing the association between LS and GOT levels,we applied the rule of GOT < 100 U/L for reliable LS assessment in a second validation cohort of 101 patients with histologically confi rmed ALD. By ex-cluding those patients with GOT > 100 U/L at the time of LS assessment from this cohort,the area under the receiver operating characteristic (AUROC) for cirrhosis detection by FS improved from 0.921 to 0.945 while specificity increased from 80% to 90% at a sensitivity of 96%. A similar AUROC could be obtained for lower F3 fibrosis stage if LS measurements were restricted to patients with GOT < 50 U/L. Histological grading of inflammation did not further improve the diagnostic accuracy of LS.CONCLUSION:Coexisting steatohepatitis markedly increases LS in patients with ALD independent of fibrosis stage.
