| 1 | An analysis of 10218 ulcerative colitis cases in China显示文摘AIM: To analyze the characteristics of ulcerative colitis(UC)in Chine.METHODS: From 1981 to 2000, a total of 10218 patients of UCreported in Chinese medical literature and including ourcases diagnosed were analyzed according to the diagnosticcriteria of Lennard-Jones.RESULTS: The number of cases increased by 3.08 times overthe past 10 years (2506 patients were diagnosed from 1981 to1990 while 7512 patients were diagnosed from 1991 to 2000).Lesion renge were described in 7966 patients, 5592 (70.20%)were proctosigmoiditis or proctitis, 1792(22.50%) left-sidedcolitis, 582(7.30%) pancolitis. Among the 8122 patients,2826 (34.8%) had first episole, 4272 (52.6%) had chronicrelapse, 869 (10.7%) were of chronic persist type, 154(1.9%) were of acuts fulminant tyle. The course of theillress were described in 5867 patients, 4427(75.5%) wereless than 5 years, 910 (15.5%) between 5 and 10 years, 530(9.1%) more then 10 years. Six hundred and sixteenpatients patients (6.1% ) had extraintestinal manifestations.The mean age at the diagnosis was 40.7 years( range 6-8 years, and the peak ages 30-49 years). The male to femaleratio was1.09. Among 270 patients diagnosed in ourhospital, 36 had histories of smoking, there was no negativeassociation between the severity of UC and smoking ( P >0.05), 21 smokers were followed up for one year, 15 of themhad given up smoking when the disease were diagnosed,and one year later, 7 patients relapsed, another 6 patientscontinued smoking, and one year later, 2 patients relapsed.Among 270 UC patients diagnosed in our hospital, 4 patients(1.48%) from 2 families had familial history of UC.Treatment was mentioned in 6859 patients, only SASP and/or corticosteroid only in 1276 patients(18.6%), only Chineseherbs in 1377 patients (20.1%), combined Chinese andwestern medicine in 4056 patients (59.1%), surgery wasperformed in 87 patients (1.3%), other treatments in 63patients(0.9%).CONCLUSIONS: In China, number of UC patients increasedsignificantly in the past 10 years. Lesions are commonlylocated to left side colon. The course is short with rereextraintestinal manifestations. The age of onset is relativelyhigh. Males and females are nearly equally affected. Nonegative relation was found between smoking and severityof the disease. | 5 | Role of laparoscopic subtotal cholecystectomy in the treatment of complicated cholecystitis显示文摘BACKGROUND: Laparoscopic cholecystectomy (LC) has become the 'gold standard' in treating benign gallbladder diseases. Increasing laparoscopic experience and techniques have made laparoscopic subtotal cholecystectomy (LSC) a feasible option in more complex procedures. In recent years, few studies with a few cases of LSC have reported good results in patients with various types of cholecystitis. This study was designed to evaluate the feasibility, indications, characteristics and benefits of LSC in patients with complicated cholecystitis. METHODS: Altogether, 3485 patients were scheduled to receive LC during the past 4 years at our institute. Among them, 168 patients with various complicated forms of cholecystitis were treated by LSC. Meanwhile, the other 3317 patients who received standard LC were enrolled as the control group. Perioperative data from the two groups were collected and retrospectively analyzed. RESULTS: In the LSC group, 135 patients suffered from acute calculic cholecystitis, 18 from chronic calculic cholecystitis with cirrhotic portal hypertention, and 15 from chronic calculic atrophy cholecystitis with severe fibrosis. These patients constituted 4.8% of the total patients who underwent LC (168/3485) in the same period at our institute. In 122 patients, the cystic duct and artery were clipped before division. In another 46 patients, the gallbladder was initially incised at Hartmann’s pouch. Five patients (3.0%) were converted to open subtotal cholecystectomy. The median operation time for LSC was 65.5±15.2 minutes, estimated operative blood loss was 71.5±15.5 ml, and the time to resume diet was 20.4±6.3 hours. Thirteen patients (7.7%) had local complications. The mean postoperative hospital stay was 4.2±2.6 days. In the LC group, 2887 had chronic calculic cholecystitis, 312had acute calculic cholecystitis, 47 had chronic calculic atrophy cholecystitis, and 71 had polypus. Seventeen patients (0.5%) were converted to open cholecystectomy. The median operation time was 32.6±10.2 minutes, the estimated operative blood loss was 24.5±8.5 ml, and the time to resume diet was 18.3±4.5 hours. Thirty- nine patients (1.2%) had local complications. Mean postoperative hospital stay was 3.8±1.4 days. There was no bile duct injury or mortality in either group. CONCLUSIONS: LSC for patients with complicated cholecystitis is difficult, with a longer operation time, more operative blood loss and higher conversion and complication rates than LC. However, it is feasible and relatively safe. LSC is advantageous over open surgery, but it remains a non-routine choice. | 6 | Application of laparoscopic cholecystectomy in patients with cirrhotic portal hypertension显示文摘BACKGROUND: Laparoscopic cholecystectomy (LC) has been widely adopted in treating benign gallbladder disea- ses. Cirrhosis and cirrhotic portal hypertension (CPH) are contraindicated for LC in its early period. In recent years, several studies have reported liberal use of LC in patients with cirrhosis. But its benefits and successful use in patients with CPH are less documented. This study was designed to evaluate the feasibility, safety and technical characteristics of LC in CPH patients. METHODS: In 38 patients with symptomatic gallbladder disease and CPH, 19 belonged to Child A class, 15 Child B class and 4 Child C class. Perioperative data of these pa- tients were collected and analyzed. RESULTS: LC was successfully performed in 36 patients, and 2 patients (5.3%) were converted to open cholecystec- tomy (OC) for difficulty in management of bleeding under laparoscopy and dense adhesion of Calot’ s triangle. The sur- gical time was 62.6±15.2 minutes. The estimated amount of intraoperative hemorrhage was 75.5±15.5 ml. No blood transfusion was necessary. The time to resume diet was 18.3 ±6.5 hours. Seven postoperative complications in 5 patients (13.2%) included port-site infection (1 patient), respiratory infection (2), upper digestive tract bleeding (1), slight hepatic encephalopathy (1) and increased ascites (2). All patients were cured and discharged from the hospital within 5.6±2.4 days after LC. CONCLUSIONS: Despite LC is difficult for CPH patients, it is feasible and relatively safe. | 7 | Effect of caffeic acid phenethyl ester on proliferation and apoptosis of hepatic stellate cells in vitro显示文摘AIM: To investigate the role of nuclear factor-κB (NF-κB)inhibitor caffeic acid phenethy1 ester (CAPE) in the proliferation, collagen synthesis and apoptosis of hepatic stellate cells (HSCs) of rats.
METHODS: The HSCs from rats were isolated and cultured in Dulbecco's Modified Eagle's Medium (DMEM) and treated with CAPE. The proliferation and collagen synthesis of HSCs were determined by 3H-TdR and 3H-proline incorporation respectively, and the expression of type Ⅰ, Ⅲ procollagen genes was further explored byin situ hybridization. Apoptosis cell indices (AIs) were examined using terminal deoxynucleotidyl transferase- mediated DIG-dUTP nick end labeling (TUNEL).
RESULTS: Tn activated HSC in culture, CAPE significantly inhibited 3H-TdR and 3H-proline incorporation by HSCs at concentrations of 5 μmol/L and 10 μmol/L respectively. CAPE also reduced the type I procollagen gene expression (P<0.05)at higher concentration. Apoptosis of HSC was induced by CAPE and the AIs were time-and dose-dependently increased from 2.82+0.73 % to 7.66±1.25 % at 12 h (P<0.01) and from 3.15±0.88 % to 10.6L±2.88 % at 24 h (P<0.01).

| 9 | Relationship between pre-TIPS hepatic hemodynamics and postoperative incidence of hepatic encephalopathy显示文摘BACKGROUND: Hepatic encephalopathy (HE) is one of the complications that have limited the effectiveness of transjugular intrahepatic portosystemic shunt (TIPS) most significantly. Up to the present, the predicting factors of HE post-TIPS have been debated controversially. This study was undertaken to verify the relationship between pre-TIPS intrahepatic hemodynamics and the incidence of post-TIPS HE. METHODS: The hepatic blood dynamics was evaluated in 41 patients with liver cirrhosis before TIPS and at one month after TIPS by ultrasonography. The patients were divided into two groups according to Doppler findings before TIPS: group 1, patients with prograde portal flow, and group 2, patients with hepatofugal or back-forth portal flow. The clinical characteristics (age, sex, etiology of liver disease, pre-TIPS Child-Pugh score, incidence of pre-TIPS HE, and portacaval pressure gradient), incidence of post TIPS HE, and pre-/post-TIPS hepatic arterial resistant index (RI) in the two groups were compared. The independent prognostic value of pre-TIPS variables for the onset of HE after TIPS, including age, Child-Pugh score, presence of HE before TIPS, and the pattern of portal flow, was tested with a multiple-factor regression analysis. RESULTS: No significant difference in age, etiology of liver disease, indications of TIPS placement, incidence of HE before TIPS, and portacaval gradient before and after TIPS was observed between the two groups; but liver failure was more severe in group 2 (P<0.05). The incidence of post-TIPS HE in group 2 was significantly lower than that in group 1 (P<0.01). Pre-TIPS, the RI of the hepatic artery in group 1 was significantly higher than that in group 2 (P<0.01). However, TIPS induced a significantly decreased RI in group 1 (P <0.01), but not in group 2. Multiple-factor regression analysis demonstrated that the pattern of portal flow before TIPS was closely associated with the onset of post TIPS HE. CONCLUSIONS: Pre-TIPS intrahepatic hetnodynamics is closely related to the incidence of post-TIPS HE. Hepatic hetnodynamics of patients with hepatofugal portal blood flow only changes a little after TIPS and still provides compensatory blood supply of the hepatic artery, and the hepatic function is less affected. Hence HE is unlikely. Hepatic hemodynamics of patients with prograde portal blood flow changes a lot after TIPS, and dual blood supply of the portal vein and hepatic artery changes into compensatory blood supply of the hepatic artery, and hepatic function suffers greatly in a short time. | 10 | Analysis of HLA alleles polymorphism in Chinese patients with primary biliary cirrhosis显示文摘BACKGROUND: A familial dustering of patients with primary biliary cirrhosis (PBC) and the presence of immunological abnormalities in family members suggest a genetic component involved in the pathogenesis of PBC. The aims of this study are to investigate the frequencies of human leukocyte antigen HLA-A, -B, and -DRB1 alleles in Chinese patients with PBC by polymerase chain reaction (PCR)-based techniques, and to assess the correlation of the above-mentioned HLA with some clinical and laboratory features. METHODS: Genotyping of HLA alleles were performed in 65 well-characterized PBC patients and 431 healthy controls with sequence-specifc primers PCR amplification. RESULTS: HLA-DRB1~*07 allele detected in 19 of the 65 (29.2%) PBC patients was subtyped as DRB1~*0701, as well as in 13.9% of controls (P_C<0.05, OR=2.55, 95% CI: 1.4-4.6). An increased frequency of DRB1~*03 (18.4% vs. 7.2% in healthy controls) and a decreased frequency of DRB1~*12 (16.9% vs. 28.8%) in PBC patients were statistically significant. There was no association with HLADRB1~*08 reported. The frequencies for HLA-A, B and the other DRB1 alleles were similar between patients and healthy controls. CONCLUSIONS: The susceptibility to PBC in Chinese individuals is associated with DRB1~*0701 allele. This association differs from that in North Americans, South Americans, North Europeans and even Japanese, but it is not restricted to any particular subgroup of patients. | Hai-Ying Liu. | 11 | Expression of liver cancer associated gene HCCA3显示文摘AIM: To study and clone a novel liver cancer reisted gene,and to explore the molecular basis of liver cancer genesis.METHODS: Using mRNA differential display polymerasechain reaction (DDPCR), we investigated the difference of mRNA in human hepatocellular carcinoma (HCC) and paired surrounding liver tissues, and got a gene probe. By screening a human placenta cDNA library and genomic homologous extend, we obtained a full-length cDNA named HCCA3. We analyzed the expression of this novel gene in 42pairs of HCC and the surrounding liver tissues, and distribution in human normal tissues by means of Northern blot assay.RESULTS: A full-length cDNA of liver cancer associated gene HCCA3 has been submitted to the GeneBank nucleotide sequence databases ( Accession No. AF276707 ). The positive expression rate of this gene was 78.6% (33/42) in HCC tissues, and the clinical pathological data showed that the HCCA3 was closely associated with the invasion of tumor capsule ( P = 0.023) and adjacant small metastasis satellite nodules lesions ( P= 0.041). The HCCA3 was widely distributed in the human normal tissues, which was intensively expressed in lungs, brain and colon tissues,while lowly expressed in the liver tissues.CONCLUSION: A novel full-length cDNA was cloned and differentiated, which was highly expressed in liver cancer tissues. | 12 | Pericardial devascularization combined with preservation of Latarjet's nerves trunk in treatment of patients with portal hypertension显示文摘OBJECTIVE: To investigate the effect of perieardial devascularization combined with preservation ofLatarjet’s innervation on portal hypertension.METHODS: Sixty-two patients (57 men and 5 women) have been undergone pericardialdevascularization combined with Latarjet’s innervation since 1984. Clinical results and postoperativecomplications were evaluated.RESULTS: The incidence of upper-digestive tract bleeding was 3.2% (2 patients) within 1 month afteroperation. Four patients (6.5%) died within 1 month atier operation (3 patients received emergencyoperation and 1 selective operation). Among them, 3 patients died from hepatic failure, and one patientfrom sepsis due to subphrentic infection. Among these deaths, 3 were evaluated Child’s C. | 14 | Study on Retinal Ganglion Cell Apoptosis after Explosive Injury of Eyeballs in Rabbits显示文摘Purpose: To explore the pathological changes of retinal ganglion cell apoptosis and its relation to the value of glutamate concentration in vitreous body after explosive injury of eyeballs in the rabbits.Method: Similar explosive injury models of eyeballs in 10 adult grey rabbits were made.The rabbits were killed on scheduled time. The retinal tissues of studied eyes and control eyes were obtained for the pathological examination with TUNEL method respectively.The value of glutamate in vitreum of injured eyes was measured and was compared with that of contralateral eyes. Statistical comparison analysis on the experiment data was performed.Result: The value of glutamate in vitreum of injured eyes was significantly higher than that of contralateral eyes in all rabbits in the study. A lot of TUNEL positive cells were observed in the injured eyes. | 15 | Hepatocellular glycogen in alleviation of liver ischemia reperfusion injury显示文摘Objective: To study the mechanism of hepatocellularglycogen in alleviation of liver ischemia-reperfusioninjury during hepatic vascular occlusion for partialhepatectomy.Methods: Seventeen patients were randomly dividedinto experimental group (n=9) and control group(n=8). In the experimental group, patients weregiven high concentration glucose intravenously during24 hours before operation. The hepatic lesion was re-sected after portal triad clamping in the two groups.Non-cancer liver tissue was biopsied to measure he-patic tissue ATP content and change of malondialde-hyde (MDA) and superoxide dismutase (SOD).Liver function of all patients was assessed beforeoperation and the first and fifth day after operation.Results: Hepatic tissue ATP content of the experi-mental group was significantly higher than that of thecontrol group both at the end of hepatic vascular oc-clusion and the point of one-hour reperfusion. Be-sides, liver function of the experimental group wassignificantly better than that of the control group thefirst and fifth day after operation. There was signifi-cant difference in SOD activity or MDA content be-tween the two groups at the end of hepatic vascularocclusion and at the point of one-hour reperfusion.Conclusions: Abundant intracellular glycogen mayreduce liver ischemia-reperfusion injury caused byhepatic vascular occlusion. | 16 | Effect of Seawater Soaking on Explosive Cornea Injury显示文摘Objective: To observe the changes in corneal tissue after explosive corneal injury and the effect of seawater soaking on the healing of cornea after explosive injury with the help ofoptical microscope. Methods: Make 10 similar explosive injury models of rabbit′s eyeball using 10 adult greyrabbits. For each rabbit, both eyes are artificially injured through explosion; its right eye is the comparison eye and after the injury the left eye is soaked in seawater for 30 minutes.Conduct slit lamp examinations, fluorescein dyeing, and ultrasonic cornea thickness metering on the corneas at intervals, that is, before the injury, 1 , 2, 3, 4, 5, 7, 9, 12, 15days after the injury respectively. Examine the corneas under the optical microscope andmake comparisons.Results: The corneal thickness of the experiment eye becomes apparently thicker than thatof the comparison eye after the injury. The corneal clouding of the former apparentlyaggravates compared with the latter. The healing of corneal epithelium in the injured eye isslower compared with that in the comparison eye. Comparative examination under the opticalmicroscope shows: after the injury, the corneal tissue of the experiment eye changesdistinctly, its healing is delayed and the vascularization degree becomes high in cornealstroma.Conclusion: Scars and vascularization of various degrees will appear in corneal stroma afterexplosive injuries. | 17 | Silencing phospholipid scramblase 1 expression by RNA interference in colorectal cancer and metastatic liver cancer显示文摘BACKGROUND:Phospholipid scramblase 1(PLSCR1) not only participates in the transbilayer movement of phospholipids,but also plays a role in the pathogenesis and progression of cancers.The present study aimed to evaluate the effect of silencing PLSCR1 expression by RNA interference in colorectal cancer(CRC) and metastatic liver cancer.METHODS:The expression of PLSCR1 in CRC and metastatic liver cancer samples was assessed by immunohistochemistry.The cultured cells with the highest expression were selected for subsequent experiments.We designed three siRNA oligonucleotide segments targeted at PLSCR1.Successful transfection was confirmed.The biological behavior of the cells in proliferation,adhesion,migration and invasion was determined.RESULTS:PLSCR1 protein expression increased significantly in the majority of CRC and metastatic liver cancer samples compared with normal samples.Lovo cells had the highest expression of PLSCR1.The siRNA-390 oligonucleotide segment had the best silencing effect.After transfection,Lovo cell proliferation was significantly inhibited compared with the controls in the MTT assay.Laminin and fibronectin adhesion assays showed Lovo cell adhesion was also significantly inhibited.In the migration assay,the number of migrating cells in the PLSCR1 siRNA-390 group was 50±12,significantly lower than the number in the siRNA-N group(115±28) and in the control group(118±31).In an invasion test,the number of invading cells in the PLSCR1 siRNA-390 group was 60±18,significantly lower than that in the siRNA-N group(97±26) and the control group(103±24).CONCLUSIONS:PLSCR1 is overexpressed in CRC and metastatic liver cancer.Silencing of PLSCR1 by siRNA inhibits the proliferation,adhesion,migration and invasion of Lovo cells,which suggests that PLSCR1 contributes to the tumorigenesis and tumor progression of CRC.PLSCR1 may be a potential gene therapy target for CRC and associated metastatic liver cancer. | Wei Cui,Shi-Yong Li,Jun-Feng Du,Zi-Man Zhu and Ping An Department of General Surgery,General Surgery Center of the PLA,General Hospital of Beijing Military Command,Beijing 100700,China Department of Hepatobiliary Surgery,First Affiliated Hospital,General Hospital of the PLA,Beijing 100037,China | 2012 | Hepatobiliary & Pancreatic Diseases International2012,11,4: | 4 |

| 20 | S-adenosylmethionine in treatment of cholestasis after total parenteral nutrition: laboratory investigation and clinical application显示文摘Objective: To observe the effects of S-adenosylmethio-nine (SAMe) in the treatment of cholestasis after totalparenteral nutrition (TPN).Methods: Thirty SD rats were randomly divided intocontrol group, hypercalorie group, hypercalorie+SAMegroup, sepsis group and sepsis+SAMe group to com-pare their states of cholestasis. Sixteen patients re-ceived SAMe because of cholestasis after prolongedTPN, and the therapeutic efficacy was observed.Results: Bile flow was obviously decreased and theserum levels of total bile acid and gamma-glutamyltranspeptidase(γ-GT) were markedly increased in thehypercalorie and sepsis groups. Meanwhile, hepatocytefatty degeneration, dilatation of cholangioles, and bilesludge could be seen microscopically. SAMe adminis-tration in the hypercalorie+SAMe and sepsis+SAMegroups could increase the bile flow, decrease theserum levels of total bile acid and γ-GT, reduce thepathological damage to the liver, and clear the bilesludge in the cholangioles. Cholestasis and abnormalliver function were the main manifestations of the 16patients before SAMe administration.
