| 1 | 区域旅游流空间结构的高铁效应及机理--以中国京沪高铁为例显示文摘交通是影响旅游流空间结构最主要因素之一。以京沪高铁线为例,探讨了区域旅游流空间结构的高铁效应及特征,在此基础上,分析区域交通可达性变化,并结合区域旅游资源禀赋、区域旅游接待设施、区域旅游交通网络密度以及区位等影响因素分析区域旅游流空间结构的高铁效应的机理。研究表明:1区域旅游流空间结构的高铁效应表现为马太效应、过滤效应、扩散效应和叠加效应等特征。2区位条件、旅游资源禀赋、旅游接待能力、交通网络密度以及时空压缩程度等影响因素共同作用均非常明显态势下,产生高铁马太效应;旅游资源禀赋、旅游接待能力、交通网络密度均不强,时空压缩程度不显著等旅游节点产生高铁过滤效应;区位条件、旅游资源禀赋、旅游接待能力和交通网络密度均具有很强优势,且时空压缩程度明显等旅游节点可成为扩散源,高铁强化了扩散源旅游流集聚作用,然后向边缘旅游地扩散,呈现为'集聚—扩散'模式;高铁使大尺度空间的不同客源地居民出游空间范围出现叠加现象,但只有区位条件、旅游资源禀赋、旅游接待能力、交通网络密度优势较强且时空压缩程度明显旅游节点产生叠加效应。 | 汪德根 陈田 陆林 王莉 ALAN August Lew | 2015 | 地理学报2015,70,2: | 226 |
| 2 | The PI3K/Akt/m TOR pathway in ovarian cancer: therapeutic opportunities and challenges显示文摘The phosphatidylinositol 3 kinase(PI3K) pathway is frequently altered in cancer, including ovarian cancer(OC). Unfortunately, despite a sound biological rationale and encouraging activity in preclinical models, trials of first-generation inhibitors of mammalian target of rapamycin(m TOR) in OC have demonstrated negative results. The lack of patient selection as well as resistance to selective m TOR complex-1(m TORC1) inhibitors could explain the disappointing results thus far. Nonetheless, a number of novel agents are being investigated, including dual m TORC1/m TORC2, Akt, and PI3 K inhibitors. Although it is likely that inhibition of the PI3K/Akt/m TOR pathway may have little effect in unselected OC patients, certain histological types, such as clear cell or endometrioid OC with frequent phosphatidylinositol-4,5-biphosphate 3-kinase, catalytic subunit alpha(PIK3CA) and/or phosphatase and tensin homolog(PTEN) alterations, may be particularly suited to this approach. Given the complexity and redundancy of the PI3 K signaling network, PI3 K pathway inhibition may be most useful in combination with either chemotherapy or other targeted therapies, such as MEK inhibitors, anti-angiogenic therapy, and hormonal therapy, in appropriately selected OC patients. Here, we discuss the relevance of the PI3 K pathway in OC and provide an up-to-date review of clinical trials of novel PI3 K inhibitors alone or in combination with cytotoxics and novel therapies in OC. | 5 | Operative complications and economic outcomes of cholecystectomy for acute cholecystitis显示文摘BACKGROUND Recent management of acute cholecystitis favors same admission(SA)or emergent cholecystectomy based on overall shorter hospital stay and therefore cost savings.We adopted the practice of SA cholecystectomy for the treatment of acute cholecystitis at our tertiary care center and wanted to evaluate the economic benefit of this practice.We hypothesized that the existence of complications,particularly among patients with a higher degree of disease severity,during SA cholecystectomy could negate the cost savings.AIM To compare complication rates and hospital costs between SA vs delayed cholecystectomy among patients admitted emergently for acute cholecystitis.METHODS Under an IRB-approved protocol,complications and charges for were obtained for SA,later after conservative management(Delayed),or elective cholecystectomies over an 8.5-year period.Patients were identified using the acute care surgery registry and billing database.Data was retrieved via EMR,operative logs,and Revenue Cycle Operations.The severity of acute cholecystitis was graded according to the Tokyo Guidelines.TG18 categorizes acute cholecystitis by Grades 1,2,and 3 representing mild,moderate,and severe,respectively.Comparisons were analyzed withχ2,Fisher’s exact test,ANOVA,ttests,and logistic regression;significance was set at P<0.05.RESULTS Four hundred eighty-six(87.7%)underwent a SA while 68 patients(12.3%)received Delayed cholecystectomy.Complication rates were increased after SA compared to Delayed cholecystectomy(18.5%vs 4.4%,P=0.004).The complication rates of patients undergoing delayed cholecystectomy was similar to the rate for elective cholecystectomy(7.4%,P=0.35).Mortality rates were 0.6%vs 0%for SA vs Delayed.Patients with moderate disease(Tokyo 2)suffered more complications among SA while none who were delayed experienced a complication(16.1%vs 0.0%,P<0.001).Total hospital charges for SA cholecystectomy were increased compared to a Delayed approach($44500±$59000 vs$35300±$16700,P=0.019).The relative risk of developing a complication was 4.2x[95%confidence interval(CI):1.4-12.9]in the SA vs Delayed groups.Among eight patients(95%CI:5.0-12.3)with acute cholecystitis undergoing SA cholecystectomy,one patient will suffer a complication.CONCLUSION Patients with Tokyo Grade 2 acute cholecystitis had more complications and increased hospital charges when undergoing SA cholecystectomy.This data supports a selective approach to SA cholecystectomy for acute cholecystitis. | Christopher P Rice Krishnamurthy B Vaishnavi Celia Chao Daniel Jupiter August B Schaeffer Whitney R Jenson Lance W Griffin William J Mileski | 2019 | World Journal of Gastroenterology2019,25,48: | 8 |
| 6 | 区域旅游流空间结构的高铁效应及机理(英文)显示文摘Chinese railway has entered the 'HSR era', while the structure of 'four vertical and four horizontal' railways for transit passengers is almost completed. Taking the Beijing-Shanghai High-Speed Rail(hereinafter referred to as HSR) as an example, this paper first explores HSR's effects on the spatial structure of regional tourist flows using the social network analysis. Next, it notes changes in the accessibility of regional transportation. After analyzing the factors including initial endowment of regional tourism resources, hospitality facilities, the density of the regional tourism transportation network, and locations, the paper discusses the mechanisms through which HSR affects regional tourist flows. The study shows the following:(1) HSR's effects on the spatial structure of regional tourist flows are manifested through the Matthew effect, the filtering effect, the diffusion effect and the overlying effect, and(2) the Matthew effect of HSR is manifested under an obvious interaction of the location, the initial endowment of tourism resources, hospitality capacity, tourist transportation network density and 'time-space compression'. The filtering effect of HSR is manifested for those tourism nodes without favorable location conditions, endowment of tourism resources, hospitality capacity, or tourist transportation network density and without obvious benefits from 'time-space compression'. Those tourism nodes that boast advantages in terms of location condition, endowment of tourism resources, hospitality capacity, tourist transportation network density and obvious 'time-space compression' will become sources for the diffusion effect. HSR will strengthen the aggregation effects of tourist flow in these diffusion sources, which will thereafter diffuse to peripheral tourist areas, manifesting 'aggregation-diffusion'. HSR has overlapped tourists' spatial traveling range over large-scale spaces. It can be noted from the results that, the process for biosorption of heavy metals (copper, cadmium) by immobilized Laminaria japonica can be described by the Banerm model. According to the model, the adsorption rate constant calculated was 0.107 8 and 0.030 28 min-1 for Cu2+ and Cd2+ respectively. The experimental biosorption equilibrium data for Cu2+ and Cd2+ were in good agreement with those calculated by the Langmuir model. The maximum uptake capacity calculated was 83.3 and 112.4 mg/g for Cu2+ and Cd2+ according to the Langmuir model, respectively. | 13 | Stem cell autotomy and niche interaction in different systems显示文摘The best known cases of cell autotomy are the formation of erythrocytes and thrombocytes(platelets) from progenitor cells that reside in special niches. Recently, autotomy of stem cells and its enigmatic interaction with the niche has been reported from male germline stem cells(GSCs) in several insect species. First described in lepidopterans, the silkmoth, followed by the gipsy moth and consecutively in hemipterans, foremost the milkweed bug. In both, moths and the milkweed bug, GSCs form finger-like projections toward the niche, the apical cells(homologs of the hub cells in Drosophila). Whereas in the milkweed bug the projection terminals remain at the surface of the niche cells, in the gipsy moth they protrude deeply into the singular niche cell. In both cases, the projections undergo serial retrograde fragmentation with progressing signs of autophagy. In the gipsy moth, the autotomized vesicles are phagocytized and digested by the niche cell. In the milkweed bug the autotomized vesicles accumulate at the niche surface and disintegrate. Autotomy and sprouting of new projections appears to occur continuously. The significance of the GSC-niche interactions, however, remains enigmatic. Our concept on the signaling relationship between stem cell-niche in general and GSC and niche(hub cells and cyst stem cells) in particular has been greatly shaped by Drosophila melanogaster. In comparing the interactions of GSCs with their niche in Drosophila with those in species exhibiting GSC autotomy it is obvious that additional or alternative modes of stem cell-niche communication exist. Thus, essential signaling pathways, including niche-stem cell adhesion(E-cadherin) and the direction of asymmetrical GSC division- as they were found in Drosophila- can hardly be translated into the systems where GSC autotomywas reported. It is shown here that the serial autotomy of GSC projections shows remarkable similarities with Wallerian axonal destruction, developmental axon pruning and dying-back degeneration in neurodegenerative diseases. Especially the hypothesis of an existing evolutionary conserved 'autodestruction program' in axons that might also be active in GSC projections appears attractive. Investigations on the underlying signaling pathways have to be carried out. There are two other well known cases of programmed cell autotomy: the enucleation of erythroblasts in the process of erythrocyte maturation and the segregation of thousands of thrombocytes(platelets) from one megakaryocyte. Both progenitor cell types- erythroblasts and megakaryocytes- are associated with a niche in the bone marrow, erythroblasts with a macrophage, which they surround, and the megakaryocytes with the endothelial cells of sinusoids and their extracellular matrix. Although the regulatory mechanisms may be specific in each case, there is one aspect that connects all described processes of programmed cell autotomy and neuronal autodestruction: apoptotic pathways play always a prominent role. Studies on the role of male GSC autotomy in stem cell-niche interaction have just started but are expected to reveal hitherto unknown ways of signal exchange. Spermatogenesis in mammals advance our understanding of insect spermatogenesis. Mammal and insect spermatogenesis share some broad principles, but a comparison of the signaling pathways is difficult. We have intimate knowledge from Drosophila, but of almost no other insect, and we have only limited knowledge from mammals. | 17 | Biliary leakage after urgent cholecystectomy: Optimization of endoscopic treatment显示文摘AIM: To investigate the results of endoscopic treatment of postoperative biliary leakage occurring after urgent cholecystectomy with a long-term follow-up.METHODS: This is an observational database study conducted in a tertiary care center. All consecutive patients who underwent endoscopic retrograde cholangiography(ERC) for presumed postoperative biliary leakage after urgent cholecystectomy in the period between April 2008 and April 2013 were considered for this study. Patients with bile duct transection and biliary strictures were excluded. Biliary leakage was suspected in the case of bile appearance from either percutaneous drainage of abdominal collection or abdominal drain placed at the time of cholecystectomy. Procedural and main clinical characteristics of all consecutive patients with postoperative biliary leakage after urgent cholecystectomy, such as indication for cholecystectomy, etiology and type of leakage, ERC findings and post-ERC complications, were collected from our electronic database. All patients in whom the leakage was successfully treated endoscopically were followed-up after they were discharged from the hospital and the main clinical characteristics, laboratory data and common bile duct diameter were electronically recorded. RESULTS: During a five-year period, biliary leakage was recognized in 2.2% of patients who underwent urgent cholecystectomy. The median time from cholecystectomy to ERC was 6 d(interquartile range, 4-11 d). Endoscopic interventions to manage biliary leakage included biliary stent insertion with or without biliary sphincterotomy. In 23(77%) patients after first endoscopic treatment bile flow through existing surgical drain ceased within 11 d following biliary therapeutic endoscopy(median, 4 d; interquartile range, 2-8 d). In those patients repeat ERC was not performed andthe biliary stent was removed on gastroscopy. In seven(23%) patients repeat ERC was done within one to fourth week after their first ERC, depending on the extent of the biliary leakage. In two of those patients common bile duct stone was recognized and removed. Three of those seven patients had more complicated clinical course and they were referred to surgery and were excluded from long-term follow-up. The median interval from endoscopic placement of biliary stent to demonstration of resolution of bile leakage for ERC treated patients was 32 d(interquartile range, 28-43 d). In a CuCl/bpy/RCl/St system, a bimodal GPC peak at the early stage of polymerization was observed, and a concept of multi active species was proposed to explain this phenomenon. In a CuCl/phen (DPP)/RCl/St system, the \%M\%\-n of polystyrene (PS) increased linearly with St conversion and ln[M] o/[M] also increased linearly with time, indicating the living nature of this system.
