| 2 | Severe chest pain in a pediatric ulcerative colitis patient after 5-aminosalicylic acid therapy显示文摘Severe reactions to mesalamine products are rarely seen in pediatric patients. We report a case of a 12-year-old boy who had a severe cardiac reaction to a mesalamine product Asacol. Past medical history is significant for ulcerative colitis (UC) diagnosed at 9 years of age. Colo- noscopy one week prior to admission revealed pancoli- tis. He was treated with Asacol 800 mg three times per day and prednisone 20 mg/d. He was subsequently ad- mitted to the hospital for an exacerbation of his UC and started on intravenous solumedrol. He had improvement of his abdominal pain and diarrhea. The patient com- plained of new onset of chest pain upon initiating Asacol therapy. Electrocardiogram (ECG) revealed non-specific ST-T wave changes with T-wave inversion in the lateral leads. Echocardiogram (ECHO) revealed low-normal to mildly depressed left ventricular systolic function. The left main coronary artery and left anterior descending artery were mildly prominent measuring 5 mm and 4.7 mm, respectively. His chest pain completely resolved within 24-36 h of discontinuing Asacol. A repeat echo- cardiogram performed two days later revealed normal left ventricular function with normal coronary arteries (< 3.5 mm). | 3 | Ti leaching from activated ilmenite–Fe mixture at different milling energy levels显示文摘Mechanical activation processes on ilmenite concentrate were performed in three different energy levels. Iron powder as a reducing agent was added to ilmenite in the milling stage and the mechanically activated mixture was subjected to acid leaching. The leaching experiments were designed using the Taguchi method, and the optimum ranges were obtained. Furthermore, response surface methodology(RSM) was used to optimize the critical parameters in the leaching system to achieve the highest titanium(Ti) leachability. Based on the inductively coupled plasma-optical emission spectrometry(ICP-OES) results, maximum leaching recovery of Ti(80%) was obtained using activated Ti concentrates at a medium activation energy level, which is calculated to be 25.38 kJ/g, using 15 vol% hydrochloric acid(HCl), a temperature of 70°C, leaching time of 3 h, and a solid-to-liquid ratio of 0.05 g×mL^(–1). Intensifying the milling energy from a low to high level led to a decrease in the mean crystallite size and also structure homogenization at the high energy level. According to the transmission electron microscopy(TEM) images, the mean grain size of the ilmenite/Fe nanocomposite was about 30 nm at the medium energy level sample. | 4 | Is hemodialysis a reason for unresponsiveness to hepatitis B vaccine? Hepatitis B virus and dialysis therapy显示文摘Impaired renal function is associated with a high risk of chronicity of hepatitis B virus(HBV) infection.Patients on hemodialysis(HD) or peritoneal dialysis are at an increased risk of viral transmission due to frequent necessity of blood product transfer as well as use of contaminated dialysate or dialysis materials.Additionally,health professionals may cause viral spread via contaminated hands and carelessness against hygiene rules.The frequency of chronic HBV infection may be as high as 80% in patients on renal replacement therapies.This is because HBV vaccination is essential to eliminate chronic HBV infection.However,response rates of HD patients to HBV vaccination vary between 10%-50%.Dialysis adequacy and early vaccination before the onset of dialysis therapy seem to be major determinants of high seroconversion rates.Older age,male gender,duration of dialysis therapy and nutritional status are other well-known factors associated with seroconversion rate.There are controversial reports regarding the role of the presence of diabetes mellitus,HCV positivity,erythropoietin resistance,hyperparathyroidism,and vitamin D inadequacy.The role of genetic alteration in the functions or production of cytokines still needs to be elucidated. | Dede Sit Bennur Esen Ahmet Engin Atay Hasan Kayabasi | 2015 | World Journal of Hepatology2015，7，5： | 3 |
