1三倍体毛白杨KP和EMCC蒸煮及TCF漂白过程中木素分子质量的变化显示文摘以三倍体毛白杨为研究对象,进行了常规和深度脱木素蒸煮,并对纸浆进行OQP漂序的全无氯漂白。分别从原料、KP纸浆、EMCC纸浆、以及其氧脱木素后的半漂浆和全漂浆中分离出原料木素和纸浆残余木素。木素分离采用酶-弱酸解的两段法。采用酸析法从蒸煮黑液中分离出溶出木素样品,经过弱酸水解得到纯的木素试样。通过凝胶渗透色谱(GPC),对所有试样分别进行了分子质量的检测,得出各种木素样品的分子质量及其分布,分析了木素溶出过程中分子质量的变化,并且对KP和EMCC两种纸浆中残余木素的平均分子质量、黑液中溶出木素的平均分子质量进行了对比。刘玉 詹怀宇 陈嘉川 Dimitris Argyropoulos 2005中国造纸学报2005,20,2:11
2Establishing the presence or absence of chronic kidney disease:Uses and limitations of formulas estimating the glomerular filtration rate显示文摘The development of formulas estimating glomerular filtration rate(eG FR) from serum creatinine and cystatin C and accounting for certain variables affecting the production rate of these biomarkers, including ethnicity, gender and age, has led to the current scheme of diagnosing and staging chronic kidney disease(CKD),which is based on e GFR values and albuminuria.This scheme has been applied extensively in various populations and has led to the current estimates of prevalence of CKD. In addition, this scheme is applied in clinical studies evaluating the risks of CKD and the efficacy of various interventions directed towards improving its course. Disagreements between creatinine-based and cystatin-based e GFR values and between e GFR values and measured GFR have been reported in various cohorts. These disagreements are the consequence of variations in the rate of production and in factors, other than GFR, affecting the rate of removal of creatinine and cystatin C. The disagreements create limitations for all e GFR formulas developed so far. The main limitations are low sensitivity in detecting early CKD in several subjects, e.g., those with hyperfiltration, and poor prediction of the course of CKD. 3结肠的腺癌和麦芽淋巴瘤的同时的出现: 一系列三个盒子显示文摘 Simultaneous development of adenocarcinoma and primary B cell lymphoma of mucosa-associated lymphoid tissue(MALT) lymphoma of the colon is rare;only one case has so far been reported out of 13 cases with the coexistence of colonic adenocarcinoma with involvement of the colon by lymphoma.We hereby present three more cases,two females(aged 75 and 71 years) and a male(aged 72 years).All three underwent colectomy based on a preoperative biopsy revealing colonic carcinoma.Histological examination of the resection specimens disclosed a colonic adenocarcinoma in two cases,whereas a tubulovillous adenoma with superficial foci of intraepithelial adenocarcinoma was seen in the thirdcase.Moreover,in all three cases,a coexisting MALT lymphoma was diagnosed in the colon(1 case),in both colon and adjacent lymph nodes(1 case) or in colonic lymph nodes and omentum(1 case).In the last case,a post-operative bone marrow biopsy revealed extensive infiltration of the bone marrow,due to which the patient received postoperative chemotherapy.Diagnostic and treatment issues are briefly discussed.Theodoros Argyropoulos Periklis Foukas Maria Kefala Panagiotis Xylardistos Sotirios Papageorgiou Nikolaos Machairas Evmorfia Boltetsou Anastasios Machairas Ioannis G Panayiotides 2012World Journal of Gastrointestinal Oncology2012,4,4:4

5Fluid balance concepts in medicine:Principles and practice显示文摘The regulation of body fluid balance is a key concern in health and disease and comprises three concepts. The first concept pertains to the relationship between total body water(TBW) and total effective solute and is expressed in terms of the tonicity of the body fluids. Disturbances in tonicity are the main factor responsible for changes in cell volume, which can critically affect brain cell function and survival. Solutes distributed almost exclusively in the extracellular compartment(mainly sodium salts) and in the intracellular compartment(mainly potassium salts) contribute to tonicity, while solutes distributed in TBW have no effect on tonicity. The second body fluid balance concept relates to the regulation and measurement of abnormalities of sodium salt balance and extracellular volume. Estimation of extracellular volume is more complex and error prone than measurement of TBW. A key function of extracellular volume, which is defined as the effective arterial blood volume(EABV), is to ensure adequate perfusion of cells and organs. Other factors, including cardiac output, total and regional capacity of both arteries and veins, Starling forces in the capillaries, and gravity also affect the EABV. Collectively, these factors interact closely with extracellular volume and some of them undergo substantial changes in certain acute and chronic severe illnesses. Their changes result not only in extracellular volume expansion, but in the need for a larger extracellular volume compared with that of healthy individuals. Assessing extracellular volume in severe illness is challenging because the estimates of this volume by commonly used methods are prone to large errors in many illnesses. In addition, the optimal extracellular volume may vary from illness to illness, is only partially based on volume measurements by traditional methods, and has not been determined for each illness. Further research is needed to determine optimal extracellular volume levels in several illnesses. 6Hospital readmissions in decompensated cirrhotics: Factors pointing toward a prevention strategy显示文摘AIM To reduce readmissions and improve patient outcomes in cirrhotic patients through better understanding of readmission predictors.METHODS We performed a single-center retrospective study of patients admitted with decompensated cirrhosis from January 1, 2011 to December 31, 2013(n = 222). Primary outcomes were time to first readmission and 30-d readmission rate due to complications of cirrhosis. Clinical and demographic data were collected to help describe predictors of readmission, along with care coordination measures such as post-discharge status and outpatient follow-up. Univariate and multivariateanalyses were performed to describe variables associated with readmission.RESULTS One hundred thirty-two patients(59.4%) were readmitted at least once during the study period. Median time to first and second readmissions were 54 and 93 d, respectively. Thirty and 90-d readmission rates were 20.7 and 30.1 percent, respectively. Predictors of 30-d readmission included education level, hepatic encephalopathy at index, ALT more than upper normal limit and Medicare coverage. There were no statistically significant differences in readmission rates when stratified by discharge disposition, outpatient follow-up provider or time to first outpatient visit.CONCLUSION Readmissions are challenging aspect of care for cirrhotic patients and risk continues beyond 30 d. 7Hypertonicity:Clinical entities,manifestations and treatment显示文摘Hypertonicity causes severe clinical manifestations and is associated with mortality and severe short-term and longterm neurological sequelae. The main clinical syndromes of hypertonicity are hypernatremia and hyperglycemia.Hypernatremia results from relative excess of body sodium over body water. Loss of water in excess of intake,gain of sodium salts in excess of losses or a combination of the two are the main mechanisms of hypernatremia.Hypernatremia can be hypervolemic,euvolemic or hypovolemic. The management of hypernatremia addresses both a quantitative replacement of water and,if present,sodium deficit,and correction of the underlying pathophysiologic process that led to hypernatremia.Hypertonicity in hyperglycemia has two components,solute gain secondary to glucose accumulation in the extracellular compartment and water loss through hyperglycemic osmotic diuresis in excess of the losses of sodium and potassium. Differentiating between these two components of hypertonicity has major therapeutic implications because the first component will be reversed simply by normalization of serum glucose concentration while the second component will require hypotonic fluid replacement. 8Influence of colour changes and moisture content during banana drying on laser backscattering显示文摘Pre-drying treatments are frequently employed to preserve fruit quality.The objective of this research was to monitor colour changes of banana during drying by laser backscattering and to determine the influence of the fruit discolouration on the light distribution into banana tissue.Moreover,to examine the influence of drying on the laser backscatter,the relationship between moisture content and relative laser area of banana slices was analyzed with different degrees of colour degradation.The experiments were conducted at drying air temperature of 63℃with various pre-treatments like chilling,soaking in ascorbic/citric acid and dipping in distilled water.An untreated sample was used as a control.A laser diode emitting at 670 nm with 3 mW power was used as light source.The backscattering relative laser area was used as an indicator for the light absorption into the tissue.The high result achieved on coefficient of determination R^(2)(>0.93)confirmed linear relationship between relative laser area and moisture content.Treatment with ascorbic acid gave the best prediction of the moisture content with the standard error of 5.7 and 8.8 for the estimated intercept and slope.The results showed a significant difference of lightness(L*values)during drying according to the different treatments.As a result,colour degradation did not have a significant influence on the absorption of light at 670 nm wavelength.Giuseppe Romano Dimitrios Argyropoulos Klaus Gottschalk Emanuele Cerruto Joachim Müller 2010International Journal of Agricultural and Biological Engineering2010,3,2:2
