1Clinical epidemiology and disease burden of nonalcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease(NAFLD) is defined as the presence of hepatic fat accumulation after the exclusion of other causes of hepatic steatosis, including other causes of liver disease, excessive alcohol consumption, and other conditions that may lead to hepatic steatosis. NAFLD encompasses a broad clinical spectrum ranging from nonalcoholic fatty liver to nonalcoholic steatohepatitis(NASH), advanced fibrosis, cirrhosis, and finally hepatocellular carcinoma(HCC). NAFLD is the most common liver disease in the world and NASH may soon become the most common indication for liver transplantation. Ongoing persistence of obesity with increasing rate of diabetes will increase the prevalence of NAFLD, and as this population ages, many will develop cirrhosis and end-stage liver disease. There has been a general increase in the prevalence of NAFLD, with Asia leading the rise, yet the United States is following closely behind with a rising prevalence from 15% in 2005 to 25% within 5 years. NAFLD is commonly associated with metabolic comorbidities, including obesity, type Ⅱ diabetes, dyslipidemia, and metabolic syndrome. Our understanding of the pathophysiology of NAFLD is constantly evolving. Based on NAFLD subtypes, it has the potential to progress into advanced fibrosis, end-stage liver disease and HCC. The increasing prevalence of NAFLD with advanced fibrosis, is concerning because patients appear toexperience higher liver-related and non-liver-related mortality than the general population. 2Intrahepatic biliary cystic neoplasms:Surgical results of 9 patients and literature review显示文摘AIM: To investigate the eligible management of the cystic neplasms of the liver. METHODS: The charts of 9 patients who underwent surgery for intrahepatic biliary cystic liver neoplasms between 2003 and 2008 were reviewed retrospectively. Informed consent was obtained from the patients and approval was obtained from the designated review board of the institution. RESULTS: All patients were female with a median (range) age of 49 (27-60 years). The most frequent symptom was abdominal pain in 6 of the patients. Four patients had undergone previous laparotomy (with other diagnoses) which resulted in incomplete surgery or recurrences. Liver resection (n = 6) or enucleation (n = 3) was performed. The final diagnosis was intrahepaticbiliary cystadenoma in 8 patients and cystadenocarcinoma in 1 patient. All symptoms resolved after surgery. There has been no recurrence during a median (range) 31 (7-72) mo of follow up. CONCLUSION: In spite of the improvement in imaging modalities and increasing recognition of biliary cystadenoma and cystadenocarcinoma, accurate preoperative diagnosis may be difficult. 3Effect of harmless acute pancreatitis score, red cell distribution width and neutrophil/lymphocyte ratio on the mortality of patients with nontraumatic acute pancreatitis at the emergency department显示文摘BACKGROUND: Harmless acute pancreatitis score(HAPS), neutrophile/lymphocyte ratio and red blood cell distribution width(RDW) are used to determine the early prognosis of patients diagnosed with nontraumatic acute pancreatitis in the emergency department(ED).METHODS: Patients diagnosed with acute pancreatitis(K 85.9) in the ED according to the ICD10 coding during one year were included in the study. Patients with chronic pancreatitis and those who had missing data in their files were excluded from the study. Patients who did not have computed tomography(CT) in the ED were not included in the study.RESULTS: Ultimately, 322 patients were included in the study. The median age of the patients was 53.1(IQR=36–64). Of the patients, 68.1%(n=226) had etiological causes of the biliary tract. The mortality rate of these patients within the first 48 hours was 4.3%(n=14). In the logistic regression analysis performed by using Balthazar classification, HAPS score, RDW, neutrophile/lymphocyte ratio, age, diabetes mellitus and systolic blood pressure, the only independent variable in determining mortality was assigned as Balthazar classification(OR: 15; 95% CI: 3.5 to 64.4).CONCLUSIONS: HAPS, neutrophile/lymphocyte ratio and RDW were not effective in determining the mortality of nontraumatic acute pancreatitis cases within the first 48 hours. 4Orbital inflammatory disease:Pictorial review and differential diagnosis显示文摘Orbital inflammatory disease(OID) represents a collec tion of inflammatory conditions affecting the orbit. OID is a diagnosis of exclusion, with the differential diagno sis including infection, systemic inflammatory conditions and neoplasms, among other conditions. Inflammatory conditions in OID include dacryoadenitis, myositis, cel lulitis, optic perineuritis, periscleritis, orbital apicitis, and a focal mass. Sclerosing orbital inflammation is a rare condition with a chronic, indolent course involving dense fibrosis and lymphocytic infiltrate. Previously though to be along the spectrum of OID, it is now considered a distinct pathologic entity. Imaging plays an importan role in elucidating any underlying etiology behind orbita inflammation and is critical for ruling out other condi tions prior to a definitive diagnosis of OID. 5Therapeutic potential of flavonoids in inflammatory bowel disease: A comprehensive review显示文摘The inflammatory process plays a central role in the development and progression of numerous pathological situations,such as inflammatory bowel disease(IBD),autoimmune and neurodegenerative diseases,metabolic syndrome,and cardiovascular disorders. IBDs involve inflammation of the gastrointestinal area and mainly comprise Crohn's disease(CD) and ulcerative colitis(UC). Both pathological situations usually involve recurring or bloody diarrhea,pain,fatigue and weight loss. There is at present no pharmacological cure for CD or UC. However,surgery may be curative for UC patients. The prescribed treatment aims to ameliorate the symptoms and prevent and/or delay new painful episodes. Flavonoid compounds are a large family of hydroxylated polyphenolic molecules abundant in plants,including vegetables and fruits which are the major dietary sources of these compounds for humans,together with wine and tea. Flavonoids are becoming very popular because they have many health-promoting and disease-preventive effects. Most interest has been directed towards the antioxidant activity of flavonoids,evidencing a remarkable free-radical scavenging capacity. However,accumulating evidence suggests that flavonoids have many other biological properties,including anti-inflammatory,antiviral,anticancer,and neuroprotective activities through different mechanisms of action. 6地理信息系统技术在伤寒发热监测中应用显示文摘目的运用地理信息系统(GIS)技术,了解各居民点发热流行率和评价以医疗机构为哨点的伤寒发热监测系统的敏感性。方法收集医学人口普查数据、疾病监测系统信息数据、现场采集居民居住点全球卫星定位数据等资料,建立GIS系统和以人口为基数的发热监测系统;对所有发热就诊病人进行身份和居住地址确认,并制作各居民点年发热就诊率GIS分布图。结果共采集3776个居民居住的全球定位系统(GPS)定位点,人群发热就诊率为1.11‰,发热就诊人次占总门诊人次的16.3%,监测区内的发热就诊病人占所有发热就诊病人的83.5%。在3种类型的医疗机构中,门诊量以个体门诊类最多,但其发热就诊人次占总门诊人次的比率最低(12.6%)。监测区内发热1d或以上的病人地址和身份(ADDRESS-ID)确认率达到95%。根据居民点年发热就诊率GIS分布图,发热就诊率最高的居民点达到17.81‰,最低为0;发热就诊率高的居民点多集中在城市。结论利用GIS技术可直观准确的了解疾病流行率的分布,可迅速直观的寻找出监测系统中的薄弱环节和推断出可能存在的问题。杨进 董柏青 张杰 廖和壮 梁大斌 吴兴华 司国爱 杨宏徽 Jin Kyung Park R Leon Ochiai Camilo J Acosta Moharmmad Ali John D Clemens 2007中国公共卫生2007,23,9:8

8Application of SALTMED and HYDRUS-1D models for simulations of soil water content and soil salinity in controlled groundwater depth显示文摘Salinization is a gradual process that should be monitored.Modelling is a suitable alternative technique that saves time and cost for the field monitoring.But the performance of the models should be evaluated using the measured data.Therefore,the aim of this study was to evaluate and compare the SALTMED and HYDRUS-1D models using the measured soil water content,soil salinity and wheat yield data under different levels of saline irrigation water and groundwater depth.The field experiment was conducted in 2013 and in this research three controlled groundwater depths,i.e.,60(CD60),80(CD80)and 100(CD100)cm and two salinity levels of irrigation water,i.e.,4(EC4)and 8(EC8)dS/m were used in a complete randomized design with three replications.Soil water content and soil salinity were measured in soil profile and compared with the predicted values by the SALTMED and HYDRUS-1D models.Calibrations of the SALTMED and HYDRUS-1D models were carried out using the measured data under EC4-CD100 treatment and the data of the other treatments were used for validation.The statistical parameters including normalized root mean square error(NRMSE)and degree of agreement(d)showed that the values for predicting soil water content and soil salinity were more accurate in the HYDRUS-1D model than in the SALTMED model.The NRMSE and d values of the HYDRUS-1D model were 9.6%and 0.64 for the predicted soil water content and 6.2%and 0.98 for the predicted soil salinity,respectively.These indices of the SALTMED model were 10.6%and 0.81 for the predicted soil water content and 11.0%and 0.97 for the predicted soil salinity,respectively.According to the NRMSE and d values for the predicted wheat yield(9.8%and 0.91,respectively)and dry matter(2.9%and 0.99,respectively),we concluded that the SALTMED model predicted the wheat yield and dry matter accurately.Masoud NOSHADI Saghar FAHANDEJ-SAADI Ali R SEPASKHAH 2020Journal of Arid Land2020,12,3:4

10Comparative study of Mo2Ga2C with superconducting MAX phase Mo2GaC: First-principles calculations显示文摘The structural, electronic, optical and thermodynamic properties of Mo_2Ga_2C are investigated using density functional theory(DFT) within the generalized gradient approximation(GGA). The optimized crystal structure is obtained and the lattice parameters are compared with available experimental data. The electronic density of states(DOS) is calculated and analyzed. The metallic behavior for the compound is confirmed and the value of DOS at Fermi level is 4.2 states per unit cell per e V. Technologically important optical parameters(e.g., dielectric function, refractive index, absorption coefficient, photo conductivity, reflectivity, and loss function) are calculated for the first time. The study of dielectric constant(ε1) indicates the Drude-like behavior. The absorption and conductivity spectra suggest that the compound is metallic.The reflectance spectrum shows that this compound has the potential to be used as a solar reflector. The thermodynamic properties such as the temperature and pressure dependent bulk modulus, Debye temperature, specific heats, and thermal expansion coefficient of Mo_2Ga_2C MAX phase are derived from the quasi-harmonic Debye model with phononic effect also for the first time. 11Evaluation of immunogenicity and reactogenicity of recombinant DNA hepatitis B vaccine produced in India显示文摘AIM: (1) To gain information on immune responses to an accelerated schedule of 0, 1, and 2 mo in paramedical staff and BDS students who are at an increased risk of getting hepatitis B infection and come under high risk groups. (2) To assess the efficacy and safety of EnivacHB in different age groups, using genetically modified yeast strain Pichia pastoris, a new recombinant hepatitis B vaccine developed and manufactured in India.METHODS: A prospective, comparative, and single blinded trial of rapid (0, 1, and 2 mo) hepatitis B immunization schedulewas reported. A total of three hundred and seven (212 females and 95 males) healthy volunteers divided into three age groups (18-29, 30-39,and 40-49) were enrolled after screening for markers of hepatitis B. All the volunteers received 20 mg of the vaccine intramuscularly at 0, 1, and 2 mo.RESULTS: Geometric mean titers were calculated pre and post vaccination. Before immunization the GMT was 0.0124 mIU/mL. One month after the administration of the third dose of recombinant vaccine 296/307 (96.5%)subjects achieved seroprotective levels of anti-HBs. The geometric mean anti-HBs titers achieved after one month of the third dose was 2 560.0 mIU/mL. The geometric mean anti-HBs titer of males was 2 029.0 mIU/mL, while that of the females was 2 759.0 mIU/mL. In the age group of 18-29 years, anti-HBs titer was 3 025.0 mIU/mL, while that in the age group of 30-39 years was 2096.0 mIU/mL. In third age group of 40-49 years, antiHBs titer was 1 592.0 mIU/mL. Hyper-responses (antiHBs≥100 mIU/mL) were shown in 88.0% (271/307) of subjects. Eleven (3.5%) subjects responded poorly to the vaccine in the age group of 40-49 years. 12Comparative induction of controlled circulation by magnesium and remifentanil in spine surgery显示文摘AIM: To evaluate the efficacy of magnesium sulfate(MGS) in comparison with remifentanil for induction of relative hypotension in posterior fusion of spine(PSF).METHODS: In this randomized clinical trial, 40 patients with the American Society of Anesthesiologists Ⅰ and Ⅱ physical status undergoing lumbar PSF were randomized to receive remifentanil(REM) 0.15 μg/kg or MGS 50 mg/kg for controlled hypotension. The administering anesthesiologist was blinded to the medication. Continuous infusion was maintained at a fixed volume rate to deliver precalculated doses of either study drugs. All other aspects of anesthesia and surgery were similar in the two groups. The target mean arterial pressure(MAP) range used in this study was 60-70 mm Hg. In the course of surgery, the hemodynamic variables, volumeof blood loss, urine output, fluid intake and surgeon's satisfaction were recorded. Data was analyzed with SPSS version 13.0 and P values less than 0.05 were considered significant.RESULTS: Twenty patients in the MGS group and 19 patients in the REM group were studied. There was no difference between the two groups in the hemodynamic variables, blood loss, urine output, fluid requirement and surgeon's satisfaction for exposure. The target MAP was achieved in 75% of Mg and 58% of remifentanil groups. 14Design of a multiplex PCR method for detection of toxigenicpathogenic in Vibrio cholerae显示文摘Objective:To study virulence and regulatory genes(hlyA,ctxB,tcpI) in clinical strains of Vibrio ckolerae(V.cholerae),simultaneously.Methods:Three important genes,tepI,hlyA and ctxB were used for detection of toxigenic and pathogenic V.cholera by chain reaction assay method. 15Effect of fluence and ambient environment on the surface and structural modification of femtosecond laser irradiated Ti显示文摘Under certain conditions, ultrafast pulsed laser interaction with matter leads to the formation of self-organized conical as well as periodic surface structures(commonly reffered to as, laser induced periodic surface structures, LIPSS). The purpose of the present investigations is to explore the effect of fsec laser fluence and ambient environments(Vacuum &O_2) on the formation of LIPSS and conical structures on the Ti surface. The surface morphology was investigated by scanning electron microscope(SEM). The ablation threshold with single and multiple(N = 100) shots and the existence of an incubation effect was demonstrated by SEM investigations for both the vacuum and the O_2 environment. The phase analysis and chemical composition of the exposed targets were performed by x-ray diffraction(XRD) and energy dispersive x-ray spectroscopy(EDS), respectively. SEM investigations reveal the formation of LIPSS(nano & micro). FFT d-spacing calculations illustrate the dependence of periodicity on the fluence and ambient environment. The periodicity of nano-scale LIPSS is higher in the case of irradiation under vacuum conditions as compared to O_2. Furthermore, the O2 environment reduces the ablation threshold. XRD data reveal that for the O_2 environment, new phases(oxides of Ti) are formed. EDS analysis exhibits that after irradiation under vacuum conditions, the percentage of impurity element(Al) is reduced. 17Micronucleus analysis in patients with colorectal adenocarcinoma and colorectal polyps显示文摘AIM:To determine,by counting micronucleus (MN) frequencies,whether chromosomal or DNA damage have an effect on the pathogenesis of early colorectal adenocarcinoma (CRC). METHODS:We analyzed MN frequencies in 21 patients with CRC,24 patients with colon polyps [10 neoplastic polyps (NP) and 14 non-neoplastic polyps (NNP)] and 20 normal controls. RESULTS:MN frequency was significantly increased in CRC patients and in NP patients compared with controls (3.72 ± 1.34,3.58 ± 1.21 vs 1.97 ± 0.81,P < 0.001). However,there was no difference in the MN frequency between CRC patients and NP patients (P > 0.05). Similarly,there was no difference in the MN frequency between NNP patients (2.06 ± 0.85) and controls (P > 0.05). 19Malingering or simulation in ophthalmology-visual acuity显示文摘Simulation can be defined as malingering,or sometimes functional visual loss(FVL).It manifests as either simulating an ophthalmic disease(positive simulation),or denial of ophthalmic disease(negative simulation).Conscious behavior and compensation or indemnity claims are prominent features of simulation.Since some authors suggest that this is a manifestation of underlying psychopathology,even conversion is included in this context.In today's world,every ophthalmologist can face with simulation of ophthalmic disease or disorder.In case of simulation suspect,the physician's responsibility is to prove the simulation considering the disease/disorder first,and simulation as an exclusion.In simulation examinations,the physician should be firm and smart to select appropriate test(s) to convince not only the subject,but also the judge in case of indemnity or compensation trials.Almost all ophthalmic sensory and motor functions including visual acuity,visual field,color vision and night vision can be the subject of simulation.Examiner must be skillful in selecting the most appropriate test.Apart from those in the literature,we included all kinds of simulation in ophthalmology.In addition,simulation examination techniques,such as,use of OCT(optical coherence tomography),frequency doubling perimetry(FDP),and modified polarization tests were also included.In this review,we made a thorough literature search,and added our experiences to give the readers up-to-date information on malingering or simulation in ophthalmology.Ali Ihsan Incesu Güngr Sobaci 2011International Journal of Ophthalmology(English edition)2011,4,5:2
