1Cardiovascular risk factors for acute stroke: Risk profiles in the different subtypes of ischemic stroke显示文摘Timely diagnosis and control of cardiovascular risk factors is a priority objective for adequate primary and secondary prevention of acute stroke. Hypertension, atrial fibrillation and diabetes mellitus are the most common risk factors for acute cerebrovascular events, although novel risk factors, such as sleep-disordered breathing, inflammatory markers or carotid intima-media thickness have been identified. However, the cardiovascular risk factors profile differs according to the different subtypes of ischemic stroke. Atrial fibrillation and ischemic heart disease are more frequent in patients with cardioembolic infarction, hypertension and diabetes in patients with lacunar stroke, and vascular peripheral disease, hypertension, diabetes, previous transient ischemic attack and chronic obstructive pulmonary disease in patients with atherothrombotic infarction. 2Diagnosis and treatment of hepatocellular carcinoma: An update显示文摘Hepatocellular carcinoma(HCC) is one of the most common malignancies leading to high mortality rates in the general population; in cirrhotic patients, it is the primary cause of death. The diagnosis is usually delayed in spite of at-risk population screening recommendations, i.e., patients infected with hepatitis B or C virus. Hepatocarcinogenesis hinges on a great number of genetic and molecular abnormalities that lead to tumor angiogenesis and foster their dissemination potential. The diagnosis is mainly based on imaging studies such as computed tomography and magnetic resonance, in which lesions present a characteristic classical pattern of early arterial enhancement followed by contrast medium 'washout' in late venous phase. On occasion, when imaging studies are not conclusive, biopsy of the lesion must be performed to establish the diagnosis. The Barcelona Clinic Liver Cancer staging method is the most frequently used worldwide and recommended by the international guidelines of HCC management. Currently available treatments include tumor resection, liver transplant, sorafenib and locoregional therapies(alcoholization, radiofrequency ablation, chemoembolization). The prognosis of hepatocarcinoma is determined according to the lesion's stage and in cirrhotic patients, on residual liver function. 5Drawing the Yongquan protocol into the different stages of the cardiopulmonary resuscitation sequence显示文摘AIM: To introduce new applications into the ILCORcardiopulmonary resuscitation(CPR) 'chain' sequence.METHODS: Stages of the CPR sequence('chain'): prior to the application of chest massage: assess the victim's state of consciousness and lung-heart failure;seek help(call 911), or in situations in which it is impossible to start the ILCOR protocol:(1) if the victim is trapped in car crash, overturned car, landslide, massive number of victims or catastrophe; or(2) delayed CPR.During chest compression: Yongquan is simultaneously stimulated by a third rescuer. During defibrillator application: activate K-1 Yongquan through needles before defibrillation. Unsuccessful CPR: 'gold standard'for legal clinical death.RESULTS: Implies comparing two hypotheses: Ho(null hypothesis) demonstrates no association between the two variables studied; Ha(alternative hypothesis) implies some degree of relation between them. Difference between the two treatments is observed. If it is greater than the standard error multiplied by a coefficient of security, the difference is significant: Ha will be accepted and Ho rejected. First we will compare CPR without defibrillator(method 'A') and K-1 Yongquan method(method 'B'), using percentages of representative samples(treatment 'A': 6.4% response, treatment 'B': 85% response). If │PA- PB│ is greater than the product of 1.96 times the standard error, the difference is significant. Because │PA- PB│ = 0.786 is greater than 0.098, the difference between 0.064 and 0.85 is statistically significant. Thus, we reject Ho and accept Ha as correct. Thus, it is improbable that chance was responsible for this association. This analysis shows that K-1 Yongquan method has a 'quality guarantee'. Second, we compare defibrillators('A') with K-1 Yongquan method('B')(treatment 'A': 48%, treatment 'B': 84%, │PA- PB│= 0.36; │PA- PB│ = 0.36 is greater than SE × 1.96 = 0.0148 and also statistically significant, demonstrating again the comparative value of the Yongquan method. 6Current knowledge on the laboratory diagnosis of Clostridium difficile infection显示文摘Clostridium difficile (C. difficile) is a spore-forming, toxin-producing, gram-positive anaerobic bacterium that is the principal etiologic agent of antibiotic-associated diarrhea. Infection with C. difficile (CDI) is characterized by diarrhea in clinical syndromes that vary from selflimited to mild or severe. Since its initial recognition as the causative agent of pseudomembranous colitis, C. difficile has spread around the world. CDI is one of the most common healthcare-associated infections and a significant cause of morbidity and mortality among older adult hospitalized patients. Due to extensive antibiotic usage, the number of CDIs has increased. Diagnosis of CDI is often difficult and has a substantial impact on the management of patients with the disease, mainly with regards to antibiotic management. The diagnosis of CDI is primarily based on the clinical signs and symptoms and is only confirmed by laboratory testing. Despite the high burden of CDI and the increasing interest in the disease, episodes of CDI are often misdiagnosed. The reasons for misdiagnosis are the lack of clinical suspicion or the use of inappropriate tests. The proper diagnosis of CDI reduces transmission, prevents inadequate or unnecessary treatments, and assures best antibiotic treatment. 9Stroke and sleep-disordered breathing: A relationship under construction显示文摘The association between sleep-disordered breathing(SDB) and cardiovascular risk has been the focus of attention in recent years.Sleep disorders are emerging risk factors for cardiovascular disease and have been related to the whole spectrum of stroke,including transient ischemic attack,ischemic cerebral infarction and intracerebral haemorrhage.It has been shown that lacunar stroke or lacunar infarctions affecting the internal capsule or the protuberance are associated with a higher frequency of SDB.Acute stroke patients with associated SDB have a worse prognosis and a higher mortality as compared to patients with first-ever stroke without SDB.Preliminary studies provide evidence of the usefulness of treatment with continuous positive airway pressure when SDB is present in stroke patients.Olga Parra Adrià Arboix 2016World Journal of Clinical Cases2016,4,2:5

11Imatinib-induced fatal acute liver failure显示文摘Imatinib mesylate is a drug that has been approved for treatment of chronic myeloid leukemia (CML) in blast crisis, accelerated or chronic phase, and also for advanced gastrointestinal stromal tumors. Severe hepatic toxicity and three deaths from hepatic failure have been reported. We report the case of a 51-year-old woman who was admitted to our institution with severe acute hepatitis. She was diagnosed with CML and began treatment with imatinib mesylate at a dose of 400 mg/d. Five months after beginning treatment, she developed severe hepatitis associated with coagulopathy, and was admitted to our institution. She had been consuming acetaminophen 500-1000 mg/d after the onset of symptoms. She had a progressive increase in bilirubin level and a marked decrease of clotting factor Ⅴ. Five days after admission, grade Ⅱ encephalopathy developed and she was referred for liver transplantation. Her clinical condition progressively deteriorated, and 48 h after being referred for transplantation she suffered a cardiac arrest and died. This report adds concern about the possibility of imatinib-mesylate-induced hepatotoxicity and liver failure, particularly in the case of concomitant use with acetaminophen. 13Using GEDI lidar data and airborne laser scanning to assess height growth dynamics in fast-growing species:a showcase in Spain显示文摘Background:The NASA’s Global Ecosystem Dynamics Investigation(GEDI)satellite mission aims at scanning forest ecosystems on a multi-temporal short-rotation basis.The GEDI data can validate and update statistics from nationwide airborne laser scanning(ALS).We present a case in the Northwest of Spain using GEDI statistics and nationwide ALS surveys to estimate forest dynamics in three fast-growing forest ecosystems comprising 211,346 ha.The objectives were:i)to analyze the potential of GEDI to detect disturbances,ii)to investigate uncertainty source regarding non-positive height increments from the 2015–2017 ALS data to the 2019 GEDI laser shots and iii)to estimate height growth using polygons from the Forest Map of Spain(FMS).A set of 258 National Forest Inventory plots were used to validate the observed height dynamics.Results:The spatio-temporal assessment from ALS surveying to GEDI scanning allowed the large-scale detection of harvests.The mean annual height growths were 0.79(SD=0.63),0.60(SD=0.42)and 0.94(SD=0.75)m for Pinus pinaster,Pinus radiata and Eucalyptus spp.,respectively.The median annual values from the ALS-GEDI positive increments were close to NFI-based growth values computed for Pinus pinaster and Pinus radiata,respectively.The effect of edge border,spatial co-registration of GEDI shots and the influence of forest cover in the observed dynamics were important factors to considering when processing ALS data and GEDI shots.Discussion:The use of GEDI laser data provides valuable insights for forest industry operations especially when accounting for fast changes.However,errors derived from positioning,ground finder and canopy structure can introduce uncertainty to understand the detected growth patterns as documented in this study.The analysis of forest growth using ALS and GEDI would benefit from the generalization of common rules and data processing schemes as the GEDI mission is increasingly being utilized in the forest remote sensing community.Juan Guerra-Hernández Adrián Pascual 2021Forest Ecosystems2021,8,1:4

15Glycoprotein biomarkers for the detection of pancreatic ductal adenocarcinoma显示文摘Pancreatic cancer(Pa C) shows a clear tendency to increase in the next years and therefore represents an important health and social challenge. Currently, there is an important need to find biomarkers for PaC early detection because the existing ones are not useful for that purpose. Recent studies have indicated that there is a large window of time for PaC early detection, which opens the possibility to find early biomarkers that could greatly improve the dismal prognosis of this tumor. The present manuscript reviews the state of the art of the existing PaC biomarkers. It focuses on the anomalous glycosylation process and its role in PaC. Glycan structures of glycoconjugates such as glycoproteins are modified in tumors and these modifications can be detected in biological fluids of the cancer patients. Several studies have found serum glycoproteins with altered glycan chains in PaC patients, but they have not shown enough specificity for PaC. To find more specific cancer glycoproteins we propose to analyze the glycan moieties of a battery of glycoproteins that have been reported to increase in PaC tissues and that can also be found in serum. 16Cortical laminar necrosis related to migrainous cerebral infarction显示文摘We present a 29-year-old woman with a long history of attacks of migraine with and without visual aura.She was a heavy smoker(20 cigarettes/d) and was currently taking oral contraceptives. During a typical migraine attack with aura, she developed dysarthria,left brachial hemiparesis and hemihypoesthesia and brief and autolimited left clonic facial movements. Four hours after onset, vascular headache and focal sensorimotor neurological deficit were the only persisting symptoms and, on seventh day, she was completely recovered. Brain magnetic resonance imaging on day 20 after onset showed a subacute ischemic lesion in the right temporo-parietal cortex compatible with cortical laminar necrosis(CLN). Extensive neurological work-up done to rule out other known causes of cerebral infarct with CLN was unrevealing. Only ten of 3.808 consecutive stroke patients included in our stroke registry over a 19-year period fulfilled the strictly defined International Headache Society criteria for migrainous stroke. The present case is the unique one in our stroke registry that presents CLN related to migrainous cerebral infarction. 17杂原子修饰的碳纳米管对高级氧化过程的催化性能(英文)显示文摘Multi-walled carbon nanotubes (CNTs) were submitted to chemical and thermal treatments in order to incorporate different heteroatoms on the surface. O-, S- and N-containing groups were successfully introduced onto the CNTs without significant changes of the textural properties. The cata-lytic activity of these heteroatom-modified CNTs was studied in two liquid phase oxidation processes: catalytic ozonation and catalytic wet air oxidation (CWAO), using oxalic acid and phenol as model compounds. In both cases, the presence of strongly acidic O-containing groups was found to decrease the catalytic activity of the CNTs. On the other hand, the introduction of S species (mainly sulfonic acids) enhanced the removal rate of the model compounds, particularly in the CWAO of phenol. Additional experiments were performed with a radical scavenger and sodium persulfate, in order to clarify the reaction mechanism. 18Genetic diversity,population structure,and relationships in a collection of pepper(Capsicum spp.)landraces from the Spanish centre of diversity revealed by genotypingby-sequencing(GBS)显示文摘Pepper(Capsicum spp.)is one of the most important vegetable crops;however,pepper genomic studies lag behind those of other important Solanaceae.Here we present the results of a high-throughput genotyping-by-sequencing(GBS)study of a collection of 190 Capsicum spp.accessions,including 183 of five cultivated species(C.annuum,C.chinense,C.frutescens,C.baccatum,and C.pubescens)and seven of the wild form C.annuum var.glabriusculum.Sequencing generated 6,766,231 high-quality read tags,of which 40.7%were successfully aligned to the reference genome.SNP calling yielded 4083 highly informative segregating SNPs.Genetic diversity and relationships of a subset of 148 accessions,of which a complete passport information was available,was studied using principal components analysis(PCA),discriminant analysis of principal components(DAPC),and phylogeny approaches.C.annuum,C.baccatum,and C.chinense were successfully separated by all methods.Our population was divided into seven clusters by DAPC,where C.frutescens accessions were clustered together with C.chinense.C.annuum var.glabriusculum accessions were spread into two distinct genetic pools,while European accessions were admixed and closely related.Separation of accessions was mainly associated to differences in fruit characteristics and origin.Phylogeny studies showed a close relation between Spanish and Mexican accessions,supporting the hypothesis that the first arose from a main genetic flow from the latter.Tajima’s D statistic values were consistent with positive selection in the C.annuum clusters,possibly related to domestication or selection towards traits of interest.This work provides comprehensive and relevant information on the origin and relationships of Spanish landraces and for future association mapping studies in pepper.Leandro Pereira-Dias Santiago Vilanova Ana Fita Jaime Prohens Adrián Rodríguez-Burruezo 2019Horticulture Research2019,6,1:4
19Fatty liver disease,an emerging etiology of hepatocellular carcinoma in Argentina显示文摘AIM To investigate any changing trends in the etiologies of hepatocellular carcinoma(HCC) in Argentina during the last years. METHODS A longitudinal cohort study was conducted by 14 regional hospitals starting in 2009 through 2016. All adult patients with newly diagnosed HCC either with pathology or imaging criteria were included. Patients were classified as presenting non-alcoholic fatty liver disease(NAFLD) either by histology or clinically, provided that all other etiologies of liver disease were ruled out, fatty liver was present on abdominal ultrasound and alcohol consumption was excluded. Complete follow-up was assessed in all included subjects since the date of HCC diagnosis until death or last medical visit.RESULTS A total of 708 consecutive adults with HCC were included. Six out of 14 hospitals were liver transplant centers(n = 484). The prevalence of diabetes mellitus was 27.7%. Overall, HCV was the main cause of liver disease related with HCC(37%) including cirrhotic and non-cirrhotic patients, followed by alcoholic liver disease 20.8%, NAFLD 11.4%, cryptogenic 9.6%, HBV 5.4% infection, cholestatic disease and autoimmune hepatitis 2.2%, and other causes 9.9%. A 6-fold increase in the percentage corresponding to NAFLDHCC was detected when the starting year, i.e., 2009 was compared to the last one, i.e., 2015(4.3% vs 25.6%; P < 0.0001). Accordingly, a higher prevalence of diabetes mellitus was present in NAFLD-HCC group 61.7% when compared to other than NAFLD-HCC 23.3%(P < 0.0001). Lower median AFP values at HCC diagnosis were observed between NAFLD-HCC and non-NAFLD groups(6.6 ng/m L vs 26 ng/m L; P = 0.02).
