1Carbamazepine solubility enhancement in tandem with swellable polymer osmotic pump tablet: A promising approach for extended delivery of poorly water-soluble drugs显示文摘Elementary osmotic pump(EOP)is a unique extended release(ER)drug delivery system based on the principle of osmosis.It has the ability to minimize the amount of the drug,accumulation and fluctuation in drug level during chronic uses.Carbamazepine(CBZ),a poorly water-soluble antiepileptic drug,has serious side effects on overdoses and chronic uses.The aim of the present study was to design a new EOP tablet of CBZ containing a solubility enhancers and swellable polymer to reduce its side effects and enhance the patient compliance.Firstly,a combination of solubilizing carriers was selected to improve the dissolution of the slightly soluble drug.Then,designing the new EOP tablet and investigating the effect of different variables of core and coat formulations on drug release behavior by single parameter optimization and by Taguchi orthogonal design with analysis of variance(ANOVA),respectively.The results showed that CBZ solubility was successfully enhanced by a minimum amount of combined polyvinyl pyrrolidone(PVP K30)and sodium lauryl sulfate(SLS).The plasticizer amount and molecular weight(MW)together with the osmotic agent amount directly affect the release rate whereas the swellable polymer amount and viscosity together with the semi-permeable membrane(SPM)thickness inversely influence the release rate.In addition,the tendency of following zero order kinetics was mainly affected by the coat components rather than those of the core.Further,orifice size does not have any significant effect on the release behavior within the range of 0.1 mm to 0.8 mm.In this study we report the successful formulation of CBZ-EOP tablets,which were similar to the marketed product Tegretol CR 200 and able to satisfy the USP criterion limits and to deliver about 80%of CBZ at a rate of approximately zero order for up to 12 h.Hadjira Rabti Jumah Masoud Mohammed Salmani Eltayeb Sulimen Elamin Narimane Lammari Jie Zhang Qineng Ping 2014Asian Journal of Pharmaceutical Sciences2014,9,3:4
23种新型复配剂对菜青虫的增效作用及生理生化机制显示文摘[方法]为明确乙酰甲胺磷.噻虫嗪水乳剂、水分散粒剂和毒死蜱.噻虫嗪水分散粒剂3种新型复配剂的增效作用及增效机理,室内采用喷雾法测定了对菜青虫的增效作用;通过生化测定法研究了其对菜青虫体内羧酸酯酶(CarE)、谷胱甘肽S-转移酶(GSTs)和乙酰胆碱酯酶(AChE)活力的影响;采用石蜡切片显微观察法分析了3种复配剂处理后菜青虫中肠组织细胞的变化。[结果]3种复配剂对菜青虫都有很好的杀虫活性,处理48 h后的共毒系数分别为131.10、459.04、204.94,都表现为明显的增效作用;乙酰甲胺磷.噻虫嗪水乳剂显著抑制CarE、GSTs和AChE活力,抑制率分别为90.04%、74.98%、71.59%;乙酰甲胺磷.噻虫嗪水分散粒剂对3种酶活力的抑制率分别为72.11%、74.98%、90.60%;毒死蜱.噻虫嗪水分散粒剂则仅抑制CarE和GSTs活力,抑制率为71.16%和88.55%;3种复配剂明显影响菜青虫中肠肠壁细胞的层次结构以及肠壁细胞中的杯状细胞形状及结构。[结论]3种新型复配剂可能是通过抑制菜青虫体内关键酶活性和破坏中肠组织细胞形状和结构,进而影响其正常的消化和吸收功能,达到增效和杀虫的目的。米凤玉 王永模 ELTAYEB E Mansour 张国安 2011农药2011,50,9:3
3Green synthesis, antimicrobial and cytotoxic effects of silver nanoparticles mediated by Eucalyptus camaldulensis leaf extract显示文摘Objective: To investigate the environmental-friendly extracellular biosynthetic technique for the production of the silver nanoparticles(AgN Ps) by using leaf extract of Eucalyptus camaldulensis(E. camaldulensis). Methods: The NP were characterized by colour changes and the UV-visible spectroscopy. The cytotoxic effects of prepared AgN Ps was detected against four types of pathogenic bacteria, including two Gram-negative bacteria(Pseudomonas aeruginosa and Escherichia coli) and two Gram-positive bacteria(Staphylococcus aureus and Bacillus subtilis) by using agar well diffusion method. Results: A peak absorption value between 400-450 nm for the extract and the colour change to dark brown were corresponding to the plasmon absorbance of AgN Ps. On the other hand, aqueous extract of E. camaldulensis leaves could be effective against tested microorganisms which showed inhibition zones of 9.0-14.0 mm. Furthermore, biologically synthesized AgN Ps had higher ability to suppress the growth of the tested microorganisms(12.0-19.0 mm). Conclusions: Our findings indicated that extracellular synthesis of Ag NPs mediated by E. camaldulensis leaf extract had an efficient bactericidal activity against the bacterial species tested. The exact mechanism of the extracellular biosynthesis of metal NP was not well understood. 4Mortality and rebleeding following variceal haemorrhage in liver cirrhosis and periportal fibrosis显示文摘AIM To investigate mortality and rebleeding rate and identify associated risk factors at 6 wk and 5 d following acute variceal haemorrhage in patients with liver cirrhosis and schistosomal periportal fibrosis.METHODS This is a prospective study conducted during the period from March to December 2014. Patients with portal hypertension presenting with acute variceal haemorrhage secondary to either liver cirrhosis(group A) or schistosomal periportal fibroses(group B) presenting within 24 h of the onset of the bleeding were enrolled in the study and followed for a period of 6 wk. Analysis of data was done by Microsoft Excel and comparison between groups was done by Statistical Package of Social Sciences version 20 to calculate means and find the levels of statistical differences and define the mortality rates, the P value of < 0.05 was considered to be significant. RESULTS A total of 94 patients were enrolled in the study. Thirtytwo patients(34%) had liver cirrhosis(group A) and62(66%) patients had periportal fibrosis(group B).Mortality: The 6-wk and 5-d mortality were 53% and16% respectively in group A compared to 10% and 0%in group B(P value < 0.000 and < 0.004). In group A;a Child-Turcotte-Pugh class C and rebleeding within 5 d were significantly associated with 5-d mortality(P value< 0.029 and < 0.049 respectively) and Child- TurcottePugh class C was also a significant risk factor for 6-wk mortality(P value < 0.018). In group B; mortality was significantly associated with rebleeding within the 6-wk follow-up period and requirement for blood transfusion on admission(P value < 0.005 and < 0.049). Rebleeding:The 6-wk and 5-d rebleeding rate in group A were 56%and 25% respectively compared to 32% and 3% in group B(P value < 0.015 and < 0.002). Clinical presentation with encephalopathy was a significant risk factor for 5 d rebleeding in group A(P value < 0.005) while grade Ⅲperiportal fibrosis and requirement for blood transfusion on admission were significant risk factors for 6-wk rebleeding in group B(P value < 0.004 and < 0.02).CONCLUSION The 6-wk and 5-d mortality and rebleeding rate were significantly higher in patients with liver cirrhosis compared to patients with schistosomal periportal fibrosis.Sara Elfadil Abbas Mohammed Abdelmunem Eltayeb Abdo Hatim Mohamed Yousif Mudawi 2016World Journal of Hepatology2016,8,31:3
5Vitamin D status and viral response to therapy in hepatitis C infected children显示文摘AIM: To study the frequency of vitamin D deficiency in patients with hepatitis C virus(HCV) infection and to evaluate the role of vitamin D supplementation in improving antiviral therapy.METHODS: Sixty-six children aged from 7-14 years(mean ± SD, 11.17 ± 2.293) diagnosed with HCV infection were matched to 28 healthy controls. Serum levels of 25(OH) D3, calcium, phosphorus, alkaline phosphatase and plasma level of parathormone were measured. Quantitative PCR for HCV was performed Bone density was determined by dual energy X-ray absorptiometry. All cases received conventional therapy, and only 33 patients received vitamin D supplementation.RESULTS: Children with HCV showed significantly increased levels of HCV RNA(P < 0.001), parathormone(P < 0.01) and decreased vitamin D levels(P < 0.05)(33.3% deficient and 43.3% insufficient) compared with controls. Abnormal bone status(Z score-1.98 ± 0.75) was found in ribs, L-spine, pelvis and total body. Cases treated with vitamin D showed significant higher early(P < 0.04) and sustained(P < 0.05) virological response. There was a high frequency of vitamin D deficiency among the Egyptian HCV children, with significant decrease in bone density. The vitamin D level should be assessed before the start of antiviral treatment with the correction of any detected deficiency. 15Prevalence and risk factors associated with drug resistant TB in South West,Nigeria显示文摘Objective:To determine the prevalence and risk factors associated with drug resistant tuberculosis(TB) in South West Nigeria.Methods:A retrospective study conducted among pulmonary tuberculosis(PTB) patients from Oyo and Osun States in South West Nigeria who had their culture and drug susceptibility test performed at the institute of tropical medicine Antwerp, Belgium between 2007 and 2009.Data on the patient’s characteristics were retrieved from the TB treatment card.Univariate analysis was performed to assess the risk factors for drug resistant tuberculosis.The Level of significance was at P<0.05.Results:Among the 88 patients who had drug-susceptibility test result,there were 50 males and 38 females.Of the 88 patients,55(62.5%) had strains resistant to at least one or more anti-drugs.The proportion of TB cases with resistance to a single drug was 12.7%.The multi-drug resistant TB(MDR-TB) rate was 76.4%.The only significant factor for the development of drug resistance and MDR was the history of previous anti TB treatment(P<0.01).Other factors such as age[OR 0.86(0.35-2.13);P=0.72]and gender[OR 1.24 (0.49-3.14);P=0.62]were not significantly associated with drug resistance TB.Conclusions:The study highlighted a high prevalence of MDR-TB among the study population.History of previous TB treatment was associated with MDR-TB.There is an urgent need to conduct a national TB drug resistance survey to determine the actual burden and risk factors associated with drug resistance TB in the country.Olusoji Daniel Eltayeb Osman 2011Asian Pacific Journal of Tropical Medicine2011,4,2:1
